    Improvement strategies for construction materials commercialization companies with a Sales methodology - Company in the municipality of Cúcuta.

    La finalidad del presente trabajo ha sido la generación de estrategias de mejora para empresas comercializadoras de materiales de construcción con metodología Venta – Empresa en el municipio de Cúcuta. Para ello se analizaron ocho empresas ferreteras que emplean la metodología ya mencionada; y diez empresas constructoras teniendo en cuenta la clasificación de los tres grandes grupos; con una representación del 40% para construcción de edificios, el 30% para obras de ingeniería civil, y el 30% restante para actividades especializadas para la construcción de edificios y obras de ingeniería civil; se aplicaron instrumentos de recolección de información que permitieron conocer y profundizar en los elementos internos del sector ferretero, tales como su proceso de gestión de pedidos y despachos, de cada una de las organizaciones, de su planeación, personal, dirección e investigación y desarrollo; y, de igual manera, información externa a las empresas del sector constructor, aquí se tuvieron en cuenta las variables tecnológicas, competencia en el mercado, satisfacción del cliente, priorización de necesidades del cliente. Como resultado, los autores pudieron definir que las empresas ferreteras están implementando servicios personalizados para las constructoras teniendo en cuenta sus necesidades individuales, sin embargo, aún existen vacíos en esta metodología Venta – Empresa debido a la poca inversión en sistemas de gestión integrados lo que produce que la información no sea aprovechada de forma adecuada y que se incurra en reprocesos y atrasos en los procedimientos de pedido y despacho de materiales. Además, se identificó que la mayor inconformidad por parte del sector constructor es el incumplimiento en los plazos de entrega por la escasez del insumo requerido, por lo que se hace importante prestar mayor atención a los modelos de stock implementados en cada una de las ferreterías.Universidad Libre - Seccional Cúcuta - Facultad de Ingeniería - Especialización en Gerencia de ProyectosThe purpose of this work has been the generation of improvement strategies for construction material commercialization companies using the Sale - Company methodology in the municipality of Cúcuta. For this purpose, a non-probabilistic sampling model was used, where eight hardware companies that use the aforementioned methodology were analyzed; and ten construction companies taking into account the classification of the three large groups; with a representation of 40% for building construction, 30% for civil engineering works, and the remaining 30% for specialized activities for the construction of buildings and civil engineering works; instruments were applied to collect information that allowed to know and deepen in the internal elements of the hardware sector, such as its management process of orders and dispatches, of each one of the organizations, its planning, personnel, management and research and development; and, likewise, external information to the companies of the construction sector, here were taken into account the technological variables, market competition, customer satisfaction, prioritization of customer needs. As a result, the authors were able to define that the hardware companies are implementing personalized services for the construction companies, taking into account their individual needs; however, there are still gaps in this Sale - Company methodology due to the low investment in integrated management systems, which means that the information is not adequately used and that reprocesses and delays are incurred in the procedures for ordering and dispatching materials. In addition, it was identified that the greatest nonconformity on the part of the construction sector is the failure to meet delivery deadlines due to the shortage of the required input, so it is important to pay more attention to the stock models implemented in each of the hardware stores

    Area-specific synapse structure in branched posterior nucleus axons reveals a new level of complexity in thalamocortical networks

    Thalamocortical posterior nucleus (Po) axons innervating the vibrissal somatosensory (S1) and motor (MC) cortices are key links in the brain neuronal network that allows rodents to explore the environment whisking with their motile snout vibrissae. Here, using fine-scale high-end 3D electron microscopy, we demonstrate in adult male C57BL/6 wild-type mice marked differences between MC versus S1 Po synapses in (1) bouton and active zone size, (2) neurotransmitter vesicle pool size, (3) distribution of mitochondria around synapses, and (4) proportion of synapses established on dendritic spines and dendritic shafts. These differences are as large, or even more pronounced, than those between Po and ventro-posterior thalamic nucleus synapses in S1. Moreover, using single-axon transfection labeling, we demonstrate that the above differences actually occur on the MC versus the S1 branches of individual Po cell axons that innervate both areas. Along with recently-discovered divergences in efficacy and plasticity, the synaptic structure differences reported here thus reveal a new subcellular level of complexity.     Development, characterization and sterilisation of Nanocellulose-alginate-(hyaluronic acid)- bioinks and 3D bioprinted scaffolds for tissue engineering

