    Comparison of self-reported health & healthcare utilisation between asylum seekers and refugees: an observational study

    <p>Abstract</p> <p>Background</p> <p>Adult refugees and asylum seekers living in Western countries experience a high prevalence of mental health problems, especially post traumatic stress disorder (PTSD), depression and anxiety. This study compares and contrasts the prevalence of health problems, and potential risk factors as well as the utilisation of health services by asylum seekers and refugees in the Irish context.</p> <p>Methods</p> <p>Cross sectional study using validated self reported health status questionnaires of adult asylum seekers (n = 60) and refugees (n = 28) from 30 countries, living in Ireland. Outcome measures included: general health status (SF-36), presence of PTSD symptoms and anxiety/depression symptoms. Data on chronic conditions and pre or post migration stressors are also reported. The two groups are compared for utilisation of the health care system and the use of over the counter medications.</p> <p>Results</p> <p>Asylum seekers were significantly more likely than refugees to report symptoms of PTSD (OR 6.3, 95% CI: 2.2–17.9) and depression/anxiety (OR 5.8, 95% CI: 2.2–15.4), while no significant difference was found in self-reported general health. When adjusted by multivariable regression, the presence of more than one chronic disease (OR 4.0, 95%CI: 1.3–12.7; OR 3.4, 95% CI: 1.2–10.1), high levels of pre migration stressors (OR 3.6, 95% CI: 1.1–11.9; OR 3.3, 95% CI: 1.0–10.4) or post migration stressors (OR 17.3, 95% CI: 4.9–60.8; OR 3.9, 95% CI: 1.2–12.3) were independent predictors of self reported PTSD or depression/anxiety symptoms respectively, however, residence status was no longer significantly associated with PTSD or depression/anxiety. Residence status may act as a marker for other explanatory variables; our results show it has a strong relationship with post migration stressors (χ<sup>2 </sup>= 19.74, df = 1, P < 0.001).</p> <p>In terms of health care utilisation, asylum seekers use GP services more often than refugees, while no significant difference was found between these groups for use of dentists, medication, hospitalisation or mental health services.</p> <p>Conclusion</p> <p>Asylum seekers have a higher level of self reported PTSD and depression/anxiety symptoms compared to refugees. However, residence status appears to act as a marker for post migration stressors. Compared to refugees, asylum seekers utilise GP services more often, but not mental health services.</p

    Yellow-necked mice (Apodemus flavicollis) and bank voles (Myodes glareolus) as zoomonitors of environmental contamination at a polluted area in Slovakia

    <p>Abstract</p> <p>Background</p> <p>Free-living wild rodents are often used as zoomonitors of environmental contamination. In the present study, accumulation of cadmium (Cd), copper (Cu), iron (Fe), and zinc (Zn) in critical organs of yellow-necked mice (<it>Apodemus flavicollis</it>) and bank voles (<it>Myodes glareolus</it>) trapped in a polluted area in Nováky, Slovakia was investigated.</p> <p>Methods</p> <p>Yellow-necked mice (n = 8) and bank voles (n = 10) were collected using standard theriological methods for wood ecosystems. All animals were adult males in good physical condition. The concentrations of Cd, Cu, Fe, and Zn in the liver, kidney, and bone were determined by atomic absorption spectrophotometry.</p> <p>Results</p> <p>The highest concentrations of Cd and Zn were found in the bone of both species while Cu and Fe accumulated mainly in kidney or liver. Significant higher concentrations of Cd and Cu were detected in the liver of bank voles than in yellow-necked mice. Similar significant higher levels of Cd and Zn were found in the bone of bank voles. In contrast, significant higher concentrations of Cu and Fe were present in the kidney of yellow-necked mice.</p> <p>Conclusions</p> <p>In the yellow-necked mouse and bank vole, bone seems to accumulate Cd and Zn following prolonged exposure. On the contrary, kidney and liver store Cu and Fe after a long-term environmental exposure. In the present study, bank voles seemed to be more heavy metal loaded zoomonitors than yellow-necked mice.</p

    Doctors' learning habits: CME activities among Norwegian physicians over the last decade

