    An innovative solid liquid extraction technology: use of the naviglio extractor for the production of lemon liquor.

    This document is a review on solid-liquid extractive techniques and describes an innovative solid-liquid extraction technology using the Naviglio Extractor®. Also explained is an application for the production of alcoholic extract from lemon peel. The alcoholic extract, mixed with a sugar and water solution in the right proportions is used to make a well known Italian lemon liquor commonly named "limoncello". Lemon liquor is obtained utilizing the Naviglio Extractor®; the procedure used is fast and efficient and uses about half the weight of lemon peel per volume of ethyl alcohol used for the extraction of the odorous and taste responsible compounds, compared to the commonly used extraction procedures. To test the consumer’s preference and compare the taste of the liquor obtained with that obtained by peel maceration from the same lot of lemons and obtained using the traditional recipe, a consumer test has been carried out. One hundred people, chosen from among frequent consumers of limoncello, tasted the two lemon liquors, and evaluated intensity of aroma, colour, alcohol taste and sweetness. In about 81% of the preferences, the liquor obtained using the Naviglio Extractor® was preferred. The extraction process used allows the ethanol from used up lemon peel to be totally recovered so that these can be disposed of as non toxic waste or used in agriculture or as cattle feed

    Appropriateness of learning curve for carotid artery stenting: An analysis of periprocedural complications

    ObjectivesCerebral embolism is the first cause of neurologic complications of carotid artery stenting (CAS). A large debate has been raised to identify the caseload necessary for an appropriate learning curve before systematic use of CAS. This study examined (1) the timing of periprocedural complications during CAS and how these complications vary over time to identify factors that contribute to neurologic morbidity and (2) a sufficient number of procedures for adequate training.MethodsDuring 2001 to 2006, 627 CAS procedures with cerebral protection devices (CPD) were performed in a single vascular surgery center by a team including a vascular surgeon and an interventional radiologist. These represented 38% of a total of 1598 carotid revascularizations performed in the same interval. CAS procedures were divided into two groups according to time interval: the first period, 2001 to 2003, included 195 CAS procedures, and the second period, 2004 to 2006, included 432 CAS procedures. During each CAS procedure, five major steps were considered: phase 1, or the catheterization phase, included the passage of the aortic arch, catheterization of the target vessel, and introduction of a guiding catheter or sheath. Phase 2, or the crossing stenosis phase, included the placement of a CPD. Phase 3, or the stent ballooning phase, included predilation (when indicated), stent implantation, postdilation, and recovery of the protection system. Phase 4, or the early postinterventional phase, included the first 24 hours after leaving the catheterization table. Phase 5, or the late postinterventional phase, included the interval from the first postoperative day to 30 days.ResultsAt 30 days, 10 major strokes (2 of which were fatal) and 1 cardiac death occurred, for an overall major stroke/death rate of 1.75%. Furthermore, 18 minor strokes (2.9%) were recorded. By analyzing the occurrence of major strokes according to the three intraprocedural phases, four occurred in phase 1 and six in phase 3. All strokes but one were ischemic; six were ipsilateral, three were contralateral, and one was posterior. Minor strokes occurred prevalently after the procedure (11 in phase 4, 5 in phase 5, and 1 for phases 1 and 3). Comparing the first with the second interval of the study period, the 30-day major stroke and death rate decreased from 3.1% to 0.9% (P = .047), and the 30-day any stroke and death rate decreased from 8.2% to 2.7% (P = .005). According to multivariate analysis, study interval (hazard ratio, 3.68; 95% confidence interval, 1.49-9.01; P = .005) and age (hazard ratio, 1.06; 95% confidence interval, 1.00-1.12; P = .05) were significant predictors of stroke.ConclusionsA large proportion of major strokes (4/10) from CAS cannot be prevented by using CPD, because these strokes occur during catheterization (phase 1). This finding, together with the significant decrease in the overall stroke/death rate between the first and the last interval of the study period, enhances the importance of an appropriate learning curve that involves a caseload larger than that generally accepted for credentialing. The noticeable number of postprocedural cerebral embolizations leading to minor strokes and occurring in the early and late postinterventional phases (16/18) is likely due to factors less strictly related to the learning-curve effect, such as stent design and medical therapy. Moreover, expertise in selecting material and design of the stents according to different vessel morphology, in association with correct medical treatment, may be useful in reducing the number of minor strokes that occur in the later postinterventional phases of CAS

    Mountain Resilience:A Systematic Literature Review and Paths to the Future

    Mountains are home to a considerable share of the human population. Around a billion people live in mountainous areas, which harbor rich natural and sociocultural diversity. Today, many people living in mountainous areas worldwide face fundamental changes to their cultural and economic living conditions. At the same time, mountain communities have defied harsh environments in the past by adapting to changing natural conditions and showing remarkable levels of resilience. In this review paper, we provide a comprehensive overview of English-language scientific literature on resilience-related topics in mountain areas based on a systematic review of the Scopust literature database. We propose a structured starting point for science–practice interactions and concrete action-based activities to support livelihoods and strengthen resilience in mountain areas. We suggest that existing knowledge gaps can be addressed by relying on local knowledge and cocreating solutions with communities. In this way, we can build innovative capacity and actively buffer against the impact of crises while supporting deliberate transformation toward sustainability and regeneration to further enhance resilience.ISSN:0276-4741ISSN:1994-715

