    Presentamos en este trabajo los instrumentos de evaluación propuestos por estudiantes de 2º curso del Grado de Educación Primaria mientras cursaban la asignatura de Didáctica de las Ciencias Experimentales en la Universidad de Sevilla. El desarrollo del curso se ha realizado a partir de una propuesta de enseñanza de las ciencias por investigación escolar, diseñada en el seno de un Proyecto de I+D+i1. A partir del análisis del contenido de las tres propuestas de enseñanza que los estudiantes elaboran a lo largo del curso, resulta: 1) un replanteamiento de la propuesta inicial. 2) diversificación de instrumentos. 3) escaso uso de instrumentos de evaluación para favorecer la autonomía del estudiante.Ministerio de Ciencia e Innovación EDU2011-2355

    Measuring universal health coverage based on an index of effective coverage of health services in 204 countries and territories, 1990–2019: a systematic analysis for the Global Burden of Disease Study 2019

    Background Achieving universal health coverage (UHC) involves all people receiving the health services they need, of high quality, without experiencing financial hardship. Making progress towards UHC is a policy priority for both countries and global institutions, as highlighted by the agenda of the UN Sustainable Development Goals (SDGs) and WHO's Thirteenth General Programme of Work (GPW13). Measuring effective coverage at the health-system level is important for understanding whether health services are aligned with countries' health profiles and are of sufficient quality to produce health gains for populations of all ages. Methods Based on the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, we assessed UHC effective coverage for 204 countries and territories from 1990 to 2019. Drawing from a measurement framework developed through WHO's GPW13 consultation, we mapped 23 effective coverage indicators to a matrix representing health service types (eg, promotion, prevention, and treatment) and five population-age groups spanning from reproductive and newborn to older adults (≥65 years). Effective coverage indicators were based on intervention coverage or outcome-based measures such as mortality-to-incidence ratios to approximate access to quality care; outcome-based measures were transformed to values on a scale of 0–100 based on the 2·5th and 97·5th percentile of location-year values. We constructed the UHC effective coverage index by weighting each effective coverage indicator relative to its associated potential health gains, as measured by disability-adjusted life-years for each location-year and population-age group. For three tests of validity (content, known-groups, and convergent), UHC effective coverage index performance was generally better than that of other UHC service coverage indices from WHO (ie, the current metric for SDG indicator 3.8.1 on UHC service coverage), the World Bank, and GBD 2017. We quantified frontiers of UHC effective coverage performance on the basis of pooled health spending per capita, representing UHC effective coverage index levels achieved in 2019 relative to country-level government health spending, prepaid private expenditures, and development assistance for health. To assess current trajectories towards the GPW13 UHC billion target—1 billion more people benefiting from UHC by 2023—we estimated additional population equivalents with UHC effective coverage from 2018 to 2023. Findings Globally, performance on the UHC effective coverage index improved from 45·8 (95% uncertainty interval 44·2–47·5) in 1990 to 60·3 (58·7–61·9) in 2019, yet country-level UHC effective coverage in 2019 still spanned from 95 or higher in Japan and Iceland to lower than 25 in Somalia and the Central African Republic. Since 2010, sub-Saharan Africa showed accelerated gains on the UHC effective coverage index (at an average increase of 2·6% [1·9–3·3] per year up to 2019); by contrast, most other GBD super-regions had slowed rates of progress in 2010–2019 relative to 1990–2010. Many countries showed lagging performance on effective coverage indicators for non-communicable diseases relative to those for communicable diseases and maternal and child health, despite non-communicable diseases accounting for a greater proportion of potential health gains in 2019, suggesting that many health systems are not keeping pace with the rising non-communicable disease burden and associated population health needs. In 2019, the UHC effective coverage index was associated with pooled health spending per capita (r=0·79), although countries across the development spectrum had much lower UHC effective coverage than is potentially achievable relative to their health spending. Under maximum efficiency of translating health spending into UHC effective coverage performance, countries would need to reach 1398pooledhealthspendingpercapita(US1398 pooled health spending per capita (US adjusted for purchasing power parity) in order to achieve 80 on the UHC effective coverage index. From 2018 to 2023, an estimated 388·9 million (358·6–421·3) more population equivalents would have UHC effective coverage, falling well short of the GPW13 target of 1 billion more people benefiting from UHC during this time. Current projections point to an estimated 3·1 billion (3·0–3·2) population equivalents still lacking UHC effective coverage in 2023, with nearly a third (968·1 million [903·5–1040·3]) residing in south Asia. Interpretation The present study demonstrates the utility of measuring effective coverage and its role in supporting improved health outcomes for all people—the ultimate goal of UHC and its achievement. Global ambitions to accelerate progress on UHC service coverage are increasingly unlikely unless concerted action on non-communicable diseases occurs and countries can better translate health spending into improved performance. Focusing on effective coverage and accounting for the world's evolving health needs lays the groundwork for better understanding how close—or how far—all populations are in benefiting from UHC

