    Antenna with artificial magnetic conductor for wireless application

    — This paper discussed the recent works on Artificial Magnetic Conductor (AMC) with the antenna. Two different application for antenna with AMC have been discussed. The first application is the antenna with AMC that has been used for RFID system to detect the metal object. Without incorporating AMC with antenna the system cannot be detected. When the AMC is attached with the RFID Tag antenna the system is back to normal where longer distance is achieved. The second application is used for flexible AMC using textile material for signal transmission enhancement. Using two antennas for on body application the system has been tested using AMC and without AMC. It shows that the transmission is better when there is an AMC attach to the body compared without AMC.     Symmetries and exact solutions of some integrable Haldane-Shastry like spin chains

    By using a class of `anyon like' representations of permutation algebra, which pick up nontrivial phase factors while interchanging the spins of two lattice sites, we construct some integrable variants of SU(M)SU(M) Haldane-Shastry (HS) spin chain. Lax pairs and conserved quantities for these spin chains are also found and it is established that these models exhibit multi-parameter deformed or nonstandard variants of Y(glM)Y(gl_M) Yangian symmetry. Moreover, by projecting the eigenstates of Dunkl operators in a suitable way, we derive a class of exact eigenfunctions for such HS like spin chain and subsequently conjecture that these exact eigenfunctions would lead to the highest weight states associated with a multi-parameter deformed or nonstandard variant of Y(glM)Y(gl_M) Yangian algebra. By using this conjecture, and acting descendent operator on the highest weight states associated with a nonstandard Y(gl2)Y(gl_2) Yangian algebra, we are able to find out the complete set of eigenvalues and eigenfunctions for the related HS like spin-12{1\over 2} chain. It turns out that some additional energy levels, which are forbidden due to a selection rule in the case of SU(2) HS model, interestingly appear in the spectrum of above mentioned HS like spin chain having nonstandard Y(gl2)Y(gl_2) Yangian symmetry.Comment: 35 pages, latex, no figures, minor type errors are corrected, version to appear in Nucl.     Spin dependent extension of Calogero-Sutherland model through anyon like representations of permutation operators

    We consider a AN1A_{N-1} type of spin dependent Calogero-Sutherland model, containing an arbitrary representation of the permutation operators on the combined internal space of all particles, and find that such a model can be solved as easily as its standard su(M)su(M) invariant counterpart through the diagonalisation of Dunkl operators. A class of novel representations of the permutation operator PijP_{ij}, which pick up nontrivial phase factors along with interchanging the spins of ii-th and jj-th particles, are subsequently constructed. These `anyon like' representations interestingly lead to different variants of spin Calogero-Sutherland model with highly nonlocal interactions. We also explicitly derive some exact eigenfunctions as well as energy eigenvalues of these models and observe that the related degeneracy factors crucially depend on the choice of a few discrete parameters which characterise such anyon like representations.Comment: 25 pages, plain LaTex file, the results of sec.4 are presented in a more explicit way, to appear in Nucl.     Perceptions, Barriers, and Enablers on Salt Reduction in the Out-of-Home Sectors in Malaysia (MySaltOH) from the Perspective of Street Food Vendors, Caterers, and Consumers.

    OBJECTIVE: To explore the perspectives, barriers, and enablers on salt reduction in out-of-home sectors in Malaysia among street food vendors, caterers, and consumers. DESIGN: A qualitative study involving 22 focus group discussions and six in-depth interviews was conducted, recorded and transcribed verbatim. An inductive thematic analysis approach was employed to analyze the data. SETTING: Two in-depth interviews and 22 focus group discussions were conducted face-to-face. Four in-depth interviews were conducted online. PARTICIPANTS: Focus group discussions were conducted among 23 street food vendors, 21 caterers, and 76 consumers of various eateries. In-depth interviews were conducted among two street food vendors, and four caterers, individually. RESULTS: Consumers and food operators perceived a high salt intake within Malaysia's out-of-home food sectors. Food operators emphasized the necessity for a comprehensive salt reduction policy in the out-of-home sector involving all stakeholders. Consumers faced limited awareness and knowledge, counterproductive practices among food operators, and challenges in accessing affordable low-sodium food products; whereas food operators faced the lack of standardized guidelines and effective enforcement mechanisms, and uncooperative consumer practices. Both groups expressed that food quality and price of salt were also the barriers, and they advocated for awareness promotion, enhanced regulation of manufactured food products, and stricter enforcement targeting vendors. Consumers also suggested promoting and recognizing health-conscious food premises; whereas food operators suggested on knowledge enhancement tailored to them, strategies for gaining consumers acceptance, and maintaining food quality. CONCLUSIONS: These findings provide valuable insights that serve as foundational evidence for developing and implementing salt reduction policies within Malaysia's out-of-home sectors

    Comparative evaluation of methods for estimating retinal ganglion cell loss in retinal sections and wholemounts

    To investigate the reliability of different methods of quantifying retinal ganglion cells (RGCs) in rat retinal sections and wholemounts from eyes with either intact optic nerves or those axotomised after optic nerve crush (ONC). Adult rats received a unilateral ONC and after 21 days the numbers of Brn3a+ , bIII-tubulin+ and Islet-1+ RGCs were quantified in either retinal radial sections or wholemounts in which FluoroGold (FG) was injected 48 h before harvesting. Phenotypic antibody markers were used to distinguish RGCs from astrocytes, macrophages/microglia and amacrine cells. In wholemounted retinae, counts of FG+ and Brn3a+ RGCs were of similar magnitude in eyes with intact optic nerves and were similarly reduced after ONC. Larger differences in RGC number were detected between intact and ONC groups when images were taken closer to the optic nerve head. In radial sections, Brn3a did not stain astrocytes, macrophages/microglia or amacrine cells, whereas βIII-tubulin and Islet-1 did localize to amacrine cells as well as RGCs. The numbers of βIII-tubulin+ RGCs was greater than Brn3a+ RGCs, both in retinae from eyes with intact optic nerves and eyes 21 days after ONC. Islet-1 staining also overestimated the number of RGCs compared to Brn3a, but only after ONC. Estimates of RGC loss were similar in Brn3astained radial retinal sections compared to both Brn3a-stained wholemounts and retinal wholemounts in which RGCs were backfilled with FG, with sections having the added advantage of reducing experimental animal usage

Nonetheless, the underlying mechanisms are still elusive.     Inflammation and breast cancer. Metalloproteinases as common effectors of inflammation and extracellular matrix breakdown in breast cancer

    Two rapidly evolving fields are converging to impact breast cancer: one has identified novel substrates of metalloproteinases that alter immune cell function, and the other has revealed a role for inflammation in human cancers. Evidence now shows that the mechanisms underlying these two fields interact in the context of breast cancer, providing new opportunities to understand this disease and uncover novel therapeutic strategies. The metalloproteinase class of enzymes is well studied in mammary gland development and physiology, but mostly in the context of extracellular matrix modification. Aberrant metalloproteinase expression has also been implicated in breast cancer progression, where these genes act as tumor modifiers. Here, we review how the metalloproteinase axis impacts mammary physiology and tumorigenesis and is associated with inflammatory cell influx in human breast cancer, and evaluate its potential as a regulator of inflammation in the mammary gland

    Global, regional, and national cancer incidence, mortality, years of life lost, years lived with disability, and disability-Adjusted life-years for 29 cancer groups, 1990 to 2017 : A systematic analysis for the global burden of disease study

    Importance: Cancer and other noncommunicable diseases (NCDs) are now widely recognized as a threat to global development. The latest United Nations high-level meeting on NCDs reaffirmed this observation and also highlighted the slow progress in meeting the 2011 Political Declaration on the Prevention and Control of Noncommunicable Diseases and the third Sustainable Development Goal. Lack of situational analyses, priority setting, and budgeting have been identified as major obstacles in achieving these goals. All of these have in common that they require information on the local cancer epidemiology. The Global Burden of Disease (GBD) study is uniquely poised to provide these crucial data. Objective: To describe cancer burden for 29 cancer groups in 195 countries from 1990 through 2017 to provide data needed for cancer control planning. Evidence Review: We used the GBD study estimation methods to describe cancer incidence, mortality, years lived with disability, years of life lost, and disability-Adjusted life-years (DALYs). Results are presented at the national level as well as by Socio-demographic Index (SDI), a composite indicator of income, educational attainment, and total fertility rate. We also analyzed the influence of the epidemiological vs the demographic transition on cancer incidence. Findings: In 2017, there were 24.5 million incident cancer cases worldwide (16.8 million without nonmelanoma skin cancer [NMSC]) and 9.6 million cancer deaths. The majority of cancer DALYs came from years of life lost (97%), and only 3% came from years lived with disability. The odds of developing cancer were the lowest in the low SDI quintile (1 in 7) and the highest in the high SDI quintile (1 in 2) for both sexes. In 2017, the most common incident cancers in men were NMSC (4.3 million incident cases); tracheal, bronchus, and lung (TBL) cancer (1.5 million incident cases); and prostate cancer (1.3 million incident cases). The most common causes of cancer deaths and DALYs for men were TBL cancer (1.3 million deaths and 28.4 million DALYs), liver cancer (572000 deaths and 15.2 million DALYs), and stomach cancer (542000 deaths and 12.2 million DALYs). For women in 2017, the most common incident cancers were NMSC (3.3 million incident cases), breast cancer (1.9 million incident cases), and colorectal cancer (819000 incident cases). The leading causes of cancer deaths and DALYs for women were breast cancer (601000 deaths and 17.4 million DALYs), TBL cancer (596000 deaths and 12.6 million DALYs), and colorectal cancer (414000 deaths and 8.3 million DALYs). Conclusions and Relevance: The national epidemiological profiles of cancer burden in the GBD study show large heterogeneities, which are a reflection of different exposures to risk factors, economic settings, lifestyles, and access to care and screening. The GBD study can be used by policy makers and other stakeholders to develop and improve national and local cancer control in order to achieve the global targets and improve equity in cancer care. © 2019 American Medical Association.     Global patient outcomes after elective surgery: prospective cohort study in 27 low-, middle- and high-income countries.

