    Enhancing Social Support through Adventure Education: The Case of Fathers and Sons

    Adventure education (AE) research often utilizes social support as an outcome variable associated with participation in extended outdoor adventure trips. Social support is defined as “the degree to which individuals have access to social resources, in the form of relationships, on which they can rely” (Johnson & Sarason, 1979). Using an adventure-based program offering a treatment specifically designed for father and son participants, significant changes in pre-post comparison scores on the variables of trust, communication, and social support were reported for both fathers and sons. Qualitative analysis revealed four major themes: communication enhancement, instructor influence, course components, and shared time. Based on these results, AE programming can be effective in increasing social support between fathers and sons, enabling them both to learn valuable life lessons about father-son interactions and communication benefits

    802-6 The Cost-Effectiveness of Pravastatin in Secondary Prevention of Coronary Heart Disease

    To determine the cost-effectiveness of pravastatin therapy in patients with coronary heart disease, a projected risk model was developed that used the results of the three-year, double blind, placebo controlled clinical trials: Pravastatin Limitation ofAtherosclerosis in the Coronary Arteries (PLAC I) and Pravastatin, Upids and Atherosclerosis in the Carotid Arteries (PLAC II). In addition to measuring atherosclerotic progression, the PLAC studies evaluated four outcome variables: coronary heart disease death, non-coronary heart disease death, fatal myocardial infarction, and non-fatal myocardial infarction in a patient population (mean age 60 years) with established coronary heart disease and moderate low-density-lipoprotein cholesterol levels, Pooled PLAC data analysis (n=559) revealed a statistically significant (p<0.05) difference in male non-fatal myocardial infarctions between the pravastatin and placebo groups. The projected risk model utilized Framingham data to project the risk of mortality 10 years post myocardial infarction. Markov Process was used to estimate the life-years saved and cost.     <p>Abstract</p> <p>Background</p> <p>In gnathostomes, chemosensory receptors (CR) expressed in olfactory epithelia are encoded by evolutionarily dynamic gene families encoding odorant receptors (OR), trace amine-associated receptors (TAAR), V1Rs and V2Rs. A limited number of OR-like sequences have been found in invertebrate chordate genomes. Whether these gene families arose in basal or advanced vertebrates has not been resolved because these families have not been examined systematically in agnathan genomes.</p> <p>Results</p> <p><it>Petromyzon </it>is the only extant jawless vertebrate whose genome has been sequenced. Known to be exquisitely sensitive to several classes of odorants, lampreys detect fewer amino acids and steroids than teleosts. This reduced number of detectable odorants is indicative of reduced numbers of CR gene families or a reduced number of genes within CR families, or both, in the sea lamprey. In the lamprey genome we identified a repertoire of 59 intact single-exon CR genes, including 27 OR, 28 TAAR, and four V1R-like genes. These three CR families were expressed in the olfactory organ of both parasitic and adult life stages.</p> <p>Conclusion</p> <p>An extensive search in the lamprey genome failed to identify potential orthologs or pseudogenes of the multi-exon V2R family that is greatly expanded in teleost genomes, but did find intact calcium-sensing receptors (CASR) and intact metabotropic glutamate receptors (MGR). We conclude that OR and V1R arose in chordates after the cephalochordate-urochordate split, but before the diversification of jawed and jawless vertebrates. The advent and diversification of V2R genes from glutamate receptor-family G protein-coupled receptors, most likely the CASR, occurred after the agnathan-gnathostome divergence.</p

    Landscape - wildfire interactions in southern Europe: implications for landscape management

    ReviewEvery year approximately half a million hectares of land are burned by wildfires in southern Europe, causing large ecological and socio-economic impacts. Climate and land use changes in the last decades have increased fire risk and danger. In this paper we review the available scientific knowledge on the relationships between landscape and wildfires in the Mediterranean region, with a focus on its application for defining landscape management guidelines and policies that could be adopted in order to promote landscapes with lower fire hazard. The main findings are that (1) socio-economic drivers have favoured land cover changes contributing to increasing fire hazard in the last decades, (2) large wildfires are becoming more frequent, (3) increased fire frequency is promoting homogeneous landscapes covered by fire-prone shrublands; (4) landscape planning to reduce fuel loads may be successful only if fire weather conditions are not extreme.     Large expert-curated database for benchmarking document similarity detection in biomedical literature search

    Document recommendation systems for locating relevant literature have mostly relied on methods developed a decade ago. This is largely due to the lack of a large offline gold-standard benchmark of relevant documents that cover a variety of research fields such that newly developed literature search techniques can be compared, improved and translated into practice. To overcome this bottleneck, we have established the RElevant LIterature SearcH consortium consisting of more than 1500 scientists from 84 countries, who have collectively annotated the relevance of over 180 000 PubMed-listed articles with regard to their respective seed (input) article/s. The majority of annotations were contributed by highly experienced, original authors of the seed articles. The collected data cover 76% of all unique PubMed Medical Subject Headings descriptors. No systematic biases were observed across different experience levels, research fields or time spent on annotations. More importantly, annotations of the same document pairs contributed by different scientists were highly concordant. We further show that the three representative baseline methods used to generate recommended articles for evaluation (Okapi Best Matching 25, Term Frequency-Inverse Document Frequency and PubMed Related Articles) had similar overall performances. Additionally, we found that these methods each tend to produce distinct collections of recommended articles, suggesting that a hybrid method may be required to completely capture all relevant articles. The established database server located at https://relishdb.ict.griffith.edu.au is freely available for the downloading of annotation data and the blind testing of new methods. We expect that this benchmark will be useful for stimulating the development of new powerful techniques for title and title/abstract-based search engines for relevant articles in biomedical research.Peer reviewe

