    Estudo das isotermas de adsorção da polpa de coco verde

    Brazil is considered one of the largest producers and consumers of tropical fruits. Green coconut (Cocos nucifera L.) stands out not only for its production and consumption, but also for the high amount of waste produced by coconut water industry and in natura consumption. Therefore, there is a need for utilization of this by-product. This study aims to study the adsorption isotherms of green coconut pulp and determine its isosteric heat of sorption. The adsorption isotherms at temperatures of 30, 40, 50, 60, and 70 °C were analyzed, and they exhibit type III behavior, typical of sugar rich foods. The experimental results of equilibrium moisture content were correlated by models present in the literature. The Guggenheim, Anderson and De Boer (GAB) model proved particularly good overall agreement with the experimental data. The heat of sorption determined from the adsorption isotherms increased with the decrease in moisture content. The heat of sorption is considered as indicative of intermolecular attractive forces between the sorption sites and water vapor, which is an important factor to predict the shelf life of dried products.O Brasil é considerado um dos maiores produtores e consumidores de frutas tropicais. O coco verde (Cocos nucifera L.) se destaca tanto em termos de produção e consumo quanto em quantidade de resíduos gerada por indústrias de água de coco e pelo consumo in natura. Portanto, existe uma necessidade de aproveitamento deste subproduto. Este trabalho teve por objetivo estudar as isotermas de adsorção da polpa de coco verde e determinação do calor isostérico de sorção. As isotermas de adsorção para as temperaturas de 30, 40, 50, 60 e 70 °C foram analisadas e evidenciaram curvas do tipo III, típicas de alimentos ricos em açúcares. Os dados experimentais de umidade de equilíbrio foram correlacionados por modelos da literatura. O modelo de GAB apresentou melhor concordância com os dados experimentais, entre os modelos avaliados. O calor isostérico de sorção é considerado um indicativo de forças atrativas intermoleculares entre os sítios de sorção de vapor de água, consequentemente, um importante fator para predizer a vida de prateleira de produtos desidratados.The authors wish to thank the Bank of Brazil Foundation (FBB) for financial support (Procs.     Em meados do mês de dezembro de 2019, em Wuhan, na China, foi detectada uma síndrome respiratória nominada Coronavírus (SARS-CoV-2), ou COVID-19, que com grande velocidade foi disseminada por todo o mundo, levando a OMS a declarar a situação pandêmica mundial. Sendo incontestável que imunização da população mundial pela vacinação é a mais eficaz, estratégia para controle e prevenção desta doença mortal, no Brasil foi editada a Lei Federal n.° 14.125/2021, dispondo sobre a aquisição e distribuição de vacinas por pessoas jurídicas de direito privado, que deverá ser alterada pelo Projeto de Lei nº 948/2021 facilitando o acesso da iniciativa privada ao imunizante para vacinação de empregadores e empregados. Até que ponto é um dever/direito constitucional da iniciativa privada, no cumprimento de sua função social adquirir as vacinas para voltar a fomentar sua atividade empresarial, e é claro, a economia do país, e até onde é um dever/direito do Estado de garantir que a vacinação almejada por todos seja monopolizada pela Administração Pública, para garantir a gratuidade, igualdade e universalização da sua distribuição, sem que haja influência do capital, criando castas privilegiadas? O artigo apresentará os argumentos favoráveis e contrários a liberação da compra das vacinas para imunização contra o Coronavírus pela iniciativa privada apresentando, ao final, suas conclusões

    Percutaneous endoscopic versus surgical gastrostomy in patients with benign and malignant diseases: a systematic review and meta-analysis

