    Understanding the retreat of the Jurassic Cantabrian coast (N. Spain): Comprehensive monitoring and 4D evolution model of the Tazones Lighthouse landslide

    Forecasting coastal dynamics and sea cliff retreat under different sea level rise scenarios requires a good understanding of the conditioning factors and their relative contribution to cliff stability. The so-called Jurassic Cantabrian Coast extends along 76 km of the coastline of the Asturias region (N Spain) and is well-known worldwide due to its paleontological heritage, in particular the presence of dinosaur remains and footprints. The abundance of stratigraphic, paleontological and tectonic studies contrasts with the scarcity of studies focused on the stability of this rocky coastline where cliffs predominate, sometimes exceeding 120 m in height. In fact, evidence of current and recent instability processes can be observed along the entire coastline. In this regard, continuous monitoring is crucial to understand ongoing instabilities in rocky coastlines, as in these settings some instabilities might initiate as slow movements that induce subtle topographic changes whose detection from either satellite or aerial imagery is problematic due to the spatial and temporal resolutions. This contribution presents a 4D evolution model of a key site, the Tazones Lighthouse landslide, located on the Cantabrian Coast of Asturias (N Spain), which affects subvertical rocky cliffs sculpted in the Jurassic bedrock made of alternating sandstone and marl. A high resolution multiapproach methodology was developed in order to understand its structure and kinematic characteristics, including: i) interpretation of aerial photographs and unmanned aerial photogrammetric surveys (UAV); ii) 22 monthly monitoring campaigns by total station; iii) 5 manual boreholes; iv) geomechanical characterization of the cliff bedrock; v) geomorphological evidence mapping; vi) analysis of landscape deformations obtained from UAV; and vii) precipitation, soil moisture and significant wave height (Hs) data analysis. The results show that the slope evolves by means of a complex-type mass movement, which combines translational and sliding mechanisms, and occupies tens of thousands of square meters. DTM and fieldwork analysis indicate that mass movement is mainly controlled by bedrock discontinuities (S0, 360/15-17; J1, 262/85; J2 166/75). The most important accelerations of slope movement correlate very well with rainfall, soil moisture and waves. Thus, the largest displacements occurring in January and October–November 2019, coincide with 2 periods of storms (maximum 24-h rainfall of 64.5 mm and 82.1 mm and maximum Hs of 6.54 and 9.09, respectively) and soil moisture values above 90%. Half of the markers moved more than 1 m and one of them exceeded 15 m.     [ResumenLa aplicación de los archivos de prensa al estudio de inestabilidades de ladera en Asturias plantea diversas limitaciones metodológicas. Entre ellas destacan la baja frecuencia de noticias referentes a inestabilidades, la recogida prácticamente exclusiva de aquellas que afectan a infraestructuras y la ausencia de una toma de datos de carácter científico. Sin embargo, el análisis de la información de 209 noticias pertenecientes al período 1980-94 ha puesto de manifiesto su utilidad en diversos aspectos, como la ubicación global de las inestabilidades de ladera en la Cuenca Carbonífera Central, y la valoración de su impacto sobre infraestructuras. Es destacable asimismo la posibilidad que ofrece este tipo de archivos como regis·tro cronológico de inestabilidades, lo que ha permitido establecer una aproximación a la relación temporal entre los eventos de precipitación e inestabilidad.[Abstract] The application of newspaper archives to the study of slope instabilities in Asturias has several methodological limitations. Sorne of them are the low frequency of news refering to slope instability and the publication of only those affecting people and infrastructures without a scientifical data collection. Nevertheless, the analysis of the information about 209 news in the period 198094 has evidenced its utility in showing the spatiallocation of instabilities in the Central Coal Basin, and giving an approach to its impact over infrastructures. It is also worth to point out the possibility of using these archives as chronological instability records. This has allowed the establishment of an approach to the temporal relationships between rainfal1 and instability events for the studied interval

    Modeling and simulating for the treatment of subjectivity in the process of choosing personnel using fuzzy logic

    Every day organizations pay more attention to Human Resources Management, because the human factor is preponderant in the results of it. One of the important policies is the Selection of Personnel, these are needed for their decision-making results, which in many organizations is done in a subjective manner and which brings consequences not very favorable to them. Taking this problem into account, it is decided to design and apply procedures and tools of fuzzy mathematics to reduce subjectivity and uncertainty in decision-making, creating work algorithms for this policy that includes multifactorial weights and analysis with measurement indicators that they allow tangible and reliable results. In this case of personnel selection, eight candidates were taken into account and by applying a diffuse evaluation system, the candidate with the highest rating of 98% was chosen. This indicates that subjectivity was reduced when choosing the best evaluated candidate

    Validation of score in mna scale like nutritional risk factor in institutionalized geriatric patients with moderate and severe cognitive impairment

