    Pnömonili sığırlarda miyeloid hücrelerde eksprese edilen mid-regional pro-adrenomedullin ve soluble tetikleyici reseptörün serum düzeyleri

    This study aimed to determine the serum levels of two inflammatory biomarkers, named, mid-regional pro-adrenomedullin (MR-Pro ADM) and soluble triggering receptor expressed on myeloid cells (sTREM-1), in cattle diagnosed with pneumonia. For this purpose, 40 patient female cattle, which were aged 2-7 years and displayed coughing, dyspnea, nasal discharge, anorexia and abdominal respiration, and 15 healthy female cattle within the same age range, were evaluated. The diseased cattle underwent clinical and radiological examinations and were sampled for blood prior to receiving treatment. The healthy subjects also underwent clinical examination and were sampled for blood once. Blood samples were used for biochemical and hematological measurements. While the diseased group had higher serum levels of MR-Pro ADM (86.38±6.33), compared to the healthy control group (61.81±4.96); the pneumonic cattle had lower levels of sTREM-1 (75.93±1.86), in comparison to the healthy group (96.55±9.13). In conclusion: MR-Pro ADM and sTREM-1 levels are very important diagnostically in cattle with pneumonia.Bu çalışma, pnömoni tanısı konulan sığırlarda mid-regional pro-adrenomedullin (MR-Pro ADM) ve miyeloid hücrelerde eksprese soluble tetikleyici reseptör olmak üzere iki enflamatuvar biyobelirteç ile bazı biyokimyasal ve hematolojik parametrelerin serum düzeylerinin belirlenmesini amaçladı. Bu amaçla 2-7 yaş arası öksürük, nefes darlığı, burun akıntısı, iştahsızlık ve abdominal solunumu gösteren 40 hasta dişi sığır ve aynı yaş aralığında 15 sağlıklı dişi sığır değerlendirildi. Hasta sığırlar tedavi edilmeden önce klinik ve radyolojik muayenelere tabi tutuldu ve kan örnekleri alındı. Sağlıklı sığırlar ayrıca klinik muayeneye tabi tutuldu ve bir kez kan örnekleri alındı. Kan örnekleri biyokimyasal ve hematolojik ölçümler için kullanıldı. Hasta grupta MR-Pro ADM serum düzeyleri (86,38±6,33) kontrol grubuna (61,81±4,96) göre daha yüksek bulundu. Hasta sığırların sTREM-1 seviyeleri (75,93±1,86) sağlıklı gruba kıyasla (96,55±9,13) karşılaştırıldığında istatistiksel olarak anlamlı düşük bulundu. Sonuç olarak: MR-Pro ADM ve sTREM-1 seviyeleri pnömonili sığırlarda diagnostik açıdan oldukça önemlidir

    Clinical management and treatment of polydactyly

    Amaç: Polidaktili sık görülen doğumsal el ve ayak anomalilerinden biri olup, ilk defa 1670 yılında elde rapor edilmiştir. Fazla olan parmak genellikle sadece yumuşak doku içerikli olmakla beraber nadir olarak kemik yapı da içermektedir. Polidaktili sindaktiliye oranla üç kat daha fazla görülmektedir. Bu çalışmada polidaktiliye yaklaşım ve tedavi sonuçlarımız değerlendirilmiştir. Gereç ve Yöntemler: Son 5 yılda 27 hastada elde, 20 hastada ayakta ve 3 hastada hem elde hem ayakta (24 erkek, 26 kadın) toplam 50 hasta polidaktili tanısıyla değerlendirildi. Bulgular: Hastaların ortalama yaşı altı (1-14) idi. Tüm hastalar genel anestezi altında opere edildi. Lezyonlara uygulanan lokal anestezi sonrası total derin eksizyon uygulandı. Dikişler 10-12. günlerde alındı. Opere edilen hastalarda uzun dönemde fonksiyonel sorun görülmedi. Sonuç: Polidaktili tedavisinde temel hedef fazla olan dokuların çıkarılması ve etkilenen dokuların anatomik olarak uygun şekilde onarılmasıdır. Polidaktili sonrası hastaların yaklaşık %25'inde farklı nedenlerle yeniden operasyon yapılmaktadır. Bu çalışmada el ve/ veya ayakta polidaktili nedeniyle opere edilen hastalar sonuçlarıyla birlikte değerlendirilmiştir. Sonuç olarak eksizyon ve uygun kapama basit olgular için yeterli iken, kompleks olgularda tendon, kemik ve ligamentlerin uygun şekilde repozisyonu temel amaç olmalıdır