    To compare the complications and mortality related to gastrostomy procedures performed using surgical and percutaneous endoscopic gastrostomy techniques, this review covered seven studies. Five of these were retrospective and two were randomized prospective studies. In total, 406 patients were involved, 232 of whom had undergone percutaneous endoscopic gastrostomy and 174 of whom had undergone surgical gastrostomy. The analysis was performed using Review Manager. Risk differences were computed using a fixed-effects model and forest and funnel plots. Data on risk differences and 95% confidence intervals were obtained using the Mantel-Haenszel test. There was no difference in major complications in retrospective (95% CI (-0.11 to 0.10)) or randomized (95% CI (-0.07 to 0.05)) studies. Regarding minor complications, no difference was found in retrospective studies (95% CI (-00.17 to 0.09)), whereas a difference was observed in randomized studies (95% CI (-0.25 to -0.02)). Separate analyses of retrospective and randomized studies revealed no differences between the methods in relation to mortality and major complications. Moreover, low levels of minor complications were observed among endoscopic procedures in randomized studies, with no difference observed compared with retrospective studies

    Preliminary development of lapachol gel: in vitro permeation study

    O objetivo do presente estudo foi desenvolver formulações geleificadas do lapachol, substância de conhecida atividade antiinflamatória. Para otimização das formulações, foi verificada a influência da resina de Carbopol® empregada (934P, 940 e 941), do pH da formulação (5, 7 e 8), da concentração do princípio ativo (0,2%, 0,5% e 1,0%) bem como da incorporação de um promotor de absorção (Polissorbato 80) na liberação in vitro do lapachol utilizando células de difusão tipo Franz. Para tanto foram usadas como barreiras membrana sintética Durapore® e pele de rato albino tipo Wistar. Os resultados demonstraram que a adição do Polissorbato 80 na formulação promoveu aumento significativo na permeação do princípio ativo através da pele estudada, exceto para os géis contendo 0,2% de lapachol utilizando Carbopol® 934P como agente geleificante. Estes géis apresentaram as melhores taxas difusionais de lapachol, principalmente quando incorporados em formulações com pH 8, nas concentrações de 0,5% ou 1,0% de princípio ativo. Todavia as preparações contendo 1,0 % de princípio ativo apresentaram pequenos cristais de lapachol não solubilizados.The scope of this work was to develop two gel formulations containing lapachol, substance with known anti-inflammatory activity. In order to achieve a more effective topical delivery, the effects of vehicles and enhancers were evaluated. The influence of the Carbopol® resin (934P, 940 and 941), the pH of the formulation (5, 7 and 8), the concentration of the active (0.2%, 0.5% and 1.0%) and the incorporation of Tween® 80 were investigated. A series in vitro permeation studies were conduct in classical Franz-type diffusion cells using Durapore® synthetic membranes and Wistar mouse skins as barriers. Through the careful analyse of the data obtained, we could observe that the addition of Tween® 80 leads to a significant increase in the in vitro diffusion of the active, except for the formulations containing Carbopol® 934P and 0.2% of lapachol. The formulations assayed presented best in vitro diffusion rates of lapachol at pH 8 when 0.5% or 1.0% of the active was added. However, the preparations containing 1.0% of drug presented little insoluble crystals of lapachol

    Perspectivas atuais sobre o uso de psilocibina no manejo da depressão resistente: revisão sistemática

    A depressão resistente ao tratamento é um desafio global, impactando negativamente a qualidade de vida dos pacientes. Nesse contexto, a psilocibina, um composto psicodélico presente em certos cogumelos, desperta interesse como possível intervenção terapêutica. Seu potencial para influenciar positivamente o humor e a cognição, através da ativação dos receptores de serotonina no cérebro, sugere uma nova abordagem no tratamento da depressão resistente. Este estudo busca analisar as perspectivas atuais sobre o uso da psilocibina nesse contexto, destacando a necessidade de mais pesquisas sobre seus efeitos e segurança para sua integração clínica. Este estudo, baseado em uma revisão sistemática da literatura científica, abrange o período de 2016 a 2024, utilizando as bases de dados PubMed (Medline), Cochrane Library e Scientific Electronic Library Online (SciELO). No primeiro estudo, os efeitos agudos da psilocibina foram detectáveis de 30 a 60 minutos após a administração, atingindo o pico em 2 a 3 horas e diminuindo após pelo menos 6 horas. A substância foi bem tolerada, com eventos adversos leves e transitórios. Houve uma redução significativa nos sintomas depressivos, ansiedade e anedonia após o tratamento com doses altas. O segundo estudo envolveu 233 participantes distribuídos em grupos de doses diferentes. Houve uma redução significativa nos sintomas depressivos após o tratamento, com doses mais altas apresentando uma diferença estatisticamente maior em comparação com a dose mais baixa e o grupo de controle. Eventos adversos, como dor de cabeça e náusea, foram comuns entre os participantes. O terceiro estudo abordou as perspectivas futuras para o tratamento com psilocibina para depressão resistente. Recomendações incluíram equilibrar o tempo dos pacientes e terapeutas, aumentar gradualmente a intensidade das sessões e integrar a terapia sustentada ao tratamento. O envolvimento de pacientes experientes e estudos naturalísticos adicionais foi destacado como importante para abordagens mais personalizadas. Em resumo, a psilocibina mostra potencial como tratamento para a depressão resistente, com redução significativa dos sintomas depressivos e boa tolerabilidade. No entanto, são necessárias mais pesquisas para confirmar sua eficácia e segurança, destacando a importância de estudos adicionais e ensaios clínicos controlados

