    Breast and Cervical Cancer Screening Practices for Low-Income Asian American Women in Ethnic-Specific Clinics

    Introduction: Early detection and screening are the most effective means to reduce cancer mortality in all populations. Asian American (AA) women have among the lowest rates in aggregate for use of early detection, and screening practices of all ethnic populations. The only nationally disaggregated populationbased data on these ethnic groups at the time of the study was the 1993-1994 National Health Interview Survey (NHIS) of English speaking AA, but 70% of the AA population is non-English speaking. Our study presents heretofore unavailable data for cancer screening for monolingual AA women for a comparable time period in California between 1992 and 1994, prior to initiation of the state and Federal programs targeting this group of women. Methods: Retrospective chart reviews of randomly selected medical records were conducted for the breast and cervical cancer screening practices of low-income, non-English speaking Chinese, Korean, and Thai women attending ethnic specific community-based health clinics. All women seen in the clinics between 1992-1994 who were within the appropriate screening age categories were eligible. Results: Asian American women utilizing ethnic specific clinics had equal to or better screening rates for mammography and Pap tests than mainstream services for English-speaking AA women in a national survey. These screening rates, however, were still well below nationally recommended screening rates for breast and cervical cancer according to Healthy People 2000 or 2010 goals. Conclusion: These data support the effectiveness of Community Based Clinics (CBOs) to reach these hard to reach women and address the call for the elimination of health disparities.     Gravitating toward the quadruple helix: International connections for the enhancement of a regional innovation system in Northeast Italy

    The majority of previous Regional Innovation System (RIS) studies generally provide a rather static overview of the roles of innovation-creating actors. This article explores a single RIS in Trentino in Italy. The case shows that the roles of three actors (i.e., the provincial government, academia/research centers, and firms) are vital in creating the RIS, and that the provincial-level government policy is important in supporting the innovation activities of regional research institutions and firms aimed at developing their international connections. The public-private research collaboration and international connections of these actors are the key determinants of the development of an advanced RIS, but have largely been ignored in the extant RIS literature. This article extends the existing RIS and Triple Helix research to an international dimension, highlighting the complementary role of international connections within the RIS, thus reflecting a shift toward Quadruple Helix

    Midlife suicide:A systematic review and meta-analysis of socioeconomic, psychiatric and physical health risk factors

    Suicide is an increasing contributing cause of mortality in middle-aged adults; however, knowledge to guide prevention is limited. This first systematic review and meta-analysis of studies on midlife suicide has provided an overview of published research on this issue and synthesized the evidence on socioeconomic and physical and mental health factors associated with this mortality. Using PRISMA guidelines MEDLINE, Embase, PsycINFO, Scopus and Web of Science were searched for English-language publications that involved persons aged 35 to 65, used individual-level data, and reported prevalence of exposure(s) or relative risks. The search identified 62 studies on midlife suicides and associated factors (28 for SES, 22 for psychiatric disorder and 23 for physical illness). All studies were from high income countries, and most (80.6%) used data from population registries. Meta-analyses showed that the pooled prevalence of exposure in suicide decedents was 57.8% for psychiatric disorder, 56.3% for low income, 43.2% for unemployment, and 27.3% for physical illness. The associated pooled risk ratio was 11.68 (95% confidence intervals: 5.82–23.47) for psychiatric illness of any type, 12.59 (8.29–19.12) for mood disorders, 3.91 (2.72–5.59) for unemployment, 3.18 (2.72–3.72) for being separated or divorced, 2.64 (2.26–3.10) for cancer, 2.50 (0.96–6.38) for central nervous system illness, and 2.26 (1.16–4.41) for low income. In conclusion, midlife suicide is strongly associated with socioeconomic difficulties and physical and psychiatric illnesses that are common in this age population. Future investigations should consider the interactions between risk factors, the intersectionality of sex and ethnicity, and include data from low- and middle-income countries