    <p>Abstract</p> <p>Background</p> <p>Coping with the increasing body of medical knowledge is a main challenge to all doctors. The aim of this study was to investigate self reported reading and learning habits among Norwegian doctors and their subjective ability to keep professionally updated.</p> <p>Methods</p> <p>A cross sectional survey among a randomised sample of Norwegian doctors was undertaken in 2004 (n = 1005, response rate 71%). A similar study with many identical questions was done in 1993 (n = 1041, response rate 71%) and a comparison of the results was made.</p> <p>Results</p> <p>Attending courses/congresses and reading medical literature were reported to be the most important sources of professional information in 2004, just like in 1993. Less time was spent on courses/congresses in 2004 than in 1993, and more time was spent on medical reading. The internet was regarded as useful for their professional life for three out of five, mostly among the younger and least among GPs.</p> <p>Two out of three doctors felt that they could obtain sufficient information for keeping updated in 2004, the same proportion as in 1993. A correlation was found between subjective coping with the information and a high level of continuing medical education (CME)-activities. The information copers had a higher level of job satisfaction than non-copers.</p> <p>Conclusion</p> <p>Over the last decade Norwegian doctors spend less time on attending courses/congresses and more time on medical reading, while the level of self perceived coping with information has been unchanged. The changing pattern of professional updating may reflect a more general individualistic trend in society. The consistent finding of a correlation between reading and attending courses, subjective coping and job satisfaction gives good reasons for recommending a high level of CME-activities among doctors.</p

    Guaranteed violation of a Bell inequality without aligned reference frames or calibrated devices

    Bell tests---the experimental demonstration of a Bell inequality violation---are central to understanding the foundations of quantum mechanics, underpin quantum technologies, and are a powerful diagnostic tool for technological developments in these areas. To date, Bell tests have relied on careful calibration of the measurement devices and alignment of a shared reference frame between the two parties---both technically demanding tasks in general. Surprisingly, we show that neither of these operations are necessary, violating Bell inequalities with near certainty with (i) unaligned, but calibrated, measurement devices, and (ii) uncalibrated and unaligned devices. We demonstrate generic quantum nonlocality with randomly chosen local measurements on a singlet state of two photons implemented with reconfigurable integrated optical waveguide circuits based on voltage-controlled phase shifters. The observed results demonstrate the robustness of our schemes to imperfections and statistical noise. This new approach is likely to have important applications in both fundamental science and in quantum technologies, including device independent quantum key distribution.Comment: 7 pages, 7 figure

    Solar Orbiter observations of the Kelvin-Helmholtz waves in the solar wind

    Context. The Kelvin-HeImholtz (KH) instability is a nonlinear shear-driven instability that develops at the interface between shear flows in plasmas. KH waves have been inferred in various astrophysical plasmas, and have been observed in situ at the magnetospheric boundaries of solar-system planets and through remote sensing at the boundaries of coronal mass ejections. // Aims. KH waves are also expected to develop at flow shear interfaces in the solar wind. While they were hypothesized to play an important role in the mixing of plasmas and in triggering solar wind fluctuations, their direct and unambiguous observation in the solar wind was still lacking. // Methods. We report in-situ observations of quasi-periodic magnetic and velocity field variations plausibly associated with KH waves using Solar Orbiter during its cruise phase. They are found in a shear layer in the slow solar wind in the close vicinity of the Heliospheric Current Sheet. Analysis is performed to derive the local configuration of the waves. A 2-D MHD simulation is also set up with approximate empirical values to test the stability of the shear layer. In addition, magnetic spectra of the event are analyzed. Results. We find that the observed conditions satisfy the KH instability onset criterion from the linear theory analysis, and its de- velopment is further confirmed by the simulation. The current sheet geometry analyses are found to be consistent with KH wave development, albeit with some limitations likely owing to the complex 3D nature of the event and solar wind propagation. Addition- ally, we report observations of an ion jet consistent with magnetic reconnection at a compressed current sheet within the KH wave interval. The KH activity is found to excite magnetic and velocity fluctuations with power law scalings that approximately follow k−5/3 and k−2.8 in the inertial and dissipation ranges, respectively. Finally, we discuss reasons for the lack of in-situ KH wave detection in past data. // Conclusions. These observations provide robust evidence of KH wave development in the solar wind. This sheds new light on the process of shear-driven turbulence as mediated by the KH waves with implications for the driving of solar wind fluctuations

    Flux rope and dynamics of the heliospheric current sheet Study of the Parker Solar Probe and Solar Orbiter conjunction of June 2020

    Context: Solar Orbiter and Parker Solar Probe jointly observed the solar wind for the first time in June 2020, capturing data from very different solar wind streams: calm, Alfvénic wind and also highly dynamic large-scale structures. Context. Our aim is to understand the origin and characteristics of the highly dynamic solar wind observed by the two probes, particularly in the vicinity of the heliospheric current sheet (HCS). Methods: We analyzed the plasma data obtained by Parker Solar Probe and Solar Orbiter in situ during the month of June 2020. We used the Alfvén-wave turbulence magnetohydrodynamic solar wind model WindPredict-AW and we performed two 3D simulations based on ADAPT solar magnetograms for this period. Results: We show that the dynamic regions measured by both spacecraft are pervaded by flux ropes close to the HCS. These flux ropes are also present in the simulations, forming at the tip of helmet streamers, that is, at the base of the heliospheric current sheet. The formation mechanism involves a pressure-driven instability followed by a fast tearing reconnection process. We further characterize the 3D spatial structure of helmet streamer born flux ropes, which appears in the simulations to be related to the network of quasi-separatrices

    Low-concentration, continuous brachial plexus block in the management of Purple Glove Syndrome: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Purple Glove Syndrome is a devastating complication of intravenous phenytoin administration. Adequate analgesia and preservation of limb movement for physiotherapy are the two essential components of management.</p> <p>Case presentation</p> <p>A 26-year-old Tamil woman from India developed Purple Glove Syndrome after intravenous administration of phenytoin. She was managed conservatively by limb elevation, physiotherapy and oral antibiotics. A 20G intravenous cannula was inserted into the sheath of her brachial plexus and a continuous infusion of bupivacaine at a low concentration (0.1%) with fentanyl (2 μg/ml) at a rate of 1 to 2 ml/hr was given. She had adequate analgesia with preserved motor function which helped in physiotherapy and functional recovery of the hand in a month.</p> <p>Conclusion</p> <p>A continuous blockade of the brachial plexus with a low concentration of bupivacaine and fentanyl helps to alleviate the vasospasm and the pain while preserving the motor function for the patient to perform active movements of the finger and hand.</p

    Resistance to the cereal cyst nematode (Heterodera avenae) transferred from the wild grass Aegilops ventricosa to hexaploid wheat by a "stepping-stone" procedure

    Transfer of resistance toHeterodera avenae, the cereal cyst nematode (CCN), by a stepping-stoneprocedure from the wild grassAegilops ventricosa to hexaploid wheat has been demonstrated. The number of nematodes per plant was lower, and reached a plateau much earlier, in the resistant introgression line H93-8 (1–2 nematodes per plant) than in the recipient H10-15 wheat (14–16 nematodes per plant). Necrosis (hypersensitive reaction) near the nematode, little cell fusion, and few, often degraded syncytia were observed in infested H93-8 roots, while abundant, well-formed syncytia were present in the susceptible H10-15 wheat. Line H93-8 was highly resistant to the two Spanish populations tested, as well as the four French races (Fr1-Fr4), and the British pathotype Hall, but was susceptible to the Swedish pathotypes HgI and HgIII. Resistance was inherited as though determined by a single quasi-dominant factor in the F2 generations resulting from crosses of H93-8 with H10-15 and with Loros, a resistant wheat carrying the geneCre1 (syn.Ccn1). The resistance gene in H93-8 (Cre2 orCcn2) is not allelic with respect to that in Loros. RFLPs and other markers, together with the cytogenetical evidence, indicate that theCre2 gene has been integrated into a wheat chromosome without affecting its meiotic pairing ability. Introduction ofCre2 by backcrossing into a commercial wheat backgroud increases grain yield when under challenge by the nematode and is not detrimental in the absence of infestation

    GiSAO.db: a database for ageing research

    <p>Abstract</p> <p>Background</p> <p>Age-related gene expression patterns of <it>Homo sapiens </it>as well as of model organisms such as <it>Mus musculus</it>, <it>Saccharomyces cerevisiae</it>, <it>Caenorhabditis elegans </it>and <it>Drosophila melanogaster </it>are a basis for understanding the genetic mechanisms of ageing. For an effective analysis and interpretation of expression profiles it is necessary to store and manage huge amounts of data in an organized way, so that these data can be accessed and processed easily.</p> <p>Description</p> <p>GiSAO.db (Genes involved in senescence, apoptosis and oxidative stress database) is a web-based database system for storing and retrieving ageing-related experimental data. Expression data of genes and miRNAs, annotation data like gene identifiers and GO terms, orthologs data and data of follow-up experiments are stored in the database. A user-friendly web application provides access to the stored data. KEGG pathways were incorporated and links to external databases augment the information in GiSAO.db. Search functions facilitate retrieval of data which can also be exported for further processing.</p> <p>Conclusions</p> <p>We have developed a centralized database that is very well suited for the management of data for ageing research. The database can be accessed at <url>https://gisao.genome.tugraz.at</url> and all the stored data can be viewed with a guest account.</p
