    Avaliação da produção lacrimal em gambĂĄs filhotes por trĂȘs diferentes mĂ©todos

    O estabelecimento do parĂąmetro de produção lacrimal nas diferentes espĂ©cies Ă© importante para o entendimento da saĂșde do olho e Ă© um dos componentes da semiotĂ©cnica oftĂĄlmica. Particularidades derivadas da anatomofisiologia das espĂ©cies nĂŁo domĂ©sticas induzem a busca de metodologias que sejam mais fidedignas aos parĂąmetros. Objetivou-se com este estudo avaliar e comparar a produção lacrimal de gambĂĄs-de-orelha-branca (Didelphis albiventris) e gambĂĄs-de-orelha-preta (Didelphis aurita) por trĂȘs diferentes mĂ©todos. Quinze indivĂ­duos de cada espĂ©cie, juvenis, hĂ­gidos, de ambos os sexos, com 60 a 90 dias de vida, foram contidos fisicamente para realização do teste lacrimal do vermelho de fenol (TLVF), da ponta de papel absorvente estĂ©ril e do teste lacrimal de Schirmer modificado (TLSm). O TLVF foi o mais difĂ­cil de ser executado devido Ă  maleabilidade do fio, enquanto a TEPA se mostrou mais exequĂ­vel para ambas as espĂ©cies. A mediana ± intervalo semi-interquartil para o TLVF foi de 19,79±2,61mm/15” e 5,22±2,92mm/15”, para a TEPA foram de 16,25±1,82mm/min e 10,93±3,04mm/min, e para o TLSm foram de 0±1,63mm/min e 0±1,63mm/min, para gambĂĄs-de-orelha-branca e gambĂĄs-de-orelha-preta, respectivamente. NĂŁo houve diferença entre o olho direito e esquerdo e nem quanto ao sexo. Obteve-se diferença significativa para um mesmo teste entre as espĂ©cies. NĂŁo foi encontrada correlação significativa entre os testes para ambas as espĂ©cies. A quantificação da porção aquosa da lĂĄgrima poderĂĄ auxiliar no diagnĂłstico precoce de doenças oculares nas espĂ©cies estudadas.The establishment of parameters for tear production in different species is important for better understanding eyeÂŽs health and is one of the components of the ophthalmic semiological technique. Particularities derived from the anatomophysiology of non-domestic species induce the search for more reliable methodologies. The aim was to evaluate and compare tear production of white-eared opossum (Didelphis albiventris) and Brazilian common opossum (Didelphis aurita) by three different methods. Fifteen individuals of each species, juveniles, healthy, of both sexes, with 60 to 90 days of life, were physically restrained. Phenol red thread test (PRTT), endodontic absorbent paper point tear test (EAPPTT) and modified -Schirmer tear test (mSTT) were performed. PRTT was the most difficult to perform because of the wire malleability, while EAPPTT was more feasible for both species. The median ± semi-quartile range for PRTT were 19.79±2.61mm/15 “and 5.22±2.92mm/15”, for EAPPTT were 16.25±1.82mm/min and 10.9±3.04mm/min, and for STTm were 0±1.63mm/min and 0±1.63mm/min for white-eared opossum and Brazilian common opossum respectively. There was no difference between the right and left eye neither sex. A significant difference was obtained for the same test to different species. No significant correlation was found between the tests for both species. The description of tear production parameters for juvenile white-eared opossum and Brazilian common opossum may be used as a tool, which will allow the early diagnosis of ocular diseases

