    Is More Always Better? A Reflection on the Dynamic Nature of Nationally and Regionally Focused Funder Collaboratives

    Funding collaboratives allow foundations to align, increase effectiveness, and collectively address systemic issues. Such alliances are increasingly important in the quest for social change in the face of large-scale challenges like climate change, political polarization, and inequity, which require contributions from across sectors to create meaningful impact. An exploration of why such collaboratives form, how they evolve, and what impact they have offers insights for foundations interested in tackling such complex challenges. The BUILD Health Challenge® is a funding collaborative that includes national and regionally focused funders working together to drive sustainable improvements in health. Through a reflective survey and conversation with its funders, BUILD documented the evolving composition and outcomes related to the funding collaborative. This structure, rooted in speed, flexibility, and reach, creates a symbiotic relationship between the two groups of funders, and allows regional philanthropies the opportunity to inform the national agenda and contribute deep insights as to what works locally. This article discusses how The BUILD Health Challenge’s funding collaborative was created; the challenges and opportunities created from its structure; and outcomes from the program. Emerging evidence on the benefits of such collaboratives are explored, including shifts in the confidence to take risks and new approaches to grantmaking

    The ‘Ombuds Watchers’: Collective Dissent and Legal Protest Among Users of Public Services Ombuds

    This article examines the phenomenon of the ‘ombuds watchers’. These are groups of dissatisfied users of public service ombuds schemes who engage in legal protest against the current system of redress for citizen-state complaints. Through the lens of legal consciousness scholarship we propose a framework that conceptualizes the collectivized protest of the ombuds watchers. Based on an empirical dataset, our analysis has shown that the ombuds watchers meet each of the defining characteristics of dissenting collectivism and demonstrates the existence of forms of legal consciousness which present ‘opportunities to build alternative imaginaries and institutions’ (Morgan and Kutch 2015, p. 567). Our case study provides an insight into the potential for dissenting collectives to challenge the hegemonic structures of state law, while at the same time emphasising the continuing power of legal ideology in shaping popular understandings of justice. The article also suggests a pathway for future empirical research into ombuds

    Assessing the Accuracy of Tree Diameter Measurements Collected at a Distance

    The ability to measure trees remotely or at a distance may be of value to forest inventory processes. Within three forest types (young coniferous, old coniferous, and deciduous), we compared laser caliper measurements collected at distances up to 12 m from each tree, to direct contact caliper measurements. Bitterlich sector-fork measurements and diameter tape measurements were also collected for reference purposes. We used non-parametric tests to evaluate three of our four hypotheses that suggest there are no significant differences between direct and remote diameter measurements, between caliper measurements and sector-fork measurements, and between diameter measurement errors across forest types. In general, most of the differences in diameters were small (≤0.8 cm) and were observed within the 0–6 m measurement distance from each tree. These results suggest that forest characteristics and measurement distance may play a role in remote diameter measurement accuracy. We also performed a correlation analysis between light conditions and remote measurements. The correlation analysis suggested light conditions were not significantly correlated to diameter measurement accuracy

    Utilization of Jackfruit (Artocarpus heterophyllus) Seed as a Bread Spread

    Jackfruit is the largest tree-borne fruit in the world.  It is a widely cultivated and well-consumed fruit in the Philippines and other parts of South-east Asia.  The seed makes up 15-18% of the fruit weight. However, the seeds are underutilized due to the lack of information about its nutritional and food product potential. Hence, the purpose of this research was to utilize the jackfruit seed as a bread spread that is nutritious, affordable, and acceptable in terms of consistency, texture, aroma and taste. The standardized recipe for the jackfruit seed spread was obtained after three trial formulations. The jackfruit seeds were cleaned and boiled for 30 minutes, then mixed with jackfruit rags, raw sugar, olive oil, lemon juice, lemon rind, turmeric powder, and salt in a blender until a paste-like consistency and smooth texture was achieved. The blended ingredients were cooked in low heat for 15 minutes to remove excess moisture, increase shelf-life, and attain a spreadable consistency. The final product was placed in sterile airtight jars. The jackfruit seed spread has the following characteristics: canary yellow color, smooth texture, lemony aroma, and sweet acidulous taste. One serving (21g) provides 82 kcal and is a source of potassium (59mg), phosphorus (8mg), calcium (26mg), magnesium (4mg), thiamin (0.02mg), riboflavin (0.02mg), niacin (0.2mg), and vitamin C (2mg). The product was rated as like very much by 30 evaluators from both genders using a nine-point hedonic scale. The jackfruit seed spread is affordable with a selling price of ₱58.00 for a jar of 200g net weight. The shelf-life is 30 days in refrigerator temperature. It was recommended that further studies be done on producing jackfruit seed spread for a large scale business in regions were jackfruit is widely cultivated

