    An Interdisciplinary Approach for the Historical and Technical Characterization of Medieval and Modern Mortars

    The study concerns Italian masonries and focuses on historical, medieval and modern mortars. Within the context of the different regions under examination (Piedmont, the Po Valley area, Latium, the Umbria-Marche region, and Apulia and Sardinia) a wide variety of materials with different chemical-physical characteristics were used in masonry work, determining different structural behaviors. The project aims at improving our knowledge about historical mortars in order to further the conservation of Italian built heritage, especially in zones with seismic risk. To achieve these results, we took samples and carried out analyses to investigate the different mechanical and cohesion properties that influence the vulnerability of ruined or collapsed structures. This information has enabled advances to be made in the prevention, maintenance, protection and preservation of historical buildings. Further details will concern the history of construction techniques, with particular regard to the relationship between local resources and construction sites. Another important topic is the role of different components and additives during the preparation of the mortars and their level of hydraulicity.Lo studio riguarda le strutture murarie in Italia e si concentra sulle malte storiche, medievali e moderne. Nei contesti regionali oggetto d’indagine (Piemonte e Pianura Padana, Lazio, Umbria-Marche, Puglia e Sardegna) è stata utilizzata una grande varietà di materiali, con caratteristiche chimico-fisiche diverse, e queste differenze determinano anche comportamenti strutturali diversi. Il progetto mira a migliorare la nostra conoscenza delle malte storiche per approfondire il tema della conservazione del patrimonio edilizio italiano, soprattutto nelle zone a rischio sismico. Per ottenere questi risultati, sono stati prelevati campioni sui quali sono state effettuate analisi per indagare quanto le diverse proprietà meccaniche e di coesione incidano sulla vulnerabilità delle strutture in rovina o collassate. Grazie a queste informazioni è stato possibile contribuire alla prevenzione, manutenzione, protezione e conservazione dell'edilizia storica. Ulteriori conoscenze hanno riguardato la storia delle tecniche di costruzione, con particolare riguardo al rapporto tra risorse locali e i cantieri. Un altro importante tema è stato il ruolo dei diversi componenti e additivi durante la preparazione delle malte e il loro livello di idraulicità

    Petrographic-Mineralogical Characterization of Archaeological Materials from “Casa di Diana” Mithraeum Sited in the Open Museum of Ostia Antica

    Mithraea, religious Roman buildings, are very common in Italian archeological sites. There are sixteen in Ostia Antica (Rome, Italy)The poor state of conservation, due to the intrinsic environmental conditions, characterized them: they consist of open-air museums and caves simultaneously. These places of worship are characterized by the presence of heterogeneous materials, such as wall building materials (bricks and mortars) and others used for furnishings and fittings. This increases the risk of accelerated damage because the materials ‘rheology is different. Here, a full petrographic-mineralogical characterization with polarized light microscopy (PLM), X-ray diffraction (XRD), scanning electron microscope with energy dispersive X-ray (SEM-EDS) and isotopic analysis (δ13C, δ18O) is carried out on materials like travertine, marble, pumice, ceramic, and wall-building materials in “Casa di Diana” Mithraeum (Ostia Antica). Their characterization gives provenance information as well as conservation and restoration purposes. The prevalence of siliciclastic or carbonate components discriminates between red and yellow bricks, as well as different textures and minerals in the aggregate of the red ones. The mortars are typically pozzolanic, and the aggregate is mostly made up of black and red pozzolanic clasts. In the altar, apse, and aedicule, which constitute the principal place of the Mithraeum, a variety of materials used for the ornamental purpose are represented by pumices, travertine, marble, and limestone.     Panarea natural-lab: eight years of underwater research

    The volcanic island of Panarea is characterized by the presence of continuous emission of volcanic gases from the seafloor. In November 2002 the system increased its flow of some order of magnitude and since then a steady release of gas is present generating bubble plumes that, in the shallowest points, reach the sea surface. Due to the environmental conditions, the area close to the shore and shallow water can be utilized as a “natural lab” to study the effect of high levels of CO2on the marine realm by surface techniques and directly by means of SCUBA diving. Since 2002, over one hundred hours of diving have been utilized to collect samples of fluids and sediments and to study the biota. After a first phase of volcanic surveillance, the island is more recently studied as a “natural analogue” for the development of reliable monitoring techniques for potential seepage from sub-seabed carbon storage sites.     Rationalist architecture in rural villages: petro-morphological characterization of natural asbestos fibers in decorative plasters used for artificial stones

    At the beginning of the twentieth century, artificial stones became commonly used as structural building materials, masonry materials, and architectural decorative elements. The main purpose of this work was to characterize the building materials used in a historical building in Codigoro, a small town near Ferrara, northeastern Italy. This building was constructed according to the Italian rationalist rules for monumental architecture and is an example of the rationalist architecture found in rural villages, which uses innovative materials and decorations including artificial stone. The samples analyzed in this work were imitation natural stone, such as “fake travertine” and “fake Ceppo di Grè”, composed of reinforced concrete slabs coated with white mortar. Asbestos minerals have been observed in the fillers in these materials. Petrographic, mineralogical, and chemical characterization of samples of artificial stones and concrete was performed with optical transmitted light microscopy, scanning electron microscopy–energy dispersive spectroscopy, and X-ray diffraction analyses. The results revealed chemical and biological degradation in almost all the artificial stones examined and enabled identification of the presence of calcite, iron oxychloride complexes, and asbestos lizardites used as fillers in two of the analyzed samples. This study aimed to describe a general investigation methodology used to examine the artificial stone façades of the Ex Casa del Fascio of Codigoro, whose architecture is particularly representative of the rationalist architecture in rural villages. The results highlight the relevance of compatible and suitable conservation measures that should be monitored during all restoration interventions to minimize post-degradation fracturing and transformation into elements harmful to human health and the environment