    L’Amazonie amérindienne dans l’ère du numérique : le portail multilingue WATAU

    L’élaboration et la réalisation du portail numérique WATAU à destination de deux communautés amérindiennes, les Wayana et les Apalaï, ont représenté des défis multiples. D’abord, parce qu’il s’agissait de créer un outil en ligne qui soit accessible en région amazonienne où le réseau Internet, quand il existe, n’est pas très performant. Ensuite, ce travail réalisé à plusieurs mains, à plusieurs têtes, en plusieurs langues, s’appuie sur deux cultures : l’une à tradition orale et l’autre fortement ancrée dans les technologies de l’information et de la communication. Pour ce faire, des membres des communautés wayana et apalaï et des professionnels (chercheurs, graphistes, muséologues, spécialistes des humanités numériques) ont œuvré pour créer un dialogue entre la culture amérindienne et la culture occidentale. Le travail collaboratif et l’échange de connaissances a permis un véritable enrichissement réciproque reposant sur des compétences complémentaires. Ainsi est né le projet « Savoirs autochtones wayana et apalaï : une nouvelle approche de la restitution et ses implications sur les formes de transmission », SAWA, qui a su conjuguer la recherche scientifique et la recherche appliquée dans le but de restituer aux Wayana et Apalaï via un portail numérique, des collections muséales et des corpus documentaires collectés auprès de leurs ancêtres. Le présent texte retrace le parcours de ce projet, mettant en lumière l’accessibilité interactive à ces ressources.The development and implementation of the WATAU digital portal for two Amerindian communities, the Wayana and the Apalai, presented multiple challenges. First of all because the challenge was to create an online tool that would be accessible in the Amazon region where internet, when present, is not always efficient. Secondly, the work, carried out by several persons using different languages, is based on two cultures: one with an oral tradition and the other strongly rooted in information and communication technologies. To this end, members of the Wayana and Apalai communities along with professionals (researchers, graphic designers, museologists, digital humanities specialists) have worked to build dialogue between the Amerindian and Western cultures. Collaborative work and knowledge sharing have led to truly reciprocal enrichment based on complementary skills, giving rise to the SAWA project (Wayana and Apalai Indigenous Knowledge): a new approach to restitution and its implications on forms of transmission. This combined scientific and applied research for restitution of museum collections and documentary corpuses collected from ancestors to the Wayana and Apalai via a digital portal.     Designing tools to predict and mitigate impacts on water quality following the Australian 2019/2020 wildfires: Insights from Sydney's largest water supply catchment

    The 2019/2020 Australian bushfires (or wildfires) burned the largest forested area in Australia's recorded history, with major socio-economic and environmental consequences. Among the largest fires was the 280 000 ha Green Wattle Creek Fire, which burned large forested areas of the Warragamba catchment. This protected catchment provides critical ecosystem services for Lake Burragorang, one of Australia's largest urban supply reservoirs delivering ~85% of the water used in Greater Sydney. Water New South Wales (WaterNSW) is the utility responsible for managing water quality in Lake Burragorang. Its postfire risk assessment, done in collaboration with researchers in Australia, the UK, and United States, involved (i) identifying pyrogenic contaminants in ash and soil; (ii) quantifying ash loads and contaminant concentrations across the burned area; and (iii) estimating the probability and quantity of soil, ash, and associated contaminant entrainment for different rainfall scenarios. The work included refining the capabilities of the new WEPPcloud-WATAR-AU model (Water Erosion Prediction Project cloud-Wildfire Ash Transport And Risk-Australia) for predicting sediment, ash, and contaminant transport, aided by outcomes from previous collaborative postfire research in the catchment. Approximately two weeks after the Green Wattle Creek Fire was contained, an extreme rainfall event (~276 mm in 72 h) caused extensive ash and sediment delivery into the reservoir. The risk assessment informed on-ground monitoring and operational mitigation measures (deployment of debris-catching booms and adjustment of the water supply system configuration), ensuring the continuity of safe water supply to Sydney. WEPPcloud-WATAR-AU outputs can prioritize recovery interventions for managing water quality risks by quantifying contaminants on the hillslopes, anticipating water contamination risk, and identifying areas with high susceptibility to ash and sediment transport. This collaborative interaction among scientists and water managers, aimed also at refining model capabilities and outputs to meet managers' needs, exemplifies the successful outcomes that can be achieved at the interface of industry and science.     A qualitative exploration of the healthcare challenges and pharmaceutical care needs of people with Parkinson's and their caregivers

