Rapid testing may not improve uptake of HIV testing and same day results in a rural South African community: a cohort study of 12,000 women
<p>Background: Rapid testing of pregnant women aims to increase uptake of HIV testing and results and thus optimize care. We report on the acceptability of HIV counselling and testing, and uptake of results, before and after the introduction of rapid testing in this area.</p>
<p>Methods and Principal Findings: HIV counsellors offered counselling and testing to women attending 8 antenatal clinics, prior to enrolment into a study examining infant feeding and postnatal HIV transmission. From August 2001 to April 2003, blood was sent for HIV ELISA testing in line with the Prevention of Mother-to-Child Transmission (PMTCT) programme in the district. From May 2003 to September 2004 women were offered a rapid HIV test as part of the PMTCT programme, but also continued to have ELISA testing for study purposes. Of 12,323 women counselled, 5,879 attended clinic prior to May 2003, and 6,444 after May 2003 when rapid testing was introduced; of whom 4,324 (74.6%) and 4,810 (74.6%) agreed to have an HIV test respectively. Of the 4,810 women who had a rapid HIV test, only 166 (3.4%) requested to receive their results on the same day as testing, the remainder opted to return for results at a later appointment. Women with secondary school education were less likely to agree to testing than those with no education (AOR 0.648, p<0.001), as were women aged 21â35 (AOR 0.762, p<0.001) and >35 years (AOR 0.756, p<0.01) compared to those <20 years.</p>
<p>Conclusions: Contrary to other reports, few women who had rapid tests accepted their HIV results the same day. Finding strategies to increase the proportion of pregnant women knowing their HIV results is critical so that appropriate care can be given.</p>
Effects of Cynodon dactylon on Stress-Induced Infertility in Male Rats
Cynodon dactylon (Family: Poaceae) is known to be a tackler in Indian mythology and is offered to Lord Ganesha. It is found everywhere, even on waste land, road side, dry places, and spreads vigorously on cultivated ground. This study was carried out with an objective to test if the constituents of this plant are useful in coping stress-induced sexual In this study, we considered immobilization stress to induce male infertility and the effect of C. dactylon in restoration of the dysfunction was evaluated by considering sexual behavioral observations, sexual performance, fructose content of the seminal vesicles, epididymal sperm concentration and histopathological examinations as parameters. Treatment of rats under stress with methanolic extract of C. dactylon has shown a promising effect in overcoming stress-induced sexual dysfunction, sexual performance, fructose content, sperm concentration and its effect on accessory sexual organs and body weight. We conclude that active constituents of C. dactylon present in methanolic extract have a potent aphrodisiac and male fertility activity
PIK3CA mutation in HPV-associated OPSCC patients receiving deintensified chemoradiation
PIK3CA is the most frequently mutated gene in human papillomavirus (HPV)-associated oropharyngeal squamous cell carcinoma (OPSCC). Prognostic implications of such mutations remain unknown. We sought to elucidate the clinical significance of PIK3CA mutations in HPV-associated OPSCC patients treated with definitive chemoradiation (CRT). Seventyseven patients with HPV-associated OPSCC were enrolled on two phase II clinical trials of deintensified CRT (60 Gy intensitymodulated radiotherapy with concurrent weekly cisplatin). Targeted next-generation sequencing was performed. Of the 77 patients, nine had disease recurrence (two regional, four distant, three regional and distant). Thirty-four patients had mutation( s) identified; 16 had PIK3CA mutations. Patients with wild-type-PIK3CA had statistically significantly higher 3-year disease-free survival than PIK3CA-mutant patients (93.4%, 95% confidence interval [CI] = 85.0% to 99.9% vs 68.8%, 95% CI = 26.7% to 89.8%; P=.004). On multivariate analysis, PIK3CA mutation was the only variable statistically significantly associated with disease recurrence (hazard ratio = 5.71, 95% CI = 1.53 to 21.3; P=.01). PIK3CA mutation is associated with worse diseasefree survival in a prospective cohort of newly diagnosed HPV-associated OPSCC patients treated with deintensified CRT
Myocardial fibrosis in asymptomatic and symptomatic chronic severe primary mitral regurgitation and relationship to tissue characterisation and left ventricular function on cardiovascular magnetic resonance
Background: Myocardial fbrosis occurs in end-stage heart failure secondary to mitral regurgitation (MR), but it is not
known whether this is present before onset of symptoms or myocardial dysfunction. This study aimed to characterise
myocardial fbrosis in chronic severe primary MR on histology, compare this to tissue characterisation on cardiovascular magnetic resonance (CMR) imaging, and investigate associations with symptoms, left ventricular (LV) function, and
exercise capacity.
