    Endoscopic endonasal ligation of the esphenopalatine artery for severe epistaxis

    Introduction: Severe epistaxis, usually associated to hypertension and blood dyscrasia, may benefit from surgery when the conservative treatment fails. Aim: to evaluate the results of endoscopic endonasal ligation of the sphenopalatine artery for non-responsive severe epistaxis. Study design: Clinical prospective. Material and Method: chart review of twelve patients submitted to endoscopic endonasal ligation of the sphenopalatine artery regarding clinical aspects, predisposing factors and complications of the procedure. Results: the mean age was 50.9 years, and the male:female: ratio was 2:1. Hypertension was presented in 33% and hematologic disorders in 16.6% of the patients. One patient, 8.3%, presented a new bleeding episode after the surgery. Discussion: the endoscopic endonasal ligation of the sphenopalatine artery, proved to be a safe surgical method and presented 8.3% of bleeding after the procedure in our study. Conclusion: avoiding the complications of previous surgical treatments for severe epistaxis, the endoscopic endonasal ligation of the sphenopalatine artery is an effective technique affordable for otolaryngologists with practice in endoscopic endonasal surgery.Introdução: A epistaxe severa, geralmente associada a fatores predisponentes como hipertensão arterial sistêmica e coagulopatia, é uma doença desafiadora, e pode necessitar de uma abordagem cirúrgica nos casos que não respondem ao tratamento conservador, como cauterização e tamponamento nasal. Objetivo: avaliar os resultados da ligadura endoscópica endonasal da artéria esfenopalatina no tratamento da epistaxe severa refratária à abordagem terapêutica conservadora. Forma de estudo: Clínico prospectivo. Material e Método: foram avaliados doze casos de pacientes submetidos à ligadura endoscópica endonasal da artéria esfenopalatina no tratamento da epistaxe severa não responsiva ao tratamento conservador, observando-se a história clínica, os fatores predisponentes, a evolução e as complicações deste procedimento. Resultados: a idade média foi de 50,9 anos, e a distribuição por sexo foi de 33% do sexo feminino e 67% do sexo masculino; 33% apresentaram HAS e 16,6% coagulopatia (hepatopatia) como fatores predisponentes. Um paciente (8,3%) apresentou ressangramento após o procedimento cirúrgico. Discussão: a ligadura endoscópica endonasal da artéria esfenopalatina representa uma abordagem segura e garante um controle satisfatório do sangramento, com índice de ressangramento de 8,3% entre os nossos casos.     Acoustics reflex abnormalities in auditory processing disorder

    Introduction: The acoustic reflex threshold is defined as the lowest sound intensity capable of starting the middle ear protection mechanism due to intense sounds (Metz, 1952; Jerger, 1970), being necessary the structural and functional integrity of the periferic and central auditory system. These structures are also responsable for the central processing of auditory information. Aim: We intend to verify whether acoustic reflex abnormalities can also appear in auditory processing (AP) disorders, resulting in symptoms related to speech and language disorders. Study design: Prospective randomized. Material and method: Data were analyzed from one hundred AP assessments, using Pereira (1997) method. Patients, male and female, with ages ranging from 07 to 18 years, had normal hearing thresholds and normal tympanograms patterns. The difference between the acoustic reflex and hearing thresholds defined the acoustic reflex level (ARL), considered normal between 70-90dB and altered when above of this range or when absent in one or more frequencies (Carvallo, 1996; 1997; Metz, 1952). Results: Disorders of AP were found in 97% of the patients. In this group, 62% showed ARL abnormalities, being statistically significant. Furthermore, patients with AP disorders showed ARL alterations, more frequently on severe degree disorders, on patients with combined auditory gnosis impairments and on patients with more than one auditory ability altered. Conclusion: Patients with acoustic reflex alterations and normal audiometry should perform the AP assessment, as these symptoms could unmask pathologies of the central nervous system.Introdução: O limiar do reflexo acústico corresponde à menor intensidade de um som capaz de desencadear o mecanismo de proteção da orelha média frente a sons intensos (Metz, 1952; Jerger, 1970), sendo necessária a integridade estrutural e funcional do sistema auditivo periférico e central em nível de tronco encefálico. Algumas dessas estruturas são também responsáveis pelo processamento central da informação auditiva. Objetivo: Procuramos verificar se alterações do reflexo acústico também estariam presentes na Desordem do Processamento Auditivo Central (DPA), levando a queixas relacionadas aos distúrbios da comunicação. Forma de estudo: Prospectivo randomizado. Material e método: Foram analisados cem protocolos de avaliações do Processamento Auditivo Central (PAC), realizadas segundo Pereira (1997), de indivíduos dos sexos masculino e feminino, de 7 a 18 anos, com limiares de audibilidade normais e timpanograma tipo A. A diferença entre os limiares do reflexo acústico e os de audibilidade forneceu os níveis de reflexos acústicos (NRA), considerados normais entre 70-90dB e alterados quando acima deste intervalo ou quando ocorreu ausência em uma ou mais freqüências (Carvallo, 1996; 1997; Metz, 1952). Resultados: Das crianças avaliadas 97% apresentaram algum tipo de DPA. Destas, 62% tiveram NRA alterados, numa relação estatisticamente significante. Além disso, pacientes com DPA mostraram NRA alterados, mais freqüentemente, nas desordens de grau severo, naquelas com prejuízos gnósicos auditivos combinados e naquelas com mais de uma habilidade auditiva alterada.     The value of prognostic clinical data in Bell’s palsy

