    Vólvulo ileal secundario a síndrome adherencial posquirúrgico. Presentación de un caso

    Se presentó el caso de una paciente de 71 años que tuvo durante tres meses previo a su ingreso dolor abdominal tipo cólico difuso a predominio de hemiabdomen inferior, acompañado de distensión abdominal intermitente, vómitos en cantidad y frecuencia no precisada de contenido alimentario. Al examen físico se apreció paciente en regulares condiciones, facies álgica, afebril, deshidratada. En la evaluación abdominal se evidenció abdomen distendido, con ruidos hidroaéreos disminuidos en intensidad, tono y frecuencia. Se decidió ingresar para manejo clínico y realización de estudios de apoyo diagnóstico. Se diagnosticó un síndrome adherencial, conjunto de signos y síntomas provocados por la formación de adherencias intraabdominales. Frente a un paciente con abdomen agudo obstructivo con antecedente de intervención quirúrgica previa, debe sospecharse en una complicación secundaria a síndrome adherencial, teniendo al vólvulo de íleon como una de sus expresiones clínicas

    La tuberculosis es un problema común en el mundo, fundamentalmente en países en desarrollo; aproximadamente del 1 al 3 % del total de casos de tuberculosis son extrapulmonares, y de estos, del 11 al 16 % es tuberculosis abdominal, la sexta forma más frecuente. Es producida por diferentes tipos de mycobacterium, el agente causal más frecuente en el ser humano es el Mycobacterium tuberculosis. Se realizó una revisión sistemática a través de diversas bases de datos con artículos publicados entre el año 2010 y el año 2022. Se presenta este artículo con el objetivo de describir las características clínicas, métodos diagnósticos y tratamiento en un paciente con tuberculosis peritoneal, así como el análisis realizado a su historia clínica. En la laparotomía exploradora se evidenciaron lesiones granulomatosas y caseosas generalizadas en peritoneo parietal y visceral. El estudio histopatológico reportó la presencia de granulomas y bacilos ácido-alcohol resistentes, confirmando el diagnóstico de tuberculosis peritoneal. Se inició tratamiento antituberculoso durante seis meses con mejoría del cuadro. Frente a un paciente con abdomen agudo con signos de irritación peritoneal y evolución atípica, debe pensarse en la tuberculosis intestinal como diagnóstico diferencial, aunque no tenga antecedentes de afectación pulmonar previa

    Impact of Surgeon Experience in the Resection of Basal Cell Carcinoma of the Head and Neck Region

    Foundation: surgical excision with adequate margins is considered the standard treatment for cutaneous basal cell carcinoma, due to its association with lower rates of recurrence and postoperative metastasis. Objective: to evaluate the impact of surgical experience in a high-volume head and neck surgery department in relation to margin status in basal cell carcinoma resection. Methods: a retrospective and correlational descriptive study was carried out on 412 patients selected from those undergoing surgical treatment for basal cell carcinoma in the head and neck region in the department that treats this condition between 2018 and 2022. Quantitative variables were established to characterize the demographic composition of the sample and the values of the resections. The data were analyzed with the Statistica software package. Chi-square, Fisher, and Mann-Whitney tests were used to determine statistical significance. The results were considered significant for p value less than 0.05. The results were presented in quantities and percentages. Given its retrospective nature, the study did not require approval from the University Bioethics Committee. Results: the study included 245 men and 167 women. Most of the tumors were located in the nose and pinna. The most common histological subtype was nodular. Tumor size was up to 20 mm in 354 cases. Positive surgical margins were observed in 92 cases. No significant differences were found in terms of performing positive surgical margins between residents and specialists. Conclusions: the results of the present study demonstrate that adequate surgical training in a specialized head and neck surgery service is a determining factor in achieving free surgical margins in basal cell carcinoma of the head and neck region

    The transmembrane serine protease (TMPRSS3) mutated in deafness DFNB8/10 activates the epithelial sodium channel (ENaC) in vitro

    TMPRSS3 encodes a transmembrane serine protease that contains both LDLRA and SRCR domains and is mutated in non-syndromic autosomal recessive deafness (DFNB8/10). To study its function, we cloned the mouse ortholog which maps to Mmu17, which is structurally similar to the human gene and encodes a polypeptide with 88% identity to the human protein. RT-PCR and RNA in situ hybridization on rat and mouse cochlea revealed that Tmprss3 is expressed in the spiral ganglion, the cells supporting the organ of Corti and the stria vascularis. RT-PCR on mouse tissues showed expression in the thymus, stomach, testis and E19 embryos. Transient expression of wild-type or tagged TMPRSS3 protein showed a primary localization in the endoplasmic reticulum. The epithelial amiloride-sensitive sodium channel (ENaC), which is expressed in many sodium-reabsorbing tissues including the inner ear and is regulated by membrane-bound channel activating serine proteases (CAPs), is a potential substrate of TMPRSS3. In the Xenopus oocyte expression system, proteolytic processing of TMPRSS3 was associated with increased ENaC mediated currents. In contrast, 6 TMPRSS3 mutants (D103G, R109W, C194F, W251C, P404L, C407R) causing deafness and a mutant in the catalytic triad of TMPRSS3 (S401A), failed to undergo proteolytic cleavage and activate ENaC. These data indicate that important signaling pathways in the inner ear are controlled by proteolytic cleavage and suggest: (i) the existence of an auto-catalytic processing by which TMPRSS3 would become active, and (ii) that ENaC could be a substrate of TMPRSS3 in the inner ea

