    Long-term effectiveness of agalsidase alfa enzyme replacement in Fabry disease : A Fabry Outcome Survey analysis

    Outcomes from 5 years of treatment with agalsidase alfa enzyme replacement therapy (ERT) for Fabry disease in patients enrolled in the Fabry Outcome Survey (FOS) were compared with published findings for untreated patients with Fabry disease. Data were extracted from FOS, a Shire-sponsored database, for comparison with data from three published studies. Outcomes evaluated were the annualized rate of change in estimated glomerular filtration rate (eGFR) and left ventricular mass indexed to height (LVMI) as well as time to and ages at a composite morbidity endpoint and at death. FOS data were extracted for 740 treated patients who were followed for a median of ~ 5 years. Compared with no treatment, patients treated with agalsidase alfa demonstrated slower decline in renal function and slower progression of left ventricular hypertrophy. Treated male patients with baseline eGFR < 60 mL/min/1.73 m 2 had a mean (standard error of the mean [SEM]) annualized change in eGFR of − 2.86 (0.53) mL/min/1.73 m 2 /y compared with − 6.8 (1.5) in the published untreated cohort. The mean (SEM) rate of LVMI increase with treatment was 0.33 (0.10) g/m 2.7 /y in males and 0.48 (0.09) in females, compared with 4.07 (1.03) in untreated males and 2.31 (0.81) in untreated females. Morbidity occurred later in treated patients, with ~ 16% risk of a composite morbidity event (26% in males) after 24 months with ERT versus ~ 45% without treatment, with first events and deaths also occurring at older ages in patients administered ERT (e.g., estimated median survival in treated males was 77.5 years versus 60 years in untreated males). Findings from these retrospective comparisons of observational data and published literature support the long-term benefits of ERT with agalsidase alfa for Fabry disease in slowing the progression of renal impairment and cardiomyopathy. Treatment also appeared to delay the onset of morbidity and mortality. Interpretation of these findings should take into account that they are based on retrospective comparisons with previously published data

    Ehrhart discovered that the function that counts the number of lattice points in dilations of an integral polytope is a polynomial. We call the coefficients of this polynomial Ehrhart coefficients, and say a polytope is Ehrhart positive if all Ehrhart coefficients are positive (which is not true for all integral polytopes). The main purpose of this article is to survey interesting families of polytopes that are known to be Ehrhart positive and discuss the reasons from which their Ehrhart positivity follows.     Interseismic coupling and seismic potential along the Central Andes subduction zone

    We use about two decades of geodetic measurements to characterize interseismic strain build up along the Central Andes subduction zone from Lima, Peru, to Antofagasta, Chile. These measurements are modeled assuming a 3-plate model (Nazca, Andean sliver and South America Craton) and spatially varying interseismic coupling (ISC) on the Nazca megathrust interface. We also determine slip models of the 1996 M_w = 7.7 Nazca, the 2001 M_w = 8.4 Arequipa, the 2007 M_w = 8.0 Pisco and the M_w = 7.7 Tocopilla earthquakes. We find that the data require a highly heterogeneous ISC pattern and that, overall, areas with large seismic slip coincide with areas which remain locked in the interseismic period (with high ISC). Offshore Lima where the ISC is high, a M_w∼8.6–8.8 earthquake occurred in 1746. This area ruptured again in a sequence of four M_w∼8.0 earthquakes in 1940, 1966, 1974 and 2007 but these events released only a small fraction of the elastic strain which has built up since 1746 so that enough elastic strain might be available there to generate a M_w > 8.5 earthquake. The region where the Nazca ridge subducts appears to be mostly creeping aseismically in the interseismic period (low ISC) and seems to act as a permanent barrier as no large earthquake ruptured through it in the last 500 years. In southern Peru, ISC is relatively high and the deficit of moment accumulated since the M_w∼8.8 earthquake of 1868 is equivalent to a magnitude M_w∼8.4 earthquake. Two asperities separated by a subtle aseismic creeping patch are revealed there. This aseismic patch may arrest some rupture as happened during the 2001 Arequipa earthquake, but the larger earthquakes of 1604 and 1868 were able to rupture through it. In northern Chile, ISC is very high and the rupture of the 2007 Tocopilla earthquake has released only 4% of the elastic strain that has accumulated since 1877. The deficit of moment which has accumulated there is equivalent to a magnitude M_w∼8.7 earthquake. This study thus provides elements to assess the location, size and magnitude of future large megathurst earthquakes in the Central Andes subduction zone. Caveats of this study are that interseismic strain of the forearc is assumed time invariant and entirely elastic. Also a major source of uncertainty is due to fact that the available data place very little constraints on interseismic coupling at shallow depth near the trench, except offshore Lima where sea bottom geodetic measurements have been collected suggesting strong coupling

    Brain Dynamics Underlying the Nonlinear Threshold for Access to Consciousness

    When a flashed stimulus is followed by a backward mask, subjects fail to perceive it unless the target-mask interval exceeds a threshold duration of about 50 ms. Models of conscious access postulate that this threshold is associated with the time needed to establish sustained activity in recurrent cortical loops, but the brain areas involved and their timing remain debated. We used high-density recordings of event-related potentials (ERPs) and cortical source reconstruction to assess the time course of human brain activity evoked by masked stimuli and to determine neural events during which brain activity correlates with conscious reports. Target-mask stimulus onset asynchrony (SOA) was varied in small steps, allowing us to ask which ERP events show the characteristic nonlinear dependence with SOA seen in subjective and objective reports. The results separate distinct stages in mask-target interactions, indicating that a considerable amount of subliminal processing can occur early on in the occipito-temporal pathway (<250 ms) and pointing to a late (>270 ms) and highly distributed fronto-parieto-temporal activation as a correlate of conscious reportability

