Dilatation of Virchow-Robin spaces in children hospitalized at pediatric neurology department
Background and purpose: Dilated Virchow-Robin spaces (dVRs) have been revealed by magnetic resonance imaging (MRI) in patients with various neurological disorders. However,
their etiology and clinical importance have not been discovered yet. The aim of the study
was to estimate dVRs occurrence in hospitalized children and determine dVRs localization
and their association with different nervous system diseases.
Material and methods: Contrast-enhanced brain MRI examinations with the use of 1.5 T GE
device were performed in children with different diseases of nervous system, who were
hospitalized at Pediatric Neurology Department, Chair of Children and Adolescent Neurology, Jagiellonian University in the years 2010–2011. The mean age of examined children was
11.58 years, and the studied group included 27 boys and 26 girls.
Results: Within two years, MRI examinations of the brain were performed in 1348 children
and dVRs were found in 53 of them (3.93%). Among children with dVRs, 15 were diagnosed
with headache (28.3%) and 18 with epilepsy (33.96%). Other diagnoses were less frequent and
occurred in 37.7%. Generalized dVRs and those localized in the subcortical nuclei were most
frequently found.
Conclusions: Higher incidence of dVRs was found in children with headache and epilepsy. No
association was found between localization of dVRs and symptomatology of different
nervous system diseases except for large dVRs probably due to the pressure on the
surrounding tissues

Intragenomic diversity of Rhizobium leguminosarum bv. trifolii clover nodule isolates
BACKGROUND: Soil bacteria from the genus Rhizobium are characterized by a complex genomic architecture comprising chromosome and large plasmids. Genes responsible for symbiotic interactions with legumes are usually located on one of the plasmids, named the symbiotic plasmid (pSym). The plasmids have a great impact not only on the metabolic potential of rhizobia but also underlie genome rearrangements and plasticity. RESULTS: Here, we analyzed the distribution and sequence variability of markers located on chromosomes and extrachromosomal replicons of Rhizobium leguminosarum bv. trifolii strains originating from nodules of clover grown in the same site in cultivated soil. First, on the basis of sequence similarity of repA and repC replication genes to the respective counterparts of chromids reported in R. leguminosarum bv. viciae 3841 and R. etli CFN42, chromid-like replicons were distinguished from the pool of plasmids of the nodule isolates studied. Next, variability of the gene content was analyzed in the different genome compartments, i.e., the chromosome, chromid-like and 'other plasmids'. The stable and unstable chromosomal and plasmid genes were detected on the basis of hybridization data. Displacement of a few unstable genes between the chromosome, chromid-like and 'other plasmids', as well as loss of some markers was observed in the sampled strains. Analyses of chosen gene sequences allowed estimation of the degree of their adaptation to the three genome compartments as well as to the host. CONCLUSIONS: Our results showed that differences in distribution and sequence divergence of plasmid and chromosomal genes can be detected even within a small group of clover nodule isolates recovered from clovers grown at the same site. Substantial divergence of genome organization could be detected especially taking into account the content of extrachromosomal DNA. Despite the high variability concerning the number and size of plasmids among the studied strains, conservation of the location as well as dynamic distribution of the individual genes (especially replication genes) of a particular genome compartment were demonstrated. The sequence divergence of particular genes may be affected by their location in the given genome compartment. The 'other plasmid' genes are less adapted to the host genome than the chromosome and chromid-like genes
Ocena znaczenia klinicznego obrazowania MRI i MRA w padaczce częściowej u dzieci
