    GABAergic Mechanisms Can Redress the Tilted Balance between Excitation and Inhibition in Damaged Spinal Networks

    Correct operation of neuronal networks depends on the interplay between synaptic excitation and inhibition processes leading to a dynamic state termed balanced network. In the spinal cord, balanced network activity is fundamental for the expression of locomotor patterns necessary for rhythmic activation of limb extensor and flexor muscles. After spinal cord lesion, paralysis ensues often followed by spasticity. These conditions imply that, below the damaged site, the state of balanced networks has been disrupted and that restoration might be attempted by modulating the excitability of sublesional spinal neurons. Because of the widespread expression of inhibitory GABAergic neurons in the spinal cord, their role in the early and late phases of spinal cord injury deserves full attention. Thus, an early surge in extracellular GABA might be involved in the onset of spinal shock while a relative deficit of GABAergic mechanisms may be a contributor to spasticity. We discuss the role of GABA A receptors at synaptic and extrasynaptic level to modulate network excitability and to offer a pharmacological target for symptom control. In particular, it is proposed that activation of GABA A receptors with synthetic GABA agonists may downregulate motoneuron hyperexcitability (due to enhanced persistent ionic currents) and, therefore, diminish spasticity. This approach might constitute a complementary strategy to regulate network excitability after injury so that reconstruction of damaged spinal networks with new materials or cell transplants might proceed more successfully

    Energy Consumption, Carbon Emissions and Global Warming Potential of Wolfberry Production in Jingtai Oasis, Gansu Province, China

    During the last decade, China's agro-food production has increased rapidly and been accompanied by the challenge of increasing greenhouse gas (GHG) emissions and other environmental pollutants from fertilizers, pesticides, and intensive energy use. Understanding the energy use and environmental impacts of crop production will help identify environmentally damaging hotspots of agro-production, allowing environmental impacts to be assessed and crop management strategies optimized. Conventional farming has been widely employed in wolfberry (Lycium barbarum) cultivation in China, which is an important cash tree crop not only for the rural economy but also from an ecological standpoint. Energy use and global warming potential (GWP) were investigated in a wolfberry production system in the Yellow River irrigated Jingtai region of Gansu. In total, 52 household farms were randomly selected to conduct the investigation using questionnaires. Total energy input and output were 321,800.73 and 166,888.80 MJ ha−1, respectively, in the production system. The highest share of energy inputs was found to be electricity consumption for lifting irrigation water, accounting for 68.52%, followed by chemical fertilizer application (11.37%). Energy use efficiency was 0.52 when considering both fruit and pruned wood. Nonrenewable energy use (88.52%) was far larger than the renewable energy input. The share of GWP of different inputs were 64.52% electricity, 27.72% nitrogen (N) fertilizer, 5.07% phosphate, 2.32% diesel, and 0.37% potassium, respectively. The highest share was related to electricity consumption for irrigation, followed by N fertilizer use. Total GWP in the wolfberry planting system was 26,018.64 kg CO2 eq ha−1 and the share of CO2, N2O, and CH4 were 99.47%, 0.48%, and negligible respectively with CO2 being dominant. Pathways for reducing energy use and GHG emission mitigation include: conversion to low carbon farming to establish a sustainable and cleaner production system with options of raising water use efficiency by adopting a seasonal gradient water pricing system and advanced irrigation techniques; reducing synthetic fertilizer use; and policy support: smallholder farmland transfer (concentration) for scale production, credit (small- and low-interest credit) and tax breaks

    Site-specific seeding using multi-sensor and data fusion techniques : a review

    Site-specific seeding (SSS) is a precision agricultural (PA) practice aiming at optimizing seeding rate and depth, depending on the within field variability in soil fertility and yield potential. Unlike other site-specific applications, SSS was not adopted sufficiently by farmers due to some technological and practical challenges that need to be overcome. Success of site-specific application strongly depends on the accuracy of measurement of key parameters in the system, modeling and delineation of management zone maps, accurate recommendations and finally the right choice of variable rate (VR) technologies and their integrations. The current study reviews available principles and technologies for both map-based and senor-based SSS. It covers the background of crop and soil quality indicators (SQI), various soil and crop sensor technologies and recommendation approaches of map-based and sensor-based SSS applications. It also discusses the potential of socio-economic benefits of SSS against uniform seeding. The current review proposes prospective future technology synthesis for implementation of SSS in practice. A multi-sensor data fusion system, integrating proper sensor combinations, is suggested as an essential approach for putting SSS into practice

