    Giant fibroadenoma of the breast in a pre-pubertal girl: a case report

    Juvenile fibroadenoma comprises about 4% of the total fibroadenomas. The incidence of giant juvenile fibroadenomas is merely 0.5% of all the fibroadenomas. Bilateral giant juvenile fibroadenomas are extremely rare. We are presenting a case of giant juvenile fibroadenomas in an 11-year-old pre-pubertal girl. The diagnosis was made on fine-needle aspiration cytology which was confirmed on histopathology. As these tumors are mostly benign, breast-conserving surgery is done so that patient can lead a normal life without psychological trauma.-----------------------------------Cite this article as: Goyal S, Garg G, Narang S.     Cross Modal Distillation for Flood Extent Mapping

    The increasing intensity and frequency of floods is one of the many consequences of our changing climate. In this work, we explore ML techniques that improve the flood detection module of an operational early flood warning system. Our method exploits an unlabelled dataset of paired multi-spectral and Synthetic Aperture Radar (SAR) imagery to reduce the labeling requirements of a purely supervised learning method. Prior works have used unlabelled data by creating weak labels out of them. However, from our experiments we noticed that such a model still ends up learning the label mistakes in those weak labels. Motivated by knowledge distillation and semi supervised learning, we explore the use of a teacher to train a student with the help of a small hand labelled dataset and a large unlabelled dataset. Unlike the conventional self distillation setup, we propose a cross modal distillation framework that transfers supervision from a teacher trained on richer modality (multi-spectral images) to a student model trained on SAR imagery. The trained models are then tested on the Sen1Floods11 dataset. Our model outperforms the Sen1Floods11 baseline model trained on the weak labeled SAR imagery by an absolute margin of 6.53% Intersection-over-Union (IoU) on the test split

    Human-computer collaboration for skin cancer recognition

    The rapid increase in telemedicine coupled with recent advances in diagnostic artificial intelligence (AI) create the imperative to consider the opportunities and risks of inserting AI-based support into new paradigms of care. Here we build on recent achievements in the accuracy of image-based AI for skin cancer diagnosis to address the effects of varied representations of AI-based support across different levels of clinical expertise and multiple clinical workflows. We find that good quality AI-based support of clinical decision-making improves diagnostic accuracy over that of either AI or physicians alone, and that the least experienced clinicians gain the most from AI-based support. We further find that AI-based multiclass probabilities outperformed content-based image retrieval (CBIR) representations of AI in the mobile technology environment, and AI-based support had utility in simulations of second opinions and of telemedicine triage. In addition to demonstrating the potential benefits associated with good quality AI in the hands of non-expert clinicians, we find that faulty AI can mislead the entire spectrum of clinicians, including experts. Lastly, we show that insights derived from AI class-activation maps can inform improvements in human diagnosis. Together, our approach and findings offer a framework for future studies across the spectrum of image-based diagnostics to improve human-computer collaboration in clinical practice

    Genome-Wide Functional Profiling Reveals Genes Required for Tolerance to Benzene Metabolites in Yeast

    Benzene is a ubiquitous environmental contaminant and is widely used in industry. Exposure to benzene causes a number of serious health problems, including blood disorders and leukemia. Benzene undergoes complex metabolism in humans, making mechanistic determination of benzene toxicity difficult. We used a functional genomics approach to identify the genes that modulate the cellular toxicity of three of the phenolic metabolites of benzene, hydroquinone (HQ), catechol (CAT) and 1,2,4-benzenetriol (BT), in the model eukaryote Saccharomyces cerevisiae. Benzene metabolites generate oxidative and cytoskeletal stress, and tolerance requires correct regulation of iron homeostasis and the vacuolar ATPase. We have identified a conserved bZIP transcription factor, Yap3p, as important for a HQ-specific response pathway, as well as two genes that encode putative NAD(P)H:quinone oxidoreductases, PST2 and YCP4. Many of the yeast genes identified have human orthologs that may modulate human benzene toxicity in a similar manner and could play a role in benzene exposure-related disease

    Guidelines for the use and interpretation of assays for monitoring autophagy (3rd edition)

