    Aspectos Epidemiológicos Del Cáncer De Hígado en Costa Rica Años 1990-2014

    Today, liver cancer involvement has become more relevant, as globally and more specifically in the Americas, there has been an increase in liver cancer, which is not inconsiderable as a cause of global mortality. Liver cancer represents a pathology that affects various population groups worldwide, with hepatocellular carcinoma being the most common. While it is true that liver cancer was not included in the main cancers with the highest incidence in Costa Rica, primary liver cancer is said to be a major health problem worldwide. In Costa Rica during the period 1990 to 2014, there was a greater involvement by liver cancer in men, the hepatitis C virus presented the highest mortality rate as a cause of this condition, followed by liquor consumption and other causes. The age group with the highest mortality rate was the population over the age of 70. The overall mortality rate showed a noticeable decrease; however, compared to other Latin American countries the rate in Costa Rica is higher, similar to that of the United States. The epidemiological characteristics analyzed showed in their rates a tendency to decline. Both incidence rate and prevalence over the 25 years of study showed higher male than female numbers.En la actualidad la afectación por cáncer de hígado ha adquirido una mayor relevancia, ya que a nivel mundial y más específicamente en el Continente Americano, se ha notado un incremento en dicha patología, la cual llega a ocupar un lugar no despreciable como causa de mortalidad a nivel mundial. El cáncer de hígado representa una patología que afecta diversos grupos poblacionales a nivel mundial, siendo el carcinoma hepatocelular el más frecuente. Si bien es cierto que el cáncer de hígado no estaba incluido dentro de los principales tipos de cáncer con mayor incidencia en Costa Rica, se dice que a nivel mundial el cáncer de hígado primario es un problema de salud importante. En Costa Rica durante el período de 1990 al 2014, se presentó una mayor afectación por cáncer de hígado en los hombres, el virus de la hepatitis C presentó la tasa más alta de mortalidad como causa de este padecimiento, seguido de consumo de licor y otras causas. El grupo etario con la mayor tasa de mortalidad fue la población mayor de 70 años. La tasa general de mortalidad mostró una notoria disminución; sin embargo, en comparación con otros países latinoamericanos la tasa en Costa Rica es superior, siendo similar a la de Estados Unidos. Las características epidemiológicas analizadas, mostraron en sus tasas una tendencia a la disminución. Tanto la tasa de incidencia como de prevalencia a lo largo de los 25 años de estudio mostraron cifras mayores en el sexo masculino que en el sexo femenino

    Effectiveness of Fosfomycin for the Treatment of Multidrug-Resistant Escherichia coli Bacteremic Urinary Tract Infections

    IMPORTANCE The consumption of broad-spectrum drugs has increased as a consequence of the spread of multidrug-resistant (MDR) Escherichia coli. Finding alternatives for these infections is critical, for which some neglected drugs may be an option. OBJECTIVE To determine whether fosfomycin is noninferior to ceftriaxone or meropenem in the targeted treatment of bacteremic urinary tract infections (bUTIs) due to MDR E coli. DESIGN, SETTING, AND PARTICIPANTS This multicenter, randomized, pragmatic, open clinical trial was conducted at 22 Spanish hospitals from June 2014 to December 2018. Eligible participants were adult patients with bacteremic urinary tract infections due to MDR E coli; 161 of 1578 screened patients were randomized and followed up for 60 days. Data were analyzed in May 2021. INTERVENTIONS Patients were randomized 1 to 1 to receive intravenous fosfomycin disodium at 4 g every 6 hours (70 participants) or a comparator (ceftriaxone or meropenem if resistant; 73 participants) with the option to switch to oral fosfomycin trometamol for the fosfomycin group or an active oral drug or pa renteral ertapenem for the comparator group after 4 days. MAIN OUTCOMES AND MEASURES The primary outcome was clinical and microbiological cure (CMC) 5 to 7 days after finalization of treatment; a noninferiority margin of 7% was considered. RESULTS Among 143 patients in the modified intention-to-treat population (median [IQR] age, 72 [62-81] years; 73 [51.0%] women), 48 of 70 patients (68.6%) treated with fosfomycin and 57 of 73 patients (78.1%) treated with comparators reached CMC (risk difference, -9.4 percentage points; 1-sided 95% CI, -21.5 to infinity percentage points; P = .10). While clinical or microbiological failure occurred among 10 patients (14.3%) treated with fosfomycin and 14 patients (19.7%) treated with comparators (risk difference, -5.4 percentage points; 1-sided 95% CI. -infinity to 4.9; percentage points; P = .19), an increased rate of adverse event-related discontinuations occurred with fosfomycin vs comparators (6 discontinuations [8.5%] vs 0 discontinuations; P = .006). In an exploratory analysis among a subset of 38 patients who underwent rectal colonization studies, patients treated with fosfomycin acquired a new ceftriaxone-resistant or meropenem-resistant gram-negative bacteria at a decreased rate compared with patients treated with comparators (0 of 21 patients vs 4 of 17 patients [23.5%]; 1-sided P = .01). CONCLUSIONS AND RELEVANCE This study found that fosfomycin did not demonstrate noninferiority to comparators as targeted treatment of bUTI from MDR E coli; this was due to an increased rate of adverse event-related discontinuations. This finding suggests that fosfomycin may be considered for selected patients with these infections

