Friends or Strangers? A Feasibility Study of an Innovative Focus Group Methodology
Focus groups are useful tools for examining perceptions, feelings, and suggestions about topics, products, or issues. Typically, focus groups are held in formal facilities with “strangers” or participants who do not know each other. Recent work suggests that “friendship groups” may provide an innovative alternative for collecting group-level qualitative data. This approach involves recruiting a single “source participant” who hosts a group in his/her home and recruits friends possessing the characteristics desired for the study. In order to examine the feasibility of friendship groups as a defensible research methodology, we conducted a series of four friendship groups as a feasibility study. Our analysis examined data from questionnaires about demographics, levels of acquaintanceship, and experience taking part in the group; transcripts; observational data; and the time and costs for recruiting. Using these data, we examined group dynamics, implementation issues, and recruitment time and costs. Based on these analyses, our study determined that friendship groups have the potential to be a viable and cost-effective method of qualitative inquiry

Managing Human-Habituated Bears to Enhance Survival, Habitat Effectiveness, and Public Viewing
The negative impacts on bears (Ursus spp.) from human activities associated with roads and developments are well documented. These impacts include displacement of bears from high-quality foods and habitats, diminished habitat effectiveness, and reduced survival rates. Additionally, increased public visitations to national parks accompanied with benign encounters with bears along park roads have caused more bears to habituate to the presence of people. In some contexts, habituation can predispose bears to being exposed to and rewarded by anthropogenic foods, which can also lower survival rates. The managers and staff of Yellowstone National Park located in Wyoming, Montana, and Idaho, USA, and Grand Teton National Park in northwestern Wyoming, USA have implemented several proactive strategies to mitigate the negative aspects of bear habituation. These strategies include providing park visitors with educational information on bear viewing etiquette, managing roadside viewing opportunities, installing bear-resistant infrastructure, hazing bears from developments, enforcing food and garbage storage regulations, and making human activities as predictable as possible to bears. Under the current management strategies, thousands of visitors are still able to view, photograph, and appreciate bears while visiting these parks each year. The opportunity to view bears provides a positive visitor experience and contributes millions of dollars to the local economies of park gateway communities. Positive bear viewing experiences also help build an important appreciation and conservation ethic for bears in people that visit national parks. For many years, managers were concerned about decreasing and threatened bear populations. Now more jurisdictions are facing new challenges caused by increasing bear populations. Research Exploring Physical Activity in Care Homes (REACH): study protocol for a randomised controlled trial
Background: As life expectancy increases and the number of older people, particularly those aged 85 years and over, expands there is an increase in demand for long-term care. A large proportion of people in a care home setting spend most of their time sedentary, and this is one of the leading preventable causes of death. Encouraging residents to engage in more physical activity could deliver benefits in terms of physical and psychological health, and quality of life. This study is the final stage of a programme of research to develop and preliminarily test an evidence-based intervention designed to enhance opportunities for movement amongst care home residents, thereby increasing levels of physical activity. Methods/design: This is a cluster randomised feasibility trial, aiming to recruit at least 8–12 residents at each of 12 residential care homes across Yorkshire, UK. Care homes will be randomly allocated on a 1:1 basis to receive either the intervention alongside usual care, or to continue to provide usual care alone. Assessment will be undertaken with participating residents at baseline (prior to care home randomisation) and at 3, 6, and 9 months post-randomisation. Data relating to changes in physical activity, physical function, level of cognitive impairment, mood, perceived health and wellbeing, and quality of life will be collected. Data at the level of the home will also be collected and will include staff experience of care, and changes in the numbers and types of adverse events residents experience (for example, hospital admissions, falls). Details of National Health Service (NHS) usage will be collected to inform the economic analysis. An embedded process evaluation will obtain information to test out the theory of change underpinning the intervention and its acceptability to staff and residents. Discussion: This feasibility trial with embedded process evaluation and collection of health economic data will allow us to undertake detailed feasibility work to inform a future large-scale trial. It will provide valuable information to inform research procedures in this important but challenging area
Analysis of shared heritability in common disorders of the brain
ience, this issue p. eaap8757 Structured Abstract INTRODUCTION Brain disorders may exhibit shared symptoms and substantial epidemiological comorbidity, inciting debate about their etiologic overlap. However, detailed study of phenotypes with different ages of onset, severity, and presentation poses a considerable challenge. Recently developed heritability methods allow us to accurately measure correlation of genome-wide common variant risk between two phenotypes from pools of different individuals and assess how connected they, or at least their genetic risks, are on the genomic level. We used genome-wide association data for 265,218 patients and 784,643 control participants, as well as 17 phenotypes from a total of 1,191,588 individuals, to quantify the degree of overlap for genetic risk factors of 25 common brain disorders. RATIONALE Over the past century, the classification of brain disorders has evolved to reflect the medical and scientific communities' assessments of the presumed root causes of clinical phenomena such as behavioral change, loss of motor function, or alterations of consciousness. Directly observable phenomena (such as the presence of emboli, protein tangles, or unusual electrical activity patterns) generally define and separate neurological disorders from psychiatric disorders. Understanding the genetic underpinnings and categorical distinctions for brain disorders and related phenotypes may inform the search for their biological mechanisms. RESULTS Common variant risk for psychiatric disorders was shown to correlate significantly, especially among attention deficit hyperactivity disorder (ADHD), bipolar disorder, major depressive disorder (MDD), and schizophrenia. By contrast, neurological disorders appear more distinct from one another and from the psychiatric disorders, except for migraine, which was significantly correlated to ADHD, MDD, and Tourette syndrome. We demonstrate that, in the general population, the personality trait neuroticism is significantly correlated with almost every psychiatric disorder and migraine. We also identify significant genetic sharing between disorders and early life cognitive measures (e.g., years of education and college attainment) in the general population, demonstrating positive correlation with several psychiatric disorders (e.g., anorexia nervosa and bipolar disorder) and negative correlation with several neurological phenotypes (e.g., Alzheimer's disease and ischemic stroke), even though the latter are considered to result from specific processes that occur later in life. Extensive simulations were also performed to inform how statistical power, diagnostic misclassification, and phenotypic heterogeneity influence genetic correlations. CONCLUSION The high degree of genetic correlation among many of the psychiatric disorders adds further evidence that their current clinical boundaries do not reflect distinct underlying pathogenic processes, at least on the genetic level. This suggests a deeply interconnected nature for psychiatric disorders, in contrast to neurological disorders, and underscores the need to refine psychiatric diagnostics. Genetically informed analyses may provide important "scaffolding" to support such restructuring of psychiatric nosology, which likely requires incorporating many levels of information. By contrast, we find limited evidence for widespread common genetic risk sharing among neurological disorders or across neurological and psychiatric disorders. We show that both psychiatric and neurological disorders have robust correlations with cognitive and personality measures. Further study is needed to evaluate whether overlapping genetic contributions to psychiatric pathology may influence treatment choices. Ultimately, such developments may pave the way toward reduced heterogeneity and improved diagnosis and treatment of psychiatric disorders
The psychological impact of transition on young adults from the world of academia to the business world
Young adults are faced with many challenges, changes, demands and decisions as they grow up and become young adults. Emerging adulthood is a distinctive period of change, development and a foundation for future achievements, goals and occupation decisions. This era is marked by certain biological changes, changes in social roles, individual and psychological change, as well as legal changes. This period of adult development, plus young adults’ first experiences in the work place signifies a period of often dramatic change into adulthood, combined with life and career goal alignment.
The paradigm for the research is that South Africa is no different to the rest of the world; young adults are faced with the same challenges and demands as they move from a relative place of comfort in a University setting, to a very different setting in a commercial environment. In order to break down the complexity of this paradigm, the researcher viewed the changes and the challenges that accompany it, as a good opportunity for research. The transition from the world of academia to the world of work is one that seems to have a material impact on young adults. As such, this provides an opportunity for research, and this paper delves into the challenges and demands on young adults, looking at what impacts them the most. The research was conducted by way of semi-structured interviews and was led by the researcher on a one-on-one basis with recently hired graduates (2007) as the participants, in a Financial Services firm in Johannesburg, South Africa.
The participants’ age group ranged from 21 to 29 years of age and were from various cultures and race groups. The semi-structured interview contained a range of questions which included a number of key words that were used to probe the participants view and insight into what the psychological stressors were for them personally, including emotional, physiological and mental stressors. The resultant information was that overall, young graduates felt significant stress and pressure, which was made up of various factors culminating in psychological impact. The semi-structured interview further explored various potential stressors like the organisation’s culture, company choice, earning potential, expectation and transition; overall the psychological effect of transition from an academic environment to a working environment had the greatest effect on the participants in this research process.Dissertation (MA)--University of Pretoria, 2013.lk2014PsychologyMAUnrestricte
Symptom outcomes important to women with anal incontinence: a conceptual framework.
ObjectiveTo develop a framework that describes the most important symptom outcomes for anal incontinence treatment from the patient perspective.MethodsA conceptual framework was developed by the Pelvic Floor Disorders Network based on four semistructured focus groups and confirmed in two sets of 10 cognitive interviews including women with anal incontinence. We explored: 1) patient-preferred terminology for describing anal incontinence symptoms; 2) patient definitions of treatment "success"; 3) importance of symptoms and outcomes in the framework; and 4) conceptual gaps (defined as outcomes not previously identified as important). Sessions were conducted according to grounded theory transcribed, coded, and qualitatively and quantitatively analyzed to identify relevant themes. Content and face validity of the framework were further assessed using cognitive interviews.ResultsThirty-four women participated in focus groups and 20 in cognitive interviews. Overall, 29 (54%) were aged 60 years or older, 42 (78%) were white, and 10 (19%) had a high school degree or less. Two overarching outcome themes were identified: "primary bowel leakage symptoms" and "ancillary bowel symptoms." Subdomains important in primary bowel leakage symptoms included leakage characteristics (symptom frequency, amount of leakage, symptom bother) and conditions when bowel leakage occurs (predictability, awareness, urgency). Subdomains important under ancillary bowel symptoms included emptying disorders (constipation, obstructed defecation, and wiping issues) and discomfort (pain, burning). New outcomes identified included predictability, awareness, wiping issues, and discomfort.ConclusionWomen with anal incontinence desire a wide range of symptom outcomes after treatment. These are captured in our conceptual framework, which can aid clinicians and researchers in assessing anal incontinence. LEVEL OF EVIEDENCE: II