    Social Networks Shape the Transmission Dynamics of Hepatitis C Virus

    Hepatitis C virus (HCV) infects 170 million people worldwide, and is a major public health problem in Brazil, where over 1% of the population may be infected and where multiple viral genotypes co-circulate. Chronically infected individuals are both the source of transmission to others and are at risk for HCV-related diseases, such as liver cancer and cirrhosis. Before the adoption of anti-HCV control measures in blood banks, this virus was mainly transmitted via blood transfusion. Today, needle sharing among injecting drug users is the most common form of HCV transmission. Of particular importance is that HCV prevalence is growing in non-risk groups. Since there is no vaccine against HCV, it is important to determine the factors that control viral transmission in order to develop more efficient control measures. However, despite the health costs associated with HCV, the factors that determine the spread of virus at the epidemiological scale are often poorly understood. Here, we sequenced partial NS5b gene sequences sampled from blood samples collected from 591 patients in SĂŁo Paulo state, Brazil. We show that different viral genotypes entered SĂŁo Paulo at different times, grew at different rates, and are associated with different age groups and risk behaviors. In particular, subtype 1b is older and grew more slowly than subtypes 1a and 3a, and is associated with multiple age classes. In contrast, subtypes 1a and 3b are associated with younger people infected more recently, possibly with higher rates of sexual transmission. The transmission dynamics of HCV in SĂŁo Paulo therefore vary by subtype and are determined by a combination of age, risk exposure and underlying social network. We conclude that social factors may play a key role in determining the rate and pattern of HCV spread, and should influence future intervention policies

    Phylogenetic analysis of complete genome sequences of hepatitis B virus from an Afro-Colombian community: presence of HBV F3/A1 recombinant strain

    Background: Hepatitis B virus (HBV) infection is one of the most prevalent viral infections in humans and represents a serious public health problem. In Colombia, our group reported recently the presence of subgenotypes F3, A2 and genotype G in Bogota. The aim of this study was to characterize the HBV genotypes circulating in Quibdo, the largest Afro-descendant community in Colombia. Sixty HBsAg-positive samples were studied. A fragment of 1306 bp (S/POL) was amplified by nested PCR. Positive samples to S/POL fragment were submitted to PCR amplification of the HBV complete genome. Findings: The distribution of HBV genotypes was: A1 (52.17%), E (39.13%), D3 (4.3%) and F3/A1 (4.3%). An HBV recombinant strain subgenotype F3/A1 was found for the first time. Conclusions: This study is the first analysis of complete HBV genome sequences from Afro-Colombian population. It was found an important presence of HBV/A1 and HBV/E genotypes. A new recombinant strain of HBV genotype F3/A1 was reported in this population.     Association between Grape Yeast Communities and the Vineyard Ecosystems

    The grape yeast biota from several wine-producing areas, with distinct soil types and grapevine training systems, was assessed on five islands of Azores Archipelago, and differences in yeast communities composition associated with the geographic origin of the grapes were explored. Fifty-seven grape samples belonging to the Vitis vinifera grapevine cultivars Verdelho dos Acores (Verdelho), Arinto da Terceira (Arinto) and Terrantez do Pico (Terrantez) were collected in two consecutive years and 40 spontaneous fermentations were achieved. A total of 1710 yeast isolates were obtained from freshly crushed grapes and 1200 from final stage of fermentations. Twenty-eight species were identified, Hanseniaspura uvarum, Pichia terricola and Metschnikowia pulcherrima being the three most representative species isolated. Candida carpophila was encountered for the first time as an inhabitant of grape or wine-associated environments. In both sampling years, a higher proportion of H. uvarum in fresh grapes from Verdelho cultivar was observed, in comparison with Arinto cultivar. Qualitatively significant differences were found among yeast communities from several locations on five islands of the Archipelago, particularly in locations with distinctive agro-ecological compositions. Our results are in agreement with the statement that grape-associated microbial biogeography is non-randomly associated with interactions of climate, soil, cultivar, and vine training systems in vineyard ecosystems. Our observations strongly support a possible linkage between grape yeast and wine typicality, reinforcing the statement that different viti-cultural terroirs harbor distinctive yeast biota, in particular in vineyards with very distinctive environmental conditions.Joao Drumonde Neves is the recipient of a fellowship of the Azorean Government (M321/006/F/2008) and PROEMPREGO.     The Large Observatory For X-ray Timing: LOFT

    LOFT, the Large Observatory for X-ray Timing, is a new space mission concept devoted to observations of Galactic and extra-Galactic sources in the X-ray domain with the main goals of probing gravity theory in the very strong field environment of black holes and other compact objects, and investigating the state of matter at supra-nuclear densities in neutron stars. The instruments on-board LOFT, the Large area detector and the Wide Field Monitor combine for the first time an unprecedented large effective area (~10 m2 at 8 keV) sensitive to X-ray photons mainly in the 2-30 keV energy range and a spectral resolution approaching that of CCD-based telescopes (down to 200 eV at 6 keV).     ARIA 2016 : Care pathways implementing emerging technologies for predictive medicine in rhinitis and asthma across the life cycle