    The italian quaternary volcanism

    The peninsular and insular Italy are punctuated by Quaternary volcanoes and their rocks constitute an important aliquot of the Italian Quaternary sedimentary successions. Also away from volcanoes themselves, volcanic ash layers are a common and frequent feature of the Quaternary records, which provide us with potential relevant stratigraphic and chronological markers at service of a wide array of the Quaternary science issues. In this paper, a broad representation of the Italian volcano logical community has joined to provide an updated comprehensive state of art of the Italian Quaternary volcanism. The eruptive history, style and dynamics and, in some cases, the hazard assessment of about thirty Quaternary volcanoes, from the north ernmost Mt. Amiata, in Tuscany, to the southernmost Pantelleria and Linosa, in Sicily Channel, are here reviewed in the light of the substantial improving of the methodological approaches and the overall knowledge achieved in the last decades in the vol canological field study. We hope that the present review can represent a useful and agile document summarising the knowledege on the Italian volcanism at the service of the Quaternary community operating in central Mediterranean area

    Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     Geohazard features of the north-western Sicily and Pantelleria

These seafloor features were recognized using high-resolution bathymetry data and rely on the morphological expression of the seafloor and shallow sub-surface processes. The north-western Sicily is a complex continental margin, affected by morphodynamic, depositional, and tectonic processes. The Egadi offshore is controlled by fault escarpments and alternating retreating and progradational processes. Ustica and Pantelleria submerged edifices show the effect of volcanic activity. The Ustica seafloor is interested in volcanic, tectonic, and gravitational instability processes, while the Pantelleria offshore underwent erosive-depositional processes and the effect of bottom currents.     Surgical site infection after gastrointestinal surgery in high-income, middle-income, and low-income countries: a prospective, international, multicentre cohort study

    Background: Surgical site infection (SSI) is one of the most common infections associated with health care, but its importance as a global health priority is not fully understood. We quantified the burden of SSI after gastrointestinal surgery in countries in all parts of the world. Methods: This international, prospective, multicentre cohort study included consecutive patients undergoing elective or emergency gastrointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating centres were stratified into high-income, middle-income, and low-income groups according to the UN's Human Development Index (HDI). Data variables from the GlobalSurg 1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using Bayesian multilevel logistic regression models. This trial is registered with ClinicalTrials.gov, number NCT02662231. Findings: Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58·5%) patient were from high-HDI countries (193 hospitals in 30 countries), 3918 (31·2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (10·2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12·3%) patients had SSI within 30 days of surgery. The incidence of SSI varied between countries with high (691 [9·4%] of 7339 patients), middle (549 [14·0%] of 3918 patients), and low (298 [23·2%] of 1282) HDI (p < 0·001). The highest SSI incidence in each HDI group was after dirty surgery (102 [17·8%] of 574 patients in high-HDI countries; 74 [31·4%] of 236 patients in middle-HDI countries; 72 [39·8%] of 181 patients in low-HDI countries). Following risk factor adjustment, patients in low-HDI countries were at greatest risk of SSI (adjusted odds ratio 1·60, 95% credible interval 1·05–2·37; p=0·030). 132 (21·6%) of 610 patients with an SSI and a microbiology culture result had an infection that was resistant to the prophylactic antibiotic used. Resistant infections were detected in 49 (16·6%) of 295 patients in high-HDI countries, in 37 (19·8%) of 187 patients in middle-HDI countries, and in 46 (35·9%) of 128 patients in low-HDI countries (p < 0·001). Interpretation: Countries with a low HDI carry a disproportionately greater burden of SSI than countries with a middle or high HDI and might have higher rates of antibiotic resistance. In view of WHO recommendations on SSI prevention that highlight the absence of high-quality interventional research, urgent, pragmatic, randomised trials based in LMICs are needed to assess measures aiming to reduce this preventable complication

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Search for associated production of a Z boson with a single top quark and for tZ flavour-changing interactions in pp collisions at root s=8 TeV

    A search for the production of a single top quark in association with a Z boson is presented, both to identify the expected standard model process and to search for flavour-changing neutral current interactions. The data sample corresponds to an integrated luminosity of 19.7 fb−1 recorded by the CMS experiment at the LHC in proton-proton collisions at s√=8s=8 TeV. Final states with three leptons (electrons or muons) and at least one jet are investigated. An events yield compatible with tZq standard model production is observed, and the corresponding cross section is measured to be σ(pp → tZq → ℓνbℓ+ℓ−q) = 10− 7+ 8 fb with a significance of 2.4 standard deviations. No presence of flavour-changing neutral current production of tZq is observed. Exclusion limits at 95% confidence level on the branching fractions of a top quark decaying to a Z boson and an up or a charm quark are found to be ℬ(t → Zu) < 0.022% and ℬ(t → Zc) < 0.049%