    Effect of angiotensin-converting enzyme inhibitor and angiotensin receptor blocker initiation on organ support-free days in patients hospitalized with COVID-19

    IMPORTANCE Overactivation of the renin-angiotensin system (RAS) may contribute to poor clinical outcomes in patients with COVID-19. Objective To determine whether angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) initiation improves outcomes in patients hospitalized for COVID-19. DESIGN, SETTING, AND PARTICIPANTS In an ongoing, adaptive platform randomized clinical trial, 721 critically ill and 58 non–critically ill hospitalized adults were randomized to receive an RAS inhibitor or control between March 16, 2021, and February 25, 2022, at 69 sites in 7 countries (final follow-up on June 1, 2022). INTERVENTIONS Patients were randomized to receive open-label initiation of an ACE inhibitor (n = 257), ARB (n = 248), ARB in combination with DMX-200 (a chemokine receptor-2 inhibitor; n = 10), or no RAS inhibitor (control; n = 264) for up to 10 days. MAIN OUTCOMES AND MEASURES The primary outcome was organ support–free days, a composite of hospital survival and days alive without cardiovascular or respiratory organ support through 21 days. The primary analysis was a bayesian cumulative logistic model. Odds ratios (ORs) greater than 1 represent improved outcomes. RESULTS On February 25, 2022, enrollment was discontinued due to safety concerns. Among 679 critically ill patients with available primary outcome data, the median age was 56 years and 239 participants (35.2%) were women. Median (IQR) organ support–free days among critically ill patients was 10 (–1 to 16) in the ACE inhibitor group (n = 231), 8 (–1 to 17) in the ARB group (n = 217), and 12 (0 to 17) in the control group (n = 231) (median adjusted odds ratios of 0.77 [95% bayesian credible interval, 0.58-1.06] for improvement for ACE inhibitor and 0.76 [95% credible interval, 0.56-1.05] for ARB compared with control). The posterior probabilities that ACE inhibitors and ARBs worsened organ support–free days compared with control were 94.9% and 95.4%, respectively. Hospital survival occurred in 166 of 231 critically ill participants (71.9%) in the ACE inhibitor group, 152 of 217 (70.0%) in the ARB group, and 182 of 231 (78.8%) in the control group (posterior probabilities that ACE inhibitor and ARB worsened hospital survival compared with control were 95.3% and 98.1%, respectively). CONCLUSIONS AND RELEVANCE In this trial, among critically ill adults with COVID-19, initiation of an ACE inhibitor or ARB did not improve, and likely worsened, clinical outcomes.     Underlying Motives for Selected Adventure Recreation Activities: The Case for Eudaimonics and Hedonics

    One of the interesting behaviors practiced by citizens across the globe is the pursuit of outdoor recreational activities featuring elements of personal risk and danger. These types of activities are now becoming a global mainstay for many individuals, economies, and organizations. This study examined the underlying motivations and subsequent behaviors associated with risk-taking recreational activities and used the concepts of eudaimonics and hedonics to examine the motivations for participation from individuals engaging in three different adventure activities occurring in eight different locations. Recruitment took place in several forms, including in-person solicitation of participants at the activity areas, which consisted of mountain biking trailheads, rock climbing areas, and whitewater sites. Data were collected from three popular outdoor adventure activities (OAAs), including mountain biking, rock climbing, and whitewater boating. This study employed the use of multivariate analysis of variance (MANOVA) to investigate the relationship between two independent variable sets, including (1) the activity type, and (2) the level of experience, gender, type of activity, and the dependent variables of the Hedonic and Eudaimonic Motives for Activities (HEMA) scale (eudaimonic and hedonic). In addition, a cumulative odds ordinal logistic regression with proportional odds was utilized to determine the effects of expertise level and activity type on reported eudaimonic and hedonic motivations. A qualitative interview process was utilized to further investigate participant responses surrounding eudaimonic and hedonic motivational perspectives. The results indicated slight variations in experience level in the underlying motivations. Furthermore, qualitative inquiry revealed several motivation categories and diversity in the way those motivations were present throughout the recreation activity

    Exploring Motivations and Constraints of Minority Participation: College Outdoor Adventure Programs

    While national demographics in the United States are steadily becoming more racially diverse, participation in outdoor and adventure recreation programs is still dominated by individuals identifying as White and Caucasian. College outdoor adventure programs aiming to provide opportunities for diverse recreational and educational experiences to the student body at large generally struggle to attract minority students. While much research has discussed the disparity, little literature exists to help understand and guide recruitment, and increase participation. This study explored factors contributing to leisure choice in outdoor recreation, awareness of university outdoor programming, and the motivations or constraints that influenced individual participation. Focus group interviews and a constructivist perspective were employed using a qualitative, inductive reasoning strategy to inquire about the subject and come to the conclusions found herein. This study found that factors that constrained minority participation in COAP programming included structural and inter/intrapersonal barriers, family leisure history, lack of role models and knowledge of adventure, cultural differences, and negative perceptions related to participation in adventure activities
