    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    The integration of digital home is focused on delivering a seamless, interoperable network that enabled the applications from Consumer Electronics (CE), Personal Computing (PC) and mobile devices industries to easily and conveniently enjoy content using any electronic device anywhere anytime in the home. Digital home vision can be fulfilled through a collaborative industry effort focused on delivering an interoperability framework for networked multimedia devices. Consumers are acquiring, experiencing, and managing an increasing amount of digital media on devices in both the CE and PC domains. As such, they want to easily and conveniently content regardless of the source across different devices and locations in the home. This paper covers wireless technologies that can contribute huge role in the digital home environment. The decision model selected to determine the best technology when subjective judgement is required to make a selection with many criteria is the Hierarchical Decision Model (HDM). The HDM model used ranks each of the technology solutions with a subjective probability. This paper builds the HDM model to determine the best technology for the digital home. The subjective probability results are summarized and analyzed in their contribution to the objective of providing the best wireless network solution for the home environment

    Pyrethroid resistance and its inheritance in a field population of Hippodamia convergens (Guérin-Méneville) (Coleoptera: Coccinellidae)

Retrieved from http://krex.ksu.eduThe convergent lady beetle (CLB), Hippodamia convergens (Guérin-Méneville), a species widely distributed and used in biological control, has exhibited high survival under field and laboratory conditions when treated with field rates of the pyrethroid λ-cyhalothrin, a highly unusual phenomenon for a natural enemy. This work investigated and characterized the phenomenon of pyrethroid resistance in a population of this species collected in Georgia, USA. The mechanism and level of resistance were evaluated by treating parental populations with λ-cyhalothrin ± piperonyl butoxide (PBO). The inheritance bioassay utilized parental crosses and backcrosses between parental populations to obtain testable progenies. Adult beetles from populations and progenies were topically treated with different doses of λ-cyhalothrin (technical grade) to calculate knockdown (KD) and lethal (LD) doses, and to investigate the dominance based on a single dose and whether resistance is autosomal and monogenic (null hypothesis). Genetic variation in the parental populations was examined by applying a discriminating dose for resistant individuals (0.5 g/L). The data indicate that resistance is due to at least two factors: knockdown resistance and enzymatic detoxification of the insecticide. The knockdown effect is recessive and linked to the X-chromosome. Variability in proportions of individuals within families dying following knockdown indicated genetic variation in the resistant population. Further studies should be done to investigate the role of sex linked inheritance of resistance in the species and interactions of the various mechanisms involved in resistance

    This article analyses legal aspects of the 'war on terror'. It argues that, by making recourse to a semantic of risk, danger and, in particular, precaution, the 'war on terror' blurs crucial political and legal categories of public and private, of peace and war, of combatants and civilians, thus redefining the relationship between political responsibility, time and security. As a consequence, the extrajudicial killing of individuals becomes a form of risk management that takes place beyond established mechanisms of accountability. © Sage Publications, Inc. 2008
