    Changing the road transport for a rail transport to access a Brazilian airport

    This paper reports a study aimed to assess whether passengers would change the road transport -- whether individual transport (IT) or passenger bus transport (BT) -- for the rail transport to reach or leave the International Airport Tancredo Neves in Confins, State of Minas Gerais, Brazil. It also sought to identify the passengers’ preference between the two transport modes. The study was built on two techniques: a questionnaire-based survey that approached 296 passengers, and a stated preference survey, which was answered by 223 of them. The results pointed out that most of the respondents were keen on changing the IT and BT for rail transport to reach and leave the airport. The rail transport had a good level of acceptance (84%), which, was, however, contradicted by the stated preference results: 73% of the respondents stated they do prefer the individual transport. One of the possible reasons for the controversial results may be the way the data was collected and arranged in this study. The conclusion of the study was that the rail transport enjoyed good acceptance amongst the respondents when imagined in a static scenario

    Multi-messenger observations of a binary neutron star merger

    On 2017 August 17 a binary neutron star coalescence candidate (later designated GW170817) with merger time 12:41:04 UTC was observed through gravitational waves by the Advanced LIGO and Advanced Virgo detectors. The Fermi Gamma-ray Burst Monitor independently detected a gamma-ray burst (GRB 170817A) with a time delay of ~1.7 s with respect to the merger time. From the gravitational-wave signal, the source was initially localized to a sky region of 31 deg2 at a luminosity distance of 40+8-8 Mpc and with component masses consistent with neutron stars. The component masses were later measured to be in the range 0.86 to 2.26 Mo. An extensive observing campaign was launched across the electromagnetic spectrum leading to the discovery of a bright optical transient (SSS17a, now with the IAU identification of AT 2017gfo) in NGC 4993 (at ~40 Mpc) less than 11 hours after the merger by the One- Meter, Two Hemisphere (1M2H) team using the 1 m Swope Telescope. The optical transient was independently detected by multiple teams within an hour. Subsequent observations targeted the object and its environment. Early ultraviolet observations revealed a blue transient that faded within 48 hours. Optical and infrared observations showed a redward evolution over ~10 days. Following early non-detections, X-ray and radio emission were discovered at the transient’s position ~9 and ~16 days, respectively, after the merger. Both the X-ray and radio emission likely arise from a physical process that is distinct from the one that generates the UV/optical/near-infrared emission. No ultra-high-energy gamma-rays and no neutrino candidates consistent with the source were found in follow-up searches. These observations support the hypothesis that GW170817 was produced by the merger of two neutron stars in NGC4993 followed by a short gamma-ray burst (GRB 170817A) and a kilonova/macronova powered by the radioactive decay of r-process nuclei synthesized in the ejecta

    The effects of a muscle resistance program on the functional capacity, knee extensor muscle strength and plasma levels of IL-6 and TNF-α in pre-frail elderly women: a randomized crossover clinical trial - a study protocol

    <p>Abstract</p> <p>Background</p> <p>With the increase in the elderly population, a growing number of chronic degenerative diseases and a greater dependency on caregivers have been observed. Elderly persons in states of frailty remain more susceptible to significant health complications. There is evidence of an inverse relationship between plasma levels of inflammatory mediators and levels of functionality and muscle strength, suggesting that muscle-strengthening measures can aid in inflammatory conditions. The purpose of this study will be verified the effect of a muscle-strengthening program with load during a ten-week period in pre-frail elderly women with attention to the following outcomes: (1) plasma levels of interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-α), (2) functional capacity and (3) knee extensor muscle strength.</p> <p>Methods/Design</p> <p>The study design is a randomized crossover clinical trial evaluating 26 elderly women (regardless of their race and/or social condition) who are community residents, older than 65, and classified as pre-frail according to the criteria previously described by Fried et al. (2004). All subjects will be assessed using the Timed up and go and 10-Meter Walk Test functional tests. The plasma levels of IL-6 and TNF-α will be assessed by ELISA (<it>enzyme-linked immunosorbent assay</it>) with high sensitivity kits (Quantikine<sup>®</sup>HS, R&D Systems Minneapolis, MN, U.S.). Knee extensor muscle strength will be assessed using the <it>Byodex System 3 Pro</it><sup><it>® </it></sup>isokinetic dynamometer at angular speeds of 60 and 180°/s. The intervention will consist of strengthening exercises of the lower extremities at 50 to 70% of 1RM (maximal resistance) three times per week for ten weeks. The volunteers will be randomized into two groups: group E, the intervention group, and group C, the control group that did not initiate any new activities during the initial study period (ten weeks). After the initial period, group C will begin the intervention and group E will maintain everyday activities without exercising. At the end of the total study period, all volunteers will be reassessed.</p> <p>Discussion</p> <p>To demonstrate and discuss possible influences of load-bearing exercises on the modification of plasma levels of IL-6 and TNF-α and in the functional performance of pre-frail elderly women.</p> <p>Trial Registration</p> <p>ISRCTN62824599</p

