    An exploration of the association between khat chewing and health outcomes in UK-resident male Yemeni khat chewers

    PhDAims: 1) To identify the characteristics of a sample of Yemeni Khat chewers in Sheffield. 2) To investigate how these characteristics are associated with: a) Self-rated "compromised‟ health. b) Self-reported oral health problems. c) Self- reported health conditions and d) Self-reported "high‟ nicotine dependence. Methods: This cross sectional study recruited a purposive sample of male khat chewers aged 18 years and above, selected during random visits to khat sellers. Data were collected through face to face structured interviews and validated using saliva and expired carbon monoxide samples. Data collected were analysed using simple descriptive, univariate and hierarchical logistic regression analyses. Results: Two hundred and four khat-chewing volunteers with a mean age of 44.84 years were interviewed. Sixty five percent were unemployed and 66% had a low level of completed education. Sixty five percent were tobacco smokers. Being older (OR=4.47, 95%CI=1.46-13.66), unemployed (OR=5.49, 95%CI=1.89-15.96), living in uncrowded housing (OR=2.65, 95%CI=1.13-6.22) and reporting low social participation (OR=2.61, 95%CI=1.22-5.61) were found to be statistically significantly associated with self-rated "compromised‟ health. A low level of completed education was found to be statistically significantly associated with self-reported oral problems (OR=2.27, 95%CI=1.02-5.04). Self-reported health conditions were found to be statistically significantly associated with being older (OR=3.10, 95%CI=1.32-7.28), unemployed (OR=4.25, 95%CI=1.57-11.47) and living in uncrowded housing (OR=2.96, 95%CI=1.38-6.37). Finally, self-reported „high‟ nicotine dependence was found to be statistically significantly associated with starting smoking in the UK or elsewhere as opposed to Yemen (OR=3.18, 95%CI=1.03-9.77), being divorced, single or widowed (OR=3.29, 95%CI=1.11-9.74) and reporting low social participation 2 (OR=3.69, 95%CI=1.18-11.51). Conclusions: A range of demographic and socio-cultural factors were identified that correlated with health impacts, reflecting social inequalities amongst this sample of khat chewers

    The factors associated with the knowledge of brushing teeth with fluoridated toothpaste among high school students in Al-Madinah, Saudi Arabia

    ObjectiveThe use of fluoridated toothpaste (FT) is essential for controlling caries. This analytical cross-sectional study aimed to determine the proportion of students who brushed their teeth with fluoridated toothpaste/or do not know the content at least once a day and to determine the factors associated with the knowledge of brushing teeth with FT.MethodsAn anonymous questionnaire was distributed during the academic year 2019–2020 among 439 high school students. The data collected included sociodemographic characteristics and oral-health–related variables [e.g., brushing teeth, knowledge of the effect of fluoride on caries (KEFC) and dental service utilisation (DSU)]. The dependent variable was the knowledge of using FT when brushing teeth (Yes or do not know). Descriptive, bivariate, and logistic regression analysis were performed.ResultsThe response rate was 98% (n = 432) and usable data was 88% (n = 385). The median (IQR) age of the students was 16.00 (1) years, and 190 (47%) were males. Eighty eight percent of the students brushed their teeth with toothpaste daily with no knowledge of toothpaste content and only 86 (21.8%) knew the content of the toothpaste used for brushing their teeth i.e., FT. The multivariable analyses revealed an association of family income and KEFC with brushing teeth with FT [adjusted odds ratio (AOR): 1.98, 95% confidence interval (CI): 1.14–3.43, p = 0.015 and AOR = 6.11, 95% CI: 3.45–10.83, p < 0.001, respectively].ConclusionsWhile the brushing and use of toothpaste among high school students was common, the knowledge of the content of toothpaste used for brushing teeth was less common and was associated with family income and KEFC

    Dentists\u27 Attitudes Towards Chairside Medical Conditions Screening in a Dental Setting in Saudi Arabia: An Exploratory Cross-Sectional Study.

    BACKGROUND: Screening for medical conditions (MCs) of public health importance is a first step in disease prevention and control. Prior studies in the United States found oral health care providers (OHCPS) embrace screening for increased risk of medical conditions in the dental setting. Our objectives were to assess Saudi Arabian (SA) dentist\u27s attitudes, willingness and perceived barriers towards implementing screening for MCs into their dental practices. METHODS: A self-administered, 5-point Likert Scale (1 = very important/willing to 5 = very unimportant/unwilling) questionnaire was given to a convenience sample of 190 practicing dentists. Friedman nonparametric analysis of variance was used to compare responses within each question. RESULTS: Of the 143 responding dentists the mean age was 31 years; 102 (71%) were men. The majority felt it was important for a dentist to screen for cardiovascular disease (98.6%), hypertension (97.9%), diabetes (97.9%), human immunodeficiency virus (HIV) (97.9%), and hepatitis C virus (98.6%). Respondents were willing to refer a patient to a physician (97.9%); send samples to an outside laboratory (96.1%); conduct screening that yields immediate results (96.2%); and discuss results immediately with the patient (93.7%). Respondents were willing to measure/collect blood pressure (67.2%); weight and height (63.7%); and finger stick blood (54.6%). The whole responding dentists (100%) reported time as an important barrier. Respondents were significantly more willing to refer a patient for consultation than send samples to an outside laboratory (mean ranks: 2.32, 2.81, P \u3c 0.001); significantly more willing to measure blood pressure than take oral fluids for salivary diagnostics (mean ranks 2.22, 2.75, p = 0.003). Insurance was significantly (P \u3c 0.05) less important barrier than time, cost, patients\u27 willingness or liability (mean ranks 3.56, 2.63, 3.00, 2.79, 3.02, respectively). CONCLUSIONS: The majority of dentists in this study reported positive attitudes towards and willingness to perform medical screenings in their practice. Time was an important factor

