    Incidence and maternal outcome of twin pregnancy

    Background: Twin pregnancy is a high-risk pregnancy with different prevalences in different regions of world which is on the rise due to growing use of assisted reproductive technology. The objective of this study is to determine the incidence and maternal complication and outcome of twin pregnancies.Methods: This is a prospective observational study conducted in 2016-2017 in NSCB medical college and hospital, Jabalpur on 67 twin deliveries. Final outcome was given in terms of percentage and ratio.Results: After assessment of 5995 deliveries during the period 2016-2017 the incidence of twin pregnancy was 1.12 percent. In present study 83.6% of patients were in the age group of 21-30 years. Majority 56.7% were unbooked. 50.7% were primipara. Family history of twin pregnancy was found in 31.3% cases and 17.9% cases have history of ovulation inducing agent. Maximum 65.7% delivered between the gestational age 29-37weeks. Mostly 59.7% were dichorionic-diamniotic twins. Preterm delivery occurred 72.5% in Diamniotic Dichorionic and 85.7% in Monoamniotic Monochorionic. 49.3% cases were having both babies with cephalic presentation. 29.9% twins delivered by cesarean section. most common indication for LSCS in twin pregnancy was malpresentation. Most common complication was preterm labour (52.2%) followed by anaemia and preeclampsia.Conclusions: Twin pregnancy had higher likelihood of maternal adverse effect. The higher morbidity and mortality is due to high percentage of patient being unbooked and majority receive no therapeutic intervention until admission. So all efforts should directed towards better quality antenatal care of twin carrying mother

    In Ayurvedic classics there are various types of treatment and Shastra chikitsa is one among them. Purva Karma Pradhana Karma, Paschat Karma are Trividha Karma. According to Acharya Sushruta, Purva Karma means preparation of patient along with collecting all the materials needed during the Pradhana karma. Ashtaviddha Shastra Karma is included in Pradhana Karma. Paschat Karma includes post operative care. Sushruta division of surgical activity into three parts i.e., pre- operative, operative and post-operative based on sound scientific principles. Sushruta has also described the pre-operative appreciation of foreign body, its size, shape, and exact location within the body and appropriate instrument for its removal should be selected pre-operatively. He has also mentioned the pre-operative diet and starvation for various types of surgeries. He has also emphasized that asepsis and antisepsis precaution should be taken, wound should be protected from dangerous and invisible creatures (Nishachara). Fumigation of Vranitaagara should be done for ten days, twice a day. By virtue of this article, we can understand the systematic method of arranging the surgical experience of arranging the surgical experience of the older surgeon, about preliminary measures, principal measures and after measures. All the procedures included under these three headings i.e., Trividdha karma plays an important role in successful and complication free surgery

    Uterine fibroids embolization in a tertiary hospital: our initial experience

    Background: Uterine Fibroid Embolization (UFE) is a new modality which has appeared in the horizon for non-surgical management of many conditions including fibroids. Moderate to severe anemia with fibroids is one of the reasons for delay in definitive treatment. Need for blood transfusion and its consequent risk of adverse reactions and diseases became a necessity to consider alternative treatment. Objective: To evaluate the efficacy and safety of UFE as an alternative to major surgery.Methods: Seventeen women with symptomatic uterine fibroids who declined surgery or were at poor risk for surgery were treated by Uterine Artery Embolization (UAE). The uterine arteries were catheterised and embolized with polyvinyl alcohol particles. The patients were followed up at regular interval clinically and with transabdominal ultrasonography.Results: UAE performed on all 17 patients had no major complications. Clinical follow-up showed a significant reduction in symptoms within six months of the procedure, with 79% improvement in menorrhagia and 86%   improvement in pain. Follow-up imaging showed reduction in fibroid sizes ranging from 9-60%. The mean procedural time was 150 minutes. The mean hospital stay was 5 days (range, 3 to 10).Conclusions: Mid-term results of UAE for the treatment of symptomatic fibroids in our hospital indicate this to be a safe and effective therapeutic option.

    Angiogenic Index as a Measure of Angiogenesis in Prostate Cancer and Its Correlation with Gleason Grade and Score