    Brazilian consortium for the study on renal diseases associated with COVID-19 : a multicentric effort to understand SARS-CoV-2-related nephropathy

    Kidney involvement appears to be frequent in coronavirus disease 2019 (COVID-19). Despite this, information concerning renal involvement in COVID-19 is still scarce. Several mechanisms appear to be involved in the complex relationship between the virus and the kidney. Also, different morphological patterns have been described in the kidneys of patients with COVID-19. For some authors, however, this association may be just a coincidence. To investigate this issue, we propose assessing renal morphology associated with COVID-19 at the renal pathology reference center of federal university hospitals in Brazil. Data will come from a consortium involving 17 federal university hospitals belonging to Empresa Brasileira de Serviços Hospitalares (EBSERH) network, as well as some state hospitals and an autopsy center. All biopsies will be sent to the referral center for renal pathology of the EBSERH network. The data will include patients who had coronavirus disease, both alive and deceased, with or without pre-existing kidney disease. Kidney biopsies will be analyzed by light, fluorescence, and electron microscopy. Furthermore, immunohistochemical (IHC) staining for various inflammatory cells (i.e., cells expressing CD3, CD20, CD4, CD8, CD138, CD68, and CD57) as well as angiotensin-converting enzyme 2 (ACE2) will be performed on paraffinized tissue sections. In addition to ultrastructural assays, in situ hybridization (ISH), IHC and reverse transcription-polymerase chain reaction (RT-PCR) will be used to detect Severe Acute Respiratory Syndrome Coronavirus (SARS-CoV-2) in renal tissue. For the patients diagnosed with Collapsing Glomerulopathy, peripheral blood will be collected for apolipoprotein L-1 (APOL1) genotyping. For patients with thrombotic microangiopathy, thrombospondin type 1 motif, member 13 (ADAMTS13), antiphospholipid, and complement panel will be performed. The setting of this study is Brazil, which is second behind the United States in highest confirmed cases and deaths. With this complete approach, we hope to help define the spectrum and impact, whether immediate or long-term, of kidney injury caused by SARS-CoV-2

    Contributions of mean and shape of blood pressure distribution to worldwide trends and variations in raised blood pressure: A pooled analysis of 1018 population-based measurement studies with 88.6 million participants

