    A comparison of contralateral acoustic suppression of transient-evoked otoacoustic emissions measured in a group of individuals at risk of mild traumatic brain injury through participation in contact sports and a group of controls.

    This small scale preliminary study aims to assess whether athletes participating in sports with a high occurrence of sub-concussive injury present with reduced contralateral acoustic suppression (CAS) of transient-evoked otoacoustic emissions (TEOAEs). CAS TEOAEs has potential to be used as an objective assessment of concussion, either at the “pitch-side” or as part of a “return-to-play” protocol. Previous studies have shown reduced CAS TEOAEs in subjects experiencing auditory symptoms post-concussive injury (e.g. Nölle et al, 2004). However, the effect of repeated sub-concussive injuries on CAS TEOAEs in participants with no auditory symptoms is less well understood, though it has important implications for the use of CAS TEOAEs in sports-related concussion assessment. CAS TEOAEs was measured in accordance with the test paradigm described by Hood et al (1996). TEOAEs were recorded in response to 60 dB p.e. SPL clicks with and without a 65 dB SPL broadband noise presented to the contralateral ear. Measurements were made from both ears of nine athletes who regularly play sport with a high occurrence of sub-concussive injury and fifteen control subjects who had not previously played such sports. For each subject four CAS TEOAEs measurements were obtained. These were CAS TEOAEs in the (i) right and the (ii) left ear; (iii) the sum of CAS TEOAEs obtained in both ears; and (iv) the magnitude of the difference in CAS TEOAE between the right and left ears. Results showed no significant difference between the athlete and control group for all four measures of CAS TEOAEs. In addition, results showed large inter-subject variability in CAS TEOAEs in both groups. These preliminary findings have implications for the application of CAS TEOAEs in the assessment of sports-related concussion. First, CAS TEOAEs does not appear to be sensitive to cumulative sub-concussive injury, and second, the use of individual baseline measurements is indicated, rather than population-based normative data

    ‘Procedes Huc’: Voltaire, Newton, and Locke in Lettres Philosophiques

    In Lettres philosophiques, Letter XIII is devoted to Locke, as are Letters XIV–XVII to Newton. The ordering of these letters is not adequately explained by comparing the dates of birth or death of the two thinkers. For the Letter on Locke not only precedes but also ‘frames’ those on Newton, in the sense that it provides the reader with a guide through the philosophical intricacies of Letters XIV–XVII. This works in two ways. On the one hand, in order to defend Newton against his detractors Voltaire broadly adopts Locke’s perspective on the relation among words, ideas and things. On the other hand, he subtly and misleadingly grafts Locke’s epistemology onto the Principia, though it differs from Newton’s epistemology in significant respects. For Locke, unlike Newton, holds that we can identify fixed, permanent limits concerning what kind of thing humanity can know of matter and the universe. Voltaire presents Newton’s ideas as though they respected Locke’s limits. However, we can glimpse Voltaire’s own attitude in the final words of Letter XV: ‘Procedes huc, et non ibis amplius’: Voltaire agrees more closely with Locke than Newton concerning the limits of epistemology

    Triggering on electrons, jets and tau leptons with the CMS upgraded calorimeter trigger for the LHC RUN II

    The Compact Muon Solenoid (CMS) experiment has implemented a sophisticated two-level online selection system that achieves a rejection factor of nearly 10e5. During Run II, the LHC will increase its centre-of-mass energy up to 13 TeV and progressively reach an instantaneous luminosity of 2e34cm-2s-1. In order to guarantee a successful and ambitious physics programme under this intense environment, the CMS Trigger and Data acquisition (DAQ) system has been upgraded. A novel concept for the L1 calorimeter trigger is introduced the Time Multiplexed Trigger (TMT). In this design, nine main receive each all of the calorimeter data from an entire event provided by 18 preprocessors. This design is not different from that of the CMS DAQ and HLT systems. The advantage of the TMT architecture is that a global view and full granularity of the calorimeters can be exploited by sophisticated algortihms. The goal is to maintain the current thresholds for calorimeter objects and improve the performance for their selection. The performance of these algorithms will be demonstrated, both in terms of efficiency and rate reduction. The callenging aspects of the pile-up mitigation and firmware design will be presented