    BACKGROUND: As global initiatives increase patient access to surgical treatments, there remains a need to understand the adverse effects of surgery and define appropriate levels of perioperative care. METHODS: We designed a prospective international 7-day cohort study of outcomes following elective adult inpatient surgery in 27 countries. The primary outcome was in-hospital complications. Secondary outcomes were death following a complication (failure to rescue) and death in hospital. Process measures were admission to critical care immediately after surgery or to treat a complication and duration of hospital stay. A single definition of critical care was used for all countries. RESULTS: A total of 474 hospitals in 19 high-, 7 middle- and 1 low-income country were included in the primary analysis. Data included 44 814 patients with a median hospital stay of 4 (range 2-7) days. A total of 7508 patients (16.8%) developed one or more postoperative complication and 207 died (0.5%). The overall mortality among patients who developed complications was 2.8%. Mortality following complications ranged from 2.4% for pulmonary embolism to 43.9% for cardiac arrest. A total of 4360 (9.7%) patients were admitted to a critical care unit as routine immediately after surgery, of whom 2198 (50.4%) developed a complication, with 105 (2.4%) deaths. A total of 1233 patients (16.4%) were admitted to a critical care unit to treat complications, with 119 (9.7%) deaths. Despite lower baseline risk, outcomes were similar in low- and middle-income compared with high-income countries. CONCLUSIONS: Poor patient outcomes are common after inpatient surgery. Global initiatives to increase access to surgical treatments should also address the need for safe perioperative care. STUDY REGISTRATION: ISRCTN5181700

    Repositioning of the global epicentre of non-optimal cholesterol

    High blood cholesterol is typically considered a feature of wealthy western countries(1,2). However, dietary and behavioural determinants of blood cholesterol are changing rapidly throughout the world(3) and countries are using lipid-lowering medications at varying rates. These changes can have distinct effects on the levels of high-density lipoprotein (HDL) cholesterol and non-HDL cholesterol, which have different effects on human health(4,5). However, the trends of HDL and non-HDL cholesterol levels over time have not been previously reported in a global analysis. Here we pooled 1,127 population-based studies that measured blood lipids in 102.6 million individuals aged 18 years and older to estimate trends from 1980 to 2018 in mean total, non-HDL and HDL cholesterol levels for 200 countries. Globally, there was little change in total or non-HDL cholesterol from 1980 to 2018. This was a net effect of increases in low- and middle-income countries, especially in east and southeast Asia, and decreases in high-income western countries, especially those in northwestern Europe, and in central and eastern Europe. As a result, countries with the highest level of non-HDL cholesterol-which is a marker of cardiovascular riskchanged from those in western Europe such as Belgium, Finland, Greenland, Iceland, Norway, Sweden, Switzerland and Malta in 1980 to those in Asia and the Pacific, such as Tokelau, Malaysia, The Philippines and Thailand. In 2017, high non-HDL cholesterol was responsible for an estimated 3.9 million (95% credible interval 3.7 million-4.2 million) worldwide deaths, half of which occurred in east, southeast and south Asia. The global repositioning of lipid-related risk, with non-optimal cholesterol shifting from a distinct feature of high-income countries in northwestern Europe, north America and Australasia to one that affects countries in east and southeast Asia and Oceania should motivate the use of population-based policies and personal interventions to improve nutrition and enhance access to treatment throughout the world.Peer reviewe
