    Occurrence of Early Adverse Events After Vaccination Against Influenza at a Brazilian Reference Center

    INTRODUCTION: Since 1999, the Ministry of Health in Brazil has conducted campaigns of vaccination against influenza targeted towards the elderly, chronically-diseased people and health care workers. The vaccine against influenza is associated with adverse events of minor importance. OBJECTIVE: To investigate the early adverse events related to the vaccine against influenza. CASUISTICS AND METHODS: One hundred and ninety seven elderly individuals and health care workers vaccinated against influenza were included. An inquiry regarding adverse events related to the vaccine was applied seven days after the vaccination. RESULTS: Local adverse events were reported by 32.5% and systemic effects by 26.4% of the vaccinated subjects. Pain in the region of the injection, headache, myalgia, malaise, and coryza were more frequent in the workers than in the elderly (p<0.05). There was no statistically significant difference in the occurrence of fever. CONCLUSIONS: The belief of part of the population that credits frequent and uncomfortable adverse events to the vaccine was not confirmed. The subjective adverse events were more frequent in the health care workers, which can influence, in a negative way, the disclosure of the benefits of this vaccine due to their role as opinion makers

    Efetividade da vacina contra influenza em população de idosos no município de São Paulo, Brasil

    Com o objetivo de investigar a efetividade da vacina contra Influenza em população de pacientes ambulatoriais, com 60 anos e mais, no Município de São Paulo, foram comparados retrospectivamente 187 pacientes dos quais 131 foram vacinados e 56 não foram vacinados contra influenza em 1999. Os dois grupos foram semelhantes com respeito às condições de saúde, características demográficas, tabagismo e alcoolismo. No ano posterior à vacinação, os vacinados apresentaram número de episódios clinicamente definidos como gripe, isto é, sem confirmação laboratorial, significativamente menor do que os não vacinados. Os outros parâmetros avaliados, a saber: ocorrência de gripe, de doença respiratória que não gripe, hospitalização por qualquer motivo e por doença respiratória, não diferiram nos dois grupos. Concluímos que a vacina contra influenza contribuiu para a redução do número de episódios caracterizados como gripe. São necessários mais estudos sobre a efetividade desta vacina em populações idosas e outros grupos populacionais brasileiros.To investigate the effectiveness of the influenza vaccine in a population of elderly outpatients, we compared the occurrence and frequency of influenza like illness (ILI), respiratory illness and hospitalization in vaccinated and non-vaccinated subjects. All the outcomes were clinically defined. The two groups were similar with respect to demographics, health situation and habits. The influenza vaccine contributed to the protection of the elderly investigated, since the vaccinated subjects referred a significantly lower number of ILI than the non-vaccinated. There is a need for more studies about the effectiveness of the influenza vaccine in our country in elderly and other groups of our population

    Detection of Trypanosoma cruzi DTUs TcI and TcIV in two outbreaks of orally-transmitted Chagas disease in the Northern region of Brazi

    This study describes the laboratory investigation of two acute Chagas disease outbreaks that occurred in the riverside communities of Marimarituba and Cachoeira do Arua, in the Santarem municipality, Para State, located in the Northern region of Brazil, and occurred in March 2016 and August 2017, respectively. The generation of data regarding the diversity of Trypanosoma cruzi parasites circulating in the Amazon region is key for understanding the emergence and expansion of Chagas disease. This study aimed to identify T. cruzi Discrete Typing Units (DTUs) involved in two outbreaks of acute Chagas disease (ACD) directly from the patient’s biological sample. Nested and multiplex PCR targeting the 24Sα (rRNA) and mini-exon genes, respectively, were used to identify T. cruzi DTU in blood samples from patients diagnosed with ACD. The samples with positive cPCR were submitted for analysis for T. cruzi DTUs, which included 13 samples from the patients with ACD by oral transmission and two samples collected from two newborns of two women with ACD, from Marimarituba and Cachoeira do Arua. The samples were classified as T. cruzi TcIV, from Marimarituba’s outbreak, and T. cruzi TcI, from Cachoeira do Arua’s outbreak.     Vacinação contra o sarampo: influência da idade em sua eficácia

