Narrative literature and cancer: improving the doctor-patient relationship
The role of classical literature on the subject of pain and suffering in cancer and other serious illnesses, not only from the point of view of patients but also of hospital personnel, family, friends and family doctors, has not been deeply exploited to favor the human and professional experience of young and not so young oncologists. This manuscript is the result of an effort made by postgraduate students and faculty members at the School of Oncology at Parma University to review the literature on this subject. The aim of our work is to convey the message that before teaching relationship techniques it is important to instill a culture focused on the doctor-patient relationship. Classical literature can make an important contribution to awareness in this area
Abstract
Native women in Mexico engage in organizing dynamics that allow them to provide for their families. This paper identifies the individual and collective benefits of organizational processes for the women who belong to the Unión de Palmeadoras de la Heroica Ciudad de Tlaxiaco, Oaxaca, Mexico. These women make handmade tortillas for household consumption and sale. Data were obtained between May 2015 and January 2017 through a combination of fieldwork techniques: interviews, workshops, participant observation, sale registers during a 20-day period, and a census of the whole organization. Two individual benefits were identified: guaranteed selling spots; barter opportunities while at the market. Three collective benefits were identified: daily availability of handmade tortillas; the mobilization of the local economy; the permanence of the palmeadora trade. The paper concludes that the organization was created due to the discrimination faced by these women. However, a lot remains to be done since their selling spots are inappropriate and their working conditions continue to be precarious
Contribution of KRAS mutations and c.2369C > T (p.T790M) EGFR to acquired resistance to EGFR-TKIs in EGFR mutant NSCLC: a study on circulating tumor DNA
INTRODUCTION:
KRAS oncogene mutations (MUTKRAS) drive resistance to EGFR inhibition by providing alternative signaling as demonstrated in colo-rectal cancer. In non-small cell lung cancer (NSCLC), the efficacy of treatment with EGFR tyrosine kinase inhibitors (EGFR-TKIs) depends on activating EGFR mutations (MUTEGFR). However, inhibition of EGFR may select resistant cells displaying alternative signaling, i.e., KRAS, or restoration of EGFR activity due to additional MUTEGFR, i.e., the c.2369C > T (p.T790MEGFR).
AIM:
The aim of this study was to investigate the appearance of MUTKRAS during EGFR-TKI treatment and their contribution to drug resistance.
METHODS:
This study used cell-free circulating tumor DNA (cftDNA) to evaluate the appearance of codon 12 MUTKRAS and p.T790MEGFR mutations in 33 advanced NSCLC patients progressing after an EGFR-TKI.
RESULTS:
p.T790MEGFR was detected in 11 (33.3%) patients, MUTKRAS at codon 12 in 3 (9.1%) while both p.T790MEGFR and MUTKRAS codon 12 were found in 13 (39.4%) patients. Six patients (18.2%) were KRAS wild-type (WTKRAS) and negative for p.T790MEGFR. In 8 subjects paired tumor re-biopsy/plasma samples were available; the percent concordance of tissue/plasma was 62.5% for p.T790MEGFR and 37.5% for MUTKRAS. The analysis of time to progression (TTP) and overall survival (OS) in WTKRAS vs. MUTKRAS were not statistically different, even if there was a better survival with WTKRAS vs. MUTKRAS, i.e., TTP 14.4 vs. 11.4 months (p = 0.97) and OS 40.2 vs. 35.0 months (p = 0.56), respectively.
CONCLUSIONS:
MUTKRAS could be an additional mechanism of escape from EGFR-TKI inhibition and cftDNA is a feasible approach to monitor the molecular development of drug resistance

Hyperbaric exposure and oxidative Stress in occupational activities (HEOxS): the study protocol
Background: Hyperbaric exposure (HE) is proven to be a stressor to several mechanisms in living cells.
