    Potencial de aplicação de medidas de acalmia de tráfego na transformação do ambiente rodoviário: o caso do atravessamento de localidades

    Mestrado em Planeamento do Território - Ordenamento da CidadeO excesso de velocidade é considerado como a principal causa de morte na estrada. Corroborada pelos meios de comunicação social, a própria vivência urbana comprova que o número de acidentes, de mortos, de feridos graves e de feridos leves continua elevado e que Portugal ocupa um dos primeiros lugares europeus em matéria de sinistralidade rodoviária. A maior parte dos acidentes ocorre dentro das localidades, sendo que uma parte significativa ocorre em estradas nacionais e municipais que as atravessam. De facto, os troços de estrada que atravessam espaços urbanos, para além de constituírem eixos rodoviários onde importa salvaguardar condições de fluidez no trânsito de passagem, representam, ainda e muitas das vezes, a rua principal do aglomerado, onde se centram as actividades locais. Estes tipos de situações são comuns a vários países onde no mesmo troço de estrada coexistem, de forma nem sempre pacífica, funções ligadas à mobilidade e à acessibilidade local. Por um lado, o carácter urbano que a via adquiriu, fruto do desenvolvimento territorial, requer boas condições de acessibilidade aos serviços e comércio que a ladeiam garantindo, ainda, níveis de segurança elevados para todos os utilizadores envolvidos. Porém, face aos níveis de procura de tráfego, que podem atingir valores significativos, revela-se fundamental para a economia regional salvaguardar níveis de fluidez elevados. Esta acumulação de funções está, maioritariamente, na base dos conflitos graves gerados entre a utilização do mesmo espaço pelos vários utilizadores envolvidos. A acalmia de tráfego, na sua essência, procura atenuar a gravidade deste tipo de problemas, já que aposta na compatibilização das condições de circulação entre os diferentes modos de transporte que coexistem e partilham um determinado espaço “canal”. Este trabalho aborda a aplicação de medidas de acalmia de tráfego em trechos de atravessamento de povoações, voltadas para a minimização dos impactos do tráfego automóvel no atravessamento desses espaços. O trabalho centra-se no desenvolvimento de um conjunto de soluções integradas que funcionem como soluções padrão, que procurem responder a diferentes características de base (ao nível da procura de tráfego, das condicionantes territoriais, características do aglomerado, etc.) e que cubram a generalidade dos problemas tipificáveis em atravessamento de localidades. ABSTRACT: Excessive speed is considered as the main cause of death on the road. Supported by the media, the very urban experience shows that the number of accidents, deaths, and serious and minor injuries remains high, and that Portugal occupies one of the top places in terms of the rate of road accidents. Most accidents occur within localities, with a significant part happening in national and municipal roads that run through them. In fact, the road sections passing through urban spaces, in addition to being roads where it is important to maintain conditions to safeguard the free flow of traffic, are also often the main street of the agglomeration, where local activities are centralized. These types of situations are common to several countries where, in the same stretch of road coexist – not always peacefully – functions related to local mobility and accessibility. On one hand, the urban character that the route acquired, the result of territorial development, requires good accessibility to services and commerce that surround it, while ensuring high levels of safety for all users involved. However, given the levels of traffic demand, which can reach significant values, it is essential to the regional economy to safeguard high flow levels. This accumulation of functions is mainly on the basis of the serious conflicts generated between the usage of the same space by the various users involved. Traffic calming, in essence, tries to mitigate the severity of such problems, since it trusts in the compatibility of the conditions of movement between different modes of transport that coexist and share a given space “channel”. This paper addresses the implementation of traffic calming measures on sections crossing urban spaces, aimed at minimizing the impacts of car traffic on the crossing of those spaces. The work focuses on developing a set of integrated solutions that work as standard solutions and seeking to respond to different baseline characteristics (at the level of traffic demand, territorial constraints, agglomerate characteristics, etc.) and covering the generality of typable problems in crossing localities

    The impact of surgical delay on resectability of colorectal cancer: An international prospective cohort study