    Explicar cómo y en qué condiciones fueron construidos algunos de los instrumentos científicos más emblemáticos del mundo moderno sigue siendo un desafío para los historiadores de la ciencia y de la tecnología. Este principio afecta también a los instrumentos náuticos. Así, el objetivo del presente artículo es considerar el “cómo” y el “en qué condiciones”, en un sentido amplio, analizando uno de estos artefactos: las cartas náuticas construidas durante la expansión marítima europea. Originalidad: La originalidad del texto no reside únicamente en llevar a cabo una reconstrucción de este instrumento en Portugal y España que aporte nuevos datos sobre su proceso de fabricación y sus fabricantes, sino también en arrojar luz sobre antiguos debates historiográficos que subyacen a la génesis de la ciencia moderna, tales como la difícil colaboración entre hombres prácticos y hombres teóricos; la aportación de comunidades de artesanos al mundo del conocimiento; el papel que los saberes prácticos tuvieron en el establecimiento de nuevas formas de estandarización en ciencia; la adopción de una metodología empírica para el dominio del mundo o la construcción de grandes imperios globales cimentados sobre la base del trabajo manual. Metodología: El análisis de estos debates requiere una aproximación metodológica que atienda a las categorías y los conceptos principales puestos en circulación tanto por la historia como por la filosofía de la ciencia más reciente. En definitiva, se trata de atender a las condiciones que permitieron el establecimiento de acuerdos (y también de controversias) epistemológicos entre diferentes comunidades de conocimiento tomando un instrumento náutico como “mediador”. Conclusiones: El artículo concluye que la consideración de las culturas empíricas del conocimiento y sus realizaciones materiales son una condición sine qua non para entender la génesis de la modernidad científica europea y la construcción de un mundo globalExplaining how and under what conditions some of the most emblematic scientific instruments of the modern world were built remains a challenge for historians of science and technology. This principle also affects nautical instruments. Thus, the objective of this article is to consider the “how” and the “under what conditions”, in a broad sense, by examining one of these devices: the nautical charts developed during the European maritime expansion. Originality: The originality of this article lies not only in the reconstruction of this instrument in Portugal and Spain, providing new information about its development process and its manufacturers, but also in shedding light on old historiographical debates that underlie the genesis of modern science, such as the difficult collaboration between practical men and theoretical men; the contribution of artisan communities to the world of knowledge; the role that practical knowledge played in the establishment of new forms of standardization in science; the adoption of an empirical methodology for achieving mastery of the world or the construction of large global empires founded upon manual labor. Methodology: Analyzing these debates requires a methodological approach that addresses the main categories and concepts put into circulation by the most recent history and philosophy of science. In short, unveiling the conditions that enabled the establishment of epistemological agreements (and also disputes) between different knowledge communities by taking a nautical instrument as “mediator.” Conclusions: The article concludes that examining empirical knowledge cultures and their material achievements are a sine qua non condition to understand the genesis of European scientific modernity and the construction of a global worldExplicar como e em que condições foram construídos alguns dos instrumentos científicos mais emblemáticos do mundo moderno continua sendo um desafio para os historiadores da ciência e da tecnologia.     Memoria del II Coloquio Internacional sobre Diversidad Cultural y Estudios Regionales

