Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)
This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes. Actas del V Congreso ISUF-H Costa Rica 2021: Ciudades espontáneas versus ciudades planificadas: distintos retos, distintas realidades
En el año 2021 celebramos en Costa Rica la V edición del Congreso ISUF-H, los días 1, 2 y 3 de diciembre, con la Escuela de Arquitectura de la Universidad de Costa Rica como anfitriona del evento. El congreso “Ciudades espontáneas versus ciudades planificadas: distintos retos, distintas realidades” propuso como eje central una reflexión crítica sobre los procesos de urbanización planificada y urbanización espontánea, en el cual se fomente un abordaje de las ciudades como expresión de organización social,
económica, ambiental y cultural, enfatizando el carácter ideológico de la urbanización y subrayando su continua construcción como resultado de construcciones complejas.
La celebración de un nuevo congreso en América Latina, permitió reforzar la tradición crítica en el abordaje de las ciudades, y reforzar también la necesidad de plantear una perspectiva latinoamericana de los estudios urbanos, y por consiguiente de una teoría urbana latinoamericana. En esta ocasión el congreso se centró en ahondar en la temática de la forma urbana, desde perspectivas transversales que involucren las
amplias disciplinas que asumen como objeto de discusión las problemáticas de la ciudad contemporánea y cuestionan la dicotomía planteada entre lo espontáneo y lo planificado.
Para la Escuela de Arquitectura de la Universidad de Costa Rica y su Laboratorio de Ciudad y Territorio es un honor haber podido llevar a cabo esta nueva edición del congreso de la Asociación ISUF-H como segunda sede en un país latinoamericano. Relevante para fortalecer la temática de la forma urbana en la región, reforzando alianzas y estableciendo nuevas redes que permitan compartir conocimientos a partir de las experiencias de esas diversidades urbanas. Auspiciar el debate en torno a la morfología urbana y las diferencias entre esas ciudades espontáneas y las planificadas, fue una oportunidad para reunir a expertos de las distintas latitudes hispánicas.UCR::Vicerrectoría de Docencia::Ingeniería::Facultad de Ingeniería::Escuela de Arquitectur
La renovación de la palabra en el bicentenario de la Argentina : los colores de la mirada lingüística
El libro reúne trabajos en los que se exponen resultados de investigaciones presentadas por investigadores de Argentina, Chile, Brasil, España, Italia y Alemania en el XII Congreso de la Sociedad Argentina de Lingüística (SAL), Bicentenario: la renovación de la palabra, realizado en Mendoza, Argentina, entre el 6 y el 9 de abril de 2010. Las temáticas abordadas en los 167 capítulos muestran las grandes líneas de investigación que se desarrollan fundamentalmente en nuestro país, pero también en los otros países mencionados arriba, y señalan además las áreas que recién se inician, con poca tradición en nuestro país y que deberían fomentarse. Heterogeneous contributions of change in population distribution of body mass index to change in obesity and underweight NCD Risk Factor Collaboration (NCD-RisC)
From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions
Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study
Summary
Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally.
Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies
have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of
the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income
countries globally, and identified factors associated with mortality.
Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to
hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis,
exomphalos, anorectal malformation, and Hirschsprung’s disease. Recruitment was of consecutive patients for a
minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical
status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary
intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause,
in-hospital mortality for all conditions combined and each condition individually, stratified by country income status.
We did a complete case analysis.
Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital
diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal
malformation, and 517 with Hirschsprung’s disease) from 264 hospitals (89 in high-income countries, 166 in middleincome
countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male.
Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3).
Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income
countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups).
Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in lowincome
countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries;
p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients
combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11],
p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20
[1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention
(ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety
checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed
(ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of
parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65
[0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality.
Interpretation Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between lowincome,
middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will
be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger
than 5 years by 2030

Nuestro proyecto busca desarrollar una idea de negocio a partir de un problema que hemos detectado en la sociedad actual. Este proyecto se divide en dos etapas; la primera narrará la búsqueda y validación del problema en cuestión y la segunda se concentrará en el desarrollo de la idea de negocio. Sabemos que en la actualidad, aún existen muchos tabúes acerca de la sexualidad que aún no son normalizados, es por ello que aún existe el sentimiento de vergüenza al comprar productos sexuales en tiendas físicas. Es así que, My Sextery Box se crea para poder confrontar estos tabúes y promover experiencias sexuales divertidas, seguras y distintas. En la primera etapa, se realizaron entrevistas al público objetivo y expertos en el tema, para poder obtener información acerca del problema. Es así que, se entrevistaron a una sexóloga, ginecóloga, urólogo, trabajador del Ministerio de Salud y un vendedor de una sex shop. En la segunda etapa, se determinaron los pasos a seguir para desarrollar una empresa a partir de una idea de negocio. Dentro del análisis desarrollado, se destaca la importancia de las operaciones y marketing implementado. Nuestro diferencial es el factor sorpresa y personalización del kit. Las proyecciones indican bajo una tasa de descuento de 15.56%, lograremos un VAN de S/. 8,964.68 soles, con una tasa interna de retorno de 35.7% en un periodo de recupero de la inversión de 2.88 años. Con estos números nuestro proyecto representa una gran oportunidad de inversión.Our project has as main objective to develop a business idea based on a current problem in our society. This project is splitted in two stages; the first will narrate the research and validation of the problem and the second one will focus on the development of the business idea.
We know that nowadays, there are still many taboos about sexuality that are no yet normalized. That is why there is still a feeling of shame when people buy sexual products in physical stores. Mysextery box is created to confront these taboos and to promote fun, safety and different sexual experiences.
In the first stage, we interviewed our target audience and experts to obtain information about the problem. A sexologist, gynecologist, urologist, Ministry of Health worker, and a vendor from a sex shop were interviewed.
In the second stage, we determined the steps to follow to develop a company from a business idea. Within the analysis, it highlights the importance of the operations and marketing. Our differential is the surprise factor and the personalization of the kits.
Projections indicate that, under a discount rate of 15.56%, we will achieve a NPV of S/. 8,964.68 soles with an internal rate of return of 35.7% with a payback of 2.88 years. With these numbers, our project represents a great investment opportunity.Trabajo de investigació
Acute Respiratory Infection by Chlamydophila pneumoniae and Mycoplasma pneumoniae in a Population of Older Adults in Colombia
AbstractIntroduction: Acute Respiratory Infection (ARI) is a heterogeneous group of viral and bacterial respiratory pathologies including Chlamydophila pneumoniae (CP) and Mycoplasma pneumoniae (MP) that are not routinely identified; these infections in the older adults have mortality rates 3 to 5 times higher than that recorded in other age groups. Methods: this study was conducted prospectively to determine the proportion of atypical bacterial pathogens in older adults with ARI in Bogotá. Microbiological diagnosis was determined by real-time PCR (qPCR) in samples of respiratory origin and serology for antibodies IgG, IgA and IgM to MP and CP. Results: A total of 71 patients were enrolled from 2012 to 2013. Upper respiratory infections were diagnosed in the 69% of patients and lower respiratory infections in 31%. MP was identified in 9.8% and CP in 8.5%. Conclusions: these findings indicated that CP and MP must be viewed as a significant etiological agent of ARI in older adults in Bogotá
