    Transfusion transmitted malaria in three major blood banks of Peshawar, Pakistan

    Malaria is one of the most important transfusion associated infections in many parts of the world, particularly the developing countries where it is endemic. This study estimates the risk of acquiring malaria from a single unit of blood in North of Pakistan. A prospective study was conducted to investigate transfusion transmitted malaria in three major blood banks of Peshawar, Pakistan. A total of 1558 (1534 males and 24 females) healthy volunteer blood donors were screened for the presence of malarial parasites in thick and thin smears using microscopy. Nine donors (0.577%) were found positive for malarial parasite, with trophozoites and gametocytic stages of Plasmodium falciparum (1) and Plasmodium vivax (8). Majority of the donors (82.09%) belonged to age group 15 - 30 years. Donors who suffered from malaria in recent years were 28 in number and 3 of them were found positive for malaria in the present study. The results of the present study indicated that transfusion transmitted malaria is a risk in malaria endemic regions like Pakistan. Further research on this aspect using more sophisticated and advanced diagnostic techniques like enzyme-linked immunosorbent assay (ELISA) and polymerase chain reaction (PCR) are required for proper assessment of actual situation and control of transfusion transmitted malaria in Pakistan.Keywords: Transfusion, malaria, blood-borne infections, screening, microscopy, blood banksAfrican Journal of Biotechnology Vol. 9(33), pp. 5445-5449, 16 August, 201

    Outcome of inadvertent high dose BCG administration in newborns at a tertiary care hospital, Karachi- Case series

    Bacillus Calmette-Guérin (BCG) vaccine is given to newborns soon after birth. BCG vaccine overdose has been rarely reported. Here we report the outcome of newborns who accidently received high dose BCG at a tertiary care hospital, Karachi. We reviewed records of 26 newborns, who accidentally received intradermal high dose BCG, used for the treatment of urinary bladder cancers and 80 times higher dose than the BCG used for routine vaccination. The incident happened from 14-16th April, 2016 at Aga Khan University Hospital, Karachi. Analysis was carried out using SPSS. A total of 23/26(88.5%) newborns were followed for atleast 3 months and 11/26 (42.3%) were followed for atleast one year. 13/26 (50%) were male. All 26 patients were prescribed isoniazid and rifampicin for 3 months. 3/26 (11.5%) were lost to follow-up before completion of anti-tuberculous drugs (ATT). Lesions at the BCG site were observed in 16/26 (61.5%) infants, of which 15 (93.8%) had a papule, 3 (18.8%) developed a pustule, 3 (18.8%) had skin induration and 2 (12.5%) had skin erythema. Axillary lymphadenopathy was observed in 1/26 (3.8%) patient. Coagulation was deranged in 3/26 (11.5%) of babies. Intracranial bleeding was observed in 1/26 (3.8%) case. Localized skin lesions were the most common adverse events. None of them developed clinical tuberculosis. Chemoprophylaxis for inadvertent high dose BCG administration should be given for atleast 3 months.     Simplified antibiotic regimens for treatment of clinical severe infection in the outpatient setting when referral is not possible for young infants in Pakistan (Simplified Antibiotic Therapy Trial [SATT]): a randomised, open-label, equivalence trial