    The authors compare the serologic efficacy and the clinical protection afforded by three different measles vaccination schemes in adequately nourished children in São Paulo city, Brazil. Two hundred forty two children were divided into three groups. Group A, comprising 117 children who had received the vaccine before 12 months of age and a second dose at 12 months of age or more. Group B, comprising 46 children who had received only one dose, before 12 months of age. Group C, comprising 79 children who had received only one dose, at 12 months of age or more. The geometric mean titer of antibodies in Group A was 790.1; in Group B, 251.1; and in Group C, 550.3. There was no statistically significant difference between Groups A and C. The exposure to the measles virus was probably similar in all groups, and the children in Groups A and C had similar chances of acquiring the disease after vaccination whereas in Group B the chances were higher when compared to the other two groups. The results obtained in this study favor the use, in developing countries, of a vaccination program against measles that includes an early first dose at eight months of age and revaccination after 12 months of age.Os autores comparam a eficácia sorológica e a proteção clinica obtidas com três esquemas diferentes de vacinação contra o sarampo, em crianças eutróficas, na cidade de São Paulo, Brasil. Duzentas e quarenta e duas crianças foram divididas em três grupos. Grupo A, compreendendo 117 crianças primovacinadas antes dos 12 meses de idade e revacinadas com 12 ou mais meses de idade. Grupo B, compreendendo 46 crianças vacinadas com dose única, antes dos 12 meses de idade. Grupo C, compreendendo 79 crianças vacinadas com dose única, aos 12 ou mais meses de idade. A média geométrica do título de anticorpos no grupo A foi 790,1; no grupo B, 251,1; e no grupo C, 550,3. Não houve diferença estatisticamente significante entre os grupos A e C. A exposição ao vírus do sarampo foi provavelmente semelhante em todos os grupos. As crianças dos grupos A e C apresentaram risco de adoecimento semelhante após vacinação, enquanto que tal risco foi maior no grupo B quando comparado aos outros dois grupos. Os resultados obtidos neste estudo falam a favor do uso, em países em desenvolvimento, de um programa de vacinação contra o sarampo que inclua uma primeira dose àos oito meses de idade e revacinação após os 12 meses de idade

    This study describes difficulties in the monitoring of a child born during an oral outbreak of Chagas disease, in which there are several indications that the transmission occurred through the congenital route: 1. the mother was in the third trimester of pregnancy when she was infected; 2. She presented high parasitemia at the time of delivery; 3. In both, the mother and her daughter, T. cruzi was classified as DTU TcIV. The parasites were not found in the blood at birth and the infection was detected only three months later in an asymptomatic infant. As the mother and her child live in a highly endemic area, vector transmission could not be excluded during this period

    Heterogeneous contributions of change in population distribution of body mass index to change in obesity and underweight NCD Risk Factor Collaboration (NCD-RisC)

    From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions

    O desenvolvimento, licenciamento e comercialização recentes de uma vacina inativada contra a hepatite A (VIHA) têm possibilitado a obtenção de imunização ativa, segura e provavelmente duradoura contra essa doença. Estudos conduzidos em países desenvolvidos demonstram sua utilidade clínica na prevenção da hepatite A (HA) em viajantes susceptíveis que se dirigem a áreas de alta endemicidade, em crianças pré-escolares e trabalhadores de creches, além de avaliar o uso pós-exposição e em em surtos epidêmicos. Os autores enfocam aspectos epidemiológicos atuais da hepatite A em diferentes regiões visando, através do conhecimento da epidemiologia da doença, esclarecer a utilidade que a VIHA teria no controle dessa doença nos países em desenvolvimento, especialmente no Brasil. Com base na sua eficácia, segurança e imunogenicidade, a VIHA se mostra de extremo valor a nível de proteção individual. Porém, devido ao pouco tempo de uso clínico desta vacina, não encontramos disponíveis recomendações formais para o seu uso nos países em desenvolvimento, especialmente a nível de Saúde Pública. Dados epidemiológicos atualizados sobre a HA nas diversas regiões brasileiras são essenciais para o desenvolvimento de uma estratégia racional de imunização

    As contas de vidro estão significativamente presentes nos registros arqueológicos de origem histórica, principalmente em sítios de contato ou relacionados à diaspora africana no Brasil, EUA e Caribe. Esses objetos eram produzidos na Europa e faziam parte de uma rede comercial internacional que as destribuiam através das rotas mercantilistas da época, alcançando as colonias americanas. Destacamos o potencial das coleções de etnologia africana (MAE/USP) para estas pesquisas, possibilitando análises e correlações através da comparação destas contas  arqueológicas com as contas de vidro africanas ou de origens históricas dos candomblés do Brasil e que fazem parte do acervo etnográfico do museu