    SummaryElectroneurography (ENoG) and clinical staging are currently the methods of choice to indicate prognosis in Bell’s palsy, although ENoG is an electrophysiological test not universally available. Aim: Identify other options of prognostic evaluation based upon clinical aspects and minimal electrical stimulation test allowing prognostic measurement in almost any circumstances. Study design: historic cohort. Material and Method: Chart review of 1,521 cases of IPFP, analyzing the following clinical aspects: gender, age, paralyzed side, installation mode, previous symptoms, associated symptoms and minimal electrical stimulation test (Hilger test) and its statistical correlation to facial palsy evolution after 6 months. Results: Data indicated that patients above 60 years old had worse prognosis in comparison with patients under 30 years old. A progressive mode of paralysis installation, absence of previous symptoms, concomitant vertigo and response superior to 3.5 mA at minimum electrical stimulation test were also related to worse prognosis. On the other hand, the absence of concomitant symptoms, diminished tearing and sudden onset were related to better prognosis. Conclusion: Clinical factors and Hilger’s test can accurately indicate the prognosis in cases of Bell’s palsy when ENoG is not available

    Single hadron response measurement and calorimeter jet energy scale uncertainty with the ATLAS detector at the LHC

    The uncertainty on the calorimeter energy response to jets of particles is derived for the ATLAS experiment at the Large Hadron Collider (LHC). First, the calorimeter response to single isolated charged hadrons is measured and compared to the Monte Carlo simulation using proton-proton collisions at centre-of-mass energies of sqrt(s) = 900 GeV and 7 TeV collected during 2009 and 2010. Then, using the decay of K_s and Lambda particles, the calorimeter response to specific types of particles (positively and negatively charged pions, protons, and anti-protons) is measured and compared to the Monte Carlo predictions. Finally, the jet energy scale uncertainty is determined by propagating the response uncertainty for single charged and neutral particles to jets. The response uncertainty is 2-5% for central isolated hadrons and 1-3% for the final calorimeter jet energy scale.Comment: 24 pages plus author list (36 pages total), 23 figures, 1 table, submitted to European Physical Journal

    Measurements of Higgs boson production and couplings in diboson final states with the ATLAS detector at the LHC

    Measurements are presented of production properties and couplings of the recently discovered Higgs boson using the decays into boson pairs, H →γ γ, H → Z Z∗ →4l and H →W W∗ →lνlν. The results are based on the complete pp collision data sample recorded by the ATLAS experiment at the CERN Large Hadron Collider at centre-of-mass energies of √s = 7 TeV and √s = 8 TeV, corresponding to an integrated luminosity of about 25 fb−1. Evidence for Higgs boson production through vector-boson fusion is reported. Results of combined fits probing Higgs boson couplings to fermions and bosons, as well as anomalous contributions to loop-induced production and decay modes, are presented. All measurements are consistent with expectations for the Standard Model Higgs boson