    Naiv hepatit C enfeksiyonlu hastaların on-altı yıllık prognozu

    Objectives: In this study, we aimed to evaluate the clinical course of treatment-naive patients infected with hepatitis C virus (HCV) who were followed up in various centers in Turkey. Materials and Methods: This was a retrospective study performed with the participation of 15 centers. Patients aged 18 years and older with HCV infection were included. Results: A total of 391 treatment-naive patients infected with HCV were included in this study. During the follow-up period, the final values of alanine aminotransferase, aspartate transaminase, and total protein were significantly decreased when compared to the initial values (p<0.001, p<0.001, and p=0.005, respectively). In the study group, 19.2% of the patients underwent liver biopsy and 4.1% underwent transient elastography (FibroScan). An increased histological activity index (HAI) score and fibrosis in the second biopsy were observed in one patient, only increased HAI in two patients and increased fibrosis in one patient, as shown on the FibroScan. In the 16 years of the study period, cirrhosis was radiologically detected in only one patient. Conclusion: Even if rapid progression is not observed, close monitoring of the clinical findings related to liver failure and fibrosis with invasive or non-invasive methods may be useful.Amaç: Bu çalışmada ülkemizin çeşitli merkezlerinde takip edilen naiv hepatit C virüs (HCV) ile enfekte hastaların klinik seyrini değerlendirmeyi amaçladık. Gereç ve Yöntemler: Bu çalışma retrospektif olarak 15 merkezin katılımıyla gerçekleştirilmiştir. Çalışmaya 18 yaş üstü, HCV enfeksiyonu olan hastalar dahil edilmiştir. Bulgular: Çalışmada 391 tedavi-naiv HCV enfeksiyonlu hasta yer almıştır. Hastaların takip süresinde son alanine aminotransferase, aspartate transaminase ve total protein değerleri ilk düzeyine göre önemli düzeyde azalmıştır (sırasıyla p<0,001, p<0,001, p=0,005). Çalışma grubunda hastaların %19,2’sine karaciğer biyopsisi, %4,1’ine elastografi (FibroScan) uygulanmıştır. Takip esnasında bir hastada ikinci biyopside histolojik aktivite indeksi (HAI) ve fibroziste artma, iki hastada sadece HAI’da artma, birinde FibroScan ile fibrozis değerinde artma olduğu gözlenmiştir. Bir hastada 16 yıl içinde radyolojik olarak siroz saptanmıştır. Sonuç: Hızlı progresyon gözlenmemekle birlikte hastaların izleminde karaciğer yetmezliği ile ilgili klinik bulguların ve invaziv veya noninvaziv yöntemlerle fibrozisin yakın takibi yararlı olabilir

    The frequency of Duchenne muscular dystrophy/Becker muscular dystrophy and Pompe disease in children with isolated transaminase elevation: results from the observational VICTORIA study

    IntroductionElevated transaminases and/or creatine phosphokinase can indicate underlying muscle disease. Therefore, this study aims to determine the frequency of Duchenne muscular dystrophy/Becker muscular dystrophy (DMD/BMD) in male children and Pompe disease (PD) in male and female children with isolated hypertransaminasemia.MethodsThis multi-center, prospective study enrolled patients aged 3–216 months with serum alanine transaminase (ALT) and/or aspartate transaminase (AST) levels &gt;2× the upper limit of normal (ULN) for ≥3 months. Patients with a known history of liver or muscle disease or physical examination findings suggestive of liver disease were excluded. Patients were screened for creatinine phosphokinase (CPK) levels, and molecular genetic tests for DMD/BMD in male patients and enzyme analysis for PD in male and female patients with elevated CPK levels were performed. Genetic analyses confirmed PD. Demographic, clinical, and laboratory characteristics of the patients were analyzed.ResultsOverall, 589 patients [66.8% male, mean age of 63.4 months (standard deviation: 60.5)] were included. In total, 251 patients (188 male and 63 female) had CPK levels above the ULN. Of the patients assessed, 47% (85/182) of male patients were diagnosed with DMD/BMD and 1% (3/228) of male and female patients were diagnosed with PD. The median ALT, AST, and CPK levels were statistically significantly higher, and the questioned neurological symptoms and previously unnoticed examination findings were more common in DMD/BMD patients than those without DMD/BMD or PD (p &lt; 0.001).DiscussionQuestioning neurological symptoms, conducting a complete physical examination, and testing for CPK levels in patients with isolated hypertransaminasemia will prevent costly and time-consuming investigations for liver diseases and will lead to the diagnosis of occult neuromuscular diseases.     A multi-country test of brief reappraisal interventions on emotions during the COVID-19 pandemic.

    The COVID-19 pandemic has increased negative emotions and decreased positive emotions globally. Left unchecked, these emotional changes might have a wide array of adverse impacts. To reduce negative emotions and increase positive emotions, we tested the effectiveness of reappraisal, an emotion-regulation strategy that modifies how one thinks about a situation. Participants from 87 countries and regions (n = 21,644) were randomly assigned to one of two brief reappraisal interventions (reconstrual or repurposing) or one of two control conditions (active or passive). Results revealed that both reappraisal interventions (vesus both control conditions) consistently reduced negative emotions and increased positive emotions across different measures. Reconstrual and repurposing interventions had similar effects. Importantly, planned exploratory analyses indicated that reappraisal interventions did not reduce intentions to practice preventive health behaviours. The findings demonstrate the viability of creating scalable, low-cost interventions for use around the world

    Basal Cell Carcinoma in Young Patients

    Basal cell carcinoma (BCC) is the most common type of skin cancer and has a direct relationship with chronic sun exposure. Other risk factors include fair skin and eyes, freckling, family history, genetic disorders, immunosuppression, ionizing radiation, arsenic, and polyaromatic hydrocarbons. BCC usually progresses slowly. Lesions are usually seen over sun-exposed areas, which are most commonly on the face and neck. Local invasion may occur, but it rarely metastasizes. The incidence of BCC peaks in the seventh decade of life. BCC is an uncommon lesion during childhood, youth, and pregnancy. It has rarely been reported during childhood. BCC seen during childhood can be inherited with diseases such as xeroderma pigmentosum, albinism, Bazex syndrome, and basal cell nevus syndrome or after high-dose radiotherapy. In this study, we present the cases of a 14-year-old girl with BCC on the right popliteal region and a 23-year-old pregnant woman with BCC on the nasal tip.     Clinical Experiences in the Surgical Treatment of Accessory Tragus

    Objective: Tragus is a part of the external ear that develops from the first branchial arch. Accessory ear is a congenital external ear anomaly and has skin elevation containing remnant cartilage. The auricle develops between the 4th and 12th week of the embryonic stage, which groove the tissue from the 1st and 2nd branchial arches. Histologically, the lesions include a rugated epidermis with a thin layer stratum corneum, tiny mature hair follicles, fat lobules, and connective tissue framework that may include a central cartilage core. The aim of this study was to evaluate the accessory tragus lesions with our clinical surgical treatment results. Material and Methods: Lesions usually located anterior to the tragus and along an imaginary line drawn from the tragus to the angle of the mouth. Twelve patients admitted to our clinic between October 2011 and November 2014 were included in this study. Results: Seven boys and five girls between two–13 years old underwent operation. In total, 28 accessory ears were excised. No complications were observed during the procedure, and no complaints were noted in the postoperative period. Conclusion: Generally, limited anomaly is associated with the first and second branchial arch anomalies.