Methods: Patients with class I or IIa indications for surgery underwent CMR and cardiopulmonary exercise testing. LV
biopsies were taken at surgery and the extent of fbrosis was quantifed on histology using collagen volume fraction
(CVFmean) compared to autopsy controls without cardiac pathology.
Results: 120 consecutive patients (64±13 years; 71% male) were recruited; 105 patients underwent MV repair
while 15 chose conservative management. LV biopsies were obtained in 86 patients (234 biopsy samples in total).
MR patients had more fbrosis compared to 8 autopsy controls (median: 14.6% [interquartile range 7.4â20.3] vs. 3.3%
[2.6â6.1], P<0.001); this diference persisted in the asymptomatic patients (CVFmean 13.6% [6.3â18.8], P<0.001), but
severity of fbrosis was not signifcantly higher in NYHA II-III symptomatic MR (CVFmean 15.7% [9.9â23.1] (P=0.083).
Fibrosis was patchy across biopsy sites (intraclass correlation 0.23, 95% CI 0.08â0.39, P=0.001). No signifcant relationships were identifed between CVFmean and CMR tissue characterisation [native T1, extracellular volume (ECV)
or late gadolinium enhancement] or measures of LV function [LV ejection fraction (LVEF), global longitudinal strain
(GLS)]. Although the range of ECV was small (27.3±3.2%), ECV correlated with multiple measures of LV function (LVEF:
Rho=â0.22, P=0.029, GLS: Rho=0.29, P=0.003), as well as NTproBNP (Rho=0.54, P<0.001) and exercise capacity
(%PredVO2max: R=â0.22, P=0.030). Conclusions: Patients with chronic primary MR have increased fbrosis before the onset of symptoms. Due to the
patchy nature of fbrosis, CMR derived ECV may be a better marker of global myocardial status.

A Solvable Regime of Disorder and Interactions in Ballistic Nanostructures, Part I: Consequences for Coulomb Blockade
We provide a framework for analyzing the problem of interacting electrons in
a ballistic quantum dot with chaotic boundary conditions within an energy
(the Thouless energy) of the Fermi energy. Within this window we show that the
interactions can be characterized by Landau Fermi liquid parameters. When ,
the dimensionless conductance of the dot, is large, we find that the disordered
interacting problem can be solved in a saddle-point approximation which becomes
exact as (as in a large-N theory). The infinite theory shows a
transition to a strong-coupling phase characterized by the same order parameter
as in the Pomeranchuk transition in clean systems (a spontaneous
interaction-induced Fermi surface distortion), but smeared and pinned by
disorder. At finite , the two phases and critical point evolve into three
regimes in the plane -- weak- and strong-coupling regimes separated
by crossover lines from a quantum-critical regime controlled by the quantum
critical point. In the strong-coupling and quantum-critical regions, the
quasiparticle acquires a width of the same order as the level spacing
within a few 's of the Fermi energy due to coupling to collective
excitations. In the strong coupling regime if is odd, the dot will (if
isolated) cross over from the orthogonal to unitary ensemble for an
exponentially small external flux, or will (if strongly coupled to leads) break
time-reversal symmetry spontaneously.Comment: 33 pages, 14 figures. Very minor changes. We have clarified that we

at (anti-)neutrino energies below 1.5
GeV. We determine the single momentum bin cross section measurements, averaged
over the T2K -flux, for the detector target material (mainly
Carbon, Oxygen, Hydrogen and Copper) with phase space restricted laboratory
frame kinematics of 500 MeV/c. The

An Integrated TCGA Pan-Cancer Clinical Data Resource to Drive High-Quality Survival Outcome Analytics
For a decade, The Cancer Genome Atlas (TCGA) program collected clinicopathologic annotation data along with multi-platform molecular profiles of more than 11,000 human tumors across 33 different cancer types. TCGA clinical data contain key features representing the democratized nature of the data collection process. To ensure proper use of this large clinical dataset associated with genomic features, we developed a standardized dataset named the TCGA Pan-Cancer Clinical Data Resource (TCGA-CDR), which includes four major clinical outcome endpoints. In addition to detailing major challenges and statistical limitations encountered during the effort of integrating the acquired clinical data, we present a summary that includes endpoint usage recommendations for each cancer type. These TCGA-CDR findings appear to be consistent with cancer genomics studies independent of the TCGA effort and provide opportunities for investigating cancer biology using clinical correlates at an unprecedented scale. Analysis of clinicopathologic annotations for over 11,000 cancer patients in the TCGA program leads to the generation of TCGA Clinical Data Resource, which provides recommendations of clinical outcome endpoint usage for 33 cancer types
Search for direct production of charginos and neutralinos in events with three leptons and missing transverse momentum in âs = 7 TeV pp collisions with the ATLAS detector
A search for the direct production of charginos and neutralinos in final states with three electrons or muons and missing transverse momentum is presented. The analysis is based on 4.7 fbâ1 of protonâproton collision data delivered by the Large Hadron Collider and recorded with the ATLAS detector. Observations are consistent with Standard Model expectations in three signal regions that are either depleted or enriched in Z-boson decays. Upper limits at 95% confidence level are set in R-parity conserving phenomenological minimal supersymmetric models and in simplified models, significantly extending previous results
Jet size dependence of single jet suppression in lead-lead collisions at sqrt(s(NN)) = 2.76 TeV with the ATLAS detector at the LHC
Measurements of inclusive jet suppression in heavy ion collisions at the LHC
provide direct sensitivity to the physics of jet quenching. In a sample of
lead-lead collisions at sqrt(s) = 2.76 TeV corresponding to an integrated
luminosity of approximately 7 inverse microbarns, ATLAS has measured jets with
a calorimeter over the pseudorapidity interval |eta| < 2.1 and over the
transverse momentum range 38 < pT < 210 GeV. Jets were reconstructed using the
anti-kt algorithm with values for the distance parameter that determines the
nominal jet radius of R = 0.2, 0.3, 0.4 and 0.5. The centrality dependence of
the jet yield is characterized by the jet "central-to-peripheral ratio," Rcp.
Jet production is found to be suppressed by approximately a factor of two in
the 10% most central collisions relative to peripheral collisions. Rcp varies
smoothly with centrality as characterized by the number of participating
nucleons. The observed suppression is only weakly dependent on jet radius and
transverse momentum. These results provide the first direct measurement of
inclusive jet suppression in heavy ion collisions and complement previous

Search for Lorentz and CPT violation using sidereal time dependence of neutrino flavor transitions over a short baseline
A class of extensions of the Standard Model allows Lorentz and CPT violations, which can be identified
by the observation of sidereal modulations in the neutrino interaction rate. A search for such modulations
was performed using the T2K on-axis near detector. Two complementary methods were used in this study,
both of which resulted in no evidence of a signal. Limits on associated Lorentz and CPT-violating terms
from the Standard Model extension have been derived by taking into account their correlations in this
model for the first time. These results imply such symmetry violations are suppressed by a factor of more
than 10 20 at the GeV scale
