    Probing the sensitivity of orientational ordering as a way towards absolute enantiorecognition: Helical-particle solutes in helical-particle nematic solvents

    To inquire into whether two enantiomers can be absolutely distinguished on the sole basis of their different degrees of orientational ordering when dissolved in a chiral nematic liquid crystal, several enantiomeric pairs of hard helical particles are dissolved in a cholesteric (nematic) liquid crystal made of slender hard helical particles as well as in a screwlike nematic liquid crystal made of tortuous hard helical particles. While in the former ordinary chiral nematic solvent their nematic order parameters are (almost) coincident, in the latter new chiral nematic solvent the two enantiomeric solutes not only usually have more appreciably different nematic order parameters, but also always have significantly different screwlike order parameters. If also the latter orientational order parameter could be measurable in real experiments, it would constitute an additional decisive piece of information on the way to absolutely distinguishing two enantiomers on the sole basis of their different degrees of orientational order when dissolved in a chiral nematic liquid crystal that is in the apter screwlike nematic phase.     Post Hoc Analysis of a Randomized Controlled Trial on Fasting and Plant-Based Diet in Rheumatoid Arthritis (NutriFast): Nutritional Supply and Impact on Dietary Behavior

    This study aimed at comparing the nutrient supply and dietary behaviors during a plant-based diet (PBD) combined with time-restricted eating (TRE) to standard dietary recommendations in rheumatoid arthritis patients. In this open-label, randomized, controlled clinical trial, patients were assigned to either a 7-day fast followed by an 11-week PBD including TRE (A) or a 12-week anti-inflammatory diet following official German guidelines (German Nutrition Society, DGE) (B). Dietary habits were assessed by 3-day food records at weeks -1, 4 and 9 and food frequency questionnaires. 41 out of 53 participants were included in a post-hoc per protocol analysis. Both groups had similar energy, carbohydrate, sugar, fiber and protein intake at week 4. Group A consumed significantly less total saturated fat than group B (15.9 +/- 7.7 vs. 23.2 +/- 10.3 g/day; p = 0.02). Regarding micronutrients, group B consumed more vitamin A, B-12, D, riboflavin and calcium (each p <= 0.02). Zinc and calcium were below recommended intakes in both groups. Cluster analysis did not show clear group allocation after three months. Hence, dietary counselling for a PBD combined with TRE compared to a standard anti-inflammatory diet does not seem to lead to two different dietary clusters, i.e., actual different dietary behaviors as expected.     Divergence-free spherical harmonic gravity field modelling based on the Runge–Krarup theorem: a case study for the Moon

    Recent numerical studies on external gravity field modelling show that external spherical harmonic series may diverge near or on planetary surfaces. This paper investigates an alternative solution that is still based on external spherical harmonic series, but capable of avoiding the divergence effect. The approach relies on the Runge–Krarup theorem and the iterative downward continuation. In theory, Runge–Krarup-type solutions are able to approximate the true potential within the entire space external to the masses with an arbitrary e-accuracy, e&gt; 0. Using gravity implied by the lunar topography, we show numerically that this technique avoids indeed the divergence effect, at least at the studied 5 arc-min resolution. In the context of the iterative scheme, we show that a function expressed as a truncated solid spherical harmonic expansion on a general star-shaped surface possesses an infinite surface spherical harmonic spectrum after it is mapped onto a sphere. We also study the convergence of the gradient approach, which is a technique for efficient grid-wise synthesis on irregular surfaces. We show that the resulting Taylor series may converge slowly when analytically upward continuing from points inside the masses. The continuation from the mass-free space should therefore be preferred. As an underlying topic of the paper, spherical harmonic coefficients from spectral gravity forward modelling and their Runge–Krarup counterpart are numerically studied. Regarding their different nature, we formulate some research topics that might contribute to a deeper understanding of the current methodologies used to develop combined high-degree spherical harmonic gravity models

    Surgeons’ practice and preferences for the anal fissure treatment: results from an international survey