    In 2008 we published the first set of guidelines for standardizing research in autophagy. Since then, research on this topic has continued to accelerate, and many new scientists have entered the field. Our knowledge base and relevant new technologies have also been expanding. Accordingly, it is important to update these guidelines for monitoring autophagy in different organisms. Various reviews have described the range of assays that have been used for this purpose. Nevertheless, there continues to be confusion regarding acceptable methods to measure autophagy, especially in multicellular eukaryotes. For example, a key point that needs to be emphasized is that there is a difference between measurements that monitor the numbers or volume of autophagic elements (e.g., autophagosomes or autolysosomes) at any stage of the autophagic process versus those that measure fl ux through the autophagy pathway (i.e., the complete process including the amount and rate of cargo sequestered and degraded). In particular, a block in macroautophagy that results in autophagosome accumulation must be differentiated from stimuli that increase autophagic activity, defi ned as increased autophagy induction coupled with increased delivery to, and degradation within, lysosomes (inmost higher eukaryotes and some protists such as Dictyostelium ) or the vacuole (in plants and fungi). In other words, it is especially important that investigators new to the fi eld understand that the appearance of more autophagosomes does not necessarily equate with more autophagy. In fact, in many cases, autophagosomes accumulate because of a block in trafficking to lysosomes without a concomitant change in autophagosome biogenesis, whereas an increase in autolysosomes may reflect a reduction in degradative activity. It is worth emphasizing here that lysosomal digestion is a stage of autophagy and evaluating its competence is a crucial part of the evaluation of autophagic flux, or complete autophagy. Here, we present a set of guidelines for the selection and interpretation of methods for use by investigators who aim to examine macroautophagy and related processes, as well as for reviewers who need to provide realistic and reasonable critiques of papers that are focused on these processes. These guidelines are not meant to be a formulaic set of rules, because the appropriate assays depend in part on the question being asked and the system being used. In addition, we emphasize that no individual assay is guaranteed to be the most appropriate one in every situation, and we strongly recommend the use of multiple assays to monitor autophagy. Along these lines, because of the potential for pleiotropic effects due to blocking autophagy through genetic manipulation it is imperative to delete or knock down more than one autophagy-related gene. In addition, some individual Atg proteins, or groups of proteins, are involved in other cellular pathways so not all Atg proteins can be used as a specific marker for an autophagic process. In these guidelines, we consider these various methods of assessing autophagy and what information can, or cannot, be obtained from them. Finally, by discussing the merits and limits of particular autophagy assays, we hope to encourage technical innovation in the field

    Laparoscopic cholecystectomy in situs inversus totalis: A review article

    Background: Situs inversus totalis is a rare congenital anomaly characterized by transposition of organs to the opposite side of the body. Diagnosis and treatment of cholelithiasis in such cases pose a challenge to the operating surgeon because of an atypical clinical picture and the contra lateral disposition of the viscera. Methods: A literature search using the PubMed and Cochrane databases identified articles focusing on the key issues of laparoscopy cholecystectomy in situs inversus. Conclusions: Without doubt, laparoscopic cholecystectomy in these patients is technically more demanding but still feasible and should be performed by trained and experienced laparoscopic surgeons. Difficulty is encountered in skeletonizing the structures in Calot's triangle, which usually requires extra time than in patients with a normally located gall bladder.     Clinical and management profile of thoracic trauma patients

    Background: Field of traumatology has been developing over the past 25 years or so, it is only recently that trauma has come to be recognized as a discrete disease entity by the medical and lay communities. Trauma represents a significant portion of the world’s morbidity and mortality. Of the 11 million deaths each year in the developed world, 0.8 million are the result of trauma. Aims and Objectives: To study the clinical profile of thoracic trauma patients and compare various management options. Material and Methods: This prospective study was carried out on one hundred consecutive patients of thoracic injuries who required indoor admission due to chest trauma. Results: Males predominated females in thoracic trauma patients. Blunt trauma mainly road accidents formed the commonest cause of thoracic trauma with right side of chest as most commonly while left side was more commonly involved in penetrating injuries. Out of total 17 patients of flail chest, 3 had mortality. Duration of tube thoracostomy and its complications were more in blunt thoracic injury patients as compared to penetrating injury patients. 68% patients underwent tube thoracostomies and haemothorax was the commonest indication. Operative intervention was required in 12% cases. Six cases underwent splenectomy mainly for associated abdominal injuries. Overall mortality rate in the present study was 6% and it was more in blunt injuries as compared to penetrating injuries. Conclusion: Chest injury constitutes a significant number of trauma patients and commonly affected victims are males of productive age. Majority of these patients can be managed by conservative management and simple intervention like intercostal chest drainage