    Preventive digital mental health interventions for children and young people: a review of the design and reporting of research

    Abstract: Digital health interventions (DHIs) have frequently been highlighted as one way to respond to increasing levels of mental health problems in children and young people. Whilst many are developed to address existing mental health problems, there is also potential for DHIs to address prevention and early intervention. However, there are currently limitations in the design and reporting of the development, evaluation and implementation of preventive DHIs that can limit their adoption into real-world practice. This scoping review aimed to examine existing evidence-based DHI interventions and review how well the research literature described factors that researchers need to include in their study designs and reports to support real-world implementation. A search was conducted for relevant publications published from 2013 onwards. Twenty-one different interventions were identified from 30 publications, which took a universal (n = 12), selective (n = 3) and indicative (n = 15) approach to preventing poor mental health. Most interventions targeted adolescents, with only two studies including children aged ≤10 years. There was limited reporting of user co-design involvement in intervention development. Barriers and facilitators to implementation varied across the delivery settings, and only a minority reported financial costs involved in delivering the intervention. This review found that while there are continued attempts to design and evaluate DHIs for children and young people, there are several points of concern. More research is needed with younger children and those from poorer and underserved backgrounds. Co-design processes with children and young people should be recognised and reported as a necessary component within DHI research as they are an important factor in the design and development of interventions, and underpin successful adoption and implementation. Reporting the type and level of human support provided as part of the intervention is also important in enabling the sustained use and implementation of DHIs

    Perlecan (HSPG2) promotes structural, contractile, and metabolic development of human cardiomyocytes

    Perlecan (HSPG2), a heparan sulfate proteoglycan similar to agrin, is key for extracellular matrix (ECM) maturation and stabilization. Although crucial for cardiac development, its role remains elusive. We show that perlecan expression increases as cardiomyocytes mature in vivo and during human pluripotent stem cell differentiation to cardiomyocytes (hPSC-CMs). Perlecan-haploinsuffient hPSCs (HSPG2+/−) differentiate efficiently, but late-stage CMs have structural, contractile, metabolic, and ECM gene dysregulation. In keeping with this, late-stage HSPG2+/− hPSC-CMs have immature features, including reduced ⍺-actinin expression and increased glycolytic metabolism and proliferation. Moreover, perlecan-haploinsuffient engineered heart tissues have reduced tissue thickness and force generation. Conversely, hPSC-CMs grown on a perlecan-peptide substrate are enlarged and display increased nucleation, typical of hypertrophic growth. Together, perlecan appears to play the opposite role of agrin, promoting cellular maturation rather than hyperplasia and proliferation. Perlecan signaling is likely mediated via its binding to the dystroglycan complex. Targeting perlecan-dependent signaling may help reverse the phenotypic switch common to heart failure

    Large expert-curated database for benchmarking document similarity detection in biomedical literature search

    Document recommendation systems for locating relevant literature have mostly relied on methods developed a decade ago. This is largely due to the lack of a large offline gold-standard benchmark of relevant documents that cover a variety of research fields such that newly developed literature search techniques can be compared, improved and translated into practice. To overcome this bottleneck, we have established the RElevant LIterature SearcH consortium consisting of more than 1500 scientists from 84 countries, who have collectively annotated the relevance of over 180 000 PubMed-listed articles with regard to their respective seed (input) article/s. The majority of annotations were contributed by highly experienced, original authors of the seed articles. The collected data cover 76% of all unique PubMed Medical Subject Headings descriptors. No systematic biases were observed across different experience levels, research fields or time spent on annotations. More importantly, annotations of the same document pairs contributed by different scientists were highly concordant. We further show that the three representative baseline methods used to generate recommended articles for evaluation (Okapi Best Matching 25, Term Frequency-Inverse Document Frequency and PubMed Related Articles) had similar overall performances. Additionally, we found that these methods each tend to produce distinct collections of recommended articles, suggesting that a hybrid method may be required to completely capture all relevant articles. The established database server located at https://relishdb.ict.griffith.edu.au is freely available for the downloading of annotation data and the blind testing of new methods. We expect that this benchmark will be useful for stimulating the development of new powerful techniques for title and title/abstract-based search engines for relevant articles in biomedical research.Peer reviewe

    Treatment with 670 nm light up regulates cytochrome C oxidase expression and reduces inflammation in an age-related macular degeneration model.

