Effects of intrauterine exposure to synthetic glucocorticoids on fetal, newborn, and infant hypothalamic-pituitary-adrenal axis function in humans : a systematic review
BACKGROUND: Synthetic glucocorticoids are commonly used in reproductive medicine. Fetal organ systems are highly sensitive to changes in the intrauterine environment, including overexposure to glucocorticoids. Structural and functional alterations resulting from such changes may persist throughout life and have been associated with diverse diseases. One system that could be particularly sensitive to fetal glucocorticoid overexposure is the hypothalamic-pituitary-adrenal (hpa) axis. Many human studies have investigated this possibility, but a systematic review to identify consistent, emergent findings is lacking. METHODS: We systematically review 49 human studies, assessing the effects of intrauterine exposure to synthetic glucocorticoids on fetal, neonate, and infant hpa function. RESULTS: Study quality varied considerably, but the main findings held true after restricting the analyses to higher-quality studies: intrauterine exposure to synthetic glucocorticoids reduces offspring hpa activity under unstimulated conditions after pain but not pharmacological challenge. Although reduced unstimulated hpa function appears to recover within the first 2 wk postpartum, blunted hpa reactivity to pain is likely to persist throughout the first 4 months of life. There is some evidence that the magnitude of the effects is correlated with the total amount of glucocorticoids administered and varies with the time interval between glucocorticoid exposure and hpa assessment. CONCLUSIONS: This systematic review has allowed the demonstration of the way in which intrauterine exposure to various regimens of synthetic glucocorticoids affects various forms of hpa function. As such, it guides future studies in terms of which variables need to be focused on in order to further strengthen the understanding of such therapy, whilst continuing to profit from its clinical benefits
Distinct genetic liability profiles define clinically relevant patient strata across common diseases
Stratified medicine holds great promise to tailor treatment to the needs of individual patients. While genetics holds great potential to aid patient stratification, it remains a major challenge to operationalize complex genetic risk factor profiles to deconstruct clinical heterogeneity. Contemporary approaches to this problem rely on polygenic risk scores (PRS), which provide only limited clinical utility and lack a clear biological foundation. To overcome these limitations, we develop the CASTom-iGEx approach to stratify individuals based on the aggregated impact of their genetic risk factor profiles on tissue specific gene expression levels. The paradigmatic application of this approach to coronary artery disease or schizophrenia patient cohorts identified diverse strata or biotypes. These biotypes are characterized by distinct endophenotype profiles as well as clinical parameters and are fundamentally distinct from PRS based groupings. In stark contrast to the latter, the CASTom-iGEx strategy discovers biologically meaningful and clinically actionable patient subgroups, where complex genetic liabilities are not randomly distributed across individuals but rather converge onto distinct disease relevant biological processes. These results support the notion of different patient biotypes characterized by partially distinct pathomechanisms. Thus, the universally applicable approach presented here has the potential to constitute an important component of future personalized medicine paradigms
Prospects for combined analyses of hadronic emission from -ray sources in the Milky Way with CTA and KM3NeT
The Cherenkov Telescope Array and the KM3NeT neutrino telescopes are major
upcoming facilities in the fields of -ray and neutrino astronomy,
respectively. Possible simultaneous production of rays and neutrinos
in astrophysical accelerators of cosmic-ray nuclei motivates a combination of
their data. We assess the potential of a combined analysis of CTA and KM3NeT
data to determine the contribution of hadronic emission processes in known
Galactic -ray emitters, comparing this result to the cases of two
separate analyses. In doing so, we demonstrate the capability of Gammapy, an
open-source software package for the analysis of -ray data, to also
process data from neutrino telescopes. For a selection of prototypical
-ray sources within our Galaxy, we obtain models for primary proton and
electron spectra in the hadronic and leptonic emission scenario, respectively,
by fitting published -ray spectra. Using these models and instrument
response functions for both detectors, we employ the Gammapy package to
generate pseudo data sets, where we assume 200 hours of CTA observations and 10
years of KM3NeT detector operation. We then apply a three-dimensional binned
likelihood analysis to these data sets, separately for each instrument and
jointly for both. We find that the largest benefit of the combined analysis
lies in the possibility of a consistent modelling of the -ray and
neutrino emission. Assuming a purely leptonic scenario as input, we obtain, for
the most favourable source, an average expected 68% credible interval that
constrains the contribution of hadronic processes to the observed -ray

