    Estudio para formular el plan estratégico de seguridad vial para el instituto de tránsito y transporte de Sogamoso “INTRASOG”

    Con el desarrollo del presente plan estratégico de seguridad vial para el Instituto de Tránsito y Transporte de Sogamoso “INTRASOG” a partir del análisis de los diferentes elementos inmersos en el accidente de tránsito se pretenden detectar posibles conflictos de seguridad vial que conlleven a fortalecer la gestión institucional, en un enfoque hacia la cultura de la seguridad vial y un medio ambiente sano para los funcionarios de la Institución. “INTRASOG” dando cumplimiento a la ley, diseña una propuesta de mejora en la seguridad vial a partir de cinco enfoques, orientados al análisis sistémico de la prevención y la planeación de acciones, mecanismos, estrategias y medidas que deben hacer parte de la organización. Dichas acciones deben estar encaminadas a propiciar la seguridad vial como algo inherente al ser humano. Este estudio va encaminado a concretar objetivos y acciones específicas que se deben conducir para determinar unos propósitos en la prevención de los problemas de inseguridad vial, facilitando la administración y la gestión de la entidad. De la misma forma, se pretende definir los funcionarios involucrados, los responsables y las acciones; así como los mecanismos de evaluación, control y vigilancia para poder alcanzar los propósitos definidos en cumplimiento de este plan estratégico de seguridad vial. Lo anterior con el fin de trazar una ruta que indique los resultados que INTRASOG como órgano rector de transito se ha propuesto en materia de seguridad vial. Cuando se habla de la cultura vial, debe integrasen factores como la sicología del comportamiento, la actitud del manejo, la sociología de grupo, la disposición física, el nivel cultural y otros tantos factores que inciden en el proceso de la conducción que deben terminar en una autónoma actitud del comportamiento vial para de esa manera demostrar una autentica cultura ciudadana, en el uso seguro de las vías.Starting from the analysis of the different elements involved in the accident on, the Institute of Traffic and Transportation of Sogamoso "INTRASOG" develops road safety strategic plan intending to detect possible conflicts of road safety which leads to strengthen corporate management, aiming to encourage road safety culture and a healthy environment for members of the institution. In compliance with law, "INTRASOG" designs an improvement proposal for road safety from five approaches on, oriented to the systemic analysis of prevention and planning of actions, mechanisms, strategies and measures that should be part of the organization to encourage road safety as something inherent to the human behavior. In facilitation of the entity management, this study is getting at establishing specific objectives and actions that must be taken to determine the purposes in the prevention of problems of road insecurity. Likewise, it is intended to define the officials involved, along with actions; as well as the evaluation, control and monitoring mechanisms to achieve defined purposes in compliance with this strategic road safety plan. The aforementioned in order to set a route indicating the results that INTRASOG as the traffic governing body has accomplished in terms of road safety. In terms of road culture, factors such as the psychology of behavior, the attitude when driving, sociology group, physical layout, cultural level and many other factors affecting the process of driving should be involved; to end up with an autonomous attitude of road behavior to demonstrate a genuine civic culture, in using roads safely

    Benefits of Coffee and Therapy for Chronic Pain in Patients Diagnosed with Cancer

    El objetivo de este estudio fue determinar mediante una revisión integradora de literatura los beneficios del café y su empleabilidad como terapia para el dolor crónico en pacientes con diagnóstico de cáncer. Materiales y método: Se utilizaron las bases de datos: PubMed, ELSERVIER, Scite, Cochrane Library, Bvs LIbrary, para la adquisición de los artículos y fueron consultadas durante el periodo de marzo de 2021 a mayo de 2022 encontrando 2336 artículos  de los cuales se escogieron 24 que  cumplieron con los criterios de inclusión, a estos artículos se les aplicó la herramienta de evaluación crítica de artículos CASPE según el tipo de articulo, Resultados :  Primero se hizo una caracterización de los estudios encontrando que según el enfoque la mayoría fueron cuantitativos 79% y cualitativos 29%, se categorizaron según nivel de evidencia y grados de recomendación ,  según el país de origen un 32%  fueron realizados en EE. UU , se analizaron las profesiones de los investigadores siendo el grupo más grande con un 17%  por médicos , los hallazgos se agruparon en 5 categorías según los objetivos específicos del estudio : Describir las sustancias protectoras que tienen el café para prevención de cáncer, Identificar el mecanismo de acción de las propiedades del café para el alivio del dolor crónico, Describir los beneficios del café en pacientes con diagnóstico de cáncer , Relacionar la dosis de café requerida para generar un efecto protector para cáncer e inhibitorio del dolor  y Proponer un plan de terapias alternativas con el uso del café para el manejo del dolor en pacientes con cáncer   . Conclusiones: Los beneficios del café van a depender del tipo de fruto, preparación, procesamiento y adición de azúcar u otras sustancias procesadas, contiene múltiples sustancias beneficiosas para la salud  y sus  mayores beneficios se obtienen al consumir la bebida sin azúcar o ninguna otra sustancia añadida, los componentes del café tales como el Cafestol y Kaweol  son las moléculas  a las cuales se les atribuye principalmente  propiedades protectoras antineoplásicas y de aumento de sobrevida en enfermedad metastásica , el contenido de cafeína del café puede ayudar a mejorar el umbral del dolor y tolerancia del calor siempre y cuando las tazas bebidas de éste contenga la dosis de cafeína entre 100-200 mg.The objective of this study was to determine, through an integrative review of the literature, the benefits of coffee and its applicability as a therapy for chronic pain in patients diagnosed with cancer. Materials and method: The following databases were used: PubMed, ELSERVIER, Scite, Cochrane Library, Bvs LIbrary, for the acquisition of the articles and were consulted during the period from March 2021 to May 2022 finding 2336 articles of which 24 were chosen that met the inclusion criteria, to these articles the critical evaluation tool of articles CASPE was applied according to the type of article, Results : First a characterization of the studies was made finding that according to the approach the majority were quantitative 79% and qualitative 29% were categorized according to the level of evidence and degrees of recommendation, according to the country of origin, 32% were carried out in the USA. The findings were grouped into 5 categories according to the specific objectives of the study: To describe the protective substances that coffee has for cancer prevention, To identify the mechanism of action of the properties of coffee for the relief of chronic pain, To describe the benefits of coffee in patients diagnosed with cancer, To relate the dose of coffee required to generate a protective effect for cancer and inhibitory pain, and To propose a plan of alternative therapies with the use of coffee for pain management in cancer patients. Conclusions: The benefits of coffee are going to depend on the type of fruit, preparation, processing and addition of sugar or other processed substances, it contains multiple beneficial substances for health and its greatest benefits are obtained when consuming the beverage without sugar or any other added substance, Coffee components such as Cafestol and Kaweol are the molecules to which are mainly attributed antineoplastic protective properties and increased survival in metastatic disease. The caffeine content of coffee can help to improve the pain threshold and heat tolerance as long as the cups drunk contain a dose of caffeine between 100-200 mg

