    Is there a duty to participate in digital epidemiology?

    This paper poses the question of whether people have a duty to participate in digital epidemiology. While an implied duty to participate has been argued for in relation to biomedical research in general, digital epidemiology involves processing of non-medical, granular and proprietary data types that pose different risks to participants. We first describe traditional justifications for epidemiology that imply a duty to participate for the general public, which take account of the immediacy and plausibility of threats, and the identifiability of data. We then consider how these justifications translate to digital epidemiology, understood as an evolution of traditional epidemiology that includes personal and proprietary digital data alongside formal medical datasets. We consider the risks imposed by re-purposing such data for digital epidemiology and propose eight justificatory conditions that should be met in justifying a duty to participate for specific digital epidemiological studies. The conditions are then applied to three hypothetical cases involving usage of social media data for epidemiological purposes. We conclude with a list of questions to be considered in public negotiations of digital epidemiology, including the application of a duty to participate to third-party data controllers, and the important distinction between moral and legal obligations to participate in research

    Long-term tropospheric formaldehyde concentrations deduced from ground-based fourier transform solar infrared measurements

    We report a 13-year (1992–2005) dataset of total column measurements of formaldehyde (HCHO) over Lauder, New Zealand, inferred from solar infrared spectra measured using a high-resolution Fourier Transform Spectrometer (FTS). Ambient HCHO concentrations at this rural location are often close to levels typical of remote marine environments (\u3c250 ppt), which are close to the detection limit using standard techniques. Consequently we develop a new method that successfully produces HCHO columns with sufficient sensitivity throughout the whole season. HCHO columns over Lauder have a strong seasonal cycle (±50%), with a mean column of 4.9×1015 molecules cm−2, peaking during summer months. A simple box model of CH4 oxidation reproduces the observed broad-scale seasonal cycle, but significantly underestimates the seasonal peak HCHO ground concentrations during summer. This suggests the existence of an additional significant source of HCHO, possibly isoprene that cannot be explained by oxidation of CH4 alone. The ground-based FTS column data compare well with collocated HCHO column measurements from the Global Ozone Monitoring Experiment (GOME) satellite instrument during the operational period of GOME (1996–2001, r2=0.65, mean bias=10%, n=48)

    Development and validation of an RP-HPLC/UV Method for Determination of Cholecalciferol in Polymeric Nanoparticles Suspensions / Desenvolvimento e validação de um método RP-HPLC/UV para determinação do Cholecalciferol em nanopartículas poliméricas Suspensões

    A simple and effective RP-HPLC/UV method was developed to determine cholecalciferol (COL) in suspensions of polymeric nanoparticles. Chromatographic conditions used in the methodology were a C18 RP column using an isocratic mobile phase of methanol and water (98:2 v/v) at a flow rate of 1.2 mL/min, UV detection at 265 nm. The validation parameters evaluated were: specificity, linearity, limit of quantification, limit of detection, precision, precision and robustness. The retention time was about 7.5 min. The method was linear, specific (r=0.9992) interval 10.0 to 80.0 ?g/mL, the limit of quantification (LOQ) was 0.52 ?g/mL and the limit of detection was 0.15 ?g/mL. Intermediate precision was evaluated, expressed as inter-day variation (RSD = 1.13) and intra-day (RSD = 1.16). The accuracy obtained was the recovery of 102.87 ± 9.84, considering the exact method proposed. To evaluate the robustness, the results showed little variation, concluding that a robust method. The method was fast, robust and suitable for quantification of suspensions of COL nanoparticles and can be used to evaluate the efficiency of COL encapsulation

    Long-Term Clinical and Multimodal Imaging Findings in Patients with Disseminated Mycobacterium Chimaera Infection

    BACKGROUND To analyze long-term ophthalmic clinical and multimodal imaging findings of disseminated Mycobacterium (M.) chimaera infection after cardiothoracic surgery among the Swiss Cohort. METHODS Systemic and multimodal ophthalmic imaging and clinical findings including rate of recurrence were reviewed and correlated to a previously proposed classification system of choroidal lesions and classification of ocular disease. MAIN OUTCOMES MEASURES long-term clinical and multimodal ocular imaging findings of M. chimaera. RESULTS Twelve patients suffering from systemic infection from M. chimaera were included. Mean age at the first ophthalmic examination was 59 years (range from 48 to 66 years). Mean duration of the follow-up was 22.63 ± 17.8 months. All patients presented with bilateral chorioretinal lesions at baseline; 5 patients had additional signs, including optic disc swelling (2), choroidal neovascularization (1), retinal neovascularization (1) and cilioretinal vascular occlusion (1). Four recurrence events after discontinuation or adjustment of the antibiotic treatment were observed. Progressive choroiditis was seen in 5 patients under treatment, 4 of them deceased. CONCLUSIONS Expertise from ophthalmologists is not only relevant but also critical for the assessment of the adverse drug effect of antimycobacterial treatment along with monitoring therapeutic response and identifying recurrences