    Standalone vertex finding in the ATLAS muon spectrometer

    A dedicated reconstruction algorithm to find decay vertices in the ATLAS muon spectrometer is presented. The algorithm searches the region just upstream of or inside the muon spectrometer volume for multi-particle vertices that originate from the decay of particles with long decay paths. The performance of the algorithm is evaluated using both a sample of simulated Higgs boson events, in which the Higgs boson decays to long-lived neutral particles that in turn decay to bbar b final states, and pp collision data at √s = 7 TeV collected with the ATLAS detector at the LHC during 2011

    Measurement of the top quark-pair production cross section with ATLAS in pp collisions at \sqrt{s}=7\TeV

    A measurement of the production cross-section for top quark pairs(\ttbar) in pppp collisions at \sqrt{s}=7 \TeV is presented using data recorded with the ATLAS detector at the Large Hadron Collider. Events are selected in two different topologies: single lepton (electron ee or muon μ\mu) with large missing transverse energy and at least four jets, and dilepton (eeee, μμ\mu\mu or eμe\mu) with large missing transverse energy and at least two jets. In a data sample of 2.9 pb-1, 37 candidate events are observed in the single-lepton topology and 9 events in the dilepton topology. The corresponding expected backgrounds from non-\ttbar Standard Model processes are estimated using data-driven methods and determined to be 12.2±3.912.2 \pm 3.9 events and 2.5±0.62.5 \pm 0.6 events, respectively. The kinematic properties of the selected events are consistent with SM \ttbar production. The inclusive top quark pair production cross-section is measured to be \sigmattbar=145 \pm 31 ^{+42}_{-27} pb where the first uncertainty is statistical and the second systematic.     Measurement of the top quark pair cross section with ATLAS in pp collisions at √s=7 TeV using final states with an electron or a muon and a hadronically decaying τ lepton

    A measurement of the cross section of top quark pair production in proton-proton collisions recorded with the ATLAS detector at the Large Hadron Collider at a centre-of-mass energy of 7 TeV is reported. The data sample used corresponds to an integrated luminosity of 2.05 fb -1. Events with an isolated electron or muon and a τ lepton decaying hadronically are used. In addition, a large missing transverse momentum and two or more energetic jets are required. At least one of the jets must be identified as originating from a b quark. The measured cross section, σtt-=186±13(stat.)±20(syst.)±7(lumi.) pb, is in good agreement with the Standard Model prediction

    Measurement of χ c1 and χ c2 production with s√ = 7 TeV pp collisions at ATLAS

    The prompt and non-prompt production cross-sections for the χ c1 and χ c2 charmonium states are measured in pp collisions at s√ = 7 TeV with the ATLAS detector at the LHC using 4.5 fb−1 of integrated luminosity. The χ c states are reconstructed through the radiative decay χ c → J/ψγ (with J/ψ → μ + μ −) where photons are reconstructed from γ → e + e − conversions. The production rate of the χ c2 state relative to the χ c1 state is measured for prompt and non-prompt χ c as a function of J/ψ transverse momentum. The prompt χ c cross-sections are combined with existing measurements of prompt J/ψ production to derive the fraction of prompt J/ψ produced in feed-down from χ c decays. The fractions of χ c1 and χ c2 produced in b-hadron decays are also measured

    Measurement of the production cross section for W-bosons in association with jets in pp collisions at s=7 TeV with the ATLAS detector

    This Letter reports on a first measurement of the inclusive W + jets cross section in proton-proton collisions at a centre-of-mass energy of 7 TeV at the LHC, with the ATLAS detector. Cross sections, in both the electron and muon decay modes of the W-boson, are presented as a function of jet multiplicity and of the transverse momentum of the leading and next-to-leading jets in the event. Measurements are also presented of the ratio of cross sections sigma (W + >= n)/sigma(W + >= n - 1) for inclusive jet multiplicities n = 1-4. The results, based on an integrated luminosity of 1.3 pb(-1), have been corrected for all known detector effects and are quoted in a limited and well-defined range of jet and lepton kinematics. The measured cross sections are compared to particle-level predictions based on perturbative QCD. Next-to-leading order calculations, studied here for n <= 2, are found in good agreement with the data. Leading-order multiparton event generators, normalized to the NNLO total cross section, describe the data well for all measured jet multiplicitie
