Hygiene perception changes during the influenza A H1N1 pandemic in Germany: incorporating the results of two cross-sectional telephone surveys 2008–2009
Background: The federal campaign Wir gegen Viren [Us against viruses] promoted hygiene in Germany during the influenza A H1N1 pandemic in 2009. The intervention aimed to encourage people to protect themselves against respiratory infections by simple means of hygiene behaviour. Quantitative research was carried out to outline changes in hygiene perception of the population over time, and to find out whether the potential hygiene perception changes were consistent to the federal campaign about hygiene or not. Methods: To determine changes in the hygiene perception of the population, two cross-sectional telephone surveys were held, each one with n = 2006 participants. The initial survey was carried out before the influenza A H1N1 pandemic in calendar week 49–51 in 2008 and the second in week 48 in 2009 directly after the peak of the pandemic in Germany. The questionnaire contained indicators about perceived hand hygiene efficacy, preference for coughing into the sleeve, propensity for presenteeism while showing symptoms of a cold and acceptance of hygiene masks. Results: The proportion of people who perceive the efficacy of hand washing as “very good” increased significantly from 50.9% in 2008 to 61.1% in 2009. The proportion of people who perceive coughing into the sleeve as the best way to cough increased even more dramatically from 4.8% in 2008 to 38.3% in 2009. In contrast the propensity for presenteeism decreased significantly: The proportion of people who state that they always report to work while they show symptoms of a cold decreased from 50.8% in 2008 to 40.9% in 2009. Acceptance of hygiene masks has not changed significantly from 2008 to 2009. Conclusions: The results revealed changes in hygiene perception during influenza A H1N1 pandemic in Germany. The changes we found are in accordance with the hygiene recommendations given by the federal campaign Wir gegen Viren [Us against viruses]. Results can constitute a practical benchmark for future research about hygiene perception and hygiene promotion for adults. Ascites’ neutrophil function is significantly impaired in patients with decompensated cirrhosis but can be restored by autologous plasma incubation
Systemic immune cell dysfunction is a typical feature of liver diseases and
increases the risk of bacterial infection, especially spontaneous bacterial
peritonitis. We evaluated functional properties of neutrophil granulocytes in
blood and ascites of patients both with and without decompensated cirrhosis.
We collected blood and ascites samples from 63 patients with cirrhosis and
eight without cirrhosis. Phagocytosis activity (PA) and oxidative burst
activity (OBA) were evaluated after ex vivo stimulation with E. coli, while
fluorescence signals were measured by flow cytometry. Ascites’ neutrophil
function tests were repeated after incubation with autologous plasma. Ascites’
neutrophils showed an impaired PA and OBA (median blood PA 98.1% (86.8–99.8)
vs. ascites’ PA 50.5% (0.4–97.3), p < 0.0001; median blood OBA 98.7%
(27.5–100) vs. ascites’ OBA 27.5% (0.3–96.7), p < 0.0001). Patients with non-
cirrhotic ascites showed higher PA but equally suppressed OBA. Ascites’
neutrophil function could be partially restored after incubation with
autologous plasma (median increase PA: 22.5% (−49.7 – +93.2), p = 0.002; OBA:
22.8% (−10.4 – +48.8), p = 0.002). Ascites’ neutrophils of patients with
cirrhosis are functionally impaired, but could be partially restored after
incubation with plasma. Further investigations are needed to identify the
factors in ascites that are associated with neutrophils’ function
Suicidal Behavior and Alcohol Abuse
Suicide is an escalating public health problem, and alcohol use has consistently been implicated in the precipitation of suicidal behavior. Alcohol abuse may lead to suicidality through disinhibition, impulsiveness and impaired judgment, but it may also be used as a means to ease the distress associated with committing an act of suicide. We reviewed evidence of the relationship between alcohol use and suicide through a search of MedLine and PsychInfo electronic databases. Multiple genetically-related intermediate phenotypes might influence the relationship between alcohol and suicide. Psychiatric disorders, including psychosis, mood disorders and anxiety disorders, as well as susceptibility to stress, might increase the risk of suicidal behavior, but may also have reciprocal influences with alcohol drinking patterns. Increased suicide risk may be heralded by social withdrawal, breakdown of social bonds, and social marginalization, which are common outcomes of untreated alcohol abuse and dependence. People with alcohol dependence or depression should be screened for other psychiatric symptoms and for suicidality. Programs for suicide prevention must take into account drinking habits and should reinforce healthy behavioral patterns
