    Meeting the challenges posed by an escalating diabetes healthcare burden: A mixed methods study

Background: The ongoing escalation in the incidence of diabetes is contributing to a growing burden on health services because patients with diabetes as a co-morbidity are now spread throughout the hospital (rather than being located in specific areas). Thus, there is a need for expertise in diabetes care throughout services to ensure optimal care and reduce threats to patient safety. Aims and objectives: To identify new strategies to maintain optimal care for patients with diabetes while in hospital. Design: Mixed methods underpinned by Appreciative Inquiry. Methods: The Diabetes Knowledge Questionnaire Survey (DKQS) (n = 173); focus group interviews with nurses and midwives (n = 40), and individual interviews with recently discharged hospital patients (n = 6). Quantitative data were analysed using descriptive statistics and qualitative data by thematic analysis. Results: Highlight a need to enhance knowledge and confidence of diabetes management among nursing, midwifery, medical and ancillary staff through the introduction of focused education strategies. This, together with improved communication and team work, is required to relieve the burdens on frontline nursing staff and patients caused by delays in prescribing and reviewing insulin requirements and in accessing needed food for people with diabetes. Conclusions: In seeking solutions to the challenges in caring for hospitalised patients with diabetes there is a need to work across the entire hospital workforce and to develop effective and efficient methods for ensuring appropriate skills and knowledge of diabetes management for staff across complex and rapidly changing hospital systems. Relevance to clinical practice: The introduction and implementation of innovative educational and organisational strategies are needed to assist in meeting the challenges posed by an escalating diabetes healthcare burden. The safety of patients with diabetes can be optimised with the timely availability of appropriate meals and snacks, and enhanced coordination and communication between and within multidisciplinary teams

    Nursing leadership and clinical development units : unravelling the myth

    Australian Clinical Development Units (Nursing) (CDUs(N)) take their name from the British Nursing Development Unit (NDU) movement, which has been widely credited with introducing innovative approaches to developing nurses and nursing. A network of nine CDUs(N) was set up in an urban area health service in Australia in 1997. The aim of this project was to develop existing wards or units into centres of excellence by disseminating a new vision for Australian nurses, based on the pioneering work of the British NDU (Nursing Development Unit) movement. This research study set out to develop an understanding of the phenomena of CDU(N) leadership by interviewing these new CDU(N) leaders over time. Principles of Heideggerian hermeneutic phenomenology provided a framework for the study. Despite attempts to implement a variety of measures to nurture these CDUs(N) until they had become well established, the participants were unable to maintain the CDU(N) vision with which they had been entrusted. This thesis discusses their immediate reactions to the problems they faced, the new understandings they developed over time and the regrets and disillusionment many of them retained. The recommendations made for future projects are discussed in the light of the experiences of these participants and the current Australian health care climate

    Background. A network of nine Clinical Development Units (Nursing) (CDU(N)) were recently created in the Western Sydney Area Health Service. These units are designed to develop patient-focused nursing practice through group process and action research, based on principles of transformational leadership. Aims of the study. Although there is documented evidence from Australia and the United Kingdom (UK) that CDUs and Nursing Development Units (NDUs) are very successful in improving both patient and staff satisfaction, there is also growing evidence that the stressors experienced by nurse leaders are threatening the survival of some of these units. This study set out to develop an understanding of the phenomena of Clinical Development Unit (Nursing) leadership when these stressors were likely to impinge. Study design. Hermeneutic phenomenology was employed in order to identify how these experiences changed over time. Two rounds of interviews were conducted: approximately 4–6 months after the launch and, again, 12 months later. Findings. The Clinical Development Unit (Nursing) philosophy provided a framework on which these very motivated leaders began to enhance nursing accountability in their units through reflective practice and participatory governance. While reinforcing many previously published positive and negative aspects of Clinical Development Units and Nursing Development Units, this paper also highlights how the expectations and experiences of these leaders changed over time with␣unanticipated pressures of work, a high turnover of clinical leaders, a perceived diminution of management support and unrealistic self-expectations. A significant theme that emerged as these stressors began to impinge was the leaders' own need for leadership in order to sustain their confidence and motivation. Implications for nursing. Insights harvested from this study have since been incorporated into a revised leadership preparation programme and support mechanisms for the leaders of eight new Clinical Development Units (Nursing) in the Western Sydney Area Health Service
