    A Competitive Model of Lenggong Valley as a Rural Heritage Destination in Malaysia

    Besides rural attraction, Lenggong Valley has been acknowledged as an important archeological attraction in the state of Perak. On 30th June 2012, Lenggong Valley has been declared as the world heritage site by UNESCO for its archaeological heritage which includes four excavation sites divided into two clusters. Cluster 1 consists of the Bukit Bunuh-Kota Tampan core zone and its own buffer zone, while Cluster 2 consists of three core zones, namely Bukit Kepala Gajah, Bukit Gua Harimau and Bukit Jawa, all enclosed within a single buffer zone. For Perak, this recognition is important to attract more visitors to Perak this year, in conjunction with the Visit Perak Year 2012. From a tourism perspective, a place competitiveness can be increased by utilizing the unique resources of a particular place. The aim of this paper is to propose the competitiveness model of the Lenggong Valley. Various models on destination competitiveness will be reviewed

    Meeting the support and information needs of women with advanced breast cancer: a randomised controlled trial

    Addressing psychosocial and quality of life needs is central to provision of excellent care for people with advanced cancer. This study tested a brief nurse-delivered intervention to address the needs of urban women with advanced breast cancer. This study was conducted at four large urban hospitals in Australia. One hundred and five women with advanced breast cancer were recruited and randomised to receive the intervention or usual care, then asked to complete the European Organisation of Research and Treatment of Quality of life Q-C30 version (2.0) (EORTC Q-C30) (version 2) and Supportive Care Needs Survey (SCNS) at 1 month and 3 months postrecruitment. No significant differences were detected between intervention and usual care groups in the SCNS or the EORTC Q-C30 subscale scores. However, when the groups were divided into high needs (score of above 50) and low baseline needs (score of 50 or below) for each SCNS subscale, a significant difference between intervention and usual care groups was found in the psychological/emotional subscale among women with high baseline needs. In conclusions, this study demonstrated that a face-to-face session and follow-up phone call with a breast care nurse significantly reduced the psychological and emotional needs of those with high initial needs. There was no evidence of the intervention influencing the quality of life; or perceived needs of women with low initial psychological/emotional needs or perceived needs in other domains. Possibly, the intervention was not sufficiently intense to achieve an effect

    Noise correlation in PET, CT, SPECT and PET/CT data evaluated using autocorrelation function: a phantom study on data, reconstructed using FBP and OSEM

    BACKGROUND: Positron Emission Tomography (PET), Computed Tomography (CT), PET/CT and Single Photon Emission Tomography (SPECT) are non-invasive imaging tools used for creating two dimensional (2D) cross section images of three dimensional (3D) objects. PET and SPECT have the potential of providing functional or biochemical information by measuring distribution and kinetics of radiolabelled molecules, whereas CT visualizes X-ray density in tissues in the body. PET/CT provides fused images representing both functional and anatomical information with better precision in localization than PET alone. Images generated by these types of techniques are generally noisy, thereby impairing the imaging potential and affecting the precision in quantitative values derived from the images. It is crucial to explore and understand the properties of noise in these imaging techniques. Here we used autocorrelation function (ACF) specifically to describe noise correlation and its non-isotropic behaviour in experimentally generated images of PET, CT, PET/CT and SPECT. METHODS: Experiments were performed using phantoms with different shapes. In PET and PET/CT studies, data were acquired in 2D acquisition mode and reconstructed by both analytical filter back projection (FBP) and iterative, ordered subsets expectation maximisation (OSEM) methods. In the PET/CT studies, different magnitudes of X-ray dose in the transmission were employed by using different mA settings for the X-ray tube. In the CT studies, data were acquired using different slice thickness with and without applied dose reduction function and the images were reconstructed by FBP. SPECT studies were performed in 2D, reconstructed using FBP and OSEM, using post 3D filtering. ACF images were generated from the primary images, and profiles across the ACF images were used to describe the noise correlation in different directions. The variance of noise across the images was visualised as images and with profiles across these images. RESULTS: The most important finding was that the pattern of noise correlation is rotation symmetric or isotropic, independent of object shape in PET and PET/CT images reconstructed using the iterative method. This is, however, not the case in FBP images when the shape of phantom is not circular. Also CT images reconstructed using FBP show the same non-isotropic pattern independent of slice thickness and utilization of care dose function. SPECT images show an isotropic correlation of the noise independent of object shape or applied reconstruction algorithm. Noise in PET/CT images was identical independent of the applied X-ray dose in the transmission part (CT), indicating that the noise from transmission with the applied doses does not propagate into the PET images showing that the noise from the emission part is dominant. The results indicate that in human studies it is possible to utilize a low dose in transmission part while maintaining the noise behaviour and the quality of the images. CONCLUSION: The combined effect of noise correlation for asymmetric objects and a varying noise variance across the image field significantly complicates the interpretation of the images when statistical methods are used, such as with statistical estimates of precision in average values, use of statistical parametric mapping methods and principal component analysis. Hence it is recommended that iterative reconstruction methods are used for such applications. However, it is possible to calculate the noise analytically in images reconstructed by FBP, while it is not possible to do the same calculation in images reconstructed by iterative methods. Therefore for performing statistical methods of analysis which depend on knowing the noise, FBP would be preferred

    Differences in interproximal and occlusal tooth wear among prehistoric tennessee indians: Implications for masticatory function

    Breadth of the interproximal wear facet between lower P2 and M1 and between lower M1 and M2 was measured in human skeletal samples representing the Archaic, Woodland, and Mississippian periods of Tennessee River Valley prehistory, with the aim of assessing relative magnitudes of applied masticatory forces. When stratified by level of occlusal wear, mean interproximal facet breadth was consistently larger in the Archaic sample than in the Mississippian sample, with the Woodland sample intermediate. An analysis of covariance demonstrated that there was significant (p < 0.01) differences in facet size among the three groups even when differences in crown breadth were taken into account. Similar results were obtained in regressions of facet size on chronological age (Archaic larger than Mississippian at p < 0.01). Since rate of occlusal wear appears to be somewhat greater in the Archaic sample than in the later samples, the differences in interproximal wear are probably underestimated. It is suggested that the high levels of interproximal wear in the Archaic are indicative of the large occlusal forces and repetitive chewing required to masticate a diet of seeds, wild plant foods, and small animals, for which prior preparation (e.g., grinding, cooking) was minimal or nonexistent (as indicated by paleofecal samples). The lower amounts of interproximal wear observed in the Woodland and Mississippian samples imply considerable reductions in strenuous mastication, perhaps due to the widespread adoption during these periods of pottery and the earth oven, together with ethnographically-documented techniques of food preparation that transformed most foods to a soft consistency.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/37612/1/1330570111_ftp.pd

    State of knowledge on UK agricultural peatlands for food production and the net zero transition

    Agricultural peatlands are the most productive soils in the UK for the cultivation of many food crops. Historical drainage of peat for agriculture (i.e., cropland and managed grassland), without consideration of other associated environmental and climatic impacts, has resulted in a significant emission of greenhouse gases (GHGs). There is a need to reduce GHG emissions without compromising the rural economy and jeopardizing food security in the UK to a greater extent than is currently being experienced. In March 2023, in a bid to identify alternative land management systems for agricultural peatlands to support the UK’s commitment to achieving net zero GHG emissions by 2050, a group of forty investigators met at a workshop convened by the AgriFood4NetZero Network+. The workshop reviewed the state of knowledge surrounding the Fens of Eastern England and their importance for food provision, the economy, cultural identity, and climate change mitigation. A broad consensus emerged for research into how GHG emissions from agricultural peatlands could be reduced, whether alternative farming methods, such as seasonal farming or paludiculture, would offer a solution, and how a localized approach for the Fens could be defined. The development of a holistic, inclusive, and plausible land use scenario that considers all aspects of ecosystem services provided by the Fens is urgently needed

    A community-based, bionic leg rehabilitation program for patients with chronic stroke: clinical trial protocol

    Stroke is a major global health problem whereby many survivors have unmet needs concerning mobility during recovery. As such, the use of robotic assisted devices (i.e., a bionic leg) within a community-setting may be an important adjunct to normal physiotherapy in chronic stroke survivors. This study will be a dual-centre, randomized, parallel group clinical trial to investigate the impact of a community based, training program using a bionic leg on biomechanical, cardiovascular and functional outcomes in stroke survivors. Following a baseline assessment which will assess gait, postural sway, vascular health (blood pressure, arterial stiffness) and functional outcomes (6-minute walk), participants will be randomized to a 10-week program group, incorporating either: i) physiotherapy plus community-based bionic leg training program, ii) physiotherapy only, or iii) usual care control. The training program will involve participants engaging in a minimum of 1 hour per day of bionic leg activities at home. Follow up assessment, identical to baseline, will occur after 10-weeks, 3 and 12 months post intervention. Given the practical implications of the study, the clinical significance of using the bionic leg will be assessed for each outcome variable. The potential improvements in gait, balance, vascular health and functional status may have a meaningful impact on patients’ quality of life. The integration of robotic devices within home-based rehabilitation programs may prove to be a cost effective, practical and beneficial resource for stroke survivors

Given the limited number of surveys of geography published in academic journals since the 1970s, it is particularly timely to question and debate where the subject has evolved to, the current debates and issues facing those who work within the subject and where the subject will evolve in the next five years. The paper is structured around a number of distinct themes to emerge from the research activity of geographers, which is deliberately selective in its coverage due to the constraints of space, but focuses on: explaining spatialities; tourism planning and places; development and its discontents; tourism as an 'applied' area of research, and future prospects

    This paper examines event tourism as a field of study and area of professional practice updating the previous review article published in 2008. In this substantially extended review, a deeper analysis of the field’s evolution and development is presented, charting the growth of the literature, focusing both chronologically and thematically. A framework for understanding and creating knowledge about events and tourism is presented, forming the basis which signposts established research themes and concepts and outlines future directions for research. In addition, the review article focuses on constraining and propelling forces, ontological advances, contributions from key journals, and emerging themes and issues. It also presents a roadmap for research activity in event tourism