    Adherence to DASH-style Diet in Relation to Glioma among Iranian Adults: A Hospital-Based Case-Control Study

    Introduction and Aims Data on the association of adherence to DASH-style and glioma are scarce. This hospital-based case-control study examined the association between adherence to DASH-style diet and glioma in a group of Iranian adults. Materials and Methods Cases of glioma (n=128) and their age- and sex-matched healthy controls (n=256) were recruited from hospital. Participants’ dietary intakes were assessed using a validate food frequency questionnaire. Adherence to DASH diet was evaluated. Results After controlling for potential confounders, individuals with the greatest adherence to the DASH diet were less likely to have glioma compared with those with the lowest adherence (Odds ratio for the highest vs. the lowest tertile: 0.26; 95% CI: 0.13-0.54). Additional controlling for BMI did not influence the association significantly (0.28; 0.13-0.57). A diverse association was also seen between consumption of fruits (0.31; 0.14-0.68), legumes and nuts (0.23; 0.10-0.53) and risk of glioma. We found a significant positive association between red and processed meat intake and risk of glioma (2.60; 1.16- 5.81). Individuals in the highest tertile of salt intake were 2.87 times more likely to have glioma than those in the bottom tertile after taking all potential confounders into account (2.87; 1.30-6.34). Conclusion We found that adherence to the DASH style dietary pattern was inversely associated with glioma. In addition, some components of DASH diet, including red meats and salt intake was positively associated with glioma.     Assessment of Utility in Migraine: Mapping the Migraine-Specific Questionnaire to the EQ-5D-5L

    Introduction: There are increasing demands for studies of cost-effectiveness to allocate resources for disease prevention and treatment strategies. The aim of this study is to measure quality of life in migraineurs, based on the Migraine-Specific Questionnaire (MSQ) and EQ-5D-5L, and thereafter map an algorithm to estimate health-state utility values from the MSQ in individuals with migraine. Methods: In this cross-sectional study conducted between May and July 2018 in a tertiary headache clinic in Tehran, Iran, migraineurs diagnosed based on International Classification of Headache Disorders (ICHD)-3β were enrolled and were asked to complete the MSQ questionnaire and EQ-5D questionnaire. The Spearman correlation coefficient (�) was calculated to measure the correlation between the EQ-5D-5L and MSQ v2.1 domains� score. A P value of <.05 was considered statistically significant. After statistical analysis, several regression models were presented to map the results of the MSQ domains to the utility index, and the preferred model was achieved based on goodness of fit and the model's predictive performance. Results: The preferred MSQ algorithm had approximately the same prediction errors in all migraineurs, episodic and chronic migraine (root mean square error 0.24, 0.24, and 0.23, respectively). The preferred MSQ model explained a variance of 0.26 (R2) in episodic and 0.38 in chronic migraine in the EQ-5D-5L questionnaire. Conclusion: The preferred MSQ mapping algorithm will be suitable in estimating health state utilities in trials of patients with migraine that contain MSQ scores but lack utility values. © 202

    Economic burden of medication-overuse headache in Iran: direct and indirect costs

    Background and objective: Medication-overuse headache (MOH) as a secondary chronic headache imposes a considerable burden on both individuals and societies. Nevertheless, little is known about the burden of MOH in Iran. Therefore, in the current study, we aimed to quantify the annual cost of MOH among Iranian patients. Methods: In this cross-sectional study, 84 patients were recruited. Demographic data, headache attack characteristics, related disability, and information about the economic burden of MOH were collected through face-to-face interview. Direct medical and nonmedical costs as well as indirect costs were included in our cost analysis. The prevalence-based approach was applied to estimate the economic burden of MOH. Results: We found that MOH patients in Iran spend averagely 1046 for medical services, 132 for nonmedical services, and 1432 due to lost productivity per year. The per-person annual cost of MOH was US2610, and the total annual cost for Iran was 10,179,000,000, with direct and indirect cost accounting for 45 and 55, respectively. Conclusion: MOH leads to substantial healthcare costs and significant loss of productivity in Iran. Therefore, raising awareness in this area especially for policymakers can use in future health planning and lead to resource allocation in the field of disabling type of headache disorders such as MOH. Our findings also provide a different insight into the burden of MOH, which are likely closer to the actual costs in middle- and low-income countries, and also it could be a sample of such a study in western Asia. © 2020, Fondazione Società Italiana di Neurologia