    The best nonoperative or operative anal fissure (AF) treatment is not yet established, and several options have been proposed. Aim is to report the surgeons' practice for the AF treatment. Thirty-four multiple-choice questions were developed. Seven questions were about to participants' demographics and, 27 questions about their clinical practice. Based on the specialty (general surgeon and colorectal surgeon), obtained data were divided and compared between two groups. Five-hundred surgeons were included (321 general and 179 colorectal surgeons). For both groups, duration of symptoms for at least 6 weeks is the most important factor for AF diagnosis (30.6%). Type of AF (acute vs chronic) is the most important factor which guide the therapeutic plan (44.4%). The first treatment of choice for acute AF is ointment application for both groups (59.6%). For the treatment of chronic AF, this data is confirmed by colorectal surgeons (57%), but not by the general surgeons who prefer the lateral internal sphincterotomy (LIS) (31.8%) (p = 0.0001). Botulin toxin injection is most performed by colorectal surgeons (58.7%) in comparison to general surgeons (20.9%) (p = 0.0001). Anal flap is mostly performed by colorectal surgeons (37.4%) in comparison to general surgeons (28.3%) (p = 0.0001). Fissurectomy alone is statistically significantly most performed by general surgeons in comparison to colorectal surgeons (57.9% and 43.6%, respectively) (p = 0.0020). This analysis provides useful information about the clinical practice for the management of a debated topic such as AF treatment. Shared guidelines and consensus especially focused on operative management are required to standardize the treatment and to improve postoperative results

    Apolipoprotein B, Residual Cardiovascular Risk After Acute Coronary Syndrome, and Effects of Alirocumab.

    Background: Apolipoprotein B (apoB) provides an integrated measure of atherogenic risk. Whether apoB levels and apoB lowering hold incremental predictive information on residual risk after acute coronary syndrome beyond that provided by low-density lipoprotein cholesterol is uncertain. Methods: The ODYSSEY OUTCOMES trial (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) compared the proprotein convertase subtilisin/kexin type 9 inhibitor alirocumab with placebo in 18 924 patients with recent acute coronary syndrome and elevated atherogenic lipoproteins despite optimized statin therapy. Primary outcome was major adverse cardiovascular events (MACE; coronary heart disease death, nonfatal myocardial infarction, fatal/nonfatal ischemic stroke, hospitalization for unstable angina). Associations between baseline apoB or apoB at 4 months and MACE were assessed in adjusted Cox proportional hazards and propensity score–matched models. Results: Median follow-up was 2.8 years. In proportional hazards analysis in the placebo group, MACE incidence increased across increasing baseline apoB strata (3.2 [95% CI, 2.9–3.6], 4.0 [95% CI, 3.6–4.5], and 5.5 [95% CI, 5.0–6.1] events per 100 patient-years in strata 35–<50, and ≀35 mg/dL, respectively). Compared with propensity score–matched patients from the placebo group, treatment hazard ratios for alirocumab also decreased monotonically across achieved apoB strata. Achieved apoB was predictive of MACE after adjustment for achieved low-density lipoprotein cholesterol or non–high-density lipoprotein cholesterol but not vice versa. Conclusions: In patients with recent acute coronary syndrome and elevated atherogenic lipoproteins, MACE increased across baseline apoB strata. Alirocumab reduced MACE across all strata of baseline apoB, with larger absolute reductions in patients with higher baseline levels. Lower achieved apoB was associated with lower risk of MACE, even after accounting for achieved low-density lipoprotein cholesterol or non–high-density lipoprotein cholesterol, indicating that apoB provides incremental information. Achievement of apoB levels as low as ≀35 mg/dL may reduce lipoprotein-attributable residual risk after acute coronary syndrome.     The impact of surgical delay on resectability of colorectal cancer: An international prospective cohort study

    AimThe SARS-CoV-2 pandemic has provided a unique opportunity to explore the impact of surgical delays on cancer resectability. This study aimed to compare resectability for colorectal cancer patients undergoing delayed versus non-delayed surgery.MethodsThis was an international prospective cohort study of consecutive colorectal cancer patients with a decision for curative surgery (January-April 2020). Surgical delay was defined as an operation taking place more than 4 weeks after treatment decision, in a patient who did not receive neoadjuvant therapy. A subgroup analysis explored the effects of delay in elective patients only. The impact of longer delays was explored in a sensitivity analysis. The primary outcome was complete resection, defined as curative resection with an R0 margin.ResultsOverall, 5453 patients from 304 hospitals in 47 countries were included, of whom 6.6% (358/5453) did not receive their planned operation. Of the 4304 operated patients without neoadjuvant therapy, 40.5% (1744/4304) were delayed beyond 4 weeks. Delayed patients were more likely to be older, men, more comorbid, have higher body mass index and have rectal cancer and early stage disease. Delayed patients had higher unadjusted rates of complete resection (93.7% vs. 91.9%, P = 0.032) and lower rates of emergency surgery (4.5% vs. 22.5%, P ConclusionOne in 15 colorectal cancer patients did not receive their planned operation during the first wave of COVID-19. Surgical delay did not appear to compromise resectability, raising the hypothesis that any reduction in long-term survival attributable to delays is likely to be due to micro-metastatic disease