    Reforestation as a Transversal Axis: an Integrative Proposal

    La reforestación ha resultado en la actualidad una estrategia de alto impacto, que incentiva a la protección del medio ambiente, generando conductas proambientales y sostenibles. Por lo anterior, el presente artículo se enfoca en la integración curricular del Eje Temático de Reforestación en la IEDR Rinconada, en concordancia con la especialidad de la institución en Explotaciones Agropecuarias Ecológicas. Se asume un abordaje cualitativo, se tomó como muestra 40 estudiantes de diferentes grados. Dentro de los hallazgos encontrados, existe gran afinidad entre los estudiantes y las actividades ecológicas, en su mayoría manifiestas tener conocimientos sobre las problemáticas medio ambientales que se presentan en su comunidad, han tenido la oportunidad de participar en actividades de reforestación, lo que contribuye a la mejora del entorno. El proyecto tiene un alto nivel de aceptación, sin embargo, es necesario el compromiso de las directivas de la institución y las autoridades locales para obtener resultados significativosReforestation has now resulted in a high impact strategy, which encourages the protection of the environment, generating pro-environmental and sustainable behaviors. Therefore, this article focuses on the curricular integration of the Thematic Area of Reforestation in the IEDR Rinconada, in accordance with the specialty of the institution in Ecological Farming Operations. A qualitative approach is assumed; 40 students of different grades were taken as sample. Among the findings, there is great affinity between the students and the ecological activities, most of them manifest knowledge of the environmental problems that arise in their community, they have had the opportunity to participate in reforestation activities, which contributes to the improvement of the environment. The project has a high level of acceptance, however, it is necessary the commitment of the directives of the institution and local authorities to obtain significant result

    Common variants in Alzheimer’s disease and risk stratification by polygenic risk scores

    Genetic discoveries of Alzheimer’s disease are the drivers of our understanding, and together with polygenetic risk stratification can contribute towards planning of feasible and efficient preventive and curative clinical trials. We first perform a large genetic association study by merging all available case-control datasets and by-proxy study results (discovery n = 409,435 and validation size n = 58,190). Here, we add six variants associated with Alzheimer’s disease risk (near APP, CHRNE, PRKD3/NDUFAF7, PLCG2 and two exonic variants in the SHARPIN gene). Assessment of the polygenic risk score and stratifying by APOE reveal a 4 to 5.5 years difference in median age at onset of Alzheimer’s disease patients in APOE ɛ4 carriers. Because of this study, the underlying mechanisms of APP can be studied to refine the amyloid cascade and the polygenic risk score provides a tool to select individuals at high risk of Alzheimer’s disease.Fil: Dalmasso, Maria Carolina.     Neuropsychiatric profiles and conversion to dementia in mild cognitive impairment, a latent class analysis

Our aim was to determine profiles based on the prominent NPS in MCI patients and to explore the predictive value of these profiles on conversion to specific types of dementia. A total of 2137 MCI patients monitored in a memory clinic were included in the study. Four NPS profiles emerged (classes), which were defined by preeminent symptoms: Irritability, Apathy, Anxiety/Depression and Asymptomatic. Irritability and Apathy were predictors of conversion to dementia (HR = 1.43 and 1.56, respectively). Anxiety/depression class showed no risk effect of conversion when compared to Asymptomatic class. Irritability class appeared as the most discriminant neuropsychiatric condition to identify non-AD converters (i.e., frontotemporal dementia, vascular dementia, Parkinson's disease and dementia with Lewy Bodies). The findings revealed that consistent subgroups of MCI patients could be identified among comorbid basal NPS. The preeminent NPS showed to behave differentially on conversion to dementia, beyond AD. Therefore, NPS should be used as early diagnosis facilitators, and should also guide clinicians to detect patients with different illness trajectories in the progression of MCI

    Long runs of homozygosity are associated with Alzheimer's disease

L.M.R. is supported by Consejería de Salud de la Junta de Andalucía (Grant PI-0001/2017).Long runs of homozygosity (ROH) are contiguous stretches of homozygous genotypes, which are a footprint of inbreeding and recessive inheritance. The presence of recessive loci is suggested for Alzheimer's disease (AD); however, their search has been poorly assessed to date. To investigate homozygosity in AD, here we performed a fine-scale ROH analysis using 10 independent cohorts of European ancestry (11,919 AD cases and 9181 controls.) We detected an increase of homozygosity in AD cases compared to controls [ β (CI 95%) = 0.070 (0.037-0.104); P = 3.91 × 10 −5 ; β (CI95%) = 0.043 (0.009-0.076); P = 0.013]. ROHs increasing the risk of AD (OR > 1) were significantly overrepresented compared to ROHs increasing protection (p < 2.20 × 10 −16). A significant ROH association with AD risk was detected upstream the HS3ST1 locus (chr4:11,189,482‒11,305,456), (β (CI 95%) = 1.09 (0.48 ‒ 1.48), p value = 9.03 × 10 −4), previously related to AD. Next, to search for recessive candidate variants in ROHs, we constructed a homozygosity map of inbred AD cases extracted from an outbred population and explored ROH regions in whole-exome sequencing data (N = 1449). We detected a candidate marker, rs117458494, mapped in the SPON1 locus, which has been previously associated with amyloid metabolism. Here, we provide a research framework to look for recessive variants in AD using outbred populations. Our results showed that AD cases have enriched homozygosity, suggesting that recessive effects may explain a proportion of AD heritability

    Mendelian Randomisation Confirms the Role of Y-Chromosome Loss in Alzheimer’s Disease Aetiopathogenesis in Men

    Mosaic loss of chromosome Y (mLOY) is a common ageing-related somatic event and has been previously associated with Alzheimer’s disease (AD). However, mLOY estimation from genotype microarray data only reflects the mLOY degree of subjects at the moment of DNA sampling. Therefore, mLOY phenotype associations with AD can be severely age-confounded in the context of genome-wide association studies. Here, we applied Mendelian randomisation to construct an age-independent mLOY polygenic risk score (mloy-PRS) using 114 autosomal variants. The mloy-PRS instrument was associated with an 80% increase in mLOY risk per standard deviation unit (p = 4.22 × 10−20) and was orthogonal with age. We found that a higher genetic risk for mLOY was associated with faster progression to AD in men with mild cognitive impairment (hazard ratio (HR) = 1.23, p = 0.01). Importantly, mloy-PRS had no effect on AD conversion or risk in the female group, suggesting that these associations are caused by the inherent loss of the Y chromosome. Additionally, the blood mLOY phenotype in men was associated with increased cerebrospinal fluid levels of total tau and phosphorylated tau181 in subjects with mild cognitive impairment and dementia.     Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     Smoking cessation opportunities in severe mental illness (tobacco intensive motivational and estimate risk — TIMER—): study protocol for a randomized controlled trial

    There is an increased risk of premature death in people with severe mental illness (SMI). Respiratory disorders and cardiovascular disease are leading causes of increased mortality rates in these patients, and tobacco consumption remains the most preventable risk factor involved. Developing new tools to motivate patients towards cessation of smoking is a high priority. Information on the motivational value of giving the lung age and prevention opportunities is unknown in this high-risk population. In the context of community care, screening and early detection of lung damage could potentially be used, together with mobile technology, in order to produce a prevention message, which may provide patients with SMI with a better chance of quitting smoking.This study receives funding by the Spanish Ministry of Economy, Industry and Competitiveness, Instituto Carlos III (FIS PI16/00802)

    Genome-wide meta-analysis for Alzheimer's disease cerebrospinal fluid biomarkers

    Amyloid-beta 42 (A beta 42) and phosphorylated tau (pTau) levels in cerebrospinal fluid (CSF) reflect core features of the pathogenesis of Alzheimer's disease (AD) more directly than clinical diagnosis. Initiated by the European Alzheimer & Dementia Biobank (EADB), the largest collaborative effort on genetics underlying CSF biomarkers was established, including 31 cohorts with a total of 13,116 individuals (discovery n = 8074; replication n = 5042 individuals). Besides the APOE locus, novel associations with two other well-established AD risk loci were observed; CR1 was shown a locus for A beta 42 and BIN1 for pTau. GMNC and C16orf95 were further identified as loci for pTau, of which the latter is novel. Clustering methods exploring the influence of all known AD risk loci on the CSF protein levels, revealed 4 biological categories suggesting multiple A beta 42 and pTau related biological pathways involved in the etiology of AD. In functional follow-up analyses, GMNC and C16orf95 both associated with lateral ventricular volume, implying an overlap in genetic etiology for tau levels and brain ventricular volume.Peer reviewe
