    Friedensförderung in Zeiten von COVID-19: Risikoadaptierte Strategien von Kooperations- und Entwicklungsprojekten

    National and international cooperation and development projects (CDP) are fundamental for peacebuilding. However, unforeseen global crises, like COVID-19, can endanger such projects, requiring rapid adaption. In Colombia, the coronavirus outbreak threatens to slow the implementation of peace-related projects, while simultaneously violence over control and ownership of land increases. Although the mid- to long-term consequences for peacebuilding are unknown, exploring risk-adapted strategies of national and international CDP can help identify crucial aspects for future processes and implementations. This study explores the key challenges and coping strategies of implementing agencies and stakeholders to COVID-19, thus helping to derive and improve risk-adapted strategies. After reviewing academic and grey literature, and implementing a semi-structured survey, peacebuilding risked-adapted strategies to COVID-19 are explored with respect to conflict-affected and vulnerable areas of Colombia. Findings show that increasing complexity rooted in top down governmental measures, the rise of new local power relations (e.g. armed groups, illicit activities), and social alienation are negatively affecting peacebuilding in Colombia. Future CDP risk adapted strategies should build on local interests and needs through public-private and environmental cooperation.Nationale und internationale Kooperations- und Entwicklungsprojekte (CDP) sind für die Friedensförderung von grundlegender Bedeutung. Unvorhergesehene globale Krisen, wie COVID-19, können solche Projekte jedoch gefährden und erfordern eine rasche Anpassung. In Kolumbien droht der Ausbruch des Coronavirus die Durchführung friedensbezogener Projekte zu verlangsamen, während gleichzeitig die Gewalt über die Kontrolle und den Besitz von Land zunimmt. Obwohl die mittel- bis langfristigen Folgen für die Friedenskonsolidierung unbekannt sind, kann die Untersuchung risikoangepasster Strategien des nationalen und internationalen CDP dazu beitragen, entscheidende Aspekte für künftige Prozesse und Implementierungen zu ermitteln. Diese Studie untersucht die Herausforderungen und Bewältigungsstrategien der Durchführungsorganisationen und Interessengruppen im Kontext von COVID-19, umso risikoangepasste Strategien abzuleiten und zu verbessern. Basierend auf Literaturrecherche und der Durchführung semi-strukturierter Interviews werden risikoangepasste Strategien der Friedensförderung für COVID-19 in unterschiedlichen Teilen Kolumbiens untersucht. Die Ergebnisse zeigen, dass die zunehmende Komplexität, die in den Maßnahmen der Regierung von oben nach unten, dem Entstehen neuer lokaler Machtverhältnisse (z. B. bewaffnete Gruppen, illegale Aktivitäten) und in zunehmend sozialer Entfremdung wurzelt, die Friedensförderung in Kolumbien negativ beeinflusst.     Surgical Treatment of Hepatic Hydatidosis

    Hepatic hydatid disease has been reported from ancient times. It is a zoonotic infection caused by nine recognized species of tapeworms of the genus Echinococcus. Some of them are known to cause human echinococcosis, and there is reasonable doubt regarding the others. Diagnosis is supported by epidemiological history, clinical presentation, radiological imaging, and serological tests. Various pathological forms may become life-threatening, and in those cases, the treatment is extremely complex. The main objective of the treatment is to completely cure the illness in order to avoid further complications and recurrences. Hepatic surgery, using different techniques, achieves best results with acceptable morbidity and mortality rates. In the South of Chile, the geographical location in which we work, this illness is endemic with high incidence and prevalence. The hepatobiliopancreatic units of the regional surgical centers of Temuco have plenty of experience treating liver hydatid disease. In this chapter we shall focus especially on epidemiology, etiopathogenesis, diagnosis, and surgical treatment of hepatic hydatidosis

    Validación de la Escala Breve de Optimismo Interactivo-G en México antes de la COVID-19