    Kdr-based insecticide resistance in Anopheles gambiae s.s populations in

    <p>Abstract</p> <p>Background</p> <p>The spread of insecticide resistance in the malaria mosquito, <it>Anopheles gambiae </it>is a serious threat for current vector control strategies which rely on the use of insecticides. Two mutations at position 1014 of the S<sub>6 </sub>transmembrane segment of domain II in the voltage gated sodium channel, known as <it>kdr </it>(<it>knockdown resistance</it>) mutations leading to a change of a Leucine to a Phenylalanine (L1014F) or to a Serine (L1014S) confer resistance to DDT and pyrethroid insecticides in the insect. This paper presents the current distribution of the <it>kdr </it>alleles in wild <it>Anopheles gambiae </it>populations in Cameroon.</p> <p>Results</p> <p>A total of 1,405 anopheline mosquitoes were collected from 21 localities throughout Cameroon and identified as <it>An. gambiae </it>(N = 1,248; 88.8%), <it>An. arabiensis </it>(N = 120; 8.5%) and <it>An. melas </it>(N = 37; 2.6%). Both <it>kdr </it>alleles 1014F and 1014S were identified in the M and S molecular forms of <it>An. gambiae </it>s.s. The frequency of the 1014F allele ranged from 1.7 to 18% in the M-form, and from 2 to 90% in the S-form. The 1014S allele ranged from 3-15% in the S-form and in the M-form its value was below 3%. Some specimens were found to carry both resistant <it>kdr </it>alleles.</p> <p>Conclusion</p> <p>This study provides an updated distribution map of the <it>kdr </it>alleles in wild <it>An. gambiae </it>populations in Cameroon. The co-occurrence of both alleles in malaria mosquito vectors in diverse ecological zones of the country may be critical for the planning and implementation of malaria vector control interventions based on IRS and ITNs, as currently ongoing in Cameroon.</p

    Utilisation of an operative difficulty grading scale for laparoscopic cholecystectomy

    Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p < 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p < 0.001). Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty

    X-ray emission from the Sombrero galaxy: discrete sources

    We present a study of discrete X-ray sources in and around the bulge-dominated, massive Sa galaxy, Sombrero (M104), based on new and archival Chandra observations with a total exposure of ~200 ks. With a detection limit of L_X = 1E37 erg/s and a field of view covering a galactocentric radius of ~30 kpc (11.5 arcminute), 383 sources are detected. Cross-correlation with Spitler et al.'s catalogue of Sombrero globular clusters (GCs) identified from HST/ACS observations reveals 41 X-rays sources in GCs, presumably low-mass X-ray binaries (LMXBs). We quantify the differential luminosity functions (LFs) for both the detected GC and field LMXBs, whose power-low indices (~1.1 for the GC-LF and ~1.6 for field-LF) are consistent with previous studies for elliptical galaxies. With precise sky positions of the GCs without a detected X-ray source, we further quantify, through a fluctuation analysis, the GC LF at fainter luminosities down to 1E35 erg/s. The derived index rules out a faint-end slope flatter than 1.1 at a 2 sigma significance, contrary to recent findings in several elliptical galaxies and the bulge of M31. On the other hand, the 2-6 keV unresolved emission places a tight constraint on the field LF, implying a flattened index of ~1.0 below 1E37 erg/s. We also detect 101 sources in the halo of Sombrero. The presence of these sources cannot be interpreted as galactic LMXBs whose spatial distribution empirically follows the starlight. Their number is also higher than the expected number of cosmic AGNs (52+/-11 [1 sigma]) whose surface density is constrained by deep X-ray surveys. We suggest that either the cosmic X-ray background is unusually high in the direction of Sombrero, or a distinct population of X-ray sources is present in the halo of Sombrero.Comment: 11 figures, 5 tables, ApJ in pres

    Incidence of Schizophrenia and Other Psychoses in England, 1950–2009: A Systematic Review and Meta-Analyses