    Inflammation is an umbrella feature of ageing. It is present in the aged retina and many retinal diseases including age-related macular degeneration (AMD). In ageing and in AMD mitochondrial function declines. In normal ageing this can be manipulated by brief exposure to 670 nm light on the retina, which increases mitochondrial membrane potential and reduces inflammation. Here we ask if 670 nm exposure has the same ability in an aged mouse model of AMD, the complement factor H knockout (CFH(-/-)) where inflammation is a key feature. Further, we ask whether this occurs when 670 nm is delivered briefly in environmental lighting rather than directly focussed on the retina. Mice were exposed to 670 nm for 6 minutes twice a day for 14 days in the form of supplemented environmental light. Exposed animals had significant increase in cytochrome c oxidase (COX), which is a mitochondrial enzyme regulating oxidative phosphorylation.There was a significant reduction in complement component C3, an inflammatory marker in the outer retina. Vimetin and glial fibrillary acidic protein (GFAP) expression, which reflect retinal stress in Muller glia, were also significantly down regulated. There were also significant changes in outer retinal macrophage morphology. However, amyloid beta (Aβ) load, which also increases with age in the outer retina and is pro-inflammatory, did not change. Hence, 670 nm is effective in reducing inflammation probably via COX activation in mice with a genotype similar to that in 50% of AMD patients even when brief exposures are delivered via environmental lighting. Further, inflammation can be reduced independent of Aβ. The efficacy revealed here supports current early stage clinical trials of 670 nm in AMD patients

    Clinical Characteristics, Racial Inequities, and Outcomes in Patients with Breast Cancer and COVID-19: A COVID-19 and Cancer Consortium (CCC19) Cohort Study

    BACKGROUND: Limited information is available for patients with breast cancer (BC) and coronavirus disease 2019 (COVID-19), especially among underrepresented racial/ethnic populations. METHODS: This is a COVID-19 and Cancer Consortium (CCC19) registry-based retrospective cohort study of females with active or history of BC and laboratory-confirmed severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection diagnosed between March 2020 and June 2021 in the US. Primary outcome was COVID-19 severity measured on a five-level ordinal scale, including none of the following complications, hospitalization, intensive care unit admission, mechanical ventilation, and all-cause mortality. Multivariable ordinal logistic regression model identified characteristics associated with COVID-19 severity. RESULTS: 1383 female patient records with BC and COVID-19 were included in the analysis, the median age was 61 years, and median follow-up was 90 days. Multivariable analysis revealed higher odds of COVID-19 severity for older age (aOR per decade, 1.48 [95% CI, 1.32-1.67]); Black patients (aOR 1.74; 95 CI 1.24-2.45), Asian Americans and Pacific Islander patients (aOR 3.40; 95 CI 1.70-6.79) and Other (aOR 2.97; 95 CI 1.71-5.17) racial/ethnic groups; worse ECOG performance status (ECOG PS ≥2: aOR, 7.78 [95% CI, 4.83-12.5]); pre-existing cardiovascular (aOR, 2.26 [95% CI, 1.63-3.15])/pulmonary comorbidities (aOR, 1.65 [95% CI, 1.20-2.29]); diabetes mellitus (aOR, 2.25 [95% CI, 1.66-3.04]); and active and progressing cancer (aOR, 12.5 [95% CI, 6.89-22.6]). Hispanic ethnicity, timing, and type of anti-cancer therapy modalities were not significantly associated with worse COVID-19 outcomes. The total all-cause mortality and hospitalization rate for the entire cohort was 9% and 37%, respectively however, it varied according to the BC disease status. CONCLUSIONS: Using one of the largest registries on cancer and COVID-19, we identified patient and BC-related factors associated with worse COVID-19 outcomes. After adjusting for baseline characteristics, underrepresented racial/ethnic patients experienced worse outcomes compared to non-Hispanic White patients.