Background: Change in the prevalence of raised blood pressure could be due to both shifts in the entire distribution of blood pressure (representing the combined effects of public health interventions and secular trends) and changes in its high-blood-pressure tail (representing successful clinical interventions to control blood pressure in the hypertensive population). Our aim was to quantify the contributions of these two phenomena to the worldwide trends in the prevalence of raised blood pressure. Methods: We pooled 1018 population-based studies with blood pressure measurements on 88.6 million participants from 1985 to 2016. We first calculated mean systolic blood pressure (SBP), mean diastolic blood pressure (DBP) and prevalence of raised blood pressure by sex and 10-year age group from 20-29 years to 70-79 years in each study, taking into account complex survey design and survey sample weights, where relevant. We used a linear mixed effect model to quantify the association between (probittransformed) prevalence of raised blood pressure and age-group- and sex-specific mean blood pressure. We calculated the contributions of change in mean SBP and DBP, and of change in the prevalence-mean association, to the change in prevalence of raised blood pressure. Results: In 2005-16, at the same level of population mean SBP and DBP, men and women in South Asia and in Central Asia, the Middle East and North Africa would have the highest prevalence of raised blood pressure, and men and women in the highincome Asia Pacific and high-income Western regions would have the lowest. In most region-sex-age groups where the prevalence of raised blood pressure declined, one half or more of the decline was due to the decline in mean blood pressure. Where prevalence of raised blood pressure has increased, the change was entirely driven by increasing mean blood pressure, offset partly by the change in the prevalence-mean association. Conclusions: Change in mean blood pressure is the main driver of the worldwide change in the prevalence of raised blood pressure, but change in the high-blood-pressure tail of the distribution has also contributed to the change in prevalence, especially in older age groups

    Worldwide trends in hypertension prevalence and progress in treatment and control from 1990 to 2019: a pooled analysis of 1201 population-representative studies with 104 million participants

    Background Hypertension can be detected at the primary health-care level and low-cost treatments can effectively control hypertension. We aimed to measure the prevalence of hypertension and progress in its detection, treatment, and control from 1990 to 2019 for 200 countries and territories. Methods We used data from 1990 to 2019 on people aged 30–79 years from population-representative studies with measurement of blood pressure and data on blood pressure treatment. We defined hypertension as having systolic blood pressure 140 mm Hg or greater, diastolic blood pressure 90 mm Hg or greater, or taking medication for hypertension. We applied a Bayesian hierarchical model to estimate the prevalence of hypertension and the proportion of people with hypertension who had a previous diagnosis (detection), who were taking medication for hypertension (treatment), and whose hypertension was controlled to below 140/90 mm Hg (control). The model allowed for trends over time to be non-linear and to vary by age. Findings The number of people aged 30–79 years with hypertension doubled from 1990 to 2019, from 331 (95% credible interval 306–359) million women and 317 (292–344) million men in 1990 to 626 (584–668) million women and 652 (604–698) million men in 2019, despite stable global age-standardised prevalence. In 2019, age-standardised hypertension prevalence was lowest in Canada and Peru for both men and women; in Taiwan, South Korea, Japan, and some countries in western Europe including Switzerland, Spain, and the UK for women; and in several low-income and middle-income countries such as Eritrea, Bangladesh, Ethiopia, and Solomon Islands for men. Hypertension prevalence surpassed 50% for women in two countries and men in nine countries, in central and eastern Europe, central Asia, Oceania, and Latin America. Globally, 59% (55–62) of women and 49% (46–52) of men with hypertension reported a previous diagnosis of hypertension in 2019, and 47% (43–51) of women and 38% (35–41) of men were treated. Control rates among people with hypertension in 2019 were 23% (20–27) for women and 18% (16–21) for men. In 2019, treatment and control rates were highest in South Korea, Canada, and Iceland (treatment >70%; control >50%), followed by the USA, Costa Rica, Germany, Portugal, and Taiwan. Treatment rates were less than 25% for women and less than 20% for men in Nepal, Indonesia, and some countries in sub-Saharan Africa and Oceania. Control rates were below 10% for women and men in these countries and for men in some countries in north Africa, central and south Asia, and eastern Europe. Treatment and control rates have improved in most countries since 1990, but we found little change in most countries in sub-Saharan Africa and Oceania. Improvements were largest in high-income countries, central Europe, and some upper-middle-income and recently high-income countries including Costa Rica, Taiwan, Kazakhstan, South Africa, Brazil, Chile, Turkey, and Iran. Interpretation Improvements in the detection, treatment, and control of hypertension have varied substantially across countries, with some middle-income countries now outperforming most high-income nations. The dual approach of reducing hypertension prevalence through primary prevention and enhancing its treatment and control is achievable not only in high-income countries but also in low-income and middle-income settings