Background: Partial epilepsy is a very important problem of epileptology in childhood including clinical and therapeutic aspect especially surgery treatment. The aim of this study is to assess clinical value of neuroimagine techniques (structural MRI, MRI angiography) in partial epilepsy diagnostics in children. The relation between results of examinations with these methods and congenital and acquired risk factors related to partial epilepsy, age of its onset and clinical assessment of patients was analyzed. Material/Methods: The study group consisted of 140 children with partial epilepsy hospitalized between 1998 and 2004 in Department of Pediatric Neurology, Collegium Medicum Jagiellonian University, Kraków. The group included 70 girls and 70 boys, the age ranged from 2 months to 17 years. In study grup statistical analysis included different factors as which can be related with results of neuroimaging as age, load of pregnancy and birth period, familiar epilepsy, patient's risk factors for apperance of epilepsy, acquired risk factors of epilepsy, results of neurological examination, type of epilepsy, status epilepticus, and signs according epileptic attaks which can be related with neuroimaging results. The primary method of neuroimagine in all patients was structural MRI, in 16 cases Magnetic Resonance Angiography (MRA). The parametric tests (t-student), nonparametric Mann-Whitney's test were used in statistical analysis. The bilateral Fisher test was used to check rate in groups. There was assessed sensitivity, specificity, positive predictive value, negative predictive value; the 95% confidence interval was calculated for these parameters. Results: Abnormalities in neurological examination in children with partial epilepsy were strongly correlated with MRI findings. The structural changes in MRI were found in younger children, whose course of epilepsy was longer than children without MRI changes. Changes in hippocampus were the most common in children with partial epilepsy with abnormalities in MRI. Children with migration disorder had big variety of clinical symptoms. Conclusions: Neuroimaging by MRI is particularly important in younger children, with longer course of epilepsy and abnormal neurological examination and for detecting of migration and hippocampal changes
Wpływ spontanicznego wzrostu ciśnienia tętniczego na dyspersję odstępu QT u chorych z nadciśnieniem tętniczym i po zawale serca
Wstęp: Zwiększenie dyspersji odstępu QT u chorych z nadciśnieniem
tętniczym i po zawale serca wiąże się z występowaniem komorowych zaburzeń rytmu
serca - uznanego czynnika ryzyka nagłego zgonu sercowego. Nadciśnienie tętnicze
zwiększa ryzyko incydentów sercowo- -naczyniowych, co może być spowodowane utrzymywaniem
się podwyższonych wartości ciśnienia tętniczego. Niewyjaśniony pozostaje wpływ
spontanicznych zwyżek ciśnienia tętniczego na występowanie nagłego zgonu sercowego.
Celem badania chorych z nadciśnieniem tętniczym z przebytym zawałem serca lub
bez niego była ocena dyspersji odstępu QT podczas spontanicznych zwyżek ciśnienia
skurczowego i rozkurczowego w dzień i w nocy oraz wpływu częstości rytmu serca,
niedokrwienia, zmian minimalnej i maksymalnej długości odstępu QT na dyspersję
odstępu QT podczas wzrostów ciśnienia w dzień i w nocy.
Materiał i metody: Badaniem objęto 80 chorych: 34 osoby z nadciśnieniem
tętniczym i 46 pacjentów z nadciśnieniem tętniczym i przebytym zawałem serca.
Wykonano jednocześnie 24-godzinną rejestrację EKG i 24-godzinny automatyczny pomiar
ciśnienia tętniczego. Badanie holterowskie EKG przeprowadzono za pomocą rejestratora
12-kanałowego. Podczas jednoczesnej 24-godzinnej kontroli ciśnienia tętniczego
wyznaczano epizody zwyżki ciśnienia skurczowego powyżej 30 mm Hg i rozkurczowego
powyżej 20 mm Hg względem odpowiedniej wartości przed wzrostem. Pomiary odstępu
QT wykonywano przynajmniej w 8 odprowadzeniach elektrokardiogramu, oddzielnie
podczas zwyżek ciśnienia skurczowego i rozkurczowego, w dzień i w nocy. Dla każdej
zwyżki ciśnienia tętniczego 3-krotnie obliczano dyspersję odstępu QT; 10 minut
przed zwyżką, w trakcie zwyżki i 10 minut po zwyżce ciśnienia tętniczego. Za pomocą
badania echokardiograficznego oceniano odcinkowe zaburzenia kurczliwości, frakcję
wyrzutową i masę lewej komory serca.