    The primary objectives of this study were determining the construct, convergent, and discriminant validity of the Brief Interactive Optimism Scale-G (BIOS-G) in participants from six states of Mexico and estimating its internal consistency. In this study 3 289 Mexicans participated (2 028 men and 1 243 women). The average age was = 30.43 years and SD = 10.52. Confirmatory factor analysis (CFA) and multiple regression analysis (MRA) were applied. There were appropriate fit indexes (e. g., CFI = .99; RMSEA = .07.). Convergent validity showed ANR (3 289)=.52(p=<.01;d=medium), with the Satisfaction with Life Scale (SWLS) and the estimation of discriminant validity was r (3 289) r = -.19 with the Brief Scale for Assessing Anger Proneness (APS-G) (p = < .01; d = almost small), Alpha = .70 (3 289); p = < .01; omega = .76. The conclusion is that there is partial national evidence supporting the use of BIOS-G, because Mexico lacks a measure of this kind, being useful, at least, for research purposes.Los objetivos primarios del presente estudio fueron determinar la validez de constructo, convergente y discriminante de la Escala Breve de Optimismo Interactivo-G (EBOI-G), en participantes de seis estados de México y calcular su consistencia interna. Participaron 3289 mexicanos, 2028 hombres y 1243 mujeres (18 casos no contestaron cuál era su género). Su edad promedio = 30.43 años, de = 10.52. Se usó el análisis factorial confirmatorio y análisis de regresión múltiple, y se encontraron buenos indicadores promedio de bondad de ajuste (e. g., CFI = .99; RMSEA = .07.). Se evaluó la validez convergente, r (3 289) = .52 (p = < .01; d = mediano), con la Escala de Satisfacción con la Vida. Se estimó la validez discriminante, r (3 289) r = -.19, con la Escala Breve de Disposición a la IRA (p = < .01; d = casi pequeño). El alfa = .70 (3 289); p = < .01; el omega = .76. Se concluye que hay evidencia parcial nacional que apoya el uso de la EBOI-G, debido a la carencia de una medida de este tipo en México, útil cuando menos para propósitos de investigación.Os objetivos primários do estudo foram determinar a validade convergente e discriminante do construto da Escala Breve de Otimismo Interativo-G (EBOI-G), em participantes de seis estados do México e calcular sua consistência interna. Bons indicadores médios de qualidade de ajuste foram encontrados (por exemplo, CFI = .99; RMSEA = .07.) A validade convergente foi avaliada, r (3 289) = .52 (p = <.01; d = mediana), com a Escala de satisfação com a vida. A validade discriminante, r (3 289) = -.19 foi estimada com a Escala Breve de Disposição à Raiva (p = <.01; d = quase pequeno). O alfa = 0,70 (3 289); p = <0,01; o ômega = 0,76. Conclui-se que há evidências nacionais parciais que apoiam o uso da EBOI-G, devido à falta de uma medida desse tipo no México, sendo útil pelo menos para fins de pesquisa

    Social Cohesion as the Missing Link between Natural Resource Management and Peacebuilding: Lessons from Cocoa Production in Côte d’Ivoire and Colombia

    Social cohesion plays a key role in processes of peacebuilding and sustainable development. Fostering social cohesion might present a potential to enhance the connection of natural resource management and peacebuilding and better functioning of sustainable land use systems. This contribution explores the nexus between social cohesion, natural resource management, and peacebuilding. We do so by (1) reviewing literature on the three concepts and (2) studying four different key action areas in the context of sustainable cocoa production for their potential to enhance social cohesion, namely (a) agroforestry; (b) cooperatives; (c) certification schemes; and (d) trade policies. Research is based on experience from cocoa production in two post-conflict countries, Côte d’Ivoire and Colombia. Our findings show that by fostering environmentally sustainable agricultural practices, these key action areas have a clear potential to foster social cohesion among cocoa producers and thus provide a valuable contribution to post-conflict peacebuilding in both countries. However, the actual effects strongly depend on a multitude of local factors which need to be carefully taken into consideration. Further, the focus in implementation of some of these approaches tends to be on increasing agricultural productivity and not directly on fostering cocoa farmers’ wellbeing and societal relations, and hence a shift toward social objectives is needed in order to strengthen these approaches as a part of overall peacebuilding strategies.Peer Reviewe