    Mathematical Modeling of the Propagation of Democratic Support of Extreme Ideologies in Spain: Causes, Effects, and Recommendations for Its Stop

    This paper deals with the construction of a discrete population mathematical model for the short-term forecast until January 2016 of the electoral support of extreme ideology parties in Spain. Firstly, the nontrivial concept of extreme ideology is stated. Then, the electoral register is split in three subpopulations: supporters of extremist parties, abstentions/blank voters, and supporters of establishment parties. The model takes into account the following variables: economy measured throughout the Spanish unemployment rate; demography quantified in terms of birth and death rates and emigration; sociopolitical situation measured by the Spanish poverty indicator, trust on the Government labor indicator (GLI), and the indicator of political trust. By considering the dynamic subpopulations transits built throughout data obtained from public and private prestigious institutions and sociopolitical analysis, a system of difference equations models the electoral population behavior in Spain allowing us to compute the expected electoral support in the time horizon of January 2016.     Abacavir/Lamivudine plus Rilpivirine Is an Effective and Safe Strategy for HIV-1 Suppressed Patients: 48 Week Results of the SIMRIKI Retrospective Study.

    OBJECTIVES: Based on data from clinical practice, we evaluated the effectiveness and safety of switching to abacavir/lamivudine plus rilpivirine (ABC/3TC+RPV) treatment in virologically suppressed HIV-1-infected patients. METHODS: We performed a multicenter, non-controlled, retrospective study of HIV-1-infected patients who switched treatment to ABC/3TC+RPV. Patients had an HIV-RNA <50 copies/mL for at least 24 weeks prior to changing treatments. The primary objective was HIV-1 RNA <50 copies/mL at week 48. Effectiveness was analyzed by intention-to-treat (ITT), missing = failure and on-treatment (OT) analyses. The secondary objectives analyzed were adverse effects changes in renal, hepatic or lipid profiles, changes in CD4+ cell count and treatment discontinuations. RESULTS: Of the 205 patients included, 75.6% were men and the median age was 49. At baseline, before switching to ABC/3TC+RPV, median time since HIV diagnosis was 13.1 years, median time with undetectable HIV-1 RNA was 6.2 years and median time of previous antiretroviral regimen was 3.1 years (48.3% patients were taking efavirenz and ABC/3TC was the most frequent backbone coformulation in 69.7% of patients). The main reasons for switching were drug toxicity/poor tolerability (60.5%) and simplification (20%). At week 48, the primary objective was achieved by 187 out of 205 (91.2%) patients by ITT analysis, and 187 out of 192 (97.4%) patients by OT analysis. The CD4+ lymphocyte count and CD4+ percentage increased significantly from baseline to week 48 by a median of 48 cells/μL (-50 to 189) and 1.2% (-1.3% to 4.1%), respectively, P<0.001. Thirty-eight adverse events (AE) were detected in 32 patients. Of these, 25 had no clear association with treatment. Three patients interrupted therapy due to AE. We observed a decrease in all lipid parameters, P<0.001, and a slight improvement in the glomerular filtration rate, P<0.01. Therapy was considered to have failed in 18 patients owing to virological failure (5 [2.4%]), toxicity/poor tolerability (4 [2%]), clinical decision (3 [1.5%]), loss to follow-up (3 [1.5%]), death (1 [0.5%]), and no clinical data (2 [1%]). CONCLUSIONS: The results of this study confirms that ABC/3TC+RPV is an effective, safe, and cost-effective option for the treatment of patients with virologically stable HIV-1 infection.The study was supported by ViiV Healthcare and SEIMC-GESIDA Foundation (grant number Gesida-8314).     Este libro reúne a varios estudiosos de turismo y gastronomía de diferentes nacionalidades, universidades y/o instancias de educación e investigación, con el fin de reflexionar sobre las tendencias que se impulsan en estos campos del conocimiento, desde tres líneas de análisis: la acción, la gestión y la innovación Ante la exigencia respecto a que las actividades económicas y sociales que impulsan la actividad turística y gastronómica no dañen el ambiente, se promueve que éstas sean sustentables. Dichas exigencias no son nuevas, desde el 2000 a la fecha se incentivan, lo cual ha provocado transformaciones importantes que ameritan una revisión general. Así, este libro abre un espacio para la presentación de investigaciones relacionadas con el turismo y la gastronomía desde la sustentabilidad. El tema central y las tendencias del mismo evidencian la orientación que hemos querido darle al libro, puesto que ninguno de los temas es más importante que otro y el orden en el que aparecen obedece únicamente a cuestiones logísticas y didácticas. De esta manera, el libro se compone de tres apartados. El primero está dedicado a la gestión sustentable e integra cinco capítulos. El segundo apartado toca el tema de la innovación sustentable y está integrado por seis trabajos, a través de los cuales se busca generar la discusión y el debate sobre la innovación en diferentes contextos del turismo y la gastronomía.