Wyniki: Spoczynkowa dyspersja QT jest istotnie większa u chorych
z nadciśnieniem tętniczym i przebytym zawałem serca niż u osób z nadciśnieniem
tętniczym bez objawów choroby wieńcowej. Spontaniczne zwyżki ciśnienia tętniczego
istotnie zmniejszają dyspersję odstępu QT u chorych z nadciśnieniem tętniczym,
umiarkowanym przerostem lewej komory serca i bez objawów choroby wieńcowej oraz
powodują nieistotne zmniejszenie dyspersji odstępu QT u pacjentów z nadciśnieniem
tętniczym, umiarkowanym przerostem lewej komory i przebytym zawałem serca. Nie
stwierdzono istotnego wpływu częstości rytmu serca na dyspersję odstępu QT podczas
zwyżek ciśnienia tętniczego u chorych w badanych grupach. W grupie osób z nadciśnieniem
tętniczym zmniejszenie dyspersji odstępu QT podczas zwyżki ciśnienia było spowodowane
głównie skróceniem maksymalnego odstępu QT. Dalsze zmniejszenie dyspersji odstępu
QT po zwyżce ciśnienia tętniczego wynikało zwłaszcza z wydłużenia minimalnego
odstępu QT. Natomiast u chorych z nadciśnieniem tętniczym i po zawale serca zależności
te były mniej widoczne, chociaż zachowały się zbliżone tendencje. U osób po zawale
serca, u których występowało niedokrwienie mięśnia sercowego, podczas wzrostu
ciśnienia tętniczego, spoczynkowa dyspersja odstępu QT podczas zwyżki i po zwyżce
ciśnienia były nieistotnie większe w porównaniu z chorymi bez niedokrwienia.
Wnioski: Dyspersja odstępu QT zmniejsza się istotnie podczas
zwyżek ciśnienia tętniczego skurczowego i rozkurczowego, zarówno w dzień, jak
i w nocy, u chorych z nadciśnieniem tętniczym, a nieistotnie u pacjentów po zawale

The impact of NKG2A and NKG2D receptors and HLA-E and MICA ligands polymorphisms on post-transplant complications after paediatric allogeneic HSCT: a single-centre experience
Introduction: Natural Killer cells are the first subpopulation of lymphocytes that reconstitute after allogeneic haematopoietic stem cell transplantation (HSCT). Their activity is regulated by various receptor-ligand interactions, including stimulation of the activating NKG2D receptor by the MICA molecule, and inhibitory NKG2A receptor interacting with the HLA-E. In this study the research effort focused on the effect of selected NKG2A and NKG2D receptors and their ligands (HLA-E and MICA molecules) polymorphisms that may affect the pathomechanisms of post-transplant complications after HSCT in children.Methods: One hundred donor-recipient pairs from a single paediatric transplantation centre were investigated. Altogether six single nucleotide substitutions (NKG2A rs7301582; NKG2D rs1049174, rs1154831; HLA-E rs1264457; MICA rs1051792, rs1063635) were genotyped, and the influence of polymorphisms was analysed on acute and chronic graft-versus-host disease (GvHD), cytomegalovirus (CMV) infection incidence, disease relapse and survival.Results: The distribution of the evaluated polymorphisms did not differ between patients and their donors. The results showed a significant influence of HLA-E rs1264457 polymorphism in patients’ HLA-E*01:01 allele, which was associated with increased risk of CMV infection (p = 0.050), especially in children positive for CMV IgG before transplantation (p = 0.001). Furthermore, the effect of HLA-E*01:01 allele on CMV infections was more evident in children above the age of 7 years (p = 0.031). Strong tendencies (0.05 < p < 0.10) towards association with the risk of acute GvHD were also observed for the NKG2A or MICA polymorphisms of the recipients. In addition, NKG2D rs1154831 AA and MICA rs1063635 GG might play a protective role as they were not present in any recipient who died after transplantation.Conclusion: In summary, there is emerging evidence that genotyping results of NKG2 receptors and their ligands, may have prognostic value for the outcome of paediatric allogeneic HSCT, but more extensive studies performed on larger groups of donors and transplant recipients are required to confirm these observations