    Canagliflozin and renal outcomes in type 2 diabetes and nephropathy

    BACKGROUND Type 2 diabetes mellitus is the leading cause of kidney failure worldwide, but few effective long-term treatments are available. In cardiovascular trials of inhibitors of sodium–glucose cotransporter 2 (SGLT2), exploratory results have suggested that such drugs may improve renal outcomes in patients with type 2 diabetes. METHODS In this double-blind, randomized trial, we assigned patients with type 2 diabetes and albuminuric chronic kidney disease to receive canagliflozin, an oral SGLT2 inhibitor, at a dose of 100 mg daily or placebo. All the patients had an estimated glomerular filtration rate (GFR) of 30 to <90 ml per minute per 1.73 m2 of body-surface area and albuminuria (ratio of albumin [mg] to creatinine [g], >300 to 5000) and were treated with renin–angiotensin system blockade. The primary outcome was a composite of end-stage kidney disease (dialysis, transplantation, or a sustained estimated GFR of <15 ml per minute per 1.73 m2), a doubling of the serum creatinine level, or death from renal or cardiovascular causes. Prespecified secondary outcomes were tested hierarchically. RESULTS The trial was stopped early after a planned interim analysis on the recommendation of the data and safety monitoring committee. At that time, 4401 patients had undergone randomization, with a median follow-up of 2.62 years. The relative risk of the primary outcome was 30% lower in the canagliflozin group than in the placebo group, with event rates of 43.2 and 61.2 per 1000 patient-years, respectively (hazard ratio, 0.70; 95% confidence interval [CI], 0.59 to 0.82; P=0.00001). The relative risk of the renal-specific composite of end-stage kidney disease, a doubling of the creatinine level, or death from renal causes was lower by 34% (hazard ratio, 0.66; 95% CI, 0.53 to 0.81; P<0.001), and the relative risk of end-stage kidney disease was lower by 32% (hazard ratio, 0.68; 95% CI, 0.54 to 0.86; P=0.002). The canagliflozin group also had a lower risk of cardiovascular death, myocardial infarction, or stroke (hazard ratio, 0.80; 95% CI, 0.67 to 0.95; P=0.01) and hospitalization for heart failure (hazard ratio, 0.61; 95% CI, 0.47 to 0.80; P<0.001). There were no significant differences in rates of amputation or fracture. CONCLUSIONS In patients with type 2 diabetes and kidney disease, the risk of kidney failure and cardiovascular events was lower in the canagliflozin group than in the placebo group at a median follow-up of 2.62 years

    Risk profiles and one-year outcomes of patients with newly diagnosed atrial fibrillation in India: Insights from the GARFIELD-AF Registry.

    BACKGROUND: The Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. METHODS AND RESULTS: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012-2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P < 0.001). Diabetes and coronary artery disease (CAD) were prevalent in 36.2% and 28.1% of patients as compared with global prevalence of 22.2% and 21.6%, respectively (P < 0.001 for both). Antiplatelet therapy was the most common antithrombotic treatment in India. With increasing stroke risk, however, patients were more likely to receive oral anticoagulant therapy [mainly vitamin K antagonist (VKA)], but average international normalized ratio (INR) was lower among Indian patients [median INR value 1.6 (interquartile range {IQR}: 1.3-2.3) versus 2.3 (IQR 1.8-2.8) (P < 0.001)]. Compared with other countries, patients from India had markedly higher rates of all-cause mortality [7.68 per 100 person-years (95% confidence interval 6.32-9.35) vs 4.34 (4.16-4.53), P < 0.0001], while rates of stroke/systemic embolism and major bleeding were lower after 1 year of follow-up. CONCLUSION: Compared to previously published registries from India, the GARFIELD-AF registry describes clinical profiles and outcomes in Indian patients with AF of a different etiology. The registry data show that compared to the rest of the world, Indian AF patients are younger in age and have more diabetes and CAD. Patients with a higher stroke risk are more likely to receive anticoagulation therapy with VKA but are underdosed compared with the global average in the GARFIELD-AF.