    Background: People with Parkinson's are at higher risk of healthcare and pharmaceutical care issues. Objective: To determine the healthcare challenges, pharmaceutical care needs, and perceived need of a pharmacist-run clinic by people with Parkinson’s and their caregivers. Setting: Malaysian Parkinson's Disease Association Method: A focus group discussion adopting a descriptive qualitative approach was conducted involving people with Parkinson's and their caregivers. A semi-structured interview guide was used to determine the challenges they faced with their medications and healthcare system, their pharmaceutical care needs, and their views on a pharmacist-run clinic. Data was thematically analysed. Main outcome measure: Healthcare challenges faced by people with Parkinson's and caregivers along with their pharmaceutical care needs and perceived need of a pharmacist-run clinic. Results: Nine people with Parkinson's and four caregivers participated. Six themes were developed: 1) "It's very personalised": the need for self-experimentation, 2) "Managing it is quite difficult": challenges with medication, 3) "The doctor has no time for you": challenges with healthcare providers, 4) "Nobody can do it except me": challenges faced by caregivers, 5) "It becomes a burden": impact on quality of life, and 6) "Lack of consistency could be counterproductive": views on pharmacist-run clinic. Conclusion: The provision of pharmaceutical care services by pharmacists could help overcome issues people with Parkinson's face, however there is a need for them to first see pharmacists in their expanded roles and change their limited perception of pharmacists. This can be achieved through integration of pharmacists within multidisciplinary teams in specialist clinics which they frequent

    Flow framework for analyzing the quality of educational games

    The challenge of educational game design is to develop solutions that appeal to as many players as possible, but are still educationally effective. One foundation for analyzing and designing educational engagement is the flow theory. This article presents a flow framework that describes the dimensions of flow experience that can be used to analyze the quality of educational games. The framework also provides design-support for producing good educational games, because it can be used to reveal ways to optimize learning effects and user experience. However, the framework only works as a link between educational theory and game design, which is useful for game analysis but does not provide the means for a complete game design. To evaluate the elements included in the proposed framework, we analyzed university student’s experiences in participating in a business simulation game. We found that the students’ flow experience in the game was high and the findings indicated that sense of control, clear goals and challenge-skill dimensions of flow scored the highest. Overall, the results indicate that the flow framework is a useful tool to aid the analysis of game-based learning experiences

    Quantifying sources of variability in infancy research using the infant-directed-speech preference

    Psychological scientists have become increasingly concerned with issues related to methodology and replicability, and infancy researchers in particular face specific challenges related to replicability: For example, high-powered studies are difficult to conduct, testing conditions vary across labs, and different labs have access to different infant populations. Addressing these concerns, we report on a large-scale, multisite study aimed at (a) assessing the overall replicability of a single theoretically important phenomenon and (b) examining methodological, cultural, and developmental moderators. We focus on infants’ preference for infant-directed speech (IDS) over adult-directed speech (ADS). Stimuli of mothers speaking to their infants and to an adult in North American English were created using seminaturalistic laboratory-based audio recordings. Infants’ relative preference for IDS and ADS was assessed across 67 laboratories in North America, Europe, Australia, and Asia using the three common methods for measuring infants’ discrimination (head-turn preference, central fixation, and eye tracking). The overall meta-analytic effect size (Cohen’s d) was 0.35, 95% confidence interval = [0.29, 0.42], which was reliably above zero but smaller than the meta-analytic mean computed from previous literature (0.67). The IDS preference was significantly stronger in older children, in those children for whom the stimuli matched their native language and dialect, and in data from labs using the head-turn preference procedure. Together, these findings replicate the IDS preference but suggest that its magnitude is modulated by development, native-language experience, and testing procedure.     Conversion of biomass platform molecules into fuel additives and liquid hydrocarbon fuels

    [EN] In this work some relevant processes for the preparation of liquid hydrocarbon fuels and fuel additives from cellulose, hemicellulose and triglycerides derived platform molecules are discussed. Thus, it is shown that a series of platform molecules such as levulinic acid, furans, fatty acids and polyols can be converted into a variety of fuel additives through catalytic transformations that include reduction, esterification, etherification, and acetalization reactions.     New genetic loci link adipose and insulin biology to body fat distribution.

    Body fat distribution is a heritable trait and a well-established predictor of adverse metabolic outcomes, independent of overall adiposity. To increase our understanding of the genetic basis of body fat distribution and its molecular links to cardiometabolic traits, here we conduct genome-wide association meta-analyses of traits related to waist and hip circumferences in up to 224,459 individuals. We identify 49 loci (33 new) associated with waist-to-hip ratio adjusted for body mass index (BMI), and an additional 19 loci newly associated with related waist and hip circumference measures (P < 5 × 10(-8)). In total, 20 of the 49 waist-to-hip ratio adjusted for BMI loci show significant sexual dimorphism, 19 of which display a stronger effect in women. The identified loci were enriched for genes expressed in adipose tissue and for putative regulatory elements in adipocytes. Pathway analyses implicated adipogenesis, angiogenesis, transcriptional regulation and insulin resistance as processes affecting fat distribution, providing insight into potential pathophysiological mechanisms