    The NANOGrav 15-Year Data Set: Detector Characterization and Noise Budget

    Pulsar timing arrays (PTAs) are galactic-scale gravitational wave detectors. Each individual arm, composed of a millisecond pulsar, a radio telescope, and a kiloparsecs-long path, differs in its properties but, in aggregate, can be used to extract low-frequency gravitational wave (GW) signals. We present a noise and sensitivity analysis to accompany the NANOGrav 15-year data release and associated papers, along with an in-depth introduction to PTA noise models. As a first step in our analysis, we characterize each individual pulsar data set with three types of white noise parameters and two red noise parameters. These parameters, along with the timing model and, particularly, a piecewise-constant model for the time-variable dispersion measure, determine the sensitivity curve over the low-frequency GW band we are searching. We tabulate information for all of the pulsars in this data release and present some representative sensitivity curves. We then combine the individual pulsar sensitivities using a signal-to-noise-ratio statistic to calculate the global sensitivity of the PTA to a stochastic background of GWs, obtaining a minimum noise characteristic strain of 7×10157\times 10^{-15} at 5 nHz. A power law-integrated analysis shows rough agreement with the amplitudes recovered in NANOGrav's 15-year GW background analysis. While our phenomenological noise model does not model all known physical effects explicitly, it provides an accurate characterization of the noise in the data while preserving sensitivity to multiple classes of GW signals.Comment: 67 pages, 73 figures, 3 tables; published in Astrophysical Journal Letters as part of Focus on NANOGrav's 15-year Data Set and the Gravitational Wave Background.     The NANOGrav 15-year Data Set: Evidence for a Gravitational-Wave Background

    We report multiple lines of evidence for a stochastic signal that is correlated among 67 pulsars from the 15-year pulsar-timing data set collected by the North American Nanohertz Observatory for Gravitational Waves. The correlations follow the Hellings-Downs pattern expected for a stochastic gravitational-wave background. The presence of such a gravitational-wave background with a power-law-spectrum is favored over a model with only independent pulsar noises with a Bayes factor in excess of 101410^{14}, and this same model is favored over an uncorrelated common power-law-spectrum model with Bayes factors of 200-1000, depending on spectral modeling choices. We have built a statistical background distribution for these latter Bayes factors using a method that removes inter-pulsar correlations from our data set, finding p=103p = 10^{-3} (approx. 3σ3\sigma) for the observed Bayes factors in the null no-correlation scenario. A frequentist test statistic built directly as a weighted sum of inter-pulsar correlations yields p=5×1051.9×104p = 5 \times 10^{-5} - 1.9 \times 10^{-4} (approx. 3.54σ3.5 - 4\sigma). Assuming a fiducial f2/3f^{-2/3} characteristic-strain spectrum, as appropriate for an ensemble of binary supermassive black-hole inspirals, the strain amplitude is 2.40.6+0.7×10152.4^{+0.7}_{-0.6} \times 10^{-15} (median + 90% credible interval) at a reference frequency of 1/(1 yr). The inferred gravitational-wave background amplitude and spectrum are consistent with astrophysical expectations for a signal from a population of supermassive black-hole binaries, although more exotic cosmological and astrophysical sources cannot be excluded. The observation of Hellings-Downs correlations points to the gravitational-wave origin of this signal.Comment: 30 pages, 18 figures.     Search for dark matter produced in association with bottom or top quarks in √s = 13 TeV pp collisions with the ATLAS detector

    A search for weakly interacting massive particle dark matter produced in association with bottom or top quarks is presented. Final states containing third-generation quarks and miss- ing transverse momentum are considered. The analysis uses 36.1 fb−1 of proton–proton collision data recorded by the ATLAS experiment at √s = 13 TeV in 2015 and 2016. No significant excess of events above the estimated backgrounds is observed. The results are in- terpreted in the framework of simplified models of spin-0 dark-matter mediators. For colour- neutral spin-0 mediators produced in association with top quarks and decaying into a pair of dark-matter particles, mediator masses below 50 GeV are excluded assuming a dark-matter candidate mass of 1 GeV and unitary couplings. For scalar and pseudoscalar mediators produced in association with bottom quarks, the search sets limits on the production cross- section of 300 times the predicted rate for mediators with masses between 10 and 50 GeV and assuming a dark-matter mass of 1 GeV and unitary coupling. Constraints on colour- charged scalar simplified models are also presented. Assuming a dark-matter particle mass of 35 GeV, mediator particles with mass below 1.1 TeV are excluded for couplings yielding a dark-matter relic density consistent with measurements

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely
