    The cost-effectiveness of prophylaxis strategies for individuals with advanced HIV starting treatment in Africa

    Introduction: Many HIV-positive individuals in Africa have advanced disease when initiating antiretroviral therapy (ART) so have high risks of opportunistic infections and death. The REALITY trial found that an enhanced-prophylaxis package including fluconazole reduced mortality by 27% in individuals starting ART with CD4<100 cells/mm3. We investigated the cost-effectiveness of this enhanced-prophylaxis package versus other strategies, including using cryptococcal antigen (CrAg) testing, in individuals with CD4<200 cells/mm3 or <100 cells/mm3 at ART initiation and all individuals regardless of CD4 count. Methods: The REALITY trial enrolled from June 2013 to April 2015. A decision-analytic model was developed to estimate the cost-effectiveness of six management strategies in individuals initiating ART in the REALITY trial countries. Strategies included standard-prophylaxis, enhanced-prophylaxis, standard-prophylaxis with fluconazole; and three CrAg testing strategies, the first stratifying individuals to enhanced-prophylaxis (CrAg-positive) or standard-prophylaxis (CrAg-negative), the second to enhanced-prophylaxis (CrAg-positive) or enhanced-prophylaxis without fluconazole (CrAg-negative) and the third to standard-prophylaxis with fluconazole (CrAg-positive) or without fluconazole (CrAg-negative). The model estimated costs, life-years and quality-adjusted life-years (QALY) over 48 weeks using three competing mortality risks: cryptococcal meningitis; tuberculosis, serious bacterial infection or other known cause; and unknown cause. Results: Enhanced-prophylaxis was cost-effective at cost-effectiveness thresholds of US300andUS300 and US500 per QALY with an incremental cost-effectiveness ratio (ICER) of US157perQALYintheCD4<200cells/mm3populationprovidingenhancedprophylaxiscomponentsaresourcedatlowestavailableprices.TheICERreducedinmoreseverelyimmunosuppressedindividuals(US157 per QALY in the CD4<200 cells/mm3 population providing enhanced-prophylaxis components are sourced at lowest available prices. The ICER reduced in more severely immunosuppressed individuals (US113 per QALY in the CD4<100 cells/mm3 population) and increased in all individuals regardless of CD4 count (US722perQALY).Resultsweresensitivetopricesoftheenhancedprophylaxiscomponents.EnhancedprophylaxiswasmoreeffectiveandlesscostlythanallCrAgtestingstrategiesasenhancedprophylaxisstillconveyedhealthgainsinCrAgnegativepatientsandsavingsfromtargetingprophylaxisbasedonCrAgstatusdidnotcompensateforcostsofCrAgtesting.CrAgtestingstrategiesdidnotbecomecosteffectiveunlessthepriceofCrAgtestingfellbelowUS722 per QALY). Results were sensitive to prices of the enhanced-prophylaxis components. Enhanced-prophylaxis was more effective and less costly than all CrAg testing strategies as enhanced-prophylaxis still conveyed health gains in CrAg-negative patients and savings from targeting prophylaxis based on CrAg status did not compensate for costs of CrAg testing. CrAg testing strategies did not become cost-effective unless the price of CrAg testing fell below US2.30. Conclusions: The REALITY enhanced-prophylaxis package in individuals with advanced HIV starting ART reduces morbidity and mortality, is practical to administer and is cost-effective. Efforts should continue to ensure that components are accessed at lowest available prices.     What Are The Benefits Of Treatment Of Familial Hypercholesterolaemia? : A Cost-Effectiveness Model Based On Real-World Data Considering Cholesterol Burden

    Objectives: To develop a new cost-effectiveness model, using real-world data, to predict long-term health outcomes and costs of patients with familial hypercholesterolaemia (FH), with and without treatment, and considering the increased effect of low-density lipoprotein cholesterol (LDL-C) over time on cardiovascular risk (known as cholesterol burden). Methods: We used routinely collected healthcare and mortality data from FH patients in the UK to estimate parametric survival models for cardiovascular events given age, sex, LDL-C, prior cardiovascular history, and to estimate the effect of treatment on LDL-C. We developed a new cohort Markov model over a lifetime horizon to predict health outcomes (e.g., quality-adjusted life years, QALYs) and UK National Health Service costs. Since the routine data was from treated patients, we estimated the counterfactual risk had they not been treated with and without accounting for cholesterol burden. Results: Accounting for cholesterol burden, treatment increases event-free survival by up to 30 years with greater benefits in younger patients, men, those with higher LDL-C and prior cardiovascular history. Treatment results in savings in those with LDL-C < 4.5 mmol/L and, in most patients, is cost-effective (incremental net health benefit at £15,000/QALY up to 2.4 QALYs). If cholesterol burden is not considered, the model predicts smaller benefits from treatment (e.g., 11 years event-free survival gain) and, in some patients, it is not cost-effective (e.g., patients < 18 years with LDL-C < 4 mmol/L). Conclusions: Treatment results in large health benefits and is highly cost-effective, but magnitude of benefits depends on whether cholesterol burden is considered, particularly in younger patients. Further research is warranted on effect size of cholesterol burden and how best to consider it in cost-effectiveness modelling

