    Stellar Feedback in Galaxies and the Origin of Galaxy-scale Winds

    Feedback from massive stars is believed to play a critical role in driving galactic super-winds that enrich the IGM and shape the galaxy mass function and mass-metallicity relation. In previous papers, we introduced new numerical methods for implementing stellar feedback on sub-GMC through galactic scales in galaxy simulations. This includes radiation pressure (UV through IR), SNe (Type-I & II), stellar winds ('fast' O-star through 'slow' AGB winds), and HII photoionization. Here, we show that these feedback mechanisms drive galactic winds with outflow rates as high as ~10-20 times the galaxy SFR. The mass-loading efficiency (wind mass loss rate divided by SFR) scales inversely with circular velocity, consistent with momentum-conservation expectations. We study the contributions of each feedback mechanism to galactic winds in a range of galaxy models, from SMC-like dwarfs & MW-analogues to z~2 clumpy disks. In massive, gas-rich systems (local starbursts and high-z galaxies), radiation pressure dominates the wind generation. For MW-like spirals and dwarf galaxies the gas densities are much lower, and shock-heated gas from SNe and stellar winds dominates production of large-scale outflows. In all models, however, winds have a multi-phase structure that depends on interactions between multiple feedback mechanisms operating on different spatial & time scales: any single mechanism fails to reproduce the winds observed. We provide fitting functions for wind mass-loading and velocities as a function of galaxy properties, for use in cosmological simulations and semi-analytic models. These differ from typically-adopted formulae with explicit dependence on gas surface density that can be very important in both low-density dwarf galaxies and high-density gas-rich galaxies.Comment: 16 pages, 11 figures, accepted to MNRAS (matches accepted version).     PLEASANT: Preventing and Lessening Exacerbations of Asthma in School-age children Associated with a New Term - a cluster randomised controlled trial and economic evaluation.

    BACKGROUND: Asthma episodes and deaths are known to be seasonal. A number of reports have shown peaks in asthma episodes in school-aged children associated with the return to school following the summer vacation. A fall in prescription collection in the month of August has been observed, and was associated with an increase in the number of unscheduled contacts after the return to school in September. OBJECTIVE: The primary objective of the study was to assess whether or not a NHS-delivered public health intervention reduces the September peak in unscheduled medical contacts. DESIGN: Cluster randomised trial, with the unit of randomisation being 142 NHS general practices, and trial-based economic evaluation. SETTING: Primary care. INTERVENTION: A letter sent (n = 70 practices) in July from their general practitioner (GP) to parents/carers of school-aged children with asthma to remind them of the importance of taking their medication, and to ensure that they have sufficient medication prior to the start of the new school year in September. The control group received usual care. MAIN OUTCOME MEASURES: The primary outcome measure was the proportion of children aged 5-16 years who had an unscheduled medical contact in September 2013. Supporting end points included the proportion of children who collected prescriptions in August 2013 and unscheduled contacts through the following 12 months. Economic end points were quality-adjusted life-years (QALYs) gained and costs from an NHS and Personal Social Services perspective. RESULTS: There is no evidence of effect in terms of unscheduled contacts in September. Among children aged 5-16 years, the odds ratio (OR) was 1.09 [95% confidence interval (CI) 0.96 to 1.25] against the intervention. The intervention did increase the proportion of children collecting a prescription in August (OR 1.43, 95% CI 1.24 to 1.64) as well as scheduled contacts in the same month (OR 1.13, 95% CI 0.84 to 1.52). For the wider time intervals (September-December 2013 and September-August 2014), there is weak evidence of the intervention reducing unscheduled contacts. The intervention did not reduce unscheduled care in September, although it succeeded in increasing the proportion of children collecting prescriptions in August as well as having scheduled contacts in the same month. These unscheduled contacts in September could be a result of the intervention, as GPs may have wanted to see patients before issuing a prescription. The economic analysis estimated a high probability that the intervention was cost-saving, for baseline-adjusted costs, across both base-case and sensitivity analyses. There was no increase in QALYs. LIMITATION: The use of routine data led to uncertainty in the coding of medical contacts. The uncertainty was mitigated by advice from a GP adjudication panel. CONCLUSIONS: The intervention did not reduce unscheduled care in September, although it succeeded in increasing the proportion of children both collecting prescriptions and having scheduled contacts in August. After September there is weak evidence in favour of the intervention. The intervention had a favourable impact on costs but did not demonstrate any impact on QALYs. The results of the trial indicate that further work is required on assessing and understanding adherence, both in terms of using routine data to make quantitative assessments, and through additional qualitative interviews with key stakeholders such as practice nurses, GPs and a wider group of children with asthma.     Self-Regulated Star Formation in Galaxies via Momentum Input from Massive Stars

    Feedback from massive stars is believed to play a critical role in shaping the galaxy mass function, the structure of the interstellar medium (ISM), and the low efficiency of star formation, but the exact form of the feedback is uncertain. In this paper, the first in a series, we present and test a novel numerical implementation of stellar feedback resulting from momentum imparted to the ISM by radiation, supernovae, and stellar winds. We employ a realistic cooling function, and find that a large fraction of the gas cools to <100K, so that the ISM becomes highly inhomogeneous. Despite this, our simulated galaxies reach an approximate steady state, in which gas gravitationally collapses to form giant molecular clouds (GMCs), dense clumps, and stars; subsequently, stellar feedback disperses the GMCs, repopulating the diffuse ISM. This collapse and dispersal cycle is seen in models of SMC-like dwarfs, the Milky-Way, and z~2 clumpy disk analogues. The simulated global star formation efficiencies are consistent with the observed Kennicutt-Schmidt relation. Moreover, the star formation rates are nearly independent of the numerically imposed high-density star formation efficiency, density threshold, and density scaling. This is a consequence of the fact that, in our simulations, star formation is regulated by stellar feedback limiting the amount of very dense gas available for forming stars. In contrast, in simulations without stellar feedback, i.e. under the action of only gravity and gravitationally-induced turbulence, the ISM experiences runaway collapse to very high densities. In these simulations without feedback, the global star formation rates exceed observed galactic star formation rates by 1-2 orders of magnitude, demonstrating that stellar feedback is crucial to the regulation of star formation in galaxies.Comment: 24 pages, 13 figures, accepted to MNRAS (significantly expanded to match accepted version).     Meta-Analysis of Genome-Wide Association Studies for Abdominal Aortic Aneurysm Identifies Four New Disease-Specific Risk Loci

    Rationale: Abdominal aortic aneurysm (AAA) is a complex disease with both genetic and environmental risk factors. Together, 6 previously identified risk loci only explain a small proportion of the heritability of AAA. Objective: To identify additional AAA risk loci using data from all available genome-wide association studies (GWAS). Methods and Results: Through a meta-analysis of 6 GWAS datasets and a validation study totalling 10,204 cases and 107,766 controls we identified 4 new AAA risk loci: 1q32.3 (SMYD2), 13q12.11 (LINC00540), 20q13.12 (near PCIF1/MMP9/ZNF335), and 21q22.2 (ERG). In various database searches we observed no new associations between the lead AAA SNPs and coronary artery disease, blood pressure, lipids or diabetes. Network analyses identified ERG, IL6R and LDLR as modifiers of MMP9, with a direct interaction between ERG and MMP9. Conclusions: The 4 new risk loci for AAA appear to be specific for AAA compared with other cardiovascular diseases and related traits suggesting that traditional cardiovascular risk factor management may only have limited value in preventing the progression of aneurysmal disease

    Genetic Evidence Implicates the Immune System and Cholesterol Metabolism in the Aetiology of Alzheimer's Disease

    Background 1Late Onset Alzheimer's disease (LOAD) is the leading cause of dementia. Recent large genome-wide association studies (GWAS) identified the first strongly supported LOAD susceptibility genes since the discovery of the involvement of APOE in the early 1990s. We have now exploited these GWAS datasets to uncover key LOAD pathophysiological processes. Methodology We applied a recently developed tool for mining GWAS data for biologically meaningful information to a LOAD GWAS dataset. The principal findings were then tested in an independent GWAS dataset. Principal Findings We found a significant overrepresentation of association signals in pathways related to cholesterol metabolism and the immune response in both of the two largest genome-wide association studies for LOAD. Significance Processes related to cholesterol metabolism and the innate immune response have previously been implicated by pathological and epidemiological studies of Alzheimer's disease, but it has been unclear whether those findings reflected primary aetiological events or consequences of the disease process. Our independent evidence from two large studies now demonstrates that these processes are aetiologically relevant, and suggests that they may be suitable targets for novel and existing therapeutic approaches

    We sought to identify new susceptibility loci for Alzheimer's disease through a staged association study (GERAD+) and by testing suggestive loci reported by the Alzheimer's Disease Genetic Consortium (ADGC) in a companion paper. We undertook a combined analysis of four genome-wide association datasets (stage 1) and identified ten newly associated variants with P ≤ 1 × 10−5. We tested these variants for association in an independent sample (stage 2). Three SNPs at two loci replicated and showed evidence for association in a further sample (stage 3). Meta-analyses of all data provided compelling evidence that ABCA7 (rs3764650, meta P = 4.5 × 10−17; including ADGC data, meta P = 5.0 × 10−21) and the MS4A gene cluster (rs610932, meta P = 1.8 × 10−14; including ADGC data, meta P = 1.2 × 10−16) are new Alzheimer's disease susceptibility loci. We also found independent evidence for association for three loci reported by the ADGC, which, when combined, showed genome-wide significance: CD2AP (GERAD+, P = 8.0 × 10−4; including ADGC data, meta P = 8.6 × 10−9), CD33 (GERAD+, P = 2.2 × 10−4; including ADGC data, meta P = 1.6 × 10−9) and EPHA1 (GERAD+, P = 3.4 × 10−4; including ADGC data, meta P = 6.0 × 10−10)

    Whatever happened to compassionate Conservatism under the Coalition government?

    Following David Cameron’s election as leader of the Conservative Party in late 2005, a series of initiatives suggested that he was seeking to reposition the Conservative Party, or perhaps to introduce some new thinking to the Party and to align it with interests and issues that it had not been linked with since at least the start of the Thatcher period. At the time, views among commentators varied about whether this was a genuine attempt to change the Conservative Party, including through a more compassionate approach to some social groups and problems, or whether it was simply designed to ‘detoxify’ the Party and to make it electable once more. However, many observers were unconvinced that the five years of the Coalition government saw significant evidence of the ‘compassionate’ ideas that Cameron and others sought to highlight prior to the 2010 general election. This article explores a number of possible reasons for the apparent disappearance of compassionate Conservatism in relation to social policies under the Coalition government. It suggests that rather than any one explanation, drawing upon a number of interpretations may provide the best understanding of the role and impact of compassionate Conservative ideas from 2010 to 2015

    Global Vascular Guidelines on the Management of Chronic Limb-Threatening Ischemia

    Chronic limb-threatening ischemia (CLTI)is associated with mortality, amputation, and impaired quality of life. These Global Vascular Guidelines (GVG)are focused on definition, evaluation, and management of CLTI with the goals of improving evidence-based care and highlighting critical research needs. The term CLTI is preferred over critical limb ischemia, as the latter implies threshold values of impaired perfusion rather than a continuum. CLTI is a clinical syndrome defined by the presence of peripheral artery disease (PAD)in combination with rest pain, gangrene, or a lower limb ulceration >2 weeks duration. Venous, traumatic, embolic, and nonatherosclerotic etiologies are excluded. All patients with suspected CLTI should be referred urgently to a vascular specialist. Accurately staging the severity of limb threat is fundamental, and the Society for Vascular Surgery Threatened Limb Classification system, based on grading of Wounds, Ischemia, and foot Infection (WIfI)is endorsed. Objective hemodynamic testing, including toe pressures as the preferred measure, is required to assess CLTI. Evidence-based revascularization (EBR)hinges on three independent axes: Patient risk, Limb severity, and ANatomic complexity (PLAN). Average-risk and high-risk patients are defined by estimated procedural and 2-year all-cause mortality. The GVG proposes a new Global Anatomic Staging System (GLASS), which involves defining a preferred target artery path (TAP)and then estimating limb-based patency (LBP), resulting in three stages of complexity for intervention. The optimal revascularization strategy is also influenced by the availability of autogenous vein for open bypass surgery. Recommendations for EBR are based on best available data, pending level 1 evidence from ongoing trials. Vein bypass may be preferred for average-risk patients with advanced limb threat and high complexity disease, while those with less complex anatomy, intermediate severity limb threat, or high patient risk may be favored for endovascular intervention. All patients with CLTI should be afforded best medical therapy including the use of antithrombotic, lipid-lowering, antihypertensive, and glycemic control agents, as well as counseling on smoking cessation, diet, exercise, and preventive foot care. Following EBR, long-term limb surveillance is advised. The effectiveness of nonrevascularization therapies (eg, spinal stimulation, pneumatic compression, prostanoids, and hyperbaric oxygen)has not been established. Regenerative medicine approaches (eg, cell, gene therapies)for CLTI should be restricted to rigorously conducted randomizsed clinical trials. The GVG promotes standardization of study designs and end points for clinical trials in CLTI. The importance of multidisciplinary teams and centers of excellence for amputation prevention is stressed as a key health system initiative. © 2019 Society for Vascular Surgery and European Society for Vascular Surger