    The Allergic Rhinitis and its Impact on Asthma (ARIA) initiative commenced during a World Health Organization workshop in 1999. The initial goals were (1) to propose a new allergic rhinitis classification, (2) to promote the concept of multi-morbidity in asthma and rhinitis and (3) to develop guidelines with all stakeholders that could be used globally for all countries and populations. ARIA-disseminated and implemented in over 70 countries globally-is now focusing on the implementation of emerging technologies for individualized and predictive medicine. MASK [MACVIA (Contre les Maladies Chroniques pour un Vieillissement Actif)-ARIA Sentinel NetworK] uses mobile technology to develop care pathways for the management of rhinitis and asthma by a multi-disciplinary group and by patients themselves. An app (Android and iOS) is available in 20 countries and 15 languages. It uses a visual analogue scale to assess symptom control and work productivity as well as a clinical decision support system. It is associated with an inter-operable tablet for physicians and other health care professionals. The scaling up strategy uses the recommendations of the European Innovation Partnership on Active and Healthy Ageing. The aim of the novel ARIA approach is to provide an active and healthy life to rhinitis sufferers, whatever their age, sex or socio-economic status, in order to reduce health and social inequalities incurred by the disease.Peer reviewe

    Diretriz da Sociedade Brasileira de Cardiologia sobre Diagnóstico e Tratamento de Pacientes com Cardiomiopatia da Doença de Chagas

    This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.  Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.   Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.  The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.  The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.     Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidĂȘncias cientĂ­ficas que as suportam, quanto ao diagnĂłstico e tratamento da CDC, com especial ĂȘnfase na base de racionalidade que a embasou. A DC no sĂ©culo XXI mantĂ©m padrĂŁo epidemiolĂłgico de endemicidade em 21 paĂ­ses da AmĂ©rica Latina. Investigadores e gestores de paĂ­ses endĂȘmicos e nĂŁo endĂȘmicos indigitam a necessidade de se adotarem polĂ­ticas abrangentes, de saĂșde pĂșblica, para controle eficaz da transmissĂŁo inter-humanos da infecção pelo T. cruzi, e obter-se nĂ­vel otimizado de atendimento aos indivĂ­duos jĂĄ infectados, com foco em oportunização diagnĂłstica e terapĂȘutica. Mecanismos patogĂȘnicos e fisiopatolĂłgicos da CDC foram revisitados apĂłs atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistĂȘncia parasitĂĄria tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distĂșrbios autonĂŽmicos e microvasculares. Alguns deles recentemente constituĂ­ram alvos potenciais de terapĂȘuticas. A histĂłria natural das fases aguda e crĂŽnica foi revista, com realce para a transmissĂŁo oral, a forma indeterminada e as sĂ­ndromes crĂŽnicas. MetanĂĄlises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade apĂłs instalação da cardiomiopatia crĂŽnica. Condutas terapĂȘuticas aplicĂĄveis aos indivĂ­duos com a FIDC foram abordadas especificamente. Todos os mĂ©todos para detectar alteraçÔes estruturais e/ou funcionais com variadas tĂ©cnicas de imageamento cardĂ­aco tambĂ©m foram revisados, com recomendaçÔes de uso nos vĂĄrios cenĂĄrios clĂ­nicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por mĂ©todos que detectam fibrose miocĂĄrdica. A metodologia atual para diagnĂłstico etiolĂłgico e as consequentes implicaçÔes do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. TambĂ©m o tratamento de pacientes em risco ou com insuficiĂȘncia cardĂ­aca, arritmias e eventos tromboembĂłlicos, baseado em recursos farmacolĂłgicos e complementares, recebeu especial atenção. CapĂ­tulos suplementares subsidiaram as condutas aplicĂĄveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grĂĄvidas, na reativação da infecção apĂłs transplante cardĂ­acos, e outros.    Por fim, dois capĂ­tulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivĂ­duos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicaçÔes mĂ©dico-trabalhistas completaram esta diretriz.&nbsp