    Background: Parenteral antibiotic therapy for young infants (aged 0–59 days) with suspected sepsis is sometimes not available or feasible in countries with high neonatal mortality. Outpatient treatment could save lives in such settings. We aimed to assess the equivalence of two simplified antibiotic regimens, comprising fewer injections and oral rather than parenteral administration, compared with a reference treatment for young infants with clinical severe infection.Methods: We undertook the Simplified Antibiotic Therapy Trial (SATT), a three-arm, randomised, open-label, equivalence trial in five communities in Karachi, Pakistan. We enrolled young infants (aged 0–59 days) who either presented at a primary health-care clinic or were identified by a community health worker with signs of clinical severe infection. We included infants who were not critically ill and whose family refused admission. We randomly assigned infants to either intramuscular procaine benzylpenicillin and gentamicin once a day for 7 days (reference); oral amoxicillin twice daily and intramuscular gentamicin once a day for 7 days; or intramuscular procaine benzylpenicillin and gentamicin once a day for 2 days followed by oral amoxicillin twice daily for 5 days. The primary outcome was treatment failure within 7 days of enrolment and the primary analysis was per protocol. We judged experimental treatments as efficacious as the reference if the upper bound of the 95% CI for the difference in treatment failure was less than 5·0. This trial is registered at ClinicalTrials.gov , numberNCT01027429 . Findings: Between Jan 1, 2010, and Dec 26, 2013, 2780 infants were deemed eligible for the trial, of whom 2453 (88%) were enrolled. Because of inadequate clinical follow-up or treatment adherence, 2251 infants were included in the per-protocol analysis. 820 infants (747 per protocol) were assigned the reference treatment of procaine benzylpenicillin and gentamicin, 816 (751 per protocol) were allocated amoxicillin and gentamicin, and 817 (753 per protocol) were assigned procaine benzylpenicillin, gentamicin, and amoxicillin. Treatment failure within 7 days of enrolment was reported in 90 (12%) infants who received procaine benzylpenicillin and gentamicin (reference), 76 (10%) of those given amoxicillin and gentamicin (risk difference with reference −1·9, 95% CI −5·1 to 1·3), and 99 (13%) of those treated with procaine benzylpenicillin, gentamicin, and amoxicillin (risk difference with reference 1·1, −2·3 to 4·5). Interpretation: Two simplified antibiotic regimens requiring fewer injections are equivalent to a reference treatment for young infants with signs of clinical severe infection but without signs of critical illness. The use of these simplified regimens has the potential to increase access to treatment for sick young infants who cannot be referred to hospital

    Phytochemical, antimicrobial and antioxidant preliminary screening of a traditional Palestinian medicinal plant, Ononis pubescens L.

    Antibiotic resistance has become a serious global concern, and the discovery of novel antimicrobial herbal constituents may provide valuable solutions to overcome this problem. It is important to identify new sources of natural antioxidants and antimicrobials. The present study describes for the first time the antioxidant, and antibacterial activities of various fractions of Ononis pubescens L., a traditional Palestinian medicinal plant. Methods: Antimicrobial activity was tested against selected strains from American Type Culture Collection (ATCC) and clinical isolates including Shigella sonnie, Staphylococcus aureus, Entrococuss feacium, Escherichia coli, Pseudomonas aeruginosa, Methicillin Resistant Staphylococcus aureus (MRSA), Candida albicans and Epidermatophyto flacosum using minimum inhibitory concentration (MIC) assay, while antioxidant activity was analyzed using the 2,2-diphenyl-1-picrylhydrazyl (DPPH) free radical scavenging method. Results: A mixture of phytochemicals was found in all of the studied fractions of O. pubescens, which also showed remarkable potential with antioxidant and antimicrobial activities. Conclusion: The current study provides initial data that justify the use and importance of Ononis species in Palestinian folkloric medicine. Our results showed that Ononis pubescens n-hexane fraction has powerful antibacterial bioactivity against Staphylococcus aureus, as well as the acetone, n-hexane and methanol fractions which showed excellent potential against Epidermatophyto flacosum fungi, while the acetone fraction showed the highest antioxidant activity among other fractions. Further investigations are needed to identify and characterize these constituents.Financial support None.     Mapping local patterns of childhood overweight and wasting in low- and middle-income countries between 2000 and 2017

    A double burden of malnutrition occurs when individuals, household members or communities experience both undernutrition and overweight. Here, we show geospatial estimates of overweight and wasting prevalence among children under 5 years of age in 105 low- and middle-income countries (LMICs) from 2000 to 2017 and aggregate these to policy-relevant administrative units. Wasting decreased overall across LMICs between 2000 and 2017, from 8.4% (62.3 (55.1–70.8) million) to 6.4% (58.3 (47.6–70.7) million), but is predicted to remain above the World Health Organization’s Global Nutrition Target of <5% in over half of LMICs by 2025. Prevalence of overweight increased from 5.2% (30 (22.8–38.5) million) in 2000 to 6.0% (55.5 (44.8–67.9) million) children aged under 5 years in 2017. Areas most affected by double burden of malnutrition were located in Indonesia, Thailand, southeastern China, Botswana, Cameroon and central Nigeria. Our estimates provide a new perspective to researchers, policy makers and public health agencies in their efforts to address this global childhood syndemic
