    Relato da experiência: implementação da metodologia Lean Healthcare no atendimento aos pacientes nas Unidades de Pronto-Atendimentos da Secretaria Municipal de saúde de Belo Horizonte: Projeto Menos Espera, Mais Saúde! / Experience report: implementation of the Lean Healthcare methodology in patient care at Emergency Departments of the Municipal Health Secretariat of Belo Horizonte: Less Waiting, More Health!

    O artigo visa apresentar a experiência de implementação da metodologia “Lean” para o atendimento aos pacientes do Sistema Único de Saúde de Belo Horizonte (SUS-BH”) a fim de  melhorar o tempo-resposta, a qualidade e a eficiência dos atendimentos em urgência e emergência, por meio de pactuação de fluxos internos.  Trata-se de uma propsta que está na carta de projetos estratégicos da Secretaria Municipal de Saúde (SMSA). A melhoria da qualidade da assistência nas UPA’s está diretamente associada à aplicação de ferramentas de gestão de processos, com foco na eliminação dos desperdícios, por meio de um trabalho padronizado, treinamento contínuo e intervenção corretiva imediata frente às não conformidades encontradas.Tratando-se de pacientes em situações de urgência, o tempo é um dos principais recursos que deve ser mensurado cotidianamente, com o objetivo de oferecer ao doente uma intervenção médica ou assistencial, o mais breve possível. Apresentaremos nesse artigo, os resultados e experiências com a implantação do projeto “Menos espera, mais saúde” em algumas UPA’s de Belo Horizonte, com o uso de ferramentas Lean nos processos que tangem o atendimento ao usuário e o que identificamos como possibilidades de melhoria contínua, por meio da gestão à vista e intervenção frente aos indicadores de desempenho operacional. O projeto inovador e transformador “Menos espera, mais saúde” faz interface da gerência de urgência com outros setores da Secretaria municipal de saúde, no objetivo de atender a demanda do usuário, com qualidade, resolutilidade e segurança na espera pelo atendimento, otimizando a permanência desse usuário nas UPA’s de Belo Horizonte

    Leucine supplementation improves adiponectin and total cholesterol concentrations despite the lack of changes in adiposity or glucose homeostasis in rats previously exposed to a high-fat diet

    <p>Abstract</p> <p>Background</p> <p>Studies suggest that leucine supplementation (LS) has a therapeutic potential to prevent obesity and to promote glucose homeostasis. Furthermore, regular physical exercise is a widely accepted strategy for body weight maintenance and also for the prevention of obesity. The aim of this study was to determine the effect of chronic LS alone or combined with endurance training (ET) as potential approaches for reversing the insulin resistance and obesity induced by a high-fat diet (HFD) in rats.</p> <p>Methods</p> <p>Forty-seven rats were randomly divided into two groups. Animals were fed a control diet-low fat (<it>n = </it>10) or HFD (<it>n = </it>37). After 15 weeks on HFD, all rats received the control diet-low fat and were randomly divided according to treatment: reference (REF), LS, ET, and LS+ET (<it>n = </it>7-8 rats per group). After 6 weeks of treatment, the animals were sacrificed and body composition, fat cell volume, and serum concentrations of total cholesterol, HDL-cholesterol, triacylglycerol, glucose, adiponectin, leptin and tumor necrosis factor-alpha (TNF-α) were analyzed.</p> <p>Results</p> <p>At the end of the sixth week of treatment, there was no significant difference in body weight between the REF, LS, ET and LS+ET groups. However, ET increased lean body mass in rats (<it>P </it>= 0.019). In addition, ET was more effective than LS in reducing adiposity (<it>P </it>= 0.019), serum insulin (<it>P </it>= 0.022) and TNF-α (<it>P </it>= 0.044). Conversely, LS increased serum adiponectin (<it>P </it>= 0.021) levels and reduced serum total cholesterol concentration (<it>P </it>= 0.042).</p> <p>Conclusions</p> <p>The results showed that LS had no beneficial effects on insulin sensitivity or adiposity in previously obese rats. On the other hand, LS was effective in increasing adiponectin levels and in reducing total cholesterol concentration.</p

