Interspinous process device versus standard conventional surgical decompression for lumbar spinal stenosis: Randomized controlled trial
Abstract
Objective To assess whether interspinous process device implantation
is more effective in the short term than conventional surgical
decompression for patients with intermittent neurogenic claudication
due to lumbar spinal stenosis.
Design Randomized controlled trial.
Setting Five neurosurgical centers (including one academic and four
secondary level care centers) in the Netherlands.
Participants 203 participants were referred to the Leiden-The Hague
Spine Prognostic Study Group between October 2008 and September
2011; 159 participants with intermittent neurogenic claudication due to
lumbar spinal stenosis at one or two levels with an indication for surgery
were randomized.
Interventions 80 participants received an interspinous process device
and 79 participants underwent spinal bony decompression.
Main outcome measures The primary outcome at short term (eight
weeks) and long term (one year) follow-up was the Zurich Claudication
Questionnaire score. Repeated measurements were made to compare
outcomes over time.
Results At eight weeks, the success rate according to the Zurich
Claudication Questionnaire for the interspinous process device group
(63%, 95% confidence interval 51% to 73%) was not superior to that for
Azimuthal Correlations in the Target Fragmentation Region of High Energy Nuclear Collisions
Results on the target mass dependence of proton and pion pseudorapidity
distributions and of their azimuthal correlations in the target rapidity range
are presented. The data have been taken with the
Plastic-Ball detector set-up for 4.9 GeV p + Au collisions at the Berkeley
BEVALAC and for 200 GeV/ p-, O-, and S-induced reactions on
different nuclei at the CERN-SPS. The yield of protons at backward rapidities
is found to be proportional to the target mass. Although protons show a typical
``back-to-back'' correlations, a ``side-by-side'' correlation is observed for
positive pions, which increases both with target mass and with impact parameter
of a collision. The data can consistently be described by assuming strong
rescattering phenomena including pion absorption effects in the entire excited

The International Cancer Expert Corps: A Unique Approach for Sustainable Cancer Care in Low and Lower-Middle Income Countries
The growing burden of non-communicable diseases including cancer in low- and lower-middle income countries (LMICs) and in geographic-access limited settings within resource-rich countries requires effective and sustainable solutions. The International Cancer Expert Corps (ICEC) is pioneering a novel global mentorshipâpartnership model to address workforce capability and capacity within cancer disparities regions built on the requirement for local investment in personnel and infrastructure. Radiation oncology will be a key component given its efficacy for cure even for the advanced stages of disease often encountered and for palliation. The goal for an ICEC Center within these health disparities settings is to develop and retain a high-quality sustainable workforce who can provide the best possible cancer care, conduct research, and become a regional center of excellence. The ICEC Center can also serve as a focal point for economic, social, and healthcare system improvement. ICEC is establishing teams of Experts with expertise to mentor in the broad range of subjects required to establish and sustain cancer care programs. The Hubs are cancer centers or other groups and professional societies in resource-rich settings that will comprise the global infrastructure coordinated by ICEC Central. A transformational tenet of ICEC is that altruistic, human-service activity should be an integral part of a healthcare career. To achieve a critical mass of mentors ICEC is working with three groups: academia, private practice, and senior mentors/retirees. While in-kind support will be important, ICEC seeks support for the career time dedicated to this activity through grants, government support, industry, and philanthropy. Providing care for people with cancer in LMICs has been a recalcitrant problem. The alarming increase in the global burden of cancer in LMICs underscores the urgency and makes this an opportune time fornovel and sustainable solutions to transform cancer care globally
Ownership and control in a competitive industry
We study a differentiated product market in which an investor initially owns a controlling stake in one of two competing firms and may acquire a non-controlling or a controlling stake in a competitor, either directly using her own assets, or indirectly via the controlled firm. While industry profits are maximized within a symmetric two product monopoly, the investor attains this only in exceptional cases. Instead, she sometimes acquires a noncontrolling stake. Or she invests asymmetrically rather than pursuing a full takeover if she acquires a controlling one. Measurement of the cross section for isolated-photon plus jet production in pp collisions at âs=13 TeV using the ATLAS detector
The dynamics of isolated-photon production in association with a jet in protonâproton collisions at a centre-of-mass energy of 13 TeV are studied with the ATLAS detector at the LHC using a dataset with an integrated luminosity of 3.2 fbâ1. Photons are required to have transverse energies above 125 GeV. Jets are identified using the anti- algorithm with radius parameter and required to have transverse momenta above 100 GeV. Measurements of isolated-photon plus jet cross sections are presented as functions of the leading-photon transverse energy, the leading-jet transverse momentum, the azimuthal angular separation between the photon and the jet, the photonâjet invariant mass and the scattering angle in the photonâjet centre-of-mass system. Tree-level plus parton-shower predictions from Sherpa and Pythia as well as next-to-leading-order QCD predictions from Jetphox and Sherpa are compared to the measurements