    Asteroseismology with the Roman Galactic Bulge Time-Domain Survey

    Asteroseismology has transformed stellar astrophysics. Red giant asteroseismology is a prime example, with oscillation periods and amplitudes that are readily detectable with time-domain space-based telescopes. These oscillations can be used to infer masses, ages and radii for large numbers of stars, providing unique constraints on stellar populations in our galaxy. The cadence, duration, and spatial resolution of the Roman galactic bulge time-domain survey (GBTDS) are well-suited for asteroseismology and will probe an important population not studied by prior missions. We identify photometric precision as a key requirement for realizing the potential of asteroseismology with Roman. A precision of 1 mmag per 15-min cadence or better for saturated stars will enable detections of the populous red clump star population in the Galactic bulge. If the survey efficiency is better than expected, we argue for repeat observations of the same fields to improve photometric precision, or covering additional fields to expand the stellar population reach if the photometric precision for saturated stars is better than 1 mmag. Asteroseismology is relatively insensitive to the timing of the observations during the mission, and the prime red clump targets can be observed in a single 70 day campaign in any given field. Complementary stellar characterization, particularly astrometry tied to the Gaia system, will also dramatically expand the diagnostic power of asteroseismology.     Frequency and Prognostic Impact of ALK Amplifications and Mutations in the European Neuroblastoma Study Group (SIOPEN) High-Risk Neuroblastoma Trial (HR-NBL1)

    Purpose: In neuroblastoma (NB), the ALK receptor tyrosine kinase can be constitutively activated through activating point mutations or genomic amplification. We studied ALK genetic alterations in high-risk (HR) patients on the HR-NBL1/SIOPEN trial to determine their frequency, correlation with clinical parameters, and prognostic impact. Materials and methods: Diagnostic tumor samples were available from 1,092 HR-NBL1/SIOPEN patients to determine ALK amplification status (n = 330), ALK mutational profile (n = 191), or both (n = 571). Results: Genomic ALK amplification (ALKa) was detected in 4.5% of cases (41 out of 901), all except one with MYCN amplification (MNA). ALKa was associated with a significantly poorer overall survival (OS) (5-year OS: ALKa [n = 41] 28% [95% CI, 15 to 42]; no-ALKa [n = 860] 51% [95% CI, 47 to 54], [P 20% mutated allele fraction) in 10% of cases (76 out of 762) and at a subclonal level (mutated allele fraction 0.1%-20%) in 3.9% of patients (30 out of 762), with a strong correlation between the presence of ALKm and MNA (P < .001). Among 571 cases with known ALKa and ALKm status, a statistically significant difference in OS was observed between cases with ALKa or clonal ALKm versus subclonal ALKm or no ALK alterations (5-year OS: ALKa [n = 19], 26% [95% CI, 10 to 47], clonal ALKm [n = 65] 33% [95% CI, 21 to 44], subclonal ALKm (n = 22) 48% [95% CI, 26 to 67], and no alteration [n = 465], 51% [95% CI, 46 to 55], respectively; P = .001). Importantly, in a multivariate model, involvement of more than one metastatic compartment (hazard ratio [HR], 2.87; P < .001), ALKa (HR, 2.38; P = .004), and clonal ALKm (HR, 1.77; P = .001) were independent predictors of poor outcome. Conclusion: Genetic alterations of ALK (clonal mutations and amplifications) in HR-NB are independent predictors of poorer survival. These data provide a rationale for integration of ALK inhibitors in upfront treatment of HR-NB with ALK alterations.Key Objective: High risk neuroblastoma (HR-NB) is one of the most difficult childhood cancers to cure. This study examined whether the presence of an ALK alteration (amplification or mutation) was associated with a poor prognosis in a large patient series treated on the prospective European high-risk neuroblastoma trial (HR-NBL1). Knowledge Generated: We found that ALK amplification or clonal mutation was associated with inferior prognosis in patients with HR-NB and both are independent prognostic variables on multivariate analysis. To our knowledge, this is the first study to report the highly prognostic significance of ALK amplification in HR-NB. Relevance: As ALK can be targeted therapeutically, this study convincingly argues for the introduction of ALK inhibitors for upfront management of patients with HR-NB with ALK aberrations.     Linking cause and effect: Nanoscale vibrational spectroscopy of space weathering from asteroid Ryugu

    Airless bodies are subjected to space-weathering effects that modify the first few microns of their surface. Therefore, understanding their impact on the optical properties of asteroids is key to the interpretation of their color variability and infrared reflectance observations. The recent Hayabusa2 sample return mission to asteroid Ryugu offers the first opportunity to study these effects, in the case of the most abundant spectral type among the main-asteroid belt, C-type objects. This study employs vibrational electron energy-loss spectroscopy in the transmission electron microscope to achieve the spatial resolution required to measure the distinct mid-infrared spectral signature of Ryugu's space-weathered surface. The comparison with the spectrum of the pristine underlying matrix reveals the loss of structural -OH and C-rich components in the space-weathered layers, providing direct experimental evidence that exposure to the space environment tends to mask the optical signatures of phyllosilicates and carbonaceous matter.