    Introduction: Microvessel density, as a measure of angiogenesis, predicts prognosis in prostate cancer. Angiogenic Index (AI, numerical value of angiogenesis) minimises the possible variation concerning the width of the microscopic field, stromal epithelial relations and cellular tumour size. Aim: To study AI in prostate cancer and its correlation with Gleason Grade (GG) and Gleason Score (GS). Materials and Methods: The present cross-sectional, study was done at Postgraduate Department of Pathology, Sarojini Naidu Medical College, Agra, India, from September 2019 to December 2020. Twenty five histopathologically confirmed prostate adenocarcinoma specimens from radical prostatectomy, Transurethral Resection of Prostate (TURP), and needle biopsy were included in the study. These cases were categorised according to Gleason Grade (GG); Gleason Score (GS) was assigned to each case. The GSs were simplified into three groups: low (GS 2-6), intermediate (GS 7) and high-grade (GS 8-10). Immunohistochemical {Cluster Differentiation (CD) 31} blood vessel staining was done to calculate AI. Statistical significance was determined by Unpaired t-test. Results: All the cases were males with age range from 55- 76 years (mean age was 65.48±5.62 years). Mean AI was 13.74, 83.76, 163.27, 299.12 for the GG1, GG3, GG4, GG5, respectively. Mean AI was 29.72, 82.67, 129.15, 190.31, 206.71, 307.34 for GS 2, 6, 7, 8, 9, 10, respectively. Comparing GG among themselves, statistically significant difference in AI was found between GG3 vs GG4 (p-value=0.0056, r-value=0.5269). Difference was also statistically significant between GG3 vs GG5 (p-value=0.000011, r-value=0.8030) and GG4 vs GG5 (p-value=0.0036, r-value=0.5806). In all scores combined, the mean AI was 56.20 for low-grade (GS 2-6), 129.15 for intermediate-grade (GS 7), 247.35 for high-grade (GS 8-10). Statistically significant difference was found in between AI (p-value <0.05) in all Gleason scores. Conclusion: Positive correlation was observed between AI, GG and GS in prostatic adenocarcinoma. AI may be of immense value to predict prognosis of prostatic adenocarcinoma

    Cross-Talk Signaling in Rice During Combined Drought and Bacterial Blight Stress

    Due to climatic changes, rice crop is affected by moisture deficit stress and pathogens. Tissue water limitation besides reducing growth rates, also renders the crop susceptible to the infection by Xanthomonas oryzae pv. oryzae (Xoo) that causes bacterial leaf blight. Independently, both drought adaptation and Xoo resistance have been extensively studied. Though the cross-talk between drought and Xoo stress responses have been explored from individual stress studies, examining the combinatorial stress response is limited in rice. Recently published combined stress studies showed that under the combined stress, maintenance of carbon assimilation is hindered and such response is regulated by overlapping cellular mechanisms that are different from either of the individual stresses. Several receptors, MAP kinases, transcription factors, and ribosomal proteins, are predicted for playing a role in cellular homeostasis and protects cells from combined stress effects. Here we provide a critical analysis of these aspects using information from the recently published combined stress literature.     An embedding technique to determine ττ backgrounds in proton-proton collision data

    An embedding technique is presented to estimate standard model tau tau backgrounds from data with minimal simulation input. In the data, the muons are removed from reconstructed mu mu events and replaced with simulated tau leptons with the same kinematic properties. In this way, a set of hybrid events is obtained that does not rely on simulation except for the decay of the tau leptons. The challenges in describing the underlying event or the production of associated jets in the simulation are avoided. The technique described in this paper was developed for CMS. Its validation and the inherent uncertainties are also discussed. The demonstration of the performance of the technique is based on a sample of proton-proton collisions collected by CMS in 2017 at root s = 13 TeV corresponding to an integrated luminosity of 41.5 fb(-1).Peer reviewe

    Bose-Einstein correlations of charged hadrons in proton-proton collisions at s\sqrt s = 13 TeV

    Bose-Einstein correlations of charged hadrons are measured over a broad multiplicity range, from a few particles up to about 250 reconstructed charged hadrons in proton-proton collisions at s \sqrt{s} = 13 TeV. The results are based on data collected using the CMS detector at the LHC during runs with a special low-pileup configuration. Three analysis techniques with different degrees of dependence on simulations are used to remove the non-Bose-Einstein background from the correlation functions. All three methods give consistent results. The measured lengths of homogeneity are studied as functions of particle multiplicity as well as average pair transverse momentum and mass.     Search for dark matter in events with a leptoquark and missing transverse momentum in proton-proton collisions at 13 TeV

    A search is presented for dark matter in proton-proton collisions at a center-of-mass energy of root s= 13 TeV using events with at least one high transverse momentum (p(T)) muon, at least one high-p(T) jet, and large missing transverse momentum. The data were collected with the CMS detector at the CERN LHC in 2016 and 2017, and correspond to an integrated luminosity of 77.4 fb(-1). In the examined scenario, a pair of scalar leptoquarks is assumed to be produced. One leptoquark decays to a muon and a jet while the other decays to dark matter and low-p(T) standard model particles. The signature for signal events would be significant missing transverse momentum from the dark matter in conjunction with a peak at the leptoquark mass in the invariant mass distribution of the highest p(T) muon and jet. The data are observed to be consistent with the background predicted by the standard model. For the first benchmark scenario considered, dark matter masses up to 500 GeV are excluded for leptoquark masses m(LQ) approximate to 1400 GeV, and up to 300 GeV for m(LQ) approximate to 1500 GeV. For the second benchmark scenario, dark matter masses up to 600 GeV are excluded for m(LQ) approximate to 1400 GeV. (C) 2019 The Author(s). Published by Elsevier B.V.Peer reviewe