    Defining the Critical Hurdles in Cancer Immunotherapy

    ABSTRACT: Scientific discoveries that provide strong evidence of antitumor effects in preclinical models often encounter significant delays before being tested in patients with cancer. While some of these delays have a scientific basis, others do not. We need to do better. Innovative strategies need to move into early stage clinical trials as quickly as it is safe, and if successful, these therapies should efficiently obtain regulatory approval and widespread clinical application. In late 2009 and 2010 the Society for Immunotherapy of Cancer (SITC), convened an "Immunotherapy Summit" with representatives from immunotherapy organizations representing Europe, Japan, China and North America to discuss collaborations to improve development and delivery of cancer immunotherapy. One of the concepts raised by SITC and defined as critical by all parties was the need to identify hurdles that impede effective translation of cancer immunotherapy. With consensus on these hurdles, international working groups could be developed to make recommendations vetted by the participating organizations. These recommendations could then be considered by regulatory bodies, governmental and private funding agencies, pharmaceutical companies and academic institutions to facilitate changes necessary to accelerate clinical translation of novel immune-based cancer therapies. The critical hurdles identified by representatives of the collaborating organizations, now organized as the World Immunotherapy Council, are presented and discussed in this report. Some of the identified hurdles impede all investigators, others hinder investigators only in certain regions or institutions or are more relevant to specific types of immunotherapy or first-in-humans studies. Each of these hurdles can significantly delay clinical translation of promising advances in immunotherapy yet be overcome to improve outcomes of patients with cancer

    Zika virus vertical transmission in children with confirmed antenatal exposure.

    We report Zika virus (ZIKV) vertical transmission in 130 infants born to PCR+ mothers at the time of the Rio de Janeiro epidemic of 2015-2016. Serum and urine collected from birth through the first year of life were tested by quantitative reverse transcriptase polymerase chain reaction (PCR) and/or IgM Zika MAC-ELISA. Four hundred and seven specimens are evaluated; 161 sera tested by PCR and IgM assays, 85 urines by PCR. Sixty-five percent of children (N = 84) are positive in at least one assay. Of 94 children tested within 3 months of age, 70% are positive. Positivity declines to 33% after 3 months. Five children are PCR+ beyond 200 days of life. Concordance between IgM and PCR results is 52%, sensitivity 65%, specificity 40% (positive PCR results as gold standard). IgM and serum PCR are 61% concordant; serum and urine PCR 55%. Most children (65%) are clinically normal. Equal numbers of children with abnormal findings (29 of 45, 64%) and normal findings (55 of 85, 65%) have positive results, p = 0.98. Earlier maternal trimester of infection is associated with positive results (p = 0.04) but not clinical disease (p = 0.98). ZIKV vertical transmission is frequent but laboratory confirmed infection is not necessarily associated with infant abnormalities

    A Roadmap for HEP Software and Computing R&D for the 2020s

    Particle physics has an ambitious and broad experimental programme for the coming decades. This programme requires large investments in detector hardware, either to build new facilities and experiments, or to upgrade existing ones. Similarly, it requires commensurate investment in the R&D of software to acquire, manage, process, and analyse the shear amounts of data to be recorded. In planning for the HL-LHC in particular, it is critical that all of the collaborating stakeholders agree on the software goals and priorities, and that the efforts complement each other. In this spirit, this white paper describes the R&D activities required to prepare for this software upgrade.Peer reviewe

    Differential cross section measurements for the production of a W boson in association with jets in proton–proton collisions at √s = 7 TeV

    Measurements are reported of differential cross sections for the production of a W boson, which decays into a muon and a neutrino, in association with jets, as a function of several variables, including the transverse momenta (pT) and pseudorapidities of the four leading jets, the scalar sum of jet transverse momenta (HT), and the difference in azimuthal angle between the directions of each jet and the muon. The data sample of pp collisions at a centre-of-mass energy of 7 TeV was collected with the CMS detector at the LHC and corresponds to an integrated luminosity of 5.0 fb[superscript −1]. The measured cross sections are compared to predictions from Monte Carlo generators, MadGraph + pythia and sherpa, and to next-to-leading-order calculations from BlackHat + sherpa. The differential cross sections are found to be in agreement with the predictions, apart from the pT distributions of the leading jets at high pT values, the distributions of the HT at high-HT and low jet multiplicity, and the distribution of the difference in azimuthal angle between the leading jet and the muon at low values.United States.