    Factors associated with neural alterations and physical disabilities in patients with leprosy in São Luis, State of Maranhão, Brazil

    Introduction Leprosy is a chronic infectious disease that is caused by Mycobacterium leprae. The objective of this study was to evaluate the risk factors that are associated with neural alterations and physical disabilities in leprosy patients at the time of diagnosis. Methods A prospective cross-sectional study was conducted on 155 leprosy patients who participated in a program that aimed to eliminate leprosy from São Luis, State of Maranhão. Results Patients who were 31-45 years of age, were older than 60 years of age or had a partner were more likely to have a disability. Patients with partners were 1.14 times more likely (p = 0.025) to have disabilities of the hands. The frequency of disabilities in the feet among the patients with different clinical forms of leprosy was statistically significant. Conclusions The identification of risk factors that are associated with neural alterations and physical disabilities in leprosy patients is important for diagnosing the disease because this approach enables physicians to plan and prioritize actions for the treatment and monitoring of patients

    Cost-effectiveness of insulin analogs from the perspective of the Brazilian public health system

    ABSTRACT Human insulin is provided by the Brazilian Public Health System (BPHS) for the treatment of diabetes, however, legal proceedings to acquire insulin analogs have burdened the BPHS health system. The aim of this study was to perform a cost-effectiveness analysis to compare insulin analogs and human insulins. This is a pharmacoeconomic study of cost-effectiveness. The direct medical cost related to insulin extracted from the Ministry of Health drug price list was considered. The clinical results, i.e. reduction in glycated hemoglobin (HbA1c), were extracted by meta-analysis. Different scenarios were structured to measure the uncertainties regarding the costs and reduction in HbA1c. Decision tree was developed for sensitivity of Incremental Cost Effectiveness Ratio (ICER). A total of fifteen scenarios were structured. Given the best-case scenario for the insulin analogs, the insulins aspart, lispro, glargine and detemir showed an ICER of R1,768.59;R 1,768.59; R 3,308.54; R11,718.75andR 11,718.75 and R 2,685.22, respectively. In all scenarios in which the minimum effectiveness was proposed, lispro, glargine and detemir were dominant strategies. Sensitivity analysis showed that the aspart had R3,066.98[95 3,066.98 [95 % CI: 2339.22; 4418.53] and detemir had R 6,163.97 [95% CI: 3919.29; 11401.57] for incremental costs. We concluded there was evidence that the insulin aspart is the most cost-effective

    Relação entre autoavaliação vocal e dados da avaliação clínica em indivíduos disfônicos

    OBJETIVO: associar os índices de autoavaliação vocal aos dados da avaliação clínica de indivíduos disfônicos. MÉTODOS: estudo observacional, analítico, retrospectivo. Foram analisados os prontuários de pacientes disfônicos atendidos em uma Clínica-Escola de Fonoaudiologia no período de 2007 a 2011. Foram levantados os dados referentes à autoavaliação vocal (índices de qualidade de vida em voz, desvantagem vocal e atribuição de nota referente ao impacto vocal), à anamnese (sexo, idade, profissão, tipo de queixa, tempo de queixa, tratamentos anteriores para a disfonia), à avaliação perceptivo-auditiva (qualidade vocal, grau de alteração, pitch, loudness, ressonância, articulação e coordenação pneumofonoarticulatória) e aos dados objetivos (tempos máximos fonatórios e relação s/z). Os dados foram tabulados e analisados estatisticamente. RESULTADOS: não houve diferença na comparação dos escores do protocolo de qualidade de vida em voz e índice de desvantagem vocal com as variáveis referentes a sexo, qualidade vocal, grau de alteração, pitch, ressonância, articulação, velocidade de fala e tipo de disfonia. Indivíduos que utilizam a voz profissionalmente e que já fizeram tratamentos anteriores para a disfonia apresentaram piores índices na autoavaliação vocal. Quanto à avaliação clínica, a incoordenação penumofonoarticulatória foi o único parâmetro que interferiu negativamente na autoavaliação. Não houve correlações entre os índices de autoavaliação vocal e as demais variáveis contínuas (idade, tempo de queixa, tempos máximos fonatórios e relação s/z). CONCLUSÃO: a autoavaliação vocal é uma impressão bastante subjetiva, e independe da maior parte dos dados coletados na avaliação clínica. Ser profissional da voz, já ter buscado outros tratamentos para a disfonia e apresentar incoordenação penumofonoarticulatória parece influenciar negativamente na autoavaliação do indivíduo acerca do impacto do distúrbio vocal em sua vida diária