    MECANISMOS FISIOLÓGICOS E FISIOPATOLÓGICOS DETERMINANTES DA ATIVIDADE VASOMOTORA SIMPÁTICA

    The sympathetic vasomotor activity is one of determinants of blood pressure (BP). Understanding the mechanisms involved in the control of the cardiovascular system is important in physiological and pathophysiological condition. The principal sympathetic premotor brain nuclei are confined in the paraventricular nucleus of hypothalamus (PVN) and in the rostralventrolateral medulla (RVLM). In different patophysiological condition, there is an increase in the sympathetic vasomotor tone, in part due to an increase in the activity of the PVN and RVLM neurons. In this brief review, we discussed the major mechanisms of sympathetic activation in different experimental models: 1) renovascular hypertension, 2) renoprival hypertension, 3) cardiac failure, 4) hypertension induced by nitric oxide blockade, 5) obesity and 6) gender differences. The actions of different mediators in the PVN and in the RVLM acting in long term, can change the level of sympathetic nerve activity and blood pressure and therefore, contributing for the progression of cardiovascular disease.A atividade vasomotora simpática é um dos determinantes da pressão arterial (PA). Estabelecer quais são os mecanismos geradores dessa atividade é importante para o entendimento de como o sistema cardiovascular opera, tanto em situações fisiológicas como fisiopatológicas. Os principais grupos pré-motores do simpático estão confinados no núcleo paraventricular do hipotálamo (PVN) e região rostoventrolateral bulbar (RVLM). Em diversas situações fisiopatológicas há aumento na atividade vasomotora simpática, em parte conseqüente a maior atividade dos neurônios do PVN e RVLM. Nesta breve revisão, foram discutidos os principais mecanismos de ativação simpática em diferentes modelos experimentais: 1) hipertensão renovascular, 2) hipertensão por baixa massa renal, 3) insuficiência cardíaca, 4) hipertensão por bloqueio do óxido nítrico, 5) obesidade e 6) dimorfismo sexual. As ações de diferentes mediadores sobre o PVN e RVLM podem em longo prazo determinar novos patamares de atividade simpática, modificando os níveis tensionais e dessa forma, contribuir para a progressão da doença cardiovascular

    Timp1 interacts with beta-1 integrin and CD63 along melanoma genesis and confers anoikis resistance by activating PI3-K signaling pathway independently of Akt phosphorylation

    Background: Anoikis resistance is one of the abilities acquired along tumor progression. This characteristic is associated with metastasis development, since tumorigenic cells must survive independently of cell-matrix interactions in this process. in our laboratory, it was developed a murine melanocyte malignant transformation model associated with a sustained stressful condition. After subjecting melan-a melanocytes to 1, 2, 3 and 4 cycles of anchorage impediment, anoikis resistant cells were established and named 1C, 2C, 3C and 4C, respectively. These cells showed altered morphology and PMA independent cell growth, but were not tumorigenic, corresponding to pre-malignant cells. After limiting dilution of 4C pre-malignant cells, melanoma cell lines with different characteristics were obtained. Previous data from our group showed that increased Timp1 expression correlated with anoikis-resistant phenotype. Timp1 was shown to confer anchorage-independent growth capability to melan-a melanocytes and render melanoma cells more aggressive when injected into mice. However, the mechanisms involved in anoikis regulation by Timp1 in tumorigenic cells are not clear yet.Methods: the beta 1-integrin and Timp1 expression were evaluated by Western blotting and CD63 protein expression by flow cytometry using specific antibodies. To analyze the interaction among Timp1, CD63 and beta 1-integrin, immunoprecipitation assays were performed, anoikis resistance capability was evaluated in the presence or not of the PI3-K inhibitors, Wortmannin and LY294002. Relative expression of TIMP1 and CD63 in human metastatic melanoma cells was analyzed by real time PCR.Results: Differential association among Timp1, CD63 and beta 1-integrins was observed in melan-a melanocytes, 4C pre-malignant melanocytes and 4C11- and 4C11+ melanoma cells.     Shared heritability and functional enrichment across six solid cancers

    Correction: Nature Communications 10 (2019): art. 4386 DOI: 10.1038/s41467-019-12095-8Quantifying the genetic correlation between cancers can provide important insights into the mechanisms driving cancer etiology. Using genome-wide association study summary statistics across six cancer types based on a total of 296,215 cases and 301,319 controls of European ancestry, here we estimate the pair-wise genetic correlations between breast, colorectal, head/neck, lung, ovary and prostate cancer, and between cancers and 38 other diseases. We observed statistically significant genetic correlations between lung and head/neck cancer (r(g) = 0.57, p = 4.6 x 10(-8)), breast and ovarian cancer (r(g) = 0.24, p = 7 x 10(-5)), breast and lung cancer (r(g) = 0.18, p = 1.5 x 10(-6)) and breast and colorectal cancer (r(g) = 0.15, p = 1.1 x 10(-4)). We also found that multiple cancers are genetically correlated with non-cancer traits including smoking, psychiatric diseases and metabolic characteristics. Functional enrichment analysis revealed a significant excess contribution of conserved and regulatory regions to cancer heritability. Our comprehensive analysis of cross-cancer heritability suggests that solid tumors arising across tissues share in part a common germline genetic basis.Peer reviewe