Genetic variants associated with subjective well-being, depressive symptoms, and neuroticism identified through genome-wide analyses
Very few genetic variants have been associated with depression and neuroticism, likely because of limitations on sample size in previous studies. Subjective well-being, a phenotype that is genetically correlated with both of these traits, has not yet been studied with genome-wide data. We conducted genome-wide association studies of three phenotypes: subjective well-being (n = 298,420), depressive symptoms (n = 161,460), and neuroticism (n = 170,911). We identify 3 variants associated with subjective well-being, 2 variants associated with depressive symptoms, and 11 variants associated with neuroticism, including 2 inversion polymorphisms. The two loci associated with depressive symptoms replicate in an independent depression sample. Joint analyses that exploit the high genetic correlations between the phenotypes (|ρ^| ≈ 0.8) strengthen the overall credibility of the findings and allow us to identify additional variants. Across our phenotypes, loci regulating expression in central nervous system and adrenal or pancreas tissues are strongly enriched for association.</p
Linking the Modern Distribution of Biogenic Proxies in High Arctic Greenland Shelf Sediments to Sea Ice, Primary Production, and Arctic-Atlantic Inflow
The eastern north coast of Greenland is considered to be highly sensitive to the ongoing Arctic warming, but there is a general lack of data on modern conditions and in particular on the modern distribution of climate and environmental proxies to provide a baseline and context for studies on past variability. Here we present a detailed investigation of 11 biogenic proxies preserved in surface sediments from the remote High Arctic Wandel Sea shelf, the entrance to the Independence, Hagen, and Danmark fjords. The composition of organic matter (organic carbon, C:N ratios, C-13, N-15, biogenic silica, and IP25) and microfossil assemblages revealed an overall low primary production dominated by benthic diatoms, especially at the shallow sites. While the benthic and planktic foraminiferal assemblages underline the intrusion of chilled Atlantic waters into the deeper parts of the study area, the distribution of organic-walled dinoflagellate cysts is controlled by the local bathymetry and sea ice conditions. The distribution of the dinoflagellate cyst Polarella glacialis matches that of seasonal sea ice and the specific biomarker IP25, highlighting the potential of this species for paleo sea ice studies. The information inferred from our multiproxy study has important implications for the interpretation of the biogenic-proxy signal preserved in sediments from circum-Arctic fjords and shelf regions and can serve as a baseline for future studies. Pharmazeutische Maßnahmen wie die Impfung und die antivirale Prophylaxe sind nur 2 Bausteine zum Infektionsschutz der Bevölkerung. Ein dritter, entscheidender Baustein sind einfache, nicht-pharmazeutische Maßnahmen wie die Händehygiene. In einer qualitativen Studie wird zunächst ermittelt, wie das Händewaschen aktuell in der Bevölkerung verankert ist. Darauf aufbauend werden 7 Hypothesen formuliert, die Bedarf für weitere Forschung zur Förderung der Händehygiene aufzeigen. Das Potenzial für derartige Forschung liegt beispielsweise in der Konzeption und Evaluation von Informationskampagnen zur Händehygiene. Die 7 Hypothesen lauten: Hypothese 1 Gesundheitstipps haben eine höhere Chance auf Verhaltensänderung, wenn sie dem Rezipienten die persönliche Betroffenheit verständlich und erlebbar machen. Hypothese 2 Hygiene dient hauptsächlich dem Wohlgefühl und der Gesellschaftsfähigkeit. Hypothese 3 Eine emotionale Verbindung zwischen Hygiene und Gesundheit besteht durch das Ekelgefühl. Hypothese 4 Das Händewaschen wird vorwiegend rituell und nach gefühltem Bedarf durchgeführt. Hypothese 5 Der Ansteckungsweg von Grippe/Atemwegsinfektionen ist bisher nur so weit bewusst, wie er beobachtbar ist, also in Bezug auf Husten, Niesen und dabei entstehende Tröpfchen. Hypothese 6 Das Bewusstmachen des Übertragungswegs „Hand“ steigert