    Desde la Sede de Occidente y, específicamente, en el seno de la Coordinación de Investigación y con el apoyo de la Dirección de la Sede y las Coordinaciones de Docencia, Administración y Acción Social, así como de la Vicerrectoría de Investigación, se han celebrado en el 2011 y en el 2012 dos coloquios internacionales sobre diversidad cultural y estudios regionales. El propósito de ambos consistió principalmente en motivar a investigadores e investigadoras de las distintas unidades académicas de la Universidad de Costa Rica y de otras instituciones de Educación Superior, así como a representantes de Centros e Institutos de Investigación nacionales e internacionales, a presentar resultados de investigaciones que contribuyeran a un mayor conocimiento de los procesos culturales y que dieran a conocer resultados que permitieran la comprensión de las realidades de distintos sectores y regiones.     Risk portfolio on modern finance has become increasingly technical, requiring the use of sophisticated mathematical tools in both research and practice. Since companies cannot insure themselves completely against risk, as human incompetence in predicting the future precisely that written in Al-Quran surah Luqman verse 34, they have to manage it to yield an optimal portfolio. The objective here is to minimize the variance among all portfolios, or alternatively, to maximize expected return among all portfolios that has at least a certain expected return. Furthermore, this study focuses on optimizing risk portfolio so called Markowitz MVO (Mean-Variance Optimization). Some theoretical frameworks for analysis are arithmetic mean, geometric mean, variance, covariance, linear programming, and quadratic programming. Moreover, finding a minimum variance portfolio produces a convex quadratic programming, that is minimizing the objective function ðð¥with constraintsð ð 𥠥 ðandð´ð¥ = ð. The outcome of this research is the solution of optimal risk portofolio in some investments that could be finished smoothly using MATLAB R2007b software together with its graphic analysis

    Impacts of the Tropical Pacific/Indian Oceans on the Seasonal Cycle of the West African Monsoon

    The current consensus is that drought has developed in the Sahel during the second half of the twentieth century as a result of remote effects of oceanic anomalies amplified by local land–atmosphere interactions. This paper focuses on the impacts of oceanic anomalies upon West African climate and specifically aims to identify those from SST anomalies in the Pacific/Indian Oceans during spring and summer seasons, when they were significant. Idealized sensitivity experiments are performed with four atmospheric general circulation models (AGCMs). The prescribed SST patterns used in the AGCMs are based on the leading mode of covariability between SST anomalies over the Pacific/Indian Oceans and summer rainfall over West Africa. The results show that such oceanic anomalies in the Pacific/Indian Ocean lead to a northward shift of an anomalous dry belt from the Gulf of Guinea to the Sahel as the season advances. In the Sahel, the magnitude of rainfall anomalies is comparable to that obtained by other authors using SST anomalies confined to the proximity of the Atlantic Ocean. The mechanism connecting the Pacific/Indian SST anomalies with West African rainfall has a strong seasonal cycle. In spring (May and June), anomalous subsidence develops over both the Maritime Continent and the equatorial Atlantic in response to the enhanced equatorial heating. Precipitation increases over continental West Africa in association with stronger zonal convergence of moisture. In addition, precipitation decreases over the Gulf of Guinea. During the monsoon peak (July and August), the SST anomalies move westward over the equatorial Pacific and the two regions where subsidence occurred earlier in the seasons merge over West Africa. The monsoon weakens and rainfall decreases over the Sahel, especially in August.Peer reviewe

    Search for heavy resonances decaying to two Higgs bosons in final states containing four b quarks

    A search is presented for narrow heavy resonances X decaying into pairs of Higgs bosons (H) in proton-proton collisions collected by the CMS experiment at the LHC at root s = 8 TeV. The data correspond to an integrated luminosity of 19.7 fb(-1). The search considers HH resonances with masses between 1 and 3 TeV, having final states of two b quark pairs. Each Higgs boson is produced with large momentum, and the hadronization products of the pair of b quarks can usually be reconstructed as single large jets. The background from multijet and t (t) over bar events is significantly reduced by applying requirements related to the flavor of the jet, its mass, and its substructure. The signal would be identified as a peak on top of the dijet invariant mass spectrum of the remaining background events. No evidence is observed for such a signal. Upper limits obtained at 95 confidence level for the product of the production cross section and branching fraction sigma(gg -> X) B(X -> HH -> b (b) over barb (b) over bar) range from 10 to 1.5 fb for the mass of X from 1.15 to 2.0 TeV, significantly extending previous searches. For a warped extra dimension theory with amass scale Lambda(R) = 1 TeV, the data exclude radion scalar masses between 1.15 and 1.55 TeV

    Search for Physics beyond the Standard Model in Events with Overlapping Photons and Jets

    Results are reported from a search for new particles that decay into a photon and two gluons, in events with jets. Novel jet substructure techniques are developed that allow photons to be identified in an environment densely populated with hadrons. The analyzed proton-proton collision data were collected by the CMS experiment at the LHC, in 2016 at root s = 13 TeV, and correspond to an integrated luminosity of 35.9 fb(-1). The spectra of total transverse hadronic energy of candidate events are examined for deviations from the standard model predictions. No statistically significant excess is observed over the expected background. The first cross section limits on new physics processes resulting in such events are set. The results are interpreted as upper limits on the rate of gluino pair production, utilizing a simplified stealth supersymmetry model. The excluded gluino masses extend up to 1.7 TeV, for a neutralino mass of 200 GeV and exceed previous mass constraints set by analyses targeting events with isolated photons.Peer reviewe
