    A SARS-CoV-2 outbreak investigation at a storage and distribution centre in England: an assessment of worker- and workplace-related risk factors.

    An outbreak of SARS-CoV-2 (1 March to 10 May 2021) with an attack rate of 26.5% among approximately 1150 workers at a storage and distribution centre in England prompted a multidisciplinary outbreak investigation (5 May to 6 August 2021), with the aim of better understanding worker- and workplace-related risk factors for viral transmission in the warehousing sector. Overall, environmental factors (e.g., ventilation, humidity and temperature) were assessed to be appropriate at the facility. Nevertheless, 39 (51.3%) surface samples from across the site tested positive for low/ very low levels of SARS-CoV-2 RNA (Ct value ≥ 32.0 for all). Among the study participants, of whom 35.6% were confirmed or suspected cases, 95.5% reported having received COVID-19 prevention training, 100.0% reported handwashing, and 80.0% reported use of face coverings at work. Notably, 43.9% and 19.0% reported working with a symptomatic and a positive contact respectively. Furthermore, 80.5% and 46.3% had concerns regarding reduction in their income and future unemployment, respectively, due to self-isolation. The findings of this study suggest that, in addition to targeted workplace infection control measures and tailored work area specific risk assessments, an enhanced and equitable sick leave policy may help limit presenteeism and viral transmission in large workplaces

    Dynamics of intraoceanic subduction initiation: 1. Oceanic detachment fault inversion and the formation of supra-subduction zone ophiolites

    Subduction initiation is a critical link in the plate tectonic cycle. Intraoceanic subduction zones can form along transform faults and fracture zones, but how subduction nucleates parallel to mid-ocean ridges, as in e.g., the Neotethys Ocean during the Jurassic, remains a matter of debate. In recent years, extensional detachment faults have been widely documented adjacent to slow-spreading and ultraslow-spreading ridges where they cut across the oceanic lithosphere. These structures are extremely weak due to widespread occurrence of serpentine and talc resulting from hydrothermal alteration, and can therefore effectively localize deformation. Here, we show geochemical, tectonic, and paleomagnetic evidence from the Jurassic ophiolites of Albania and Greece for a subduction zone formed in the western Neotethys parallel to a spreading ridge along an oceanic detachment fault. With 2-D numerical modeling exploring the evolution of a detachment-ridge system experiencing compression, we show that serpentinized detachments are always weaker than spreading ridges. We conclude that, owing to their extreme weakness, oceanic detachments can effectively localize deformation under perpendicular far-field forcing, providing ideal conditions to nucleate new subduction zones parallel and close to (or at) spreading ridges. Direct implication of this, is that resumed magmatic activity in the forearc during subduction initiation can yield widespread accretion of suprasubduction zone ophiolites at or close to the paleoridge. Our new model casts the enigmatic origin of regionally extensive ophiolite belts in a novel geodynamic context, and calls for future research on three-dimensional modeling of subduction initiation and how upper plate extension is associated with that

    A Multi-Modal AI-Driven Cohort Selection Tool to Predict Suboptimal Non-Responders to Aflibercept Loading-Phase for Neovascular Age-Related Macular Degeneration: PRECISE Study Report 1

    Patients diagnosed with exudative neovascular age-related macular degeneration are commonly treated with anti-vascular endothelial growth factor (anti-VEGF) agents. However, response to treatment is heterogeneous, without a clinical explanation. Predicting suboptimal response at baseline will enable more efficient clinical trial designs for novel, future interventions and facilitate individualised therapies. In this multicentre study, we trained a multi-modal artificial intelligence (AI) system to identify suboptimal responders to the loading-phase of the anti-VEGF agent aflibercept from baseline characteristics. We collected clinical features and optical coherence tomography scans from 1720 eyes of 1612 patients between 2019 and 2021. We evaluated our AI system as a patient selection method by emulating hypothetical clinical trials of different sizes based on our test set. Our method detected up to 57.6% more suboptimal responders than random selection, and up to 24.2% more than any alternative selection criteria tested. Applying this method to the entry process of candidates into randomised controlled trials may contribute to the success of such trials and further inform personalised care

    Upper mantle electrical resistivity structure beneath the central Mariana subduction system