    Cost-effectiveness of Cardiovascular Imaging for Stable Coronary Heart Disease

    Objective: To assess the cost-effectiveness of management strategies for patients presenting with chest pain and suspected coronary heart disease (CHD): i) cardiovascular magnetic resonance (CMR); (ii) myocardial perfusion scintigraphy (MPS); and (iii) UK National Institute for Health and Care Excellence (NICE) guideline-guided care. Methods: Using UK data for 1,202 patients from the Clinical Evaluation of Magnetic Resonance Imaging in Coronary Heart Disease 2 trial, we conducted an economic evaluation to assess the cost-effectiveness of CMR, MPS and NICE guidelines. Health outcomes were expressed as quality-adjusted life-years (QALY) and costs reflected UK pounds sterling 2016-17. Cost-effectiveness results were presented as incremental cost-effectiveness ratios and incremental net health benefits overall and for low, medium, and high pre-test likelihood of CHD subgroups. Results: CMR had the highest estimated QALY gain overall (2.21 [95% credible interval 2.15,2.26] compared to 2.07 [1.92,2.20] NICE and 2.11 [2.01,2.22] MPS) and incurred comparable costs (overall £1625 [£1431,£1824] compared to £1753 [£1473,£2032] NICE and £1768 [£1572,£1989] MPS). Overall, CMR was the cost-effective strategy, being the dominant strategy (more effective less costly) with incremental net health benefits per patient of 0.146 QALYs [-0.18,0.406] compared to NICE guidelines at a cost-effectiveness threshold of £15,000/QALY (93% probability of cost-effectiveness). Results were similar in the pre-test likelihood subgroups. Conclusions: CMR guided care is cost-effective overall and across all pre-test likelihood subgroups, compared to MPS and NICE guidelines

    Prevalence and architecture of de novo mutations in developmental disorders.

    The genomes of individuals with severe, undiagnosed developmental disorders are enriched in damaging de novo mutations (DNMs) in developmentally important genes. Here we have sequenced the exomes of 4,293 families containing individuals with developmental disorders, and meta-analysed these data with data from another 3,287 individuals with similar disorders. We show that the most important factors influencing the diagnostic yield of DNMs are the sex of the affected individual, the relatedness of their parents, whether close relatives are affected and the parental ages. We identified 94 genes enriched in damaging DNMs, including 14 that previously lacked compelling evidence of involvement in developmental disorders. We have also characterized the phenotypic diversity among these disorders. We estimate that 42% of our cohort carry pathogenic DNMs in coding sequences; approximately half of these DNMs disrupt gene function and the remainder result in altered protein function. We estimate that developmental disorders caused by DNMs have an average prevalence of 1 in 213 to 1 in 448 births, depending on parental age. Given current global demographics, this equates to almost 400,000 children born per year

    Search for dark matter produced in association with bottom or top quarks in √s = 13 TeV pp collisions with the ATLAS detector

    A search for weakly interacting massive particle dark matter produced in association with bottom or top quarks is presented. Final states containing third-generation quarks and miss- ing transverse momentum are considered. The analysis uses 36.1 fb−1 of proton–proton collision data recorded by the ATLAS experiment at √s = 13 TeV in 2015 and 2016. No significant excess of events above the estimated backgrounds is observed. The results are in- terpreted in the framework of simplified models of spin-0 dark-matter mediators. For colour- neutral spin-0 mediators produced in association with top quarks and decaying into a pair of dark-matter particles, mediator masses below 50 GeV are excluded assuming a dark-matter candidate mass of 1 GeV and unitary couplings. For scalar and pseudoscalar mediators produced in association with bottom quarks, the search sets limits on the production cross- section of 300 times the predicted rate for mediators with masses between 10 and 50 GeV and assuming a dark-matter mass of 1 GeV and unitary coupling. Constraints on colour- charged scalar simplified models are also presented. Assuming a dark-matter particle mass of 35 GeV, mediator particles with mass below 1.1 TeV are excluded for couplings yielding a dark-matter relic density consistent with measurements

    Finishing the euchromatic sequence of the human genome

    The sequence of the human genome encodes the genetic instructions for human physiology, as well as rich information about human evolution. In 2001, the International Human Genome Sequencing Consortium reported a draft sequence of the euchromatic portion of the human genome. Since then, the international collaboration has worked to convert this draft into a genome sequence with high accuracy and nearly complete coverage. Here, we report the result of this finishing process. The current genome sequence (Build 35) contains 2.85 billion nucleotides interrupted by only 341 gaps. It covers ∼99% of the euchromatic genome and is accurate to an error rate of ∼1 event per 100,000 bases. Many of the remaining euchromatic gaps are associated with segmental duplications and will require focused work with new methods. The near-complete sequence, the first for a vertebrate, greatly improves the precision of biological analyses of the human genome including studies of gene number, birth and death. Notably, the human enome seems to encode only 20,000-25,000 protein-coding genes. The genome sequence reported here should serve as a firm foundation for biomedical research in the decades ahead

    Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.
