Abstract

Background

Cervical cancer is now considered the second leading cause of death among women worldwide, and its incidence has reached alarming levels, especially in developing countries. Similarly, high grade squamous intraepithelial lesion (HSIL), the precursor stage for cervical cancer, represents a growing health problem among younger women as the HSIL management regimes that have been developed are not fully effective. From the etiological point of view, the presence of Human Papillomavirus (HPV) has been demonstrated to play a crucial role for developing cervical malignancies, and viral DNA has been detected in 99.7% of cervical tumors at the later stages. CIGB-300 is a novel cyclic synthetic peptide that induces apoptosis in malignant cells and elicits antitumor activity in cancer animal models. CIGB-300 impairs the Casein Kinase (CK2) phosphorylation, by targeting the substrate's phosphoaceptor domain. Based on the perspectives of CIGB-300 to treat cancer, this "first-in-human" study investigated its safety and tolerability in patients with cervical malignancies.

Methods

Thirty-one women with colposcopically and histologically diagnosed microinvasive or pre-invasive cervical cancer were enrolled in a dose escalating study. CIGB-300 was administered sequentially at 14, 70, 245 and 490 mg by intralesional injections during 5 consecutive days to groups of 7 – 10 patients. Toxicity was monitored daily until fifteen days after the end of treatment, when patients underwent conization. Digital colposcopy, histology, and HPV status were also evaluated.

Results

No maximum-tolerated dose or dose-limiting toxicity was achieved. The most frequent local events were pain, bleeding, hematoma and erythema at the injection site. The systemic adverse events were rash, facial edema, itching, hot flashes, and localized cramps. 75% of the patients experienced a significant lesion reduction at colposcopy and 19% exhibited full histological regression. HPV DNA was negative in 48% of the previously positive patients. Long term follow-up did not reveal recurrences or adverse events.

Conclusion

CIGB 300 was safe and well tolerated. This is the first clinical trial where a drug has been used to target the CK2 phosphoaceptor domain providing an early proof-of-principle of a possible clinical benefit.</p

Neutrinos with energies above 1017 eV are detectable with the Surface Detector Array of the Pierre Auger Observatory. The identification is efficiently performed for neutrinos of all flavors interacting in the atmosphere at large zenith angles, as well as for Earth-skimming \u3c4 neutrinos with nearly tangential trajectories relative to the Earth. No neutrino candidates were found in 3c 14.7 years of data taken up to 31 August 2018. This leads to restrictive upper bounds on their flux. The 90% C.L. single-flavor limit to the diffuse flux of ultra-high-energy neutrinos with an E\u3bd-2 spectrum in the energy range 1.0 7 1017 eV -2.5 7 1019 eV is E2 dN\u3bd/dE\u3bd < 4.4 7 10-9 GeV cm-2 s-1 sr-1, placing strong constraints on several models of neutrino production at EeV energies and on the properties of the sources of ultra-high-energy cosmic rays

Heterogeneous contributions of change in population distribution of body mass index to change in obesity and underweight NCD Risk Factor Collaboration (NCD-RisC)

From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions

The violence perpetrated by the criminal economy against migrants is evident after the increase in restrictive measures implemented by the Mexican government as a strategy to stop irregular traffic. Academic texts are reviewed, as well as newspaper resources on massacres against migrants such as those that occurred in San Fernando, Tamaulipas, 2010; in Cadereyta, Nuevo León, 2012 and Camargo, Tamaulipas, 2021. Therefore, this work aims to show tolerance and political indolence in the face of the violent intervention of the criminal economy in irregular migratory transit. The conclusions point to the Mexican authorities' lack of attention and tolerance regarding the massacres of migrants.Se evidencian las violencias perpetradas por la economía criminal hacia las personas migrantes tras el aumento de las medidas restrictivas implementadas por el gobierno mexicano como estrategia para frenar el tránsito irregular. Background: healthcare-related infections are a problem for the patient’s security and can be used as quality indicators of the care process. Objective: to characterize, clinically and epidemiologically, the healthcare-related infections, at the intensive care unit of “Leon Cuervo Rubio” General Teaching Hospital from Pinar del Río in 2015.Methods: a descriptive and retrospective study was carried out at the place and during the time herein mentioned. The universe was made up of the 156 patients who developed a nosocomial infection after 48 hours of admission. The following variables were measured: age, sex, localization, microbiology, mortality, sensitivity and antimicrobial resistance. Data were obtained from the patients’ medical histories. Results: patients older than 60 years and belonging to the male sex were the most affected ones. Ventilator-associated pneumonia (32,2 %) and ventilator-non-associated pneumonia were the most frequent infections. Escherichia coli were present in 35,7 % of the samples. Mortality was 65,4 %. The sensitivity pattern for Escherichia coli showed 76,5 % for piperacillin/tazobactam, followed by meropenem with 72,3 % and amikacin with 61,4 %. Third and fourth-generation cephalosporin was the most resistance antimicrobial group in more than 65 %.Conclusions: the clinical and epidemiological variables of the healthcare-related infections in the study population were characterized.</p