Even if after homeostasis restoration, harmful effects are expected, in particular a presence of free
radicals. These latter are the stimulus to negative phenomenon as inflammation or cancer. In Italy,
with 7500 km of sea shores, a large quantity of workers is exposed to HE during occupational
activities. A deep knowledge of HE and bodily effects is not well defined; hence a multidisciplinary
assessment of risk is needed. To detect one or more indicators of HE a research group is organised,
under the INAIL sponsorship. The research project focused on the oxidative stress (OxS) and this
paper details on the possible protocol to estimate, with a large amount of techniques on several
human liquids, the relationship between OxS and HE. Specific attention will be paid to identify
confounding factors and their influence.
Methods: Blood and urine will be sampled. Several lab techniques will be performed on samples, both
targeted, to measure the level of well-known biomarkers, and untargeted. Regard the formers:
products of oxidation of DNA and RNA in urine; inflammation and temperature cytokines and
protein carbonyles in blood. Untargeted evaluation will be performed for a metabolomics analysis in
urine. Confounding factors: temperature, body fat, fitness, allergies and dietary habits. These factors
will be assessed, directly or indirectly, prior and after HE. The final scope of the project is to determine
one or more indicators that relates to HE in hits twofold nature: depth and duration.
Conclusion: The relationship between OxS and HE is not deeply investigated and literature proposes
diverging results. The project aims to define the time dependence of biomarkers related to OxS, to
rise knowledge in risk assessment in workers exposed to HE
Guidelines for the use and interpretation of assays for monitoring autophagy (3rd edition)
In 2008 we published the first set of guidelines for standardizing research in autophagy. Since then, research on this topic has continued to accelerate, and many new scientists have entered the field. Our knowledge base and relevant new technologies have also been expanding. Accordingly, it is important to update these guidelines for monitoring autophagy in different organisms. Various reviews have described the range of assays that have been used for this purpose. Nevertheless, there continues to be confusion regarding acceptable methods to measure autophagy, especially in multicellular eukaryotes. For example, a key point that needs to be emphasized is that there is a difference between measurements that monitor the numbers or volume of autophagic elements (e.g., autophagosomes or autolysosomes) at any stage of the autophagic process versus those that measure fl ux through the autophagy pathway (i.e., the complete process including the amount and rate of cargo sequestered and degraded). In particular, a block in macroautophagy that results in autophagosome accumulation must be differentiated from stimuli that increase autophagic activity, defi ned as increased autophagy induction coupled with increased delivery to, and degradation within, lysosomes (inmost higher eukaryotes and some protists such as Dictyostelium ) or the vacuole (in plants and fungi). In other words, it is especially important that investigators new to the fi eld understand that the appearance of more autophagosomes does not necessarily equate with more autophagy. In fact, in many cases, autophagosomes accumulate because of a block in trafficking to lysosomes without a concomitant change in autophagosome biogenesis, whereas an increase in autolysosomes may reflect a reduction in degradative activity. It is worth emphasizing here that lysosomal digestion is a stage of autophagy and evaluating its competence is a crucial part of the evaluation of autophagic flux, or complete autophagy. Here, we present a set of guidelines for the selection and interpretation of methods for use by investigators who aim to examine macroautophagy and related processes, as well as for reviewers who need to provide realistic and reasonable critiques of papers that are focused on these processes. These guidelines are not meant to be a formulaic set of rules, because the appropriate assays depend in part on the question being asked and the system being used. In addition, we emphasize that no individual assay is guaranteed to be the most appropriate one in every situation, and we strongly recommend the use of multiple assays to monitor autophagy. Along these lines, because of the potential for pleiotropic effects due to blocking autophagy through genetic manipulation it is imperative to delete or knock down more than one autophagy-related gene. In addition, some individual Atg proteins, or groups of proteins, are involved in other cellular pathways so not all Atg proteins can be used as a specific marker for an autophagic process. In these guidelines, we consider these various methods of assessing autophagy and what information can, or cannot, be obtained from them. Finally, by discussing the merits and limits of particular autophagy assays, we hope to encourage technical innovation in the field