The definitive version was published in Geochemistry Geophysics Geosystems 11 (2010): Q09003, doi:10.1029/2010GC003101.This paper reports on a magnetotelluric (MT) survey across the central Mariana subduction system, providing a comprehensive electrical resistivity image of the upper mantle to address issues of mantle dynamics in the mantle wedge and beneath the slow back-arc spreading ridge. After calculation of MT response functions and their correction for topographic distortion, two-dimensional electrical resistivity structures were generated using an inversion algorithm with a smoothness constraint and with additional restrictions imposed by the subducting slab. The resultant isotropic electrical resistivity structure contains several key features. There is an uppermost resistive layer with a thickness of up to 150 km beneath the Pacific Ocean Basin, 80–100 km beneath the Mariana Trough, and 60 km beneath the Parece Vela Basin along with a conductive mantle beneath the resistive layer. A resistive region down to 60 km depth and a conductive region at greater depth are inferred beneath the volcanic arc in the mantle wedge. There is no evidence for a conductive feature beneath the back-arc spreading center. Sensitivity tests were applied to these features through inversion of synthetic data. The uppermost resistive layer is the cool, dry residual from the plate accretion process. Its thickness beneath the Pacific Ocean Basin is controlled mainly by temperature, whereas the roughly constant thickness beneath the Mariana Trough and beneath the Parece Vela Basin regardless of seafloor age is controlled by composition. The conductive mantle beneath the uppermost resistive layer requires hydration of olivine and/or melting of the mantle. The resistive region beneath the volcanic arc down to 60 km suggests that fluids such as melt or free water are not well connected or are highly three-dimensional and of limited size. In contrast, the conductive region beneath the volcanic arc below 60 km depth reflects melting and hydration driven by water release from the subducting slab. The resistive region beneath the back-arc spreading center can be explained by dry mantle with typical temperatures, suggesting that any melt present is either poorly connected or distributed discontinuously along the strike of the ridge.     Large expert-curated database for benchmarking document similarity detection in biomedical literature search

    Document recommendation systems for locating relevant literature have mostly relied on methods developed a decade ago. This is largely due to the lack of a large offline gold-standard benchmark of relevant documents that cover a variety of research fields such that newly developed literature search techniques can be compared, improved and translated into practice. To overcome this bottleneck, we have established the RElevant LIterature SearcH consortium consisting of more than 1500 scientists from 84 countries, who have collectively annotated the relevance of over 180 000 PubMed-listed articles with regard to their respective seed (input) article/s. The majority of annotations were contributed by highly experienced, original authors of the seed articles. The collected data cover 76% of all unique PubMed Medical Subject Headings descriptors. No systematic biases were observed across different experience levels, research fields or time spent on annotations. More importantly, annotations of the same document pairs contributed by different scientists were highly concordant. We further show that the three representative baseline methods used to generate recommended articles for evaluation (Okapi Best Matching 25, Term Frequency-Inverse Document Frequency and PubMed Related Articles) had similar overall performances. Additionally, we found that these methods each tend to produce distinct collections of recommended articles, suggesting that a hybrid method may be required to completely capture all relevant articles. The established database server located at https://relishdb.ict.griffith.edu.au is freely available for the downloading of annotation data and the blind testing of new methods. We expect that this benchmark will be useful for stimulating the development of new powerful techniques for title and title/abstract-based search engines for relevant articles in biomedical research.Peer reviewe

    Lean thinking and the factors necessary for its success

    Lean management is becoming the standard for systematic productivity improvement, but the majority of implementations fail to sustain. Hence, the critical success factors for lean were the focus of this work. Literature review showed that the causality for lean success was not empirically developed beyond case study contextualisation. A multifaceted work was developed with contextualisation studies, survey of lean knowledge (758 responses), and a comprehensive case-study questionnaire (1253 responses from 44 countries). The statistical methods included exploratory factor analysis and path analysis by structural equation modelling (SEM). The first questionnaire revealed two different understandings of lean, and the second explored the underlying causality for lean success, including contingency for business size and product variety. Many contributions to the body of knowledge issued from this work. First of all, there was a methodological contribution, pioneering explorative structural modelling of full scope lean implementation. Second, SEMs of the lean knowledge-based view showed the profound positive effects of management knowledge on the primary factors for lean success. These factors were shown to be leadership and employee development. Third, the most beneficial lean methods were highlighted for specific scenarios. Fourth, the negligible and negative effects of a consultant-based approach to lean were uncovered. The results showed that the majority of consultants did not aid the long-term performance and sustainability of lean but significantly hindered it, except where masterful consultants acted as coaches. Fifth, a shortage of lean knowledge was observed in New Zealand; their participants averaged only half of what the USA�s did. Sixth, as culture has been emphasised in current literature, the present danger of overly focusing on it was discussed. Seventh was a conceptual contribution integrating lean and risk management, and a practical application with a risk analysis. This developed a risk matrix for the assessment and prioritisation of implementation components. Eighth, some adjustments to government lean strategies were proposed. And finally, the work integrated the findings in a tangible stage process model for implementation in SMEs. The dissemination of this knowledge has the potential to enhance productivity and commercial success of industries in New Zealand and abroad through successful lean implementations.     Implementing lean—Outcomes from SME case studies

    The purpose of this work was to identify critical success factors for lean implementation. It followed two first-time implementations of lean in small to medium-sized enterprises (SMEs). The case studies collectively spanned over four years. It was observed that the real problem with achieving lean success was not management commitment but their ignorance of what they should commit to, hence a knowledge problem. This paper provides new insights into the role of knowledge as a causal factor in the successful implementation of lean and especially in organisations with limited resources. For practitioners, management knowledge needs active consideration in the implementation phase. Management knowledge is particularly significant in SME implementations due to their resource constraints.