die Motivation zum Händewaschen. Hypothese 7 In der Pandemie erhöht sich der Informationsbedarf und die Bereitschaft zum persönlichen Infektionsschutz.Medical interventions like vaccination and antiviral prophylaxis are only two ways of protecting the population from infectious diseases. A third and decisive method is to apply non-pharmaceutical interventions like hand hygiene. A qualitative study identified the role hand washing actually plays in the daily life of the population. Based on the results, seven hypotheses are proposed which point to the need for further research about promotion of hand hygiene, e.g. the conception and evaluation of hand washing advertising campaigns. Hypothesis 1 Health tips are more likely to be followed if they communicate clearly and understandably the personal relevance of the information to the recipients. Hypothesis 2 For many, hygiene serves above all to make them feel good and more comfortable around others. Hypothesis 3 The feeling of disgust is an emotional connection between health and hygiene. Hypothesis 4 People mainly wash their hands ritually and when they actually feel the need to clean their hands. Hypothesis 5 As far as most people are aware, the means of transmission of influenza and other respiratory diseases are only the ones they can observe, namely coughing, sneezing, and the resulting visible droplets. Hypothesis 6 People are more motivated to wash their hands after learning that infections can be spread via the hands. Hypothesis 7 A pandemic situation increases the population’s demand for information and people’s willingness to protect themselves from infection
Cervical nerve root decompression by lateral approach as salvage operation after failed anterior transdiscal surgery: technical case report
Cervical nerve root compression caused by disco-osteophytic changes is classically operated by anterior transdiscal approach with disc replacement. If compression persists or recurs, reoperation via the same surgical route may be difficult, because of scar tissue and/or implants. An alternative approach may be necessary. We recommend the lateral cervical approach (retrojugular) as salvage operation in such cases. We report a patient with cervical nerve root compression operated by anterior transdiscal approach with plate and bone graft. As some compression persisted clinically and radiologically, the patient was re-operated via a lateral approach. The surgical access was free of scar tissue. The arthrodesis could be left intact and did not prevent effective nerve root decompression. The patient became asymptomatic. The lateral cervical approach (retrojugular) as reported here, is an excellent alternative pathway if reoperation after anterior transdiscal surgery with disc replacement becomes necessary
Cholesterol esterification in plasma as a biomarker for liver function and prediction of mortality
Abstract Background Advanced stages of liver cirrhosis lead to a dramatically increased mortality. For valid identification of these patients suitable biomarkers are essential. The most important biomarkers for liver function are bilirubin and prothrombin time expressed as International Normalized Ratio (INR). However, the influence of several anticoagulants on the prothrombin time limits its diagnostic value. Aim of this study was the evaluation of cholesterol esterification (CE) fraction (esterified cholesterol vs. total cholesterol) as an alternative biomarker for liver synthesis and mortality prediction. Under physiological conditions the CE fraction in blood is closely regulated by lecithin-cholesterol acyltransferase (LCAT) which is produced in the liver. Methods One hundred forty-two patients with liver disease clinically considered for orthotopic liver transplant for different indications were enrolled in the study. One patient was excluded because of the intake of a direct oral factor Xa inhibitor which has a strong impact on prothrombin time. Results Results of CE fraction were in good agreement with INR (R2 = 0.73; p < 0.001). In patients who died or survived within three months mean CE fraction was 56% vs. 74% (p < 0.001) and mean INR was 2.0 vs. 1.3 (p < 0.001), respectively. The predictive value of CE fraction for three-month mortality risk was higher compared to INR (p = 0.04). Results for one-year mortality were comparable. Conclusions The cholesterol esterification fraction is a valid biomarker for liver synthesis and allows reliable prediction of mortality. In contrast to INR, it is independent of anticoagulation and other analytical limitations of coagulation tests