Publisher Copyright: © 2022, The Author(s).Background: People with mental health problems are more vulnerable to a broad range of coercive practices and human rights abuses. There is a global campaign to eliminate, or at the very least decrease, the use of coercion in mental health care. The use of coercion in psychiatric hospitals in developing countries is poorly documented. The primary aim of this study was to explore service users’ perceptions and experiences of coercion in psychiatric hospitals in Nigeria. Methods: Four focus group discussions were carried out among 30 service users on admission in two major psychiatric hospitals in Nigeria. The audio recordings were transcribed verbatim and then analyzed thematically with the aid of MAXQDA software. Results: The Focus group participants included 19 males and 11 females with a mean age of 34.67 ± 9.54. Schizophrenia was the most common diagnosis (40%, n = 12) and had a secondary school education (60%, n = 18). The focus group participants perceived coercion to be a necessary evil in severe cases but anti-therapeutic to their own recovery, an extension of stigma and a vicious cycle of abuse. The experience of involuntary admission revolved mainly around deception, maltreatment, and disdain. Participants in both study sites narrated experiences of being flogged for refusing medication. Mechanical restraint with chains was a common experience for reasons including refusing medications, to prevent absconding and in other cases, punitively. The use of chains was viewed by participants as dehumanizing and excruciatingly painful. Conclusion: The experiences of coercion by participants in this study confirm that human rights violations occur in large psychiatric hospitals and underscore the need for mental health services reform. The use of coercion in this context reflects agelong underinvestment in the mental health care system in the country and obsolete mental health legislation that does not protect the rights of people with mental health problems. The study findings highlight an urgent need to address issues of human rights violations in psychiatric hospitals in the country.publishersversionpublishe

    Examples of risk tools for pests in Peanut (Arachis hypogaea) developed for five countries using Microsoft Excel

    Suppressing pest populations below economically-damaging levels is an important element of sustainable peanut (Arachis hypogaea L.) production. Peanut farmers and their advisors often approach pest management with similar goals regardless of where they are located. Anticipating pest outbreaks using field history and monitoring pest populations are fundamental to protecting yield and financial investment. Microsoft Excel was used to develop individual risk indices for pests, a composite assessment of risk, and costs of risk mitigation practices for peanut in Argentina, Ghana, India, Malawi, and North Carolina (NC) in the United States (US). Depending on pests and resources available to manage pests, risk tools vary considerably, especially in the context of other crops that are grown in sequence with peanut, cultivars, and chemical inputs. In Argentina, India, and the US where more tools (e.g., mechanization and pesticides) are available, risk indices for a wide array of economically important pests were developed with the assumption that reducing risk to those pests likely will impact peanut yield in a positive manner. In Ghana and Malawi where fewer management tools are available, risks to yield and aflatoxin contamination are presented without risk indices for individual pests. The Microsoft Excel platform can be updated as new and additional information on effectiveness of management practices becomes apparent. Tools can be developed using this platform that are appropriate for their geography, environment, cropping systems, and pest complexes and management inputs that are available.     Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     Differential cross section measurements for the production of a W boson in association with jets in proton–proton collisions at √s = 7 TeV

    Measurements are reported of differential cross sections for the production of a W boson, which decays into a muon and a neutrino, in association with jets, as a function of several variables, including the transverse momenta (pT) and pseudorapidities of the four leading jets, the scalar sum of jet transverse momenta (HT), and the difference in azimuthal angle between the directions of each jet and the muon. The data sample of pp collisions at a centre-of-mass energy of 7 TeV was collected with the CMS detector at the LHC and corresponds to an integrated luminosity of 5.0 fb[superscript −1]. The measured cross sections are compared to predictions from Monte Carlo generators, MadGraph + pythia and sherpa, and to next-to-leading-order calculations from BlackHat + sherpa. The differential cross sections are found to be in agreement with the predictions, apart from the pT distributions of the leading jets at high pT values, the distributions of the HT at high-HT and low jet multiplicity, and the distribution of the difference in azimuthal angle between the leading jet and the muon at low values.United States.     Juxtaposing BTE and ATE – on the role of the European insurance industry in funding civil litigation

    One of the ways in which legal services are financed, and indeed shaped, is through private insurance arrangement. Two contrasting types of legal expenses insurance contracts (LEI) seem to dominate in Europe: before the event (BTE) and after the event (ATE) legal expenses insurance. Notwithstanding institutional differences between different legal systems, BTE and ATE insurance arrangements may be instrumental if government policy is geared towards strengthening a market-oriented system of financing access to justice for individuals and business. At the same time, emphasizing the role of a private industry as a keeper of the gates to justice raises issues of accountability and transparency, not readily reconcilable with demands of competition. Moreover, multiple actors (clients, lawyers, courts, insurers) are involved, causing behavioural dynamics which are not easily predicted or influenced. Against this background, this paper looks into BTE and ATE arrangements by analysing the particularities of BTE and ATE arrangements currently available in some European jurisdictions and by painting a picture of their respective markets and legal contexts. This allows for some reflection on the performance of BTE and ATE providers as both financiers and keepers. Two issues emerge from the analysis that are worthy of some further reflection. Firstly, there is the problematic long-term sustainability of some ATE products. Secondly, the challenges faced by policymakers that would like to nudge consumers into voluntarily taking out BTE LEI