Epidemiology of surgery associated acute kidney injury (EPIS-AKI): a prospective international observational multi-center clinical study
Purpose: The incidence, patient features, risk factors and outcomes of surgery-associated postoperative acute kidney injury (PO-AKI) across different countries and health care systems is unclear. Methods: We conducted an international prospective, observational, multi-center study in 30 countries in patients undergoing major surgery (> 2-h duration and postoperative intensive care unit (ICU) or high dependency unit admission). The primary endpoint was the occurrence of PO-AKI within 72 h of surgery defined by the Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Secondary endpoints included PO-AKI severity and duration, use of renal replacement therapy (RRT), mortality, and ICU and hospital length of stay. Results: We studied 10,568 patients and 1945 (18.4%) developed PO-AKI (1236 (63.5%) KDIGO stage 1500 (25.7%) KDIGO stage 2209 (10.7%) KDIGO stage 3). In 33.8% PO-AKI was persistent, and 170/1945 (8.7%) of patients with PO-AKI received RRT in the ICU. Patients with PO-AKI had greater ICU (6.3% vs. 0.7%) and hospital (8.6% vs. 1.4%) mortality, and longer ICU (median 2 (Q1-Q3, 1-3) days vs. 3 (Q1-Q3, 1-6) days) and hospital length of stay (median 14 (Q1-Q3, 9-24) days vs. 10 (Q1-Q3, 7-17) days). Risk factors for PO-AKI included older age, comorbidities (hypertension, diabetes, chronic kidney disease), type, duration and urgency of surgery as well as intraoperative vasopressors, and aminoglycosides administration. Conclusion: In a comprehensive multinational study, approximately one in five patients develop PO-AKI after major surgery. Increasing severity of PO-AKI is associated with a progressive increase in adverse outcomes. Our findings indicate that PO-AKI represents a significant burden for health care worldwide
Global variation in diabetes diagnosis and prevalence based on fasting glucose and hemoglobin A1c
Fasting plasma glucose (FPG) and hemoglobin A1c (HbA1c) are both used to diagnose diabetes, but these measurements can identify different people as having diabetes. We used data from 117 population-based studies and quantified, in different world regions, the prevalence of diagnosed diabetes, and whether those who were previously undiagnosed and detected as having diabetes in survey screening, had elevated FPG, HbA1c or both. We developed prediction equations for estimating the probability that a person without previously diagnosed diabetes, and at a specific level of FPG, had elevated HbA1c, and vice versa. The age-standardized proportion of diabetes that was previously undiagnosed and detected in survey screening ranged from 30% in the high-income western region to 66% in south Asia. Among those with screen-detected diabetes with either test, the age-standardized proportion who had elevated levels of both FPG and HbA1c was 29-39% across regions; the remainder had discordant elevation of FPG or HbA1c. In most low- and middle-income regions, isolated elevated HbA1c was more common than isolated elevated FPG. In these regions, the use of FPG alone may delay diabetes diagnosis and underestimate diabetes prevalence. Our prediction equations help allocate finite resources for measuring HbA1c to reduce the global shortfall in diabetes diagnosis and surveillance
Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults
Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We
estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from
1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories.
Methods We used data from 3663 population-based studies with 222 million participants that measured height and
weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate
trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children
and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the
individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference)
and obesity (BMI >2 SD above the median).
Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in
11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed
changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and
140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of
underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and
countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior
probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse
was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of
thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a
posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%)
with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and
obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for
both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such
as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged
children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls
in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and
42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents,
the increases in double burden were driven by increases in obesity, and decreases in double burden by declining
underweight or thinness.
Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an
increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy
nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of
underweight while curbing and reversing the increase in obesit

Tracking Thermo-Oxidation Reaction Products and Pathways of Modified Lignin Structures from Reactive Molecular Dynamics Simulations
Thermo-oxidation of biomass is an important process that occurs through a variety of reaction pathways depending on the chemical nature of the molecules and reaction conditions. These processes can be modeled using reactive molecular dynamics to study chemical reactions and the evolution of converted molecules over time. The advantage of this approach is that many molecules can be modeled, but it is challenging to use the large amount of data obtained from such a simulation to determine reaction products and pathways. In this study, we developed a tracking approach to identify the reaction pathways of the dominant reaction products from reactive molecular dynamics simulations. We demonstrated the approach for thermo-oxidation reactions of modified model lignin compounds. For two modified lignin structures, we tracked the evolving chemical species to find the most common reaction products. Subsequently, we monitored specific bonds to determine the individual steps in the reaction process. This combined approach of reactive molecular dynamics and tracking enabled us to identify the most likely thermo-oxidation pathways. The methodology can be used to investigate the thermo-oxidative pathways of a wider range of chemical compounds
Estimular em casa a vontade de aprender: uma experiência
A educação inicia no seio familiar e visa à socialização, aprendizagem
e incorporação de valores éticos e morais. Essa relação, atualmente,
tem sofrido influência das tecnologias digitais. Frente a esse contexto,
realizamos um estudo de caso, de natureza descritiva e exploratória,
com abordagem qualitativa, que objetivou identificar se os hábitos e
costumes vivenciados no seio familiar têm contribuído para que a criança
tire partido do potencial educativo dessas ferramentas e desenvolva o
Pensamento Computacional. Participaram uma criança de 7 anos e seus
pais. Os dados consistiram nas respostas dadas pelo pai a um inquérito
e por episódios que, gravados em vídeo pela família, contam com a
participação ativa da criança que assume papel de “apresentadora” de
determinadas tecnologias. Os resultados apontaram que as tecnologias
utilizadas em casa possibilitam o maior envolvimento entre pais
e filhos, seja para passar mais tempo juntos ou para a realização dos
deveres escolares, colaboram para o desenvolvimento da autonomia
da criança e não interferem nas outras atividades realizadas por ela.
Também mostraram que ocorreu o desenvolvimento cognitivo da criança
participante da pesquisa no que diz respeito à comunicação das ideias
e aos variados termos técnicos, bem como ao seu desenvolvimento nas

γ‑Turn Mimicry with Benzodiazepinones and Pyrrolobenzodiazepinones Synthesized from a Common Amino Ketone Intermediate
To investigate diazepinone analogues
as γ-turn mimics, seven
1,4-benzodiazepin-2-ones <b>6</b> and fourteen pyrrolo[1,2-<i>d</i>][1,4]benzodiazepin-6-ones <b>4</b> and <b>5</b> were synthesized from 1-(2-aminophenyl)pent-4-en-1-one (<b>7</b>). Acylation of aniline <b>7</b> with <i>N</i>-Boc-amino
acids, olefin oxidation, Boc removal, and intramolecular Paal–Knorr
condensation gave <b>4</b> and <b>5</b>. Alternatively,
Boc removal prior to oxidation gave benzodiazepinones <b>6</b>, which were converted to <b>4</b> by ozonolysis and cyclization.
Comparison of dihedral angle values for the amino acid component from
X-ray analyses of <b>4g</b>, <b>5f</b>, and <b>6f</b> and related diazepinones has catalogued the manner by which ring
substituents affect the component’s ability to mimic the central
residues of γ-turns