    Global patterns in endemicity and vulnerability of soil fungi

    Fungi are highly diverse organisms, which provide multiple ecosystem services. However, compared with charismatic animals and plants, the distribution patterns and conservation needs of fungi have been little explored. Here, we examined endemicity patterns, global change vulnerability and conservation priority areas for functional groups of soil fungi based on six global surveys using a high-resolution, long-read metabarcoding approach. We found that the endemicity of all fungi and most functional groups peaks in tropical habitats, including Amazonia, Yucatan, West-Central Africa, Sri Lanka, and New Caledonia, with a negligible island effect compared with plants and animals. We also found that fungi are predominantly vulnerable to drought, heat and land-cover change, particularly in dry tropical regions with high human population density. Fungal conservation areas of highest priority include herbaceous wetlands, tropical forests, and woodlands. We stress that more attention should be focused on the conservation of fungi, especially root symbiotic arbuscular mycorrhizal and ectomycorrhizal fungi in tropical regions as well as unicellular early-diverging groups and macrofungi in general. Given the low overlap between the endemicity of fungi and macroorganisms, but high conservation needs in both groups, detailed analyses on distribution and conservation requirements are warranted for other microorganisms and soil organisms

    Multi-messenger observations of a binary neutron star merger

    On 2017 August 17 a binary neutron star coalescence candidate (later designated GW170817) with merger time 12:41:04 UTC was observed through gravitational waves by the Advanced LIGO and Advanced Virgo detectors. The Fermi Gamma-ray Burst Monitor independently detected a gamma-ray burst (GRB 170817A) with a time delay of ~1.7 s with respect to the merger time. From the gravitational-wave signal, the source was initially localized to a sky region of 31 deg2 at a luminosity distance of 40+8-8 Mpc and with component masses consistent with neutron stars. The component masses were later measured to be in the range 0.86 to 2.26 Mo. An extensive observing campaign was launched across the electromagnetic spectrum leading to the discovery of a bright optical transient (SSS17a, now with the IAU identification of AT 2017gfo) in NGC 4993 (at ~40 Mpc) less than 11 hours after the merger by the One- Meter, Two Hemisphere (1M2H) team using the 1 m Swope Telescope. The optical transient was independently detected by multiple teams within an hour. Subsequent observations targeted the object and its environment. Early ultraviolet observations revealed a blue transient that faded within 48 hours. Optical and infrared observations showed a redward evolution over ~10 days. Following early non-detections, X-ray and radio emission were discovered at the transient’s position ~9 and ~16 days, respectively, after the merger. Both the X-ray and radio emission likely arise from a physical process that is distinct from the one that generates the UV/optical/near-infrared emission. No ultra-high-energy gamma-rays and no neutrino candidates consistent with the source were found in follow-up searches. These observations support the hypothesis that GW170817 was produced by the merger of two neutron stars in NGC4993 followed by a short gamma-ray burst (GRB 170817A) and a kilonova/macronova powered by the radioactive decay of r-process nuclei synthesized in the ejecta

    Connecting the multiple dimensions of global soil fungal diversity

We show the impact of precipitation and temperature interactions on local fungal species richness (alpha diversity) across different climates.     Connecting the multiple dimensions of global soil fungal diversity

    A year of genomic surveillance reveals how the SARS-CoV-2 pandemic unfolded in Africa.

    The progression of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic in Africa has so far been heterogeneous, and the full impact is not yet well understood. In this study, we describe the genomic epidemiology using a dataset of 8746 genomes from 33 African countries and two overseas territories. We show that the epidemics in most countries were initiated by importations predominantly from Europe, which diminished after the early introduction of international travel restrictions. As the pandemic progressed, ongoing transmission in many countries and increasing mobility led to the emergence and spread within the continent of many variants of concern and interest, such as B.1.351, B.1.525, A.23.1, and C.1.1. Although distorted by low sampling numbers and blind spots, the findings highlight that Africa must not be left behind in the global pandemic response, otherwise it could become a source for new variants