    Background We conducted a systematic review of incidence rates in England over a sixty-year period to determine the extent to which rates varied along accepted (age, sex) and less-accepted epidemiological gradients (ethnicity, migration and place of birth and upbringing, time). Objectives To determine variation in incidence of several psychotic disorders as above. Data Sources Published and grey literature searches (MEDLINE, PSycINFO, EMBASE, CINAHL, ASSIA, HMIC), and identification of unpublished data through bibliographic searches and author communication. Study Eligibility Criteria Published 1950–2009; conducted wholly or partially in England; original data on incidence of non-organic adult-onset psychosis or one or more factor(s) pertaining to incidence. Participants People, 16–64 years, with first -onset psychosis, including non-affective psychoses, schizophrenia, bipolar disorder, psychotic depression and substance-induced psychosis. Study Appraisal and Synthesis Methods Title, abstract and full-text review by two independent raters to identify suitable citations. Data were extracted to a standardized extraction form. Descriptive appraisals of variation in rates, including tables and forest plots, and where suitable, random-effects meta-analyses and meta-regressions to test specific hypotheses; rate heterogeneity was assessed by the I2-statistic. Results 83 citations met inclusion. Pooled incidence of all psychoses (N = 9) was 31.7 per 100,000 person-years (95%CI: 24.6–40.9), 23.2 (95%CI: 18.3–29.5) for non-affective psychoses (N = 8), 15.2 (95%CI: 11.9–19.5) for schizophrenia (N = 15) and 12.4 (95%CI: 9.0–17.1) for affective psychoses (N = 7). This masked rate heterogeneity (I2: 0.54–0.97), possibly explained by socio-environmental factors; our review confirmed (via meta-regression) the typical age-sex interaction in psychosis risk, including secondary peak onset in women after 45 years. Rates of most disorders were elevated in several ethnic minority groups compared with the white (British) population. For example, for schizophrenia: black Caribbean (pooled RR: 5.6; 95%CI: 3.4–9.2; N = 5), black African (pooled RR: 4.7; 95%CI: 3.3–6.8; N = 5) and South Asian groups in England (pooled RR: 2.4; 95%CI: 1.3–4.5; N = 3). We found no evidence to support an overall change in the incidence of psychotic disorder over time, though diagnostic shifts (away from schizophrenia) were reported. Limitations Incidence studies were predominantly cross-sectional, limiting causal inference. Heterogeneity, while evidencing important variation, suggested pooled estimates require interpretation alongside our descriptive systematic results. Conclusions and Implications of Key Findings Incidence of psychotic disorders varied markedly by age, sex, place and migration status/ethnicity. Stable incidence over time, together with a robust socio-environmental epidemiology, provides a platform for developing prediction models for health service planning

    Performance of the CMS Cathode Strip Chambers with Cosmic Rays

    The Cathode Strip Chambers (CSCs) constitute the primary muon tracking device in the CMS endcaps. Their performance has been evaluated using data taken during a cosmic ray run in fall 2008. Measured noise levels are low, with the number of noisy channels well below 1%. Coordinate resolution was measured for all types of chambers, and fall in the range 47 microns to 243 microns. The efficiencies for local charged track triggers, for hit and for segments reconstruction were measured, and are above 99%. The timing resolution per layer is approximately 5 ns

    Evaluation of Clustering and Genotype Distribution for Replication in Genome Wide Association Studies: The Age-Related Eye Disease Study

    Genome-wide association studies (GWASs) assess correlation between traits and DNA sequence variation using large numbers of genetic variants such as single nucleotide polymorphisms (SNPs) distributed across the genome. A GWAS produces many trait-SNP associations with low p-values, but few are replicated in subsequent studies. We sought to determine if characteristics of the genomic loci associated with a trait could be used to identify initial associations with a higher chance of replication in a second cohort. Data from the age-related eye disease study (AREDS) of 100,000 SNPs on 395 subjects with and 198 without age-related macular degeneration (AMD) were employed. Loci highly associated with AMD were characterized based on the distribution of genotypes, level of significance, and clustering of adjacent SNPs also associated with AMD suggesting linkage disequilibrium or multiple effects. Forty nine loci were highly associated with AMD, including 3 loci (CFH, C2/BF, LOC387715/HTRA1) already known to contain important genetic risks for AMD. One additional locus (C3) reported during the course of this study was identified and replicated in an additional study group. Tag-SNPs and haplotypes for each locus were evaluated for association with AMD in additional cohorts to account for population differences between discovery and replication subjects, but no additional clearly significant associations were identified. Relying on a significant genotype tests using a log-additive model would have excluded 57% of the non-replicated and none of the replicated loci, while use of other SNP features and clustering might have missed true associations
