    The universal multiplicity function: counting halos and voids

    We present a novel combination of the excursion-set approach with the peak theory formalism in Lagrangian space and provide accurate predictions for halo and void statistics over a wide range of scales. The set-up is based on an effective moving barrier. Besides deriving the corresponding numerical multiplicity function, we introduce a new analytical formula reaching the percent level agreement with the exact numerical solution obtained via Monte Carlo realizations down to small scales, ∌1012h−1M⊙\sim 10^{12} h^{-1}\mathrm{M_\odot}. In the void case, we derive the dependence of the effective moving barrier on the void formation threshold, ÎŽv\delta_{\rm v}, by comparison against the Lagrangian void size function measured in the Dark Energy and Massive Neutrinos Universe simulations. We discuss the mapping from Lagrangian to Eulerian space for both halos and voids; adopting the spherical symmetry approximation, we obtain a strong agreement at intermediate and large scales. Finally, using the effective moving barrier, we derive Lagrangian void density profiles accurately matching measurements from cosmological simulations, a major achievement towards using void profiles for precision cosmology with the next generation of galaxy surveys.Comment: 21 pages, 10 figures, 2 tables.     A 57 year-old patient with dilated cardiomyopathy, long-standing persistent atrial fibrillation, heart failure and episodes of gastrointestinal bleeding underwent AF ablation with pulmonary vein isolation, homogeneization of septal scar, posterior wall isolation and also left atrial appendage isolation. Additionally, because of the high risk of embolism, underwent left atrial appendage occlusion.

    Lack of evidence of Chikungunya virus infection among blood donors during the Chikungunya outbreak in Lazio Region, Italy, 2017

    Background: The latest European Chikungunya virus (CHIKV) outbreak occurred in Italy in 2017, in the municipalities of Anzio and Rome (Lazio Region), with a secondary outbreak in the Calabrian Region. Most CHIKV infections are symptomatic but about 15% of people who acquire the infection may be asymptomatic. A retrospective study was conducted with the aim of assessing the prevalence of recent/ongoing CHIKV infections on the blood donor population in the Lazio Region, during the 2017 outbreak (including in the period before it was detected). Methods: The study was conducted on 4595 plasma samples from donors who donated in 14 different Blood Establishments in the Lazio Region, in the period June-November 2017. A total of 389 of these samples were collected in provinces not affected by the outbreak and were used as negative controls. All samples were tested for IgM detection by the use of an ELISA test, and positive samples were tested for confirmation through the use of a PRNT. Molecular tests were performed on sera that were found to be IgM-positive or borderline. Results: A total of 41 (0.89%) blood donors tested positive for IgM. None of these positive IgM ELISA results was confirmed either by PRNT or by molecular tests. Conclusions: Our study has shown no evidence of recent/ongoing CHIKV infection in blood donors of the affected area

    Successful Improvement of Frequency and Symptoms of Premature Complexes after Oral Magnesium Administration

    Background: Premature ventricular and supraventricular complexes (PVC and PsVC) are frequent and often symptomatic. The magnesium (Mg) ion plays a role in the physiology of cell membranes and cardiac rhythm. Objective: We evaluated whether the administration of Mg Pidolate (MgP) in patients with PVC and PsVC is superior to placebo (P) in improving symptoms and arrhythmia frequency. Methods: Randomized double-blind study with 60 consecutive symptomatic patients with more than 240 PVC or PsVC/h on 24-hour Holter monitoring who were selected to receive placebo or MgP. To evaluate symptom improvement, a categorical and a specific questionnaire for symptoms related to PVC and PsVC was made. Improvement in premature complex density (PCD) per hour was considered significant if percentage reduction was >= 70% after treatment. The dose of MgP was 3.0 g/day for 30 days, equivalent to 260mg of Mg element. None of the patients had structural heart disease or renal failure. Results: Of the 60 patients, 33 were female (55%). Ages ranged from 16 to 70 years old. In the MgP group, 76.6% of patients had a PCD reduction >70%, 10% of them >50% and only 13.4% <50%. In the P group, 40% showed slight improvement, <30%, in the premature complexes frequency (p < 0.001). Symptom improvement was achieved in 93.3% of patients in the MgP group, compared with only 16.7% in the P group (p < 0.001).     Quality indicators for the care and outcomes of adults with atrial fibrillation