    3D-bioprinting is an emerging technology of high potential in tissue engineering (TE), since it shows effective control over scaffold fabrication and cell distribution. Biopolymers such as alginate (Alg), nanofibrillated cellulose (NC) and hyaluronic acid (HA) offer excellent characteristics for use as bioinks due to their excellent biocompatibility and rheological properties. Cell incorporation into the bioink requires sterilisation assurance, and autoclave, β-radiation and γ-radiation are widely used sterilisation techniques in biomedicine; however, their use in 3D-bioprinting for bioinks sterilisation is still in their early stages. In this study, different sterilisation procedures were applied on NC-Alg and NC-Alg-HA bioinks and their effect on several parameters was evaluated. Results demonstrated that NC-Alg and NC-Alg-HA bioinks suffered relevant rheological and physicochemical modifications after sterilisation; yet, it can be concluded that the short cycle autoclave is the best option to sterilise both NC-Alg based cell-free bioinks, and that the incorporation of HA to the NC-Alg bioink improves its characteristics. Additionally, 3D scaffolds were bioprinted and specifically characterized as well as the D1 mesenchymal stromal cells (D1-MSCs) embedded for cell viability analysis. Notably, the addition of HA demonstrates better scaffold properties, together with higher biocompatibility and cell viability in comparison with the NC-Alg scaffolds. Thus, the use of MSCs containing NC-Alg based scaffolds may become a feasible tissue engineering approach for regenerative medicine.Author thanks the Basque Government for granted fellowship to S.     Clinical practice guidelines for the prevention, early detection, diagnosis, treatment and monitoring of dyslipidemia : non-pharmacological treatment with exercise

    Objetivo: Evaluar el impacto del ejerciciocomo una intervención no farmacológica parala prevención primaria y secundaria de eventoscardio y cerebrovasculares asociados a dislipidemiaen personas con hipercolesterolemia.Métodos: Se elaboró una guía de práctica clínicasiguiendo los lineamientos de la guía metodológicadel Ministerio de Salud y ProtecciónSocial para recolectar de forma sistemática laevidencia científica y formular las recomendacionesutilizando la metodología GRADE. Setomó la información específica acerca de ejerciciocomo medida terapéutica en el tratamientode dislipidemia. Resultados: Tanto el ejercicioaeróbico como el de resistencia se asocian adisminución en el colesterol con lipoproteínasde baja densidad y los triglicéridos, cambiosque fueron estadísticamente significativos; peroestos no son clínicamente relevantes, dado quese trata de una reducción mínima. No se encontróun cambio estadísticamente significativo enlos valores de colesterol de alta densidad. Conclusiones:Se presenta evidencia a favor del usode ejercicio como parte fundamental del tratamientode hipercolesterolemia. Aunque no seencontraron diferencias clínicamente significativasen los valores de colesterol o triglicéridos,el ejercicio físico es una actividad con ampliosbeneficios para el paciente en otros dominiosclínicos de interés, por lo que al considerar elriesgo y el beneficio de esta práctica, los beneficiosson claros y superan ampliamente el bajoriesgo al que se expone una persona al realizaruna rutina de ejercicio, como parte de su vidacotidiana.Artículo original34-43Objective: To evaluate the impact of exercise as a non-pharmacological intervention for primary and secondary prevention of cardiovascular events in people with hypercholesterolemia. Methods: A clinical practice guideline was developed following the guidelines of the methodological guidance of the Ministry of Health and Social Protection to collect systematically the evidence and make recommendations using the GRADE methodology. Results: Both aerobic and resistance exercise are associated with decreased LDL cholesterol and triglycerides, changes were statistically significant, but these changes are not clinically relevant since it is a minimal reduction in these values. No statistically significant change was found in HDL cholesterol values. Conclusions: Recommendations for the use of exercise as a fundamental part of the treatment of hypercholesterolemia are formulated. Although no clinically significant differences in the values of cholesterol or triglycerides, exercise is an activity with great benefits for the patient in other clinical domains of interest, being part of healthy lifestyles and exposure to low risk, so when you consider the risks and benefits of this practice, the benefits are clear and far outweigh the low risk to which a person is exposed when performing an exercise routine as part of their daily lives

Heart Institute (InCor); Brasil.OBJECTIVE: Characterize homozygous familial hypercholesterolemia (HoFH) individuals from Iberoamerica. APPROACH AND RESULTS: In a cross-sectional retrospective evaluation 134 individuals with a HoFH phenotype, 71 adults (age 39.3±15.8 years, 38.0% males), and 63 children (age 8.8±4.0 years, 50.8% males) were studied. Genetic characterization was available in 129 (96%). The majority (91%) were true homozygotes (true HoFH, n=79, 43.0% children, 46.8% males) or compound heterozygotes (compound heterozygous familial hypercholesterolemia, n=39, 51.3% children, 46.2% males) with putative pathogenic variants in the LDLR. True HoFH due to LDLR variants had higher total (P=0.015) and LDL (low-density lipoprotein)-cholesterol (P=0.008) compared with compound heterozygous familial hypercholesterolemia. Children with true HoFH (n=34) tended to be diagnosed earlier (P=0.051) and had a greater frequency of xanthomas (P=0.016) than those with compound heterozygous familial hypercholesterolemia (n=20). Previous major cardiovascular events were present in 25 (48%) of 52 children (missing information in 2 cases), and in 43 (67%) of 64 adults with LDLR variants. Children who are true HoFH had higher frequency of major cardiovascular events (P=0.02), coronary heart (P=0.013), and aortic/supra-aortic valve diseases (P=0.022) than compound heterozygous familial hypercholesterolemia. In adults, no differences were observed in major cardiovascular events according to type of LDLR variant. From 118 subjects with LDLR variants, 76 (64%) had 2 likely pathogenic or pathogenic variants. In 89 subjects with 2 LDLR variants, those with at least one null allele were younger (P=0.003) and had a greater frequency of major cardiovascular events (P=0.038) occurring at an earlier age (P=0.001). CONCLUSIONS: There was a high frequency of cardiovascular disease even in children. Phenotype and cardiovascular complications were heterogeneous and associated with the type of molecular defect

    Why Are Outcomes Different for Registry Patients Enrolled Prospectively and Retrospectively? Insights from the Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF).

    Background: Retrospective and prospective observational studies are designed to reflect real-world evidence on clinical practice, but can yield conflicting results. The GARFIELD-AF Registry includes both methods of enrolment and allows analysis of differences in patient characteristics and outcomes that may result. Methods and Results: Patients with atrial fibrillation (AF) and ≥1 risk factor for stroke at diagnosis of AF were recruited either retrospectively (n = 5069) or prospectively (n = 5501) from 19 countries and then followed prospectively. The retrospectively enrolled cohort comprised patients with established AF (for a least 6, and up to 24 months before enrolment), who were identified retrospectively (and baseline and partial follow-up data were collected from the emedical records) and then followed prospectively between 0-18 months (such that the total time of follow-up was 24 months; data collection Dec-2009 and Oct-2010). In the prospectively enrolled cohort, patients with newly diagnosed AF (≤6 weeks after diagnosis) were recruited between Mar-2010 and Oct-2011 and were followed for 24 months after enrolment. Differences between the cohorts were observed in clinical characteristics, including type of AF, stroke prevention strategies, and event rates. More patients in the retrospectively identified cohort received vitamin K antagonists (62.1% vs. 53.2%) and fewer received non-vitamin K oral anticoagulants (1.8% vs . 4.2%). All-cause mortality rates per 100 person-years during the prospective follow-up (starting the first study visit up to 1 year) were significantly lower in the retrospective than prospectively identified cohort (3.04 [95% CI 2.51 to 3.67] vs . 4.05 [95% CI 3.53 to 4.63]; p = 0.016). Conclusions: Interpretations of data from registries that aim to evaluate the characteristics and outcomes of patients with AF must take account of differences in registry design and the impact of recall bias and survivorship bias that is incurred with retrospective enrolment.     Elective cancer surgery in COVID-19-free surgical pathways during the SARS-CoV-2 pandemic: An international, multicenter, comparative cohort study

    PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks

    Risk profiles and one-year outcomes of patients with newly diagnosed atrial fibrillation in India: Insights from the GARFIELD-AF Registry.

    BACKGROUND: The Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. METHODS AND RESULTS: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012-2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P < 0.001). Diabetes and coronary artery disease (CAD) were prevalent in 36.2% and 28.1% of patients as compared with global prevalence of 22.2% and 21.6%, respectively (P < 0.001 for both). Antiplatelet therapy was the most common antithrombotic treatment in India. With increasing stroke risk, however, patients were more likely to receive oral anticoagulant therapy [mainly vitamin K antagonist (VKA)], but average international normalized ratio (INR) was lower among Indian patients [median INR value 1.6 (interquartile range {IQR}: 1.3-2.3) versus 2.3 (IQR 1.8-2.8) (P < 0.001)]. Compared with other countries, patients from India had markedly higher rates of all-cause mortality [7.68 per 100 person-years (95% confidence interval 6.32-9.35) vs 4.34 (4.16-4.53), P < 0.0001], while rates of stroke/systemic embolism and major bleeding were lower after 1 year of follow-up. CONCLUSION: Compared to previously published registries from India, the GARFIELD-AF registry describes clinical profiles and outcomes in Indian patients with AF of a different etiology. The registry data show that compared to the rest of the world, Indian AF patients are younger in age and have more diabetes and CAD. Patients with a higher stroke risk are more likely to receive anticoagulation therapy with VKA but are underdosed compared with the global average in the GARFIELD-AF.