    AIM: The SARS-CoV-2 pandemic has provided a unique opportunity to explore the impact of surgical delays on cancer resectability. This study aimed to compare resectability for colorectal cancer patients undergoing delayed versus non-delayed surgery. METHODS: This was an international prospective cohort study of consecutive colorectal cancer patients with a decision for curative surgery (January-April 2020). Surgical delay was defined as an operation taking place more than 4 weeks after treatment decision, in a patient who did not receive neoadjuvant therapy. A subgroup analysis explored the effects of delay in elective patients only. The impact of longer delays was explored in a sensitivity analysis. The primary outcome was complete resection, defined as curative resection with an R0 margin. RESULTS: Overall, 5453 patients from 304 hospitals in 47 countries were included, of whom 6.6% (358/5453) did not receive their planned operation. Of the 4304 operated patients without neoadjuvant therapy, 40.5% (1744/4304) were delayed beyond 4 weeks. Delayed patients were more likely to be older, men, more comorbid, have higher body mass index and have rectal cancer and early stage disease. Delayed patients had higher unadjusted rates of complete resection (93.7% vs. 91.9%, P = 0.032) and lower rates of emergency surgery (4.5% vs. 22.5%, P < 0.001). After adjustment, delay was not associated with a lower rate of complete resection (OR 1.18, 95% CI 0.90-1.55, P = 0.224), which was consistent in elective patients only (OR 0.94, 95% CI 0.69-1.27, P = 0.672). Longer delays were not associated with poorer outcomes. CONCLUSION: One in 15 colorectal cancer patients did not receive their planned operation during the first wave of COVID-19. Surgical delay did not appear to compromise resectability, raising the hypothesis that any reduction in long-term survival attributable to delays is likely to be due to micro-metastatic disease

    Elective cancer surgery in COVID-19-free surgical pathways during the SARS-CoV-2 pandemic: An international, multicenter, comparative cohort study

This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks

    Characterisation of microbial attack on archaeological bone

    As part of an EU funded project to investigate the factors influencing bone preservation in the archaeological record, more than 250 bones from 41 archaeological sites in five countries spanning four climatic regions were studied for diagenetic alteration. Sites were selected to cover a range of environmental conditions and archaeological contexts. Microscopic and physical (mercury intrusion porosimetry) analyses of these bones revealed that the majority (68%) had suffered microbial attack. Furthermore, significant differences were found between animal and human bone in both the state of preservation and the type of microbial attack present. These differences in preservation might result from differences in early taphonomy of the bones. © 2003 Elsevier Science Ltd. All rights reserved

    Preoperative nasopharyngeal swab testing and postoperative pulmonary complications in patients undergoing elective surgery during the SARS-CoV-2 pandemic.

    BACKGROUND: Surgical services are preparing to scale up in areas affected by COVID-19. This study aimed to evaluate the association between preoperative SARS-CoV-2 testing and postoperative pulmonary complications in patients undergoing elective cancer surgery. METHODS: This international cohort study included adult patients undergoing elective surgery for cancer in areas affected by SARS-CoV-2 up to 19 April 2020. Patients suspected of SARS-CoV-2 infection before operation were excluded. The primary outcome measure was postoperative pulmonary complications at 30 days after surgery. Preoperative testing strategies were adjusted for confounding using mixed-effects models. RESULTS: Of 8784 patients (432 hospitals, 53 countries), 2303 patients (26.2 per cent) underwent preoperative testing: 1458 (16.6 per cent) had a swab test, 521 (5.9 per cent) CT only, and 324 (3.7 per cent) swab and CT. Pulmonary complications occurred in 3.9 per cent, whereas SARS-CoV-2 infection was confirmed in 2.6 per cent. After risk adjustment, having at least one negative preoperative nasopharyngeal swab test (adjusted odds ratio 0.68, 95 per cent confidence interval 0.68 to 0.98; P = 0.040) was associated with a lower rate of pulmonary complications. Swab testing was beneficial before major surgery and in areas with a high 14-day SARS-CoV-2 case notification rate, but not before minor surgery or in low-risk areas. To prevent one pulmonary complication, the number needed to swab test before major or minor surgery was 18 and 48 respectively in high-risk areas, and 73 and 387 in low-risk areas. CONCLUSION: Preoperative nasopharyngeal swab testing was beneficial before major surgery and in high SARS-CoV-2 risk areas. There was no proven benefit of swab testing before minor surgery in low-risk areas