    Aims To develop quality indicators (QIs) that may be used to evaluate the quality of care and outcomes for adults with atrial fibrillation (AF). Methods and results We followed the ESC methodology for QI development. This methodology involved (i) the identification of the domains of AF care for the diagnosis and management of AF (by a group of experts including members of the ESC Clinical Practice Guidelines Task Force for AF); (ii) the construction of candidate QIs (including a systematic review of the literature); and (iii) the selection of the final set of QIs (using a modified Delphi method). Six domains of care for the diagnosis and management of AF were identified: (i) Patient assessment (baseline and follow-up), (ii) Anticoagulation therapy, (iii) Rate control strategy, (iv) Rhythm control strategy, (v) Risk factor management, and (vi) Outcomes measures, including patient-reported outcome measures (PROMs). In total, 17 main and 17 secondary QIs, which covered all six domains of care for the diagnosis and management of AF, were selected. The outcome domain included measures on the consequences and treatment of AF, as well as PROMs. Conclusion This document defines six domains of AF care (patient assessment, anticoagulation, rate control, rhythm control, risk factor management, and outcomes), and provides 17 main and 17 secondary QIs for the diagnosis and management of AF. It is anticipated that implementation of these QIs will improve the quality of AF care

    Common, low-frequency, rare, and ultra-rare coding variants contribute to COVID-19 severity

    The combined impact of common and rare exonic variants in COVID-19 host genetics is currently insufficiently understood. Here, common and rare variants from whole-exome sequencing data of about 4000 SARS-CoV-2-positive individuals were used to define an interpretable machine-learning model for predicting COVID-19 severity. First, variants were converted into separate sets of Boolean features, depending on the absence or the presence of variants in each gene. An ensemble of LASSO logistic regression models was used to identify the most informative Boolean features with respect to the genetic bases of severity. The Boolean features selected by these logistic models were combined into an Integrated PolyGenic Score that offers a synthetic and interpretable index for describing the contribution of host genetics in COVID-19 severity, as demonstrated through testing in several independent cohorts. Selected features belong to ultra-rare, rare, low-frequency, and common variants, including those in linkage disequilibrium with known GWAS loci. Noteworthily, around one quarter of the selected genes are sex-specific. Pathway analysis of the selected genes associated with COVID-19 severity reflected the multi-organ nature of the disease. The proposed model might provide useful information for developing diagnostics and therapeutics, while also being able to guide bedside disease management. © 2021, The Author(s)

    This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.  Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.   Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.  The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.  The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.     Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidĂȘncias cientĂ­ficas que as suportam, quanto ao diagnĂłstico e tratamento da CDC, com especial ĂȘnfase na base de racionalidade que a embasou. Alguns deles recentemente constituĂ­ram alvos potenciais de terapĂȘuticas. A histĂłria natural das fases aguda e crĂŽnica foi revista, com realce para a transmissĂŁo oral, a forma indeterminada e as sĂ­ndromes crĂŽnicas. MetanĂĄlises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade apĂłs instalação da cardiomiopatia crĂŽnica. Condutas terapĂȘuticas aplicĂĄveis aos indivĂ­duos com a FIDC foram abordadas especificamente. Todos os mĂ©todos para detectar alteraçÔes estruturais e/ou funcionais com variadas tĂ©cnicas de imageamento cardĂ­aco tambĂ©m foram revisados, com recomendaçÔes de uso nos vĂĄrios cenĂĄrios clĂ­nicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por mĂ©todos que detectam fibrose miocĂĄrdica. A metodologia atual para diagnĂłstico etiolĂłgico e as consequentes implicaçÔes do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. TambĂ©m o tratamento de pacientes em risco ou com insuficiĂȘncia cardĂ­aca, arritmias e eventos tromboembĂłlicos, baseado em recursos farmacolĂłgicos e complementares, recebeu especial atenção. CapĂ­tulos suplementares subsidiaram as condutas aplicĂĄveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grĂĄvidas, na reativação da infecção apĂłs transplante cardĂ­acos, e outros.    Por fim, dois capĂ­tulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivĂ­duos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicaçÔes mĂ©dico-trabalhistas completaram esta diretriz.&nbsp
