    Distriktssköterskans förutsättningar för att utföra en god sårvård i hemmiljö

    Bakgrund: Sårvård i hemsjukvården är ett ensamarbete för distriktssköterskan som innefattar ett stort ansvar samtidigt som andelen patienter med svårläkta sår ökar. Syfte: Att undersöka vilka förutsättningar distriktssköterskan upplever är viktiga för att kunna utföra en god sårvård i hemmiljö. Metod: Studien är kvalitativ där en semistrukturerad intervjuguide användes. Åtta distriktssköterskor verksamma inom kommunal hemsjukvård i södra Sverige intervjuades. Intervjuerna analyserades med kvalitativ innehållsanalys. Resultat: Analysen gav två kategorier: distriktssköterskans reella kompetens och distriktssköterskans sociala kompetens och fyra subkategorier: erfarenhet som ett verktyg, flexibilitet i hemmiljön, god relation med patienten och gott samarbete. Slutsats: Den viktigaste förutsättningen för sårvården var distriktssköterskans erfarenhet, kunskap och kompetens. Viktigt var även att respektera patientens autonomi och ha ett gott samarbete med andra professionella kring patiente

    Search for heavy resonances decaying to two Higgs bosons in final states containing four b quarks

    A search is presented for narrow heavy resonances X decaying into pairs of Higgs bosons (H) in proton-proton collisions collected by the CMS experiment at the LHC at root s = 8 TeV. The data correspond to an integrated luminosity of 19.7 fb(-1). The search considers HH resonances with masses between 1 and 3 TeV, having final states of two b quark pairs. Each Higgs boson is produced with large momentum, and the hadronization products of the pair of b quarks can usually be reconstructed as single large jets. The background from multijet and t (t) over bar events is significantly reduced by applying requirements related to the flavor of the jet, its mass, and its substructure. The signal would be identified as a peak on top of the dijet invariant mass spectrum of the remaining background events. No evidence is observed for such a signal. Upper limits obtained at 95 confidence level for the product of the production cross section and branching fraction sigma(gg -> X) B(X -> HH -> b (b) over barb (b) over bar) range from 10 to 1.5 fb for the mass of X from 1.15 to 2.0 TeV, significantly extending previous searches. For a warped extra dimension theory with amass scale Lambda(R) = 1 TeV, the data exclude radion scalar masses between 1.15 and 1.55 TeV

    Chronic phase shifts of the photoperiod throughout pregnancy programs glucose intolerance and insulin resistance in the rat

    Extent: 10p.Shift work during pregnancy is associated with an increased risk for preterm birth and low birth weight. However, the impact upon the long term health of the children is currently unknown. In this study, we used an animal model to determine the consequences of maternal shift work exposure on the health of the adult offspring. Pregnant rats were exposed to chronic phase shifts (CPS) in their photoperiod every 3–4 days throughout gestation and the first week after birth. Adult offspring were assessed for a range of metabolic, endocrine, circadian and neurobehavioural parameters. At 3 months of age, male pups exposed to the CPS schedule in utero had increased adiposity (+29%) and hyperleptinaemia (+99% at 0700h). By 12 months of age, both male and female rats displayed hyperleptinaemia (+26% and +41% respectively) and hyperinsulinaemia (+110% and +83% respectively). 12 month old female CPS rats displayed poor glucose tolerance (+18%) and increased insulin secretion (+29%) in response to an intraperitoneal glucose tolerance test. In CPS males the glucose response was unaltered, but the insulin response was reduced by 35%. The glucose response to an insulin tolerance test was decreased by 21% in CPS females but unaltered in males. Disruption of circadian rhythmicity during gestation resulted in gender dependent metabolic consequences for the adult offspring. These results highlight the need for a thorough analysis of shift work exposure in utero on the health of the adult offspring in humans.Tamara J. Varcoe, Nicole Wight, Athena Voultsios, Mark D.     In the setting of prior myocardial infarction, the oral antiplatelet ticagrelor added to aspirin reduced the risk of recurrent ischemic events, especially, in those with diabetes mellitus. Patients with stable coronary disease and diabetes are also at elevated risk and might benefit from dual antiplatelet therapy. The Effect of Ticagrelor on Health Outcomes in diabEtes Mellitus patients Intervention Study (THEMIS, NCT01991795) is a Phase 3b randomized, double-blinded, placebo-controlled trial of ticagrelor vs placebo, on top of low dose aspirin. Patients >= 50 years with type 2 diabetes receiving anti-diabetic medications for at least 6 months with stable coronary artery disease as determined by a history of previous percutaneous coronary intervention, bypass grafting, or angiographic stenosis of >= 50% of at least one coronary artery were enrolled. Patients with known prior myocardial infarction (MI) or stroke were excluded. The primary efficacy endpoint is a composite of cardiovascular death, myocardial infarction, or stroke. The primary safety endpoint is Thrombolysis in Myocardial Infarction major bleeding. A total of 19 220 patients worldwide have been randomized and at least 1385 adjudicated primary efficacy endpoint events are expected to be available for analysis, with an expected average follow-up of 40 months (maximum 58 months). Most of the exposure is on a 60 mg twice daily dose, as the dose was lowered from 90 mg twice daily partway into the study. The results may revise the boundaries of efficacy for dual antiplatelet therapy and whether it has a role outside acute coronary syndromes, prior myocardial infarction, or percutaneous coronary intervention

    A search for new physics is performed based on all-hadronic events with large missing transverse momentum produced in proton-proton collisions at sqrt(s)=13 TeV. The data sample, corresponding to an integrated luminosity of 2.3 inverse femtobarns, was collected with the CMS detector at the CERN LHC in 2015. The data are examined in search regions of jet multiplicity, tagged bottom quark jet multiplicity, missing transverse momentum, and the scalar sum of jet transverse momenta. The observed numbers of events in all search regions are found to be consistent with the expectations from standard model processes. Exclusion limits are presented for simplified supersymmetric models of gluino pair production. Depending on the assumed gluino decay mechanism, and for a massless, weakly interacting, lightest neutralino, lower limits on the gluino mass from 1440 to 1600 GeV are obtained, significantly extending previous limits
