    Avaliação do programa de saúde auditiva neonatal de hospital universitário em 2015

    Trabalho de Conclusão de Curso (graduação)—Universidade de Brasília, Faculdade de Ceilândia, 2018.Objetivo: Avaliar o programa de saúde auditiva de um hospital público de Brasília quanto à cobertura e índices de passa/falha da triagem auditiva neonatal, assim como verificar o diagnóstico audiológico das crianças com ou sem risco para perda auditiva que falharam no teste ou reteste de Emissões Otoacústicas Evocadas de Triagem Auditiva Neonatal Universal (TANU) de um hospital. Método: Foi realizado um estudo descritivo, quantitativo retrospectivo, realizado a partir de um levantamento de dados, baseado em planilhas de serviço do hospital, no período de janeiro de 2015 a dezembro de 2015. Resultados: A cobertura dos nascidos no referido hospital foi de 87,90%, onde 10,8% falharam na primeira etapa da triagem. Dos que foram encaminhados para a triagem, 41,25% não compareceram e, dos que compareceram, 18,09% falharam novamente o que representa 1,14% do total de nascidos, encaminhados para diagnóstico. Conclusões: Este estudo mostrou dados estatísticos do programa de saúde auditiva neonatal e também a importância de um programa de triagem auditiva neonatal realizado de forma adequada, visto que a audição é o principal sentido responsável pela aquisição da fala e linguagem da criança. O déficit desta função pode provocar prejuízo não só no desenvolvimento da linguagem, mas também, no aspecto social, emocional e cognitivo.Objective: To evaluate the auditory health program of a public hospitalregarding the coverage and pass / fail indexes of the neonatal hearing screening, as well as to verify the audiological diagnosis of children with or without risk of hearing loss who failed to test or retest EOA of Screening Auditory Neonatal Universal (SANU) of the hospital of these paper. Methods: A descriptive, retrospective quantitative study was carried out based on a data survey, based on the worksheets of the Hospital, from January 2015 to December 2015. Results: The coverage of those born in the Hospital was 87.90%, where 10.8% failed in the first stage of screening. Of those who were referred for screening, 41.25% did not attend and those who attended 18.09% returned again, which represents 1.14% of the total number of newborns referred for diagnosis. Conclusions: This study showed statistical data of the neonatal hearing health program and also the importance of a properly performed neonatal hearing screening program, since hearing is the main responsible for the acquisition of speech and language of the child. The deficit of this function can cause damage not only in the development of language, but also in the social, emotional and cognitive aspects

    A relação dos fármacos agonistas de receptores GLP-1 e a proteção de doença renal para pacientes com Diabetes Mellitus

    A diabetes mellitus compreende a incapacidade do organismo de desempenhar a função fisiológica da insulina, ocasionando uma grande carga de doenças, incluindo doença renal ou renal crônica e cardiovasculares. Os agonistas do receptor de GLP-1 são empregados em pacientes com diabetes tipo 2 como terapias de redução da glicose diversos à saúde destes pacientes. O objetivo deste estudo foi analisar a forma de ação dos agonistas de receptores GLP-1 como um fator de proteção à patologia renal crônica, em indivíduos portadores de diabetes mellitus do tipo 2, por meio de uma revisão bibliográfica sistemática. Os dados publicados demonstram que que agonistas do receptor GLP-1 reduzem a morte e incidência de doença renal terminal ou de insuficiência renal. Adicionalmente estes fármacos podem bloquear os mecanismos envolvidos em doenças renais, além de amenizar o estresse oxidativo e apoptose celular no rim. As terapias utilizadas com o uso de agonistas GLP-1 têm se mostrado agentes redutores da glicose e eficazes para pacientes com diabetes mellitus tipo 2, sendo recomendadas por instituições americanas e européias

    Zumbido e intolerância a sons : evidência e experiência de um grupo brasileiro

    Introdução Zumbido e intolerância a sons são queixas frequentes e subjetivas que podem ter impacto na qualidade de vida do paciente. Objetivo Apresentar uma revisão dos principais pontos, inclusive conceitos, fisiopatologia, diagnóstico e abordagem do paciente com zumbido e sensibilidade a sons. Método Revisão da literatura com levantamento bibliográfico na base de dados da LILACS, SciELO, Pubmed e MEDLINE. Foram selecionados artigos e capítulos de livros sobre zumbido e sensibilidade a sons. Os diversos tópicos foram discutidos por um grupo de profissionais brasileiros e as conclusões, descritas. Resultado A prevalência de zumbido tem aumentado ao longo dos anos, muitas vezes associado a perda auditiva, fatores metabólicos e erros alimentares. A avaliação médica deve ser feita minuciosamente no sentido de orientar a solicitação de exames subsidiários. Os tratamentos disponíveis atualmente variam de medicamentos ao uso de sons com características específicas e técnicas de meditação, com resultados variáveis. Conclusão Foi apresentada uma revisão sobre os temas que permitindo ao leitor uma visão ampla da abordagem dos pacientes com zumbido e sensibilidade auditiva baseada em evidências científicas e experiência nacional.Introduction Tinnitus and sound intolerance are frequent and subjective complaints that may have an impact on a patient's quality of life. Objective To present a review of the salient points including concepts, pathophysiology, diagnosis and approach of the patient with tinnitus and sensitivity to sounds. Methods Literature review with bibliographic survey in LILACS, SciELO, Pubmed and MEDLINE database. Articles and book chapters on tinnitus and sound sensitivity were selected. The several topics were discussed by a group of Brazilian professionals and the conclusions were described. Results The prevalence of tinnitus has increased over the years, often associated with hearing loss, metabolic factors and inadequate diet. Medical evaluation should be performed carefully to guide the request of subsidiary exams. Currently available treatments range from medications to the use of sounds with specific characteristics and meditation techniques, with variable results. Conclusion A review on tinnitus and auditory sensitivity was presented, allowing the reader a broad view of the approach to these patients, based on scientific evidence and national experience

    Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Safety and efficacy of the ChAdOx1 nCoV-19 vaccine (AZD1222) against SARS-CoV-2: an interim analysis of four randomised controlled trials in Brazil, South Africa, and the UK.

    BACKGROUND: A safe and efficacious vaccine against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), if deployed with high coverage, could contribute to the control of the COVID-19 pandemic. We evaluated the safety and efficacy of the ChAdOx1 nCoV-19 vaccine in a pooled interim analysis of four trials. METHODS: This analysis includes data from four ongoing blinded, randomised, controlled trials done across the UK, Brazil, and South Africa. Participants aged 18 years and older were randomly assigned (1:1) to ChAdOx1 nCoV-19 vaccine or control (meningococcal group A, C, W, and Y conjugate vaccine or saline). Participants in the ChAdOx1 nCoV-19 group received two doses containing 5 × 1010 viral particles (standard dose; SD/SD cohort); a subset in the UK trial received a half dose as their first dose (low dose) and a standard dose as their second dose (LD/SD cohort). The primary efficacy analysis included symptomatic COVID-19 in seronegative participants with a nucleic acid amplification test-positive swab more than 14 days after a second dose of vaccine. Participants were analysed according to treatment received, with data cutoff on Nov 4, 2020. Vaccine efficacy was calculated as 1 - relative risk derived from a robust Poisson regression model adjusted for age. Studies are registered at ISRCTN89951424 and ClinicalTrials.gov, NCT04324606, NCT04400838, and NCT04444674. FINDINGS: Between April 23 and Nov 4, 2020, 23 848 participants were enrolled and 11 636 participants (7548 in the UK, 4088 in Brazil) were included in the interim primary efficacy analysis. In participants who received two standard doses, vaccine efficacy was 62·1% (95% CI 41·0-75·7; 27 [0·6%] of 4440 in the ChAdOx1 nCoV-19 group vs71 [1·6%] of 4455 in the control group) and in participants who received a low dose followed by a standard dose, efficacy was 90·0% (67·4-97·0; three [0·2%] of 1367 vs 30 [2·2%] of 1374; pinteraction=0·010). Overall vaccine efficacy across both groups was 70·4% (95·8% CI 54·8-80·6; 30 [0·5%] of 5807 vs 101 [1·7%] of 5829). From 21 days after the first dose, there were ten cases hospitalised for COVID-19, all in the control arm; two were classified as severe COVID-19, including one death. There were 74 341 person-months of safety follow-up (median 3·4 months, IQR 1·3-4·8): 175 severe adverse events occurred in 168 participants, 84 events in the ChAdOx1 nCoV-19 group and 91 in the control group. Three events were classified as possibly related to a vaccine: one in the ChAdOx1 nCoV-19 group, one in the control group, and one in a participant who remains masked to group allocation. INTERPRETATION: ChAdOx1 nCoV-19 has an acceptable safety profile and has been found to be efficacious against symptomatic COVID-19 in this interim analysis of ongoing clinical trials.     Do Flavonoids from Durum Wheat Contribute to Its Bioactive Properties? A Prospective Study

    A clear gap with respect to the potential biological properties of wheat flavonoids exists in the available literature. This information is crucial for breeding programs aiming to produce new varieties presenting improved health benefits. Accordingly, advanced breeding lines of whole durum wheat were evaluated in this contribution. The highest recovery of phenolics was achieved using aqueous acetone (50:50, v/v), as verified by multi-response optimization, thus showing that phenolics could be largely underestimated by employing an inappropriate extraction. The concentration of derivatives of apigenin, the main phenolics present, ranged from 63.5 to 80.7%, as evaluated by LC–ESI-QTOF-MS. Phenolics from the breeding line 98 exhibited the highest ability in scavenging peroxyl radicals, reducing power as well as in terms of inhibition of pancreatic lipase activity, a key enzyme regulating the absorption of triacylglycerols. In contrast, none of the samples exhibited a significant anti-diabetic potential. Despite their high concentration compared to that of phenolic acids, results of this work do not support a significant antioxidant and pancreatic lipase inhibitory effect of durum wheat flavonoids. Therefore, breeding programs and animal and/or human trials related to the effect of durum wheat flavonoids on oxidative stress and absorption of triacylglycerols are discouraged at this point
