    Analysis of permanent second molar development in children born with cleft lip and palate

    The study of dental development in individuals born with cleft lip and palate (CLP) serves to determine when orthodontic intervention should start. Objective: To evaluate the permanent second molar development in children born with cleft lip and palate according to Demirjian’s and Nolla’s methods. Methodology: Out of a total of 513 digital panoramic radiographs, 113 pairs of children aged 3 to 16 years were selected. The exams were from children born with or without cleft lip and palate, of the same sex, with an age difference of up to 30 days. The images were analyzed by three examiners and reliability was checked through intra-examiner agreement by the Kappa test. The data were analyzed by Wilcoxon's and Mann-Whitney tests according to each dataset. Results: The findings indicated delayed development of the permanent second molars in children with CLP (P<0.001). The development of the right permanent second molar was delayed compared to the left molar in children with CLP. Moreover, mandibular teeth showed significantly earlier development than maxillary teeth in both the case and control groups. There was no significant difference in the development of permanent second molars between sexes. Conclusion: Children with CLP presented delay in the development of permanent second molars

    Qual conteúdo é abordado em um concurso público em odontologia? / What is required in dentistry area in public test?

    O concurso público é uma das melhores formas de inserção do cirurgião-dentista na Estratégia Saúde da Família (ESF) e em outros serviços de saúde pública. O objetivo deste estudo foi verificar o perfil de provas de concursos públicos de Odontologia no estado da Paraíba, bem como analisar a coerência entre a proposta da ESF e o conteúdo da seleção de profissionais para este cargo. Os exames foram recuperados através de pesquisa em sites bastante acessados que disponibilizam provas de concursos no Brasil, PCI Concursos e Folha Dirigida, como também em sites de empresas de maior representatividade nas licitações de concurso na Paraíba. Os resultados apontaram que no período de 2007 a 2015, 94 dos 223 municípios da Paraíba (42,0%), disponibilizaram provas para o cargo de cirurgião dentista. Para análise dos dados foram criadas três categorias: QG,  para os quesitos que englobam as questões gerais presentes nas provas (44%). QT, questões técnicas individuais que equivaleram a 73% dos quesitos relacionados aos conhecimentos específicos e QC, correpondendo a 27% dos quesitos relacionados ao caráter coletivo. Dentre as áreas que envolveram os quesitos técnicos individuais, a Dentística foi a de maior prevalência com 23,4%, seguida de Biossegurança (19,2%) e Cirurgia (11,9%). Conclui-se que os concursos para os cirurgiões dentistas que querem atuar na ESF na Paraíba, abordam em sua maioria conteúdos de especialidades clínicas e apresentam escassez de questões coletivas. O conteúdo das provas destes concursos é de extrema importância para selecionar o perfil do profissional para atuar na ESF.

    Ensino de emergências médicas para o curso de odontologia na região nordeste do Brasil / Teaching medical emergencies for the dentistry course in the northeast region of Brazil

    O objetivo desse trabalho é verificar a presença da disciplina de Emergências Médicas na grade curricular dos cursos de odontologia nas universidades da região Nordeste do Brasil. Este estudo foi do tipo transversal e descritivo, adotando como estratégia de coleta de dados a consulta de dados disponíveis no site do Ministério da Educação (MEC) e no Conselho Federal de Odontologia (CFO), por meio da visita dos sites dos cursos de odontologia nas universidades da região Nordeste do Brasil. O universo foi composto por 95 universidades, todavia após a aplicação dos critérios de inclusão e exclusão foi obtida uma amostra de 86 faculdades que dispuseram a disciplina na grade curricular nos sites correspondentes. Assim, das 86 faculdades consultadas, apenas 14 (16,27%) apresentaram a disciplina de Emergências Médicas, sendo 7 em universidade particular e as outras 7 em universidade pública. As outras 72 universidades (83,72%) não apresentaram a disciplina. Mediante aos resultados obtidos, pode-se concluir que foram encontrados um número mínimo de faculdades que oferecem a disciplina de emergências médicas em odontologia

    Pediatric dentistry content in civil service examinations in the state of paraíba: how is this specialty being addressed?

    This research study aimed at evaluating the content related to pediatric dentistry addressed in civil service examinations in the state of Paraíba through the analysis and classification of questions. Questions related to pediatric dentistry addressed in civil service examinations for the dentist position in the Family Health Program, held in the state of Paraíba, between the years 2001 and 2017 were evaluated. Across the state, 107 civil service examinations were identified and, when applying the exclusion criteria, 9 identical tests were found, consequently, being removed, totaling 98 examinations. After the analysis, 3,949 questions were obtained and only 54 questions were related to pediatric dentistry, representing 1.36% of total questions. The questions were classified according to the topic and the cognitive level according to the BLOOM's revised taxonomy. Results showed that topics related to oral rehabilitation and surgical management, including questions regarding anesthesia, surgery, pulp therapy, endodontic treatment, operative and restorative dentistry, dental materials, traumatic injuries and prosthesis/rehabilitation corresponded to 42% of the questions. The most prevalent cognitive domains were recall and analyze, corresponding to 45% and 29%, respectively. Thus, questions concerning oral rehabilitation and surgical management were the most addressed in civil service examinations, in which the domains remember and analyze were predominant.     Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050. O evento foi realizado em São Paulo, entre 19 e 21 de setembro de 2013, a partir de uma parceria entre o NCE/USP - Núcleo de Comunicação e Educação da USP, a Licenciatura em Educomunicação da ECA/USP, a ABPEducom – Associação Brasileira de Pesquisadores e Profissionais da Educomunicação e a FAPCOM – Faculdade Paulus de Tecnologia e Comunicação, que ofereceu seu campus, na Vila Mariana, para os atos do evento.     Search for dark matter produced in association with bottom or top quarks in √s = 13 TeV pp collisions with the ATLAS detector

    A search for weakly interacting massive particle dark matter produced in association with bottom or top quarks is presented. Final states containing third-generation quarks and miss- ing transverse momentum are considered. The analysis uses 36.1 fb−1 of proton–proton collision data recorded by the ATLAS experiment at √s = 13 TeV in 2015 and 2016. No significant excess of events above the estimated backgrounds is observed. The results are in- terpreted in the framework of simplified models of spin-0 dark-matter mediators. For colour- neutral spin-0 mediators produced in association with top quarks and decaying into a pair of dark-matter particles, mediator masses below 50 GeV are excluded assuming a dark-matter candidate mass of 1 GeV and unitary couplings. For scalar and pseudoscalar mediators produced in association with bottom quarks, the search sets limits on the production cross- section of 300 times the predicted rate for mediators with masses between 10 and 50 GeV and assuming a dark-matter mass of 1 GeV and unitary coupling. Constraints on colour- charged scalar simplified models are also presented. Assuming a dark-matter particle mass of 35 GeV, mediator particles with mass below 1.1 TeV are excluded for couplings yielding a dark-matter relic density consistent with measurements

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung’s disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung’s disease) from 264 hospitals (89 in high-income countries, 166 in middleincome countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in lowincome countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. Interpretation Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between lowincome, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030