    Introducción: La valoración geriátrica integral incluye el examen del apartado nutricional debido a la alta prevalencia de desnutrición en este tipo de pacientes; especialmente en los pacientes con deterioro cognitivo asociado. Las escalas de cribado del estado nutricional presentan preguntas de autopercepción subjetiva de difícil respuesta en pacientes mayores con demencia. Objetivo: Estudiar la especificidad, el valor predictivo positivo y la sensibilidad de la escala MNA para la detección de malnutrición en pacientes diagnosticados de enfermedad de Alzheimer con deterioro cognitivo avanzado. Material y métodos: Se diseñó un estudio descriptivo poblacional con una muestra de 52 pacientes mayores de 70 años, institucionalizados, con enfermedad de Alzheimer grado moderado y severo. Se estudió la sensibilidad, especificidad y valor predictivo positivo de la escala MNA respecto a los parámetros de malnutrición del American Institute of Nutrition (AIN). Resultados: Los valores de sensibilidad, especificidad y valor predictivo positivo son 60%, 94,7% y 93,8% respectivamente. Existe una correlación significativa (p < 0,001) ente la puntuación obtenida en la escala MNA y la escala de riesgo de caídas Tinneti (r = 0,577), de función Barthel (r = 0,742), de valoración cognitiva MEC (r = 0,651) y los niveles de creatinina (r = 0,402). Más del 50% de la muestra presentó al menos un parámetro de malnutrición AIN alterado. Conclusiones: La escala MNA presenta una menor sensibilidad y especificidad en estos pacientes. El diseño de una escala de valoración nutricional sin valoraciones subjetivas y sólo con parámetros objetivos podría mejorar la eficacia de la misma en ancianos institucionalizados con deterioro cognitivo moderado y severo.Introduction: comprehensive geriatric assessment includes examination of the nutritional status given the high prevalence of hyponutrition in this kind of patients, particularly in patients with associated cognitive impairment. Scales for screening the nutritional status include questions on self-perception difficult to answer by demented elder patients. Objective: To study the specificity, the positive predictive value, and the sensitivity of the MNA scale to detect malnutrition in patients diagnosed with Alzheimer’s disease with advanced cognitive impairment. Material and methods: a population-based descriptive study with a sample of 52 patients older than 70 years, institutionalized, and with moderate-severe Alzheimer’s disease was designed. The sensitivity, specificity, and positive predictive value of MNA scale were studied regarding the parameters on malnutrition of the American Institute of Nutrition (AIN). Results: the sensitivity, specificity, and positive predictive values were 60%, 94.7%, and 93.8%, respectively. There was a significant correlation (p < 0.001) between the score obtained with the MNA Scale and the Tinneti’s Risk of Fall Scale (r = 0.577), the Barthel’s function (r = 0.742), the MCT cognitive assessment (r = 0.651), and creatinine levels (r = 0.402). More than 50% of the sample presented at least one malnutrition parameter altered. Conclusions: the MNA Scale presents lower sensitivity and specificity in these patients. Designing a nutritional assessment scale without subjective evaluations and only with objective parameters might improve its efficiency in institutionalized elderly patients with moderate-severe cognitive impairment.Nutrición humana y dietétic

    The Mexican consensus on the diagnosis, treatment, and prevention of NSAID-induced gastropathy and enteropathy

    Más de 30 millones de personas consumen diariamente antiinflamatorios noesteroideos (AINE) en el mundo, y este consumo se ve incrementado anualmente. Aunque losAINE poseen propiedades analgésicas y antiinflamatorias, sus eventos adversos gastrointesti-nales son bien reconocidos. En nuestro país no existía un consenso respecto al diagnóstico,tratamiento y prevención de la gastropatía y la enteropatía por AINE, por lo que la AsociaciónMexicana de Gastroenterología reunió a un grupo de expertos para establecer recomendacionesde utilidad para la comunidad médica. En este consenso se emitieron 33 recomendaciones. Elconsenso destaca que el riesgo de toxicidad gastrointestinal de los AINE varía según el fármacoempleado y su farmacocinética, lo cual debe ser considerado al momento de su prescripción. Losfactores de riesgo de complicación gastroduodenal por AINE son: antecedente de úlcera pép-tica, edad mayor a 65 a˜nos, dosis altas del AINE, infección por Helicobacter pylori (H.pylori), ypresencia de comorbilidades graves. Los síntomas y el da˜no gastroduodenal inducido por AINEson variables ya que puede cursar asintomático o manifestarse como anemia por deficiencia dehierro, hemorragia, estenosis y perforación. La cápsula endoscópica y la enteroscopia son méto-dos diagnósticos directos en la enteropatía por AINE. Respecto a la prevención, se recomiendaprescribir la dosis mínima necesaria de un AINE para obtener el efecto deseado y durante elmenor tiempo. Finalmente, los inhibidores de la bomba de protones (IBP) representan el están-dar de oro para la profilaxis y tratamiento de los efectos gastroduodenales, mas no son útilesen la enteropatía

    Dark Matter and Fundamental Physics with the Cherenkov Telescope Array

    The Cherenkov Telescope Array (CTA) is a project for a next-generation observatory for very high energy (GeV-TeV) ground-based gamma-ray astronomy, currently in its design phase, and foreseen to be operative a few years from now. Several tens of telescopes of 2-3 different sizes, distributed over a large area, will allow for a sensitivity about a factor 10 better than current instruments such as H.E.S.S, MAGIC and VERITAS, an energy coverage from a few tens of GeV to several tens of TeV, and a field of view of up to 10 deg. In the following study, we investigate the prospects for CTA to study several science questions that influence our current knowledge of fundamental physics. Based on conservative assumptions for the performance of the different CTA telescope configurations, we employ a Monte Carlo based approach to evaluate the prospects for detection. First, we discuss CTA prospects for cold dark matter searches, following different observational strategies: in dwarf satellite galaxies of the Milky Way, in the region close to the Galactic Centre, and in clusters of galaxies. The possible search for spatial signatures, facilitated by the larger field of view of CTA, is also discussed. Next we consider searches for axion-like particles which, besides being possible candidates for dark matter may also explain the unexpectedly low absorption by extragalactic background light of gamma rays from very distant blazars. Simulated light-curves of flaring sources are also used to determine the sensitivity to violations of Lorentz Invariance by detection of the possible delay between the arrival times of photons at different energies.     Th1 and Th17 hypercytokinemia as early host response signature in severe pandemic influenza

    Abstract Introduction Human host immune response following infection with the new variant of A/H1N1 pandemic influenza virus (nvH1N1) is poorly understood. We utilize here systemic cytokine and antibody levels in evaluating differences in early immune response in both mild and severe patients infected with nvH1N1. Methods We profiled 29 cytokines and chemokines and evaluated the haemagglutination inhibition activity as quantitative and qualitative measurements of host immune responses in serum obtained during the first five days after symptoms onset, in two cohorts of nvH1N1 infected patients. Severe patients required hospitalization (n = 20), due to respiratory insufficiency (10 of them were admitted to the intensive care unit), while mild patients had exclusively flu-like symptoms (n = 15). A group of healthy donors was included as control (n = 15). Differences in levels of mediators between groups were assessed by using the non parametric U-Mann Whitney test. Association between variables was determined by calculating the Spearman correlation coefficient. Viral load was performed in serum by using real-time PCR targeting the neuraminidase gene. Results Increased levels of innate-immunity mediators (IP-10, MCP-1, MIP-1β), and the absence of anti-nvH1N1 antibodies, characterized the early response to nvH1N1 infection in both hospitalized and mild patients. High systemic levels of type-II interferon (IFN-γ) and also of a group of mediators involved in the development of T-helper 17 (IL-8, IL-9, IL-17, IL-6) and T-helper 1 (TNF-α, IL-15, IL-12p70) responses were exclusively found in hospitalized patients. IL-15, IL-12p70, IL-6 constituted a hallmark of critical illness in our study. A significant inverse association was found between IL-6, IL-8 and PaO2 in critical patients. Conclusions While infection with the nvH1N1 induces a typical innate response in both mild and severe patients, severe disease with respiratory involvement is characterized by early secretion of Th17 and Th1 cytokines usually associated with cell mediated immunity but also commonly linked to the pathogenesis of autoimmune/inflammatory diseases. The exact role of Th1 and Th17 mediators in the evolution of nvH1N1 mild and severe disease merits further investigation as to the detrimental or beneficial role these cytokines play in severe illness

    CIBERER : Spanish national network for research on rare diseases: A highly productive collaborative initiative

This innovative research structure comprises 11 different specific areas dedicated to the main public health priorities in the National Health System. CIBERER, the thematic area of CIBER focused on rare diseases (RDs) currently consists of 75 research groups belonging to universities, research centers, and hospitals of the entire country. CIBERER's mission is to be a center prioritizing and favoring collaboration and cooperation between biomedical and clinical research groups, with special emphasis on the aspects of genetic, molecular, biochemical, and cellular research of RDs. This research is the basis for providing new tools for the diagnosis and therapy of low-prevalence diseases, in line with the International Rare Diseases Research Consortium (IRDiRC) objectives, thus favoring translational research between the scientific environment of the laboratory and the clinical setting of health centers. In this article, we intend to review CIBERER's 15-year journey and summarize the main results obtained in terms of internationalization, scientific production, contributions toward the discovery of new therapies and novel genes associated to diseases, cooperation with patients' associations and many other topics related to RD research

    Is diet partly responsible for differences in COVID-19 death rates between and within countries?

Among the several explanations proposed, an early and large testing of the population was put forward. Most current debates on COVID-19 focus on the differences among countries, but little attention has been given to regional differences and diet. The low-death rate European countries (e.g. Austria, Baltic States, Czech Republic, Finland, Norway, Poland, Slovakia) have used different quarantine and/or confinement times and methods and none have performed as many early tests as Germany. Among other factors that may be significant are the dietary habits. It seems that some foods largely used in these countries may reduce angiotensin-converting enzyme activity or are anti-oxidants. Among the many possible areas of research, it might be important to understand diet and angiotensin-converting enzyme-2 (ACE2) levels in populations with different COVID-19 death rates since dietary interventions may be of great benefit.Peer reviewe